Advance clinical practitioners (ACPs) can have a big role to play in the emergency department, but their career paths are not always well understood. This episode features three papers from the journal which begin to address that - a census survey of over 1000 ACPs, a qualitative study on their experiences in emergency care, and a commentary on how to improve ACP training. There is also a paper from Canada discussing considerations to be made for patients living with dementia, as well as their care partners who may be accompanying them to the hospital. Recently, the University of Florida Institute of Food and Agricultural Sciences (UF/IFAS) reported research on the development of a new type of citrus tree that can fight off the HLB-spreading Asian citrus psyllid (ACP). The genetically modified tree has only been tested in the lab and the greenhouse, but the research looks promising. The approach involves inserting a gene into a citrus tree that produces a protein that can kill young ACPs. That gene normally occurs in a soil-borne bacterium called Bacillus thuringiensis (Bt). The use of Bt has long been used in row crops like cotton and corn to protect against certain pests. While the citrus Bt research is promising, the story was picked up by (non-agricultural) media and was portrayed as a “cure” for HLB that was right around the corner. Michael Rogers, director of the UF/IFAS Research and Education Center in Lake Alfred, Florida, addressed this during the January 2025 All In For Citrus podcast. He wanted to set proper expectations about the Bt research and potential time frames. He noted, assuming that the research progressed well, it could be up to five to eight years before it could be available to growers. “This looks great in the greenhouse, but we really need to get it out into the field to see how it does in real-world conditions,” Rogers said. “That's what the researchers are doing right now, getting more plants into the field. And we'll probably need at least three years of field data to show how these plants perform in terms of ACP control, how much HLB is reduced in the trees and if there is any resistance developing. And how do the plants perform (agronomically)? “I say all this to be sure growers understand that this is not far enough along to be making business decisions on right now. But we want to be sure folks are aware the research is moving forward.” Also in the January episode of All In For Citrus, UF/IFAS researchers who spoke during the recent Citrus Nutrition Day provide a recap of the event. Neste episódio, conversamos com Andreia Bagatin, promotora de Justiça do MPPR, sobre os limites territoriais dos efeitos da sentença nas Ações Civis Públicas. No regime jurídico brasileiro, o instituto das ações civis públicas desempenha um papel crucial na defesa dos direitos difusos e coletivos da sociedade. Essas ações são instrumentos poderosos que permitem ao Ministério Público, como defensor dos interesses coletivos, buscar a reparação de danos ambientais, consumeristas, patrimoniais e sociais, além da promoção da igualdade e justiça social. Dados do Cadastro Nacional de Ações Coletivas (o Cacol) do Conselho Nacional de Justiça apontam que, dentre os legitimados para entrar com ACPs, os Ministérios Públicos estaduais e o Federal são os principais autores, com quase 60% das ações. Os Ministérios Públicos, inclusive, têm como dever legal a promoção de ACPs, caso seja necessário adotar providências para prevenir ou reparar danos a bens e direitos que estejam sob a sua tutela. O julgamento do STF referente a constitucionalidade do art.16 da Lei de Ação Civil Pública, alterado pela Lei 9.494/1997, questiona a restrição da eficácia territorial que foi imposta no exercício desses poderes. Em abril de 2021, o Supremo decidiu que os efeitos de decisão em ACPs não devem ter limites territoriais, caso contrário, haveria restrição ao acesso à justiça e violação do princípio da igualdade. Por outro lado, críticos da decisão apontam que sentença proferida em sede de ação civil pública deveria respeitar os limites territoriais do órgão prolator da sentença, sob o risco de se aplicar uma mesma regra para distintas regiões do país, com diferentes realidades econômicas e regulatórias, causando insegurança para o ambiente de negócios. Quais as consequências que esta decisão tem acarretado para o regime jurídico brasileiro? Como o Ministério Público tem atuado na aplicação desta normativa? In this conversation, we will look at the life of an Advanced Clinical Practitioner (ACP). We will dig into what the training looks like, the autonomy involved, the various specialisms you can take, what Dan has learnt along the way and much more. As more and more alternative career