En quoi consiste exactement la méthode All in ? In this episode, Nicola speaks with Dr. Chrisandra Shufelt to discuss:How Dr. Shulfelt came to be interested in amenorrhea and heart health. Ways in which it's reasonable to make comparisons between people who are post-menopausal versus amenorrheic. Low estrogen affects the stretchiness of blood vessels, changes in cholesterol, and can be associated with insulin resistance. Dr. Shufelt's next study is looking at which factor underlying amenorrhea might have the biggest effects on heart health. She tells us about some of the results from the study she and colleagues just completed. We discuss risk factors for heart disease, traditional and "non-traditional" (i.e. things that mostly affect women).  The impact of cholesterol, and how quickly changes in cholesterol and bone health are seen.  Dr. Shufelt tells us about her recently completed study of vascular function in people with amenorrhea versus menopause versus normal menstrual cycles and shares some of the results, including some interesting notes on inflammatory factors. We chat about hormone replacement versus birth control pills  and which is preferable and when, what the effects of the placebo week might be... Factors that biological females should be aware of with regards to heart attacks, risk factors, and conversations with your doctors around your personal risk. In this episode, Nicola and Florence receive American award-winning author and leading expert on weight-based discrimination and body image, Virgie Tovar. Virgie evokes her memories of growing up in a larger body, and internalizing toxic cultural messages about her ultimate responsibility to become thin. In her early twenties, she was introduced to fat activism while studying for her Master's degree in Sexuality Studies. Together, Virgie, Nicola, and Florence explore the 3 levels of fatphobia and how fatphobia leads to serious discrimination in many areas including wages and medical care, resulting in higher weight people avoiding going to the doctor. Fatphobia also affects fashion, with the industry doing its best to keep higher weight people outside of stores, although about 70% of the population is actually plus size. Fat discrimination is in fact considered legal in most US states. Although we know from research that  a higher weight person is less than 1% likely to ever become an average size person according to BMI. And that scientifically, dieting is correlated with depression, anxiety and an increased likelihood of developing an eating disorder. Not with weight loss.Virgie unpacks the term "diet culture" for Florence and Nicola and explains it is not coincidence that it was was born at the same time as capitalism, puritanism, and colonialism. Virgie also evokes internalized gender inferiority in women and its links to self-objectification. with the works of John Berger and Phyllis Chessler.  She reminds us that everything we can turn to for pleasure in our society is heavily regulated and shamed, including hunger. She mentions a study on subjective hunger affecting long term decisions. And the podcast ends with simple ways anyone can fight fatphobia. You can find Virgie Tovar through her website, Instagram account or "Rebel Eaters Club" podcast. In this episode, Nicola and Florence interview Erin Kenney, a former HA patient who is also a Registered Dietitian, specializing in gut health and sports nutrition.Erin starts by explaining why she became a RD and what the difference is between RDs and nutritionists (who may have very little training). She explains how her recovery informed her work, and why she doesn't believe that elimination diets should be the first point of call when people complain of digestive issues. Erin also explained the immediate effect of dieting on one's microbiome's diversity and health. Short term, dieting can cause slow motility: constipation and bloating, as well as acid reflux, low appetite, gut dysbiosis. Within 24 hours, dieting can affect the composition of gut bacteria. Long term (1 to 2 months), dieting can cause atrophy of the GI muscles. When people are under fueling, that muscle needs energy to be repaired. When people don't have robust, healthy muscles in their GI tract, they can experience constipation, gastroparesis (slow movement in GI tract), food sensitivities. Find more research, shared by Erin, backing this up:- Weight gain in anorexia nervosa does not ameliorate the faecal microbiota, branched chain fatty acid profiles and gastrointestinal complaints- The Intestinal Microbiota in Acute Anorexia Nervosa and During Renourishment: Relationship to Depression, Anxiety, and Eating Disorder Psychopathology- Gut microbiota and body composition in anorexia nervosa inpatients in comparison to athletes, overweight, obese, and normal weight controls- Gut Dysbiosis in Patients with Anorexia NervosaErin also helps us understand how to distinguish real food intolerances from sensitivities caused by chronic under fueling, as well as how to test and treat SIBO without restrictive dieting behaviors. Erin shares simple advice to enhance digestion while being all in, including the fact that relaxation techniques like yoga and meditation or therapy, which have be proven to be even more effective than the low FODMAP diet!  Relaxation tools: Yoga with Adrienne can be a good resource for digestion support. If you don't find guided meditation helpful, try mindfulness practices. The Gaudiani Clinic is a HAES (Health At Every Size)®-informed provider and embraces treating people of all shapes and sizes.Together, they talked about Dr G's unique approach to support ED patients with good science and social justice, instead of condescension and assumptiveness. Dr G talked about the "cave person brain" survival mechanisms our bodies put in place to save them from starvation, and the epidemic of false health information out there. She also clarified why extreme dieting can lead to low resting heart rates and how to recognize the difference between an athlete's heart and a starved heart.Dr G also reminded us that our bodies are not the problem, diet culture and thin privilege are.  