pathways are opening, we explore if this one is an attractive option. We also look at the alignment to the Royal College of Emergency Medicine (RCEM) curriculum and how this shapes Dan's practice, skills and clinical decision making. ACPs play a vital role in the delivery of emergency care across the UK. They now make up over 10% of the college membership. There are over one thousand ACP members and this number is rapidly increasing. ACPs are represented on all major RCEM Committees. Hutchings and Reed discuss the six steps to creating an antiracist school system in this podcast. This provocative and eye-opening discussion helps listeners understand how school systems and policies have led to discrimination by sharing clear examples of discriminatory policies. Hutchings and Reed believe that racism is not always intentional, but anti-racism must be; getting into good trouble is the way forward. Hutchings, Jr. returned to Alexandria in July 2018 to serve as superintendent of Alexandria City Public Schools (ACPS). Before this role, he served as a teacher, assistant principal, and middle school principal before becoming superintendent of Shaker Heights Schools in Cleveland, Ohio, in 2013. In 2009, he was named Tennessee Middle School Principal of the Year. In 2018, he was selected for the National Superintendent of the Year Award by the National Alliance of Black School Educators. He is a recent co-author of the book, Getting Into Good Trouble: A Guide to Becoming An Antiracist School System. Dr. Hutchings holds a doctorate in educational policy, planning, and leadership from the College of William and Mary. Dr. Hutchings is also a proud graduate of T.C. Williams High School at ACPS. Doug Reed's teaching and research interests center on educational politics and policymaking, American political development, and American constitutional law. He recently wrote Building the Federal Schoolhouse: Localism and the American Education State (Oxford University Press, 2014). The book examines how changes in the federal role in public education have reshaped local politics and how, jointly, federal, state, and local actors have constructed the American education state over the past 50 years. "Politics of Proficiency: Common Core and the Inequalities of American Education." is the title of his second book. Dr. Reed is the co-founder and director of the Program on Education, Inquiry, and Justice at Georgetown. This recently-created program views education and the teaching arts as a central element of the liberal arts. Doug Reed received a Spencer Foundation Post-Doctoral Fellowship from the National Academy of Education and was named a Carnegie Scholar by the Carnegie Corporation of New York. Dr. Hatem has pioneered the application of educational theory to medical training of both hospital staff and practicing physicians. He served as the Director of the Rabkin Fellowship in Medical Education which is devoted to training faculty to become clinician-teachers. He has also served as a consultant to numerous medical schools in the development of their medical education programs in Argentina, India, Thailand, and Saudi Arabia. Dr. Hatem has additionally served as the co-editor-in-chief of ACPs's Medical Knowledge Self Assessment Program and has developed the widely used series of “Can't Miss Cases”. In life, we will face adversity. That is guaranteed. Today, Dr. Charles Hatem recalls the wise words of Viktor Frankl as he looks at the profession of medicine. The hardships we face, the struggles we encounter, and the obstacles in front of us are inevitable. The key lies in how we choose to face that adversity. That is what defines our true character. Dr. Hatem shares many wise stories about the value of kindness, of taking the time to listen and help others, and what success means to him. He believes that success is not about title; it is more so about the commitment to others, being the best physician he can be, and to be grateful and astonished by life. And that at the root of it all—is uncovering who we are as humans. Pearls of Wisdom: 1. Understand the value of kindness, and how we can apply it in our life: Allocate the time to hear someone else's story, despite anything else going on in our lives. Use that information to help that person. That is the most basic definition of kindness in medical practice. 2. The development of who we are as a person should be paralleled to our pursuit of achievement.  We need to honor love, well-being of self, and commitment for the others in our lives, even as we focus on achievements. 3. In life, we will face adversity. That is guaranteed. What is important is how we face that adversity. It's how we choose to deal with it that defines our true character.

I’ll make my pitches later in the program but for now, my sincere gratitude to each of you. In today’s newsletter:An update on COVID cases in Virginia Five students at Woodbrook Elementary in Albemarle test positiveAlbemarle Broadband Authority briefed on progress of rural internet expansionEfforts afoot to collect enough glass to attract packaging manufacturer to recycle materials into new products On this Thanksgiving Day, the seven-day average for new COVID-19 cases in Virginia is almost the same number as today’s report of 2,600 new cases. The seven-day average is 2,588. The trajectory for other indicators is also upward, with a seven-day average of new daily cases per 100,000 population at 30.4 and the total number of new cases  per 100,000 within the last fourteen days is 362.5. In the Blue Ridge Health District, there are another 59 cases, with 30 of them reported in Albemarle, and ten in Louisa. The incidence rate of seven days of new daily cases per 100,000 population in Albemarle is 15.1 and the total number of new cases per 100,000 is 166.5. Those numbers for Louisa County are 19.8 and 198.5. Elsewhere in the area, Augusta County reports another 51 cases today. Their incidence rate is 35.6 and the total number of new cases per 100,000 within the last 14 days is 409.5. In Culpeper County, there are another 75 cases and their total number of new cases per 100,000 population is 1,055.  *Five students at Albemarle County’s Woodbrook Elementary have tested positive for COVID-19 and will now isolate at home for the next ten days. That’s according to an email sent to parents and published on Twitter via one of the parents. The county school system entered Stage 3 of its Return to School Plan which returns elementary students on a hybrid basis as well as some older students.  A school dashboard with confirmed COVID-19 cases has not yet been updated, but indicates there has been one student at Albemarle High School who has been infected and one at Baker-Butler Elementary. On Tuesday, the school system sent out an email that laid out the conditions required before they will revert back to a virtual model. “If for seven consecutive days in Albemarle County the number of new cases per 100,000 persons within the last 14 days exceeds 200 and the percentage of diagnostic tests that are positive during the last 14 days exceeds 8%, ACPS will shift to Stage 1 of our Return to School Plan, which is virtual instruction for all students,” reads the email. Today, the former metric is at 166.5 and the percent positivity is at 1.46 percent. This is another reminder that percent positivity in the Blue Ridge Health District is skewed downward to the high number of testing at the University of Virginia. Source: Virginia Department of Health*This Thanksgiving, there are likely many of us who are seeing our loved ones from afar thanks to the “magic” of teleconferencing. But, many in the rural parts of our community don’t have the bandwidth to participate. The Albemarle Broadband Authority is working on several projects to expand high speed internet to more of the community. One such item comes through the Virginia Telecommunications Initiative, a program of the Virginia Department of Housing and Community Development. Mike Culp, Albemarle’s director of information technology, briefed the authority on how the county is using $291,300 in VATI funds allocated to bring service to 838 homes. “Eight project areas are under construction so most of the project areas are currently under construction and we’re hearing from many of the residents, some of them who are going up to the folks doing the work and ask them, ‘is this CenturyLink?’ and the answer is yes,” Culp said.Culp said the goal is to complete these projects by March 5. More information is available in a Frequently Asked Questions list. ABBA is applying for additional projects, but withdrew one specific location due to a request from one of CenturyLink’s competitors. “One of our project areas was successfully challenged and we have removed it from the application and that’s the Boonesville area,” Culp said. “That’s the Boonesville area. [Central Virginia Electric Cooperative] has a number of members in the Boonesville area that are off of the White Hall substation and CVEC came in and said ‘we would prefer that you not include that in your application’ so we did.”Culp said the county would have to encourage CVEC to use its Firefly subsidiary to expand coverage in that area. Albemarle is also working on three broadband projects that have been funded through the county’s portion of the federal CARES act.  These are $697,194 toward Shentel for their BEAM  wireless service that could serve up to 2,000 people, $230,245 for CVEC’s Firefly service to serve 62 locations in the Midway area, and a $32,500 contribution for Nelson Cable to extend service in the Scottsville area. (details on page 3)The Albemarle Board of Supervisors will get a report on the authority at their meeting on January 20. Supervisor Liz Palmer of the Samuel Miller District said she wants to make sure they know what is going on. “We have a lot more work to do but sometimes I think that the Board as a whole doesn’t recognize how much has actually gotten done, which is not enough, but I think it’s a reasonable amount,” Palmer said. Earlier this month, the Board of Supervisors directed staff to find additional funding to further expand broadband. The authority also talked about upcoming bills in the next General Assembly that might assist localities in providing service. Culp is a member of the 17-member Broadband Advisory Council, which advises Virginia’s governor on the issue. That body meets again on Friday, December 4.  *As the new year approaches, localities across the Commonwealth are waiting to see if their projects will receive funding in the fourth round of the Virginia Department of Transportation’s Smart Scale process. Albemarle and Charlottesville are in the Culpeper District, and their eight projects are competing with thirty other projects throughout that jurisdiction for funding. You can review the projects in a link in the newsletter. The total cost estimates for all of those projects is $374 million and $349 million is requested. The results will be announced in January. (presentation)*Though there will certainly not be as many gatherings this Thanksgiving, it is quite likely that there will be plenty of empty glass bottles left over at the conclusion of holiday meals. That’s as good a time as any to give a quick update on efforts to divert those bottles from landfills. Jesse Warren is with UVA Sustainability and a member of the Albemarle Solid Waste Alternatives Advisory Council (SWAAC). “Glass is one of the unique things about Central Virginia that we can do something about,” Warren said. That something is working with the many producers of wine, beer, and cider in the region to gather up as many of the empties together to make a viable source for new containers. In late October, SWAAC invited Scott DeFife of the Glass Packaging Institute to talk about an idea.“We could recover significantly more glass from our region than we do now by getting it out of the landfill and single-stream recyclable streams,” Warren said. “If we could get enough glass through this region, then we can potentially attract a bottle manufacturer to come and take that glass and do what they call beneficiate it into a new product.”Warren said the issue with single-stream recycling is that the glass is too contaminated with other materials to be useful, so it gets used as landfill cover. Instead, SWAAC is encouraging “source separation” where the end user keeps empties separate from other waste. Warren said three steps are involved in this process.“The first one is working with solid waste planning units to get the glass from Fluvanna, Albemarle, Nelson, Madison, Louisa to the Ivy [Material Utilization Center],” Warren said. “[Rivanna Solid Waste Authority] has negotiated a contract with a hauler to allow them to receive this glass with no tipping charge.”Warren said UVA sends its glass there, and now he is working with localities to assemble more tonnage to go to Ivy. The second step would be coming up with a unified campaign for onsite beverage manufacturers to have dedicated bins that would be picked up by a dedicated hauler for a fee. “So to put some context to this, UVA does about 40 tons of glass a year and based on our initial calculations we think we could recover hundreds of tons from the region, so our question to these individual businesses is, where are you, and how much glass do you have?” Warren said. Then the question is, how to find a hauler to do the work? Abstract: Although advancing the therapeutic alternatives for treating deadly cancers has gained much attention globally, still the primary methods such as chemotherapy have significant downsides and low specificity. Most recently, Anticancer peptides (ACPs) have emerged as a potential alternative to therapeutic alternatives with much fewer negative side-effects. However, the identification of ACPs through wet-lab experiments is expensive and time-consuming. Hence, computational methods have emerged as viable alternatives. During the past few years, several computational ACP identification techniques using hand-engineered features have been proposed to solve this problem. In this study, we propose a new multi headed deep convolutional neural network model called ACP-MHCNN, for extracting and combining discriminative features from different information sources in an interactive way. Our model extracts sequence, physicochemical, and evolutionary based features for ACP identification through simultaneous interaction with different numerical peptide representations while restraining parameter overhead. It is evident through rigorous experiments using cross-validation and independent-dataset that ACP-MHCNN outperforms other models for anticancer peptide identification by a substantial margin. ACP-MHCNN outperforms state-of-the-art model by 6.3%, 8.6%, 3.7%, 4.0%, and 0.20 in terms of accuracy, sensitivity, specificity, precision, and MCC respectively. Gunn is the granddaughter of a Chincoteague Island sharecropper who was encouraged to build a better life for herself. She traveled 100 miles each day to earn her high school diploma even though her small island had three elementary schools and two high schools. Those schools were for whites only. She went on to earn her degree from Norfolk State University, became a teacher at ACPS, and was the first black minister of Alfred Street Baptist Church. Listen to Dr. Gunn share the challenges she faced growing up as a black woman.