None of us should feel the need to change our bodies to fit a system rooted in privilege. And that, as such, we need to work to disrupt the system that identifies that only some bodies have privilege. Every body deserves abundant, satisfying, balanced foods according to taste and satisfaction, as well as treatment, self-compassion, self-care and body trust. "If someone is eating restrictively in the name of health, and realizes their periods are missing and bone density is low, they now have proof that what they're doing is not healthy for them. That whatever they're doing is not done in the name of health, but that they're in fact trapped into fear." - Dr Jennifer GaudianiDr. Gaudiani's first book, Sick Enough: A Guide to the Medical Complications of Eating Disorders, was released by Routledge in October 2018 and is available on Amazon. To work with Dr Gaudiani and her team, reach out to the Gaudiani Clinic. At the end, Florence mentioned a podcast about the Burnout book, please find it here.To find support in your HA recovery:Get the "No Period. Shanta also mentioned the Minnesota Starvation Experiment, and that our brain requires 30% of our total energy expenditure. Nicola explained that low AMH in itself doesn't indicate low fertility (more on this here). Shanta shared that to help her recover, she needed to face the fear of weight gain. She did so by getting professional help that truly understood ED behaviors, starting a self-care routine, considering new morning rituals to replace exercising and work with self-affirming questions, not self-affirmations. Shanta revealed that her biggest take away from HA recovery was to never restrict herself in any way, but also to become a better advocate for gender equality, and a better feminist. If you are looking for anti-diet, fat-positive therapists for ED recovery, please look intoAssociation for Size Diversity and Healthwww.cbte.coNational ED Association (US)BEAT ED (UK)The Butterfly Foundation (Australia)The Middle East Eating Disorder AssociationTo find support in your HA recovery:Get the "No Period. Julie specializes in helping people with PCOS move away from dieting and rewrite their relationship with food to promote health, and advocate for better (non-weight centric) care.Together, we explore the similarities and differences between HA and PCOS, and why these seem to constantly get mixed up by medical practitioners (http://noperiod.info/HAvsPCOS).We start by defining the classic criteria for PCOS diagnosis (irregular/missing periods, hyperandrogenism, and polycystic ovaries) and why diagnosis is often loaded with weight and body shaming. We also explore whether "lean PCOS" is in fact PCOS, or just misdiagnosed HA, and how PCOS "symptoms" can be NORMAL side effects of recovery from disordered eating (also see http://noperiod.info/HAvsPCOS2)  and  that it is therefore important in recovery to wait for a year before investigating PCOS as a condition.Julie confirms that the main difference between HA and PCOS is high circulating insulin levels - typical of PCOS. We then explore how the typical "PCOS recovery" recommendations to cut sugar and carbs are not evidence-based and rooted in weight stigma.Find the latest evidence-based PCOS guidelines here (2018).Julie explores her non weight centric steps for PCOS healing and recovery. Nico and Florence notice these are similar to HA recovery: restore a balanced relationship with food and movement, including eating and resting! And, unique to PCOS, finding sustainable ways to lower insulin levels without dieting.Julie reminds Nico and Florence that PCOS and diabetes are typically NOT caused by lifestyle but genetic predispositions, and that preventing weight cycling and healthcare avoidance is key for anyone with PCOS, as well as addressing other symptoms, like sleep & mood disorders. To find out more about Julie Duffy-Dillon's work and non-diet PCOS community, please visit www.juliedillonrd.com or www.pcosbodyliberation.comTo find support in your HA recovery:Get the "No Period. Now What? and is now one of the NPNW Support Group admins, where she shares with members her knowledge as an anti-diet Registered Dietitian and former HA patient.Cynthia explains she started dieting and over exercising in college, after being placed on birth control for suspected PCOS due to having painful, inconsistent periods as a teen. And how her dieting and workout routines went up a notch in the preparation of her wedding in her mid-twenties.She explores how her training as a registered dietitian gave her very little knowledge about disordered eating or hormonal issues related to living in an energy deficit, and may have even been triggering a troubled relationship with food, exercise and her body.Together, we discussed the constant confusion between HA and PCOS diagnoses (learn more at http://noperiod.info/HAvsPCOS), and how getting the right diagnosis finally helped Cynthia go #allin. We also share strategies to be a better advocate of your own health with your healthcare practitioner, by using the No Period Now What book as a reference.Cynthia reminds us all of the importance of community in recovery, as well as increasing patience and perseverance, letting go of control, and reevaluating what your dream life really looks like (hint: it is usually the opposite of what you have to do to maintain a "dream body" as per society's unrealistic standards). You can find Cynthia's work and support via @period.nutritionist on Instagram. To find support in your HA recovery:Get the "No Period. In this Episode, Nicola and Florence explore - How Florence became a certified Eating Psychology and body image coach- That body image is - in fact - an "inside job". It is related to our thoughts, feelings and emotions towards our bodies, and doesn't have much to do with the way we look.- That to increase body confidence and recover our periods, we need to work on our mindset. Because changing our bodies is doomed in 98% of cases, and cannot be a guarantee of acceptance.- Chris Sandel - Real Health Radio- If you suffer from an Eating Disorder and don't know where to get help, look into the NGOs and associations listed under "support organisations" on this page.- Ways to deal with toxic "New year, New you" messages out there while you are recovering from HA:#1 Disconnect from external messages - move from self-control to self-care. From "I should" to "what do I need?", letting go of the unattainable goal of perfection.#2 Make your social media feed a happy, diverse, inclusive and inspiring placeGet rid of accounts that make you feel "less than"Follow body positive and fat acceptance accounts: @300poundsandrunning @ragenchastainAnd a lot more on beyondbodyimage.com/resources#3 Set boundaries against fatphobic commentsDocumentaries: Embrace, Miss Representation, The Social DilemmaLearning to side step conversations, to answer fatphobic comments, to self-care when triggered are all ways to set boundaries and protect yourself and your recovery.#4 Practice self-compassion when negative self-talk is showing up#5 Remember that diets don't work. They mostly lead to weight GAIN, body dissatisfaction, and eating disorders. #6 Use body acceptance affirmations#7 Start a gratitude journalTo find support in your HA recovery:Get the "No Period.