On April 12, 2019, students were given the opportunity to volunteer at over 33 various community organizations, Amherst County Parks and Recreation sites,  and at ACPS schools.  This "Day of Giving - Reflections" episode was created by students and faculty who traveled to various locations throughout the day to interviews individuals as to their location of service. Three basic questions were asked, 1) Have you been involved in volunteering work in the past? If so, how did the Day of Giving compare? If not, describe your experience? 2) Did this experience change the way you thought about community service? 3) What advice would you give people who may be considering volunteer work in the future? Hear what they learned and how volunteering can have a positive impact on others.

Amherst County High School is sponsoring a 2nd annual community service opportunity for our students on April 12, 2019. We are calling this event “A Day of Giving” in which the school and our students can give back to the community. Students will be given the opportunity to volunteer at various community organizations, Amherst County Parks and Recreation sites, the county elementary schools, and at the high school. Students will be chaperoned by ACPS faculty and staff. Transportation to and from the service locations will be provided by the ACPS transportation department. The school will follow normal arrival and departure times. Students will be transported to their service sites between 9:00 and 9:30 AM and will return to the high school between 2:00 and 2:30 PM.

I’m your host, Dan Keller.   This week’s podcast features an interview with Dr. Richard Ransohoff about his group’s latest research. But to begin, here is a brief summary of some of the latest developments on the MS Discovery Forum at msdiscovery.org.   We reported on two research articles on B cells published in “Science Translational Medicine". The authors suggested B cells are activated in the peripheral lymph nodes before migrating to the CNS in patients with MS. It appears that after activation the B cells travel back and forth between the CNS and the lymph nodes. The findings have implications for monoclonal antibodies rituximab and ocrelizumab, which reduce peripheral B cell numbers, as well as natalizumab, which prevents lymphocyte migration across the blood-brain barrier.    Together, MSDF and our non-profit publisher, the Accelerated Cure Project – ACP – are committed to speeding the pathway toward a cure for MS. Dr. Richard Ransohoff is the director of the Neuroinflammation Research Center at the Cleveland Clinic and a member of the MSDF scientific advisory board. His research group focuses on chemokines and chemokine receptors. He met with our senior science writer, Carol Morton, to discuss his latest findings.   Interviewee – Richard Ransohoff Glad to be here with you today, Carol. The paper that we just published is the result of at least five year's of work. The fundamental question that we tried to answer in this paper I guess if we take a step back and say one looks at an active MS lesion, and one sees a tremendous infiltrate of macrophages, which are clearly involved in the removal of myelin, which is the fundamental problem in MS. And because of the fact that these macrophages have two sources – meaning that some of them come from microglia that are resident within the brain and are macrophage like; and others are differentiated from blood monocytes that have infiltrated the tissue – it's worth wondering whether all the macrophages that look identical by their size and appearance and tissue staining are also functionally identical. And in the case of an MS like disease, by functionally identical, what we mean is are some of these cells that live in the tissue or in the lesion are some of them good guys and others are bad guys, or are they all uniformly bad guys? The reason it's worth asking that question – beyond simple curiosity – is that if you can figure out who are the good guys and the bad guys the treatment approaches become much clearer to you. So for example, if the infiltrating monocytes are the bad guys, then it's possible to apply certain kinds of medications simply to keep them out of the lesion, keep them in the bloodstream. And we already have medicines that we use in MS that take that sort of general approach. So medicines like natalizumab, which blocks cells from leaving the bloodstream and going into tissue, is one. A medicine like fingolimod, which locks up some cells in the lymph nodes and also may stabilize the blood-brain barrier, is another. And a medicine like alemtuzumab that simply removes a large fraction of the cells in the bloodstream that could participate in the tissue damage is a third. So we do know how to treat the blood cells in the way that I'm describing. Most of that work is focused on lymphocytes, not monocytes. But it could certainly be refocused. On the other hand, if the microglia are the bad guys, then the approach becomes completely different, and it's important then to figure out what sort of molecular cues are the microglia responding to and how can we block that part of the MS disease process.    Interviewer – Carol Morton Because the microglia already are in the brain. So the existing medicines have to do with peripheral immune cells that are coming into the brain during disease, right?   Dr. Ransohoff Correct. It would be important, then, to get the medicine in the brain. And when you take that approach, you have to really be very careful that the medicine doesn't have adverse effects on other brain cells. So we started from position that there were no existing markers to identify these cells in the tissue. And we were lucky enough to find out that recently developed genetic models, which tagged different myeloid cells with different fluorochromes could be used to discriminate between the monocyte derived macrophages and the microglia derived macrophages right at the onset of EAE. It turned out that we could first ask which of the two cell types contained myelin debris at the onset of EAE. That seemed like the simplest way to go after our question because we really wanted to know which cell was damaging and taking up myelin. And it turned out that both cell types had myelin in them, and so that really didn't answer the question for us. And if we gave it another moment's thought, we realized that some cells could be damaging myelin, and other cells could be removing debris. And we know that debris removal is part of the repair process. So we had to think of another way to address the problem. And through a relatively long process of developing different techniques, we figured out a way to adapt the data from studying conventional fluorescence confocal microscopy images of the cells and the tissue so that we could identify the same cells in EM sections. Because we had to go to the EM level really to answer the question that we were going after. And we used a new instrument at the Cleveland Clinic, which is one of only a few of this type of instrument at the time around the world, and it allows you to construct a 3D EM picture of certain tissue elements. And so, we set about doing that, and we identified the monocytes in the tissue and the microglia in the tissue using that approach, and we also pointed out how these two cell types were interacting with what you call the axoglial unit, which is the myelin wrap around the axons in the white matter of the spinal cord of the mouse. And we did quite a few samples from quite a few mice. And the answer to our question was very clear. Just to make it simple, the monocytes were the bad guys – they were attacking myelin and removing it from axons even when the axons appeared perfectly healthy and normal, and the myelin appeared normal where it wasn't being pulled off by the monocytes. And the microglia seemed a little bit later to begin to remove the debris. And the answer was pretty unambiguous; we never found microglia attacking myelin directly. And there was a very satisfying resolution to something that had been bothering us for a long time. We also got extra bits of information beyond what we were looking for. We found that the monocytes seemed to attack the axoglial unit not at random but at only selected places, and it seemed as though they focused primarily on the nodes of Ranvier. And it seemed as though that complicated structure was particularly vulnerable to attack. We think that that's because these structures – being as complicated and delicate as they are – become somewhat disrupted in the inflammatory environment, and the monocytes recognize something and take out after it. So we're now very intent on figuring out what the molecular signals are that attract the monocytes. We think that'll be important to know. At the same time, we thought it was important to figure out whether the gene expression in the monocytes and microglia reflected the different behaviors that we saw. And we found that the gene expression profiles in the monocytes and microglia were extremely different. The monocyte profile was almost exactly what you would predict if you watched their bad behavior and that was one of increasing many, many genes associated with inflammatory signaling, production of inflammatory mediators, and most importantly phagocytosis. The microglia, by contrast, showed a lot of downregulation of genes that are expressed in microglia from the healthy brain. So the microglia seemed to be repressed from doing things that they do in the healthy brain, and their metabolism seemed to be in a sense shutoff. They showed an inhibition of the ability to express genes by making RNA to maintain cytoplasmic and cytoskeletal organization to carryout phagocytosis and to make inflammatory mediators. So the microglia seem very repressed by comparison to microglia from the healthy brain. And so, our followup plans for the microglial side of things is to understand whether that phenotype persists for very long because the microglia need to be quite active in the process of repair and make a contribution to that process. And if their metabolism is sort of slowed down and stunted, they may have troubles to do that. So we believe the way they ask they question is to do not only additional gene expression studies but epigenetic studies, as well. And the epigenetic question we're asking is whether DNA methylation, which will turn off gene expression quite efficiently and in a long-lasting way, is changing as the microglia go from healthy brain to EAE onset and finally to recovery. We have some strong preliminary data that the gene expression decreases that we've already seen are associated with DNA methylation changes so they may be quite long-lasting. And we want to follow it through until the mice begin to recover and remyelinate to see whether the microglia recover their ability to contribute to repair. The reason we think that question is crucially important is because if this is characteristic of what happens in multiple sclerosis there may be ways to intervene such that the amount of damage is limited – we already do that pretty well – but also so the amount of repair is maximized. And I'm not certain that we understand how to do that as well as we would like.    MSDF You're calling them both macrophages. Is that a certain state of activation?   Dr. Ransohoff So when I say macrophage, it's a kind of a shorthand term for a relatively activated form of a mononuclear phagocyte in a tissue doesn't tell you where it came from – doesn't tell you if it came from a monocyte or a tissue macrophage or some other source – it just tells you what its lineage is, it's a mononuclear phagocyte, and that it's in a tissue and is displaying signs of activation.    MSDF I understand that the difference in the identity between the microglia and the macrophage has been a long-standing problem, and it sounds like you leapfrogged ahead of that to look at the functional difference. So is your study the last word on that? Is there other evidence that reinforces it, or I don't want to say contradicts but…   Dr. Ransohoff Are there other ways of looking at it?   MSDF  …or unresolved issues that it raises, like new questions that it raises. And I'm curious about the implications for thinking about that in mice and then the implications in people with MS.   Dr. Ransohoff It's always important to be mindful of the limitations of one's research approach. So we try to do that. The approach that we took had a bunch of technical reasons. Our fundamental question was are monocytes and microglia different in EAE tissues? In order to ask that and in order to ask it in a sort of a meaningful and balanced way, we chose the moment of onset of the EAE. And the reason was, first of all, that we thought the most important elements of damage to the myelin were taking place at that moment. So we wanted to know how did the disease get started. And secondly, because the preliminary work that we did showed that the monocytes and microglial cells were present in those lesions absolutely gathered together in a small space, densely intermixed with each other, and in equal numbers. So we weren't biasing the case against one cell or the other; there were equal numbers of both, and they were in the same time and place. So that's a good rationale. At a technical level, the labeling that we were using is most efficient at that moment. And so, there were lots of reasons to do it that way. That tells you a good deal about what's going on at that moment; it doesn't tell you what the fate of the monocyte derived cells is. And it doesn't tell you very much about the function of the microglia. The reason that the microglial function is a open question and an important one is because two groups – one about 10 years ago, Frank Heppner and Adriano Aguzzi and others; and then more recently Marco Prinz and his colleagues – published very strong evidence that microglia play some undefined, important role in EAE. Prinz's work is, I think, the most direct and convincing. He used a way to delete a particular inflammatory gene from microglia, and he showed that in mice where the microglia lack this inflammatory – it's a major contributor to inflammatory signaling, called TAC1 – those mice are almost resistant to EAE. And so, if you put his work together with ours, you really have a fascinating, open question. And that is okay, if the microglia at the onset of EAE look very repressed and nonfunctional, but the microglial inflammatory response is necessary for EAE to occur, then it suggests that before the onset – at some early time point – there is something the microglia are doing that makes the tissue able to support the EAE process. And so, we're going to go after that; we want to understand that to some extent. And it also, as I said, it leaves open what are the microglia doing in the process of repair, which we think is very important. So I think it is always the case that new bits of research open up new questions. We hope that we've opened up good and answerable questions.    MSDF  Great. Well, thank you. This is…I appreciate you taking the time to do this. So is there anything else that I should be asking about this study or more broadly that you wanted to emphasize…add or emphasize?   Dr. Ransohoff The only thing I want to add is that this is a particularly exciting time for this type of research. It's a time when all the work we've done over the years in MS begins to pay off not just for people with MS but also for people with other types of brain disease. I think that the people who have studied MS over the years have a unique contribution to make to understanding diseases that we worry about like Alzheimer’s disease. And vice versa people that study Alzheimer’s disease and other sorts of degenerations can make a contribution understanding MS. So it's a kind of coming together of these two different types of research.   MSDF Can you elaborate a little bit more? Is that because more of the basic science is understood, and people can apply it in these different disease contexts?    Dr. Ransohoff I think it's a combination of things. One is that people with a background studying MS have spent their lives working on inflammation, and a good deal has been learned. The thing that makes that now applicable to other conditions is that it is becoming clear that every time there's any sort of injury to the brain there will be a reaction. And elements of that reaction will resemble what you call inflammation, and that will lead to either repair process or further damage depending on the nature of the disease and other factors that we don't know about. Those of us who have worked on MS can make a contribution to that research enterprise by sharing what we know about inflammation. As I say, it's a time which is exciting, and I think we'll be productive.   MSDF  You're so good at explaining all of this.
