Our guest today is Dr. Joseph Pinzone, an endocrinologist who is leading innovative work at the Hyperthermia Cancer Institute (HCI). I first learned about the HCI from a friend who had undergone treatment for stomach cancer. The HCI uses a hyperthermia-directed treatment that targets specific tissue in the body with temperatures ranging from 106 to 109 degrees Celsius. This therapy is administered alongside chemotherapy and radiation or radiotherapy and has minimal downsides. Additionally, the therapy is FDA-approved, and most insurance policies cover it. I was thrilled to learn more about this exciting new treatment. Dr. Pinzone shares how this therapy is transforming patients' lives. We discuss how men may have hormonal issues as well, but tend to put them on the back burner or ignore them. Dr. Geller shares what he sees in the area of infertility, weight gain, and sleeplessness and why women don't have to suffer through menopause. He shares possible strategies for navigating and balancing your hormones. Dr. Geller sees a lot of patients who are trying to figure out if they have a thyroid issue or hormonal imbalance and introduces the best first steps to solving the issues before medication. This week I have the pleasure of interviewing my infertility doctor, Dr. Creighton E. Likes, III, MS, MD. Dr. Likes is a double-board certified Reproductive Endocrinologist and the Medical Director of the Fertility Center of the Carolinas where he has been practicing reproductive medicine for 12 years. Dr. Likes completed medical school at the Medical University of South Carolina in Charleston, SC, completed OB/Gyn Residency in Greenville, SC, and completed his REI Fellowship at Duke University. Dr. Likes is passionate about providing hope and support to individuals and couples who desire grow their families. In this episode, Dr. Likes shares what the process of working with an infertility specialist is like, what you expect, and when individuals and couples should consider seeing a specialist. He tells us what his thought process is like when working with a new patient and what he does when he has been working with a patient for a while who has not yet had success. Dr. Likes and I talk candidly about what it looks like to have difficult conversations with your doctor, when or how to ask for a second opinion, what to do next when a treatment cycle is unsuccessful, and how patients can find emotional support for the roller coaster journey of infertility. In this episode: Dr. Chandarana joins us is today's episode to explain what endocrinologists do, and how they play a key roll in treating diseases like diabetes and thyroid disease. In this episode we will be focusing on the thyroid as it relates to pregnancy. As most of you know if you have been following me on social media, I was diagnosed with thyroiditis after having Miles. It was an uncertain time filled with so many varying symptoms that made me feel like my body was totally out of control. But once I was able to get some answers, we were able to take the correction action towards getting me better. What is the thyroid and what is it responsible for? The thyroid is a small gland in the body that is located centrally in the neck, and is responsible for synthesizing thyroid hormone. Thyroid hormone is necessary for the functioning of the body, controls metabolism in adults, and is critical for brain development in children. Some babies are actually born without a thyroid, so they would need to take thyroid hormone for proper growth and brain maturation. Early on in a pregnancy: Sometimes the pituitary gland can secrete a hormone called Beta ECG that is similar in structure to the thyroid stimulating hormone. When Beta ECG binds to the thyroid it can trick the thyroid into thinking it's being stimulated and causes a slightly overactive thyroid. Endocrinologist consider this to be normal early on in the pregnancy. Women might also experience some swelling in their neck (goiter) due to the thyroid being overactive. Endocrinologist want to be able to distinguish between these changes going on with what is considered “pathological changes” (these need to be treated/monitored). Some examples are: Underactive thyroid (hypothyroid): Some symptoms may be low energy, fatigue, muscle aches, brain fog, difficulty concentrating, weight gain, constipation, swelling of legs, cold intolerances, hair loss, irregular menses. Hashimoto's: Autoimmune thyroid disease, runs in the family Overactive thyroid (hyperthyroid): Some symptoms may be weight loss, anxiety, tremors, palpitations, increased sweating, heat intolerance, diarrhea, hair loss. Thyroiditis: Can be painless and also painful. If a woman experienced postpartum thyroiditis, they're more at risk for developing it again with subsequent pregnancies. Graves disease: overproduction of thyroid, will not usually get better without treatment. Our hormones play a huge role in the regulation of our bodies, so it's wise to rule out hormonal imbalance if you're experiencing unexplained symptoms. Indications that you may have an overactive thyroid include anxiety, increase in temperature, heat sensitivity, heart palpitations, heart racing, tremors, shakiness. Dave McNeill at time of recording this episode had recently wrapped up his Physiotherapy Studies but also just days earlier ran a 28:16 for a 4th Place at the Australian 10,000m Track Titles at the Zatopek Classic. By way of bio, in the 2012 London Olympic Games, Dave finished 28th in the 5,000m, at the 2016 Rio Olympic Games Dave finished 16th. In the final of the 10,000m in July 2019 Dave marked his comeback with a 5000km National Cross Country Title but as you'll hear during this episode, Dave's career has certainly also take him to the wilderness years of injury. In fact, injury cost Dave Australian representation at the 2018 Commonwealth Games where he was set to take part in the 5,000m and 10,000m events. It was bone stress injury which sidelined Dave. As you will hear today that took Dave down the path of exploration where he was to discover that one of the causing factors of his injury had been training through the status of low energy availability or as we've been exploring in prior episodes of the show including last week's Expert Edition with UK based Sports and Endocrinologist Dr. Nicky Keay. Many endurance athletes inadvertently and unintentionally find themselves in a state of RED-S Relative Energy Deficiency in Sport. The flow on effects can be detriments in performance but also the effects can be found through a detriment of health. One particular flow on effect of relative energy deficiency for athletes is low bone mineral density. Dave had to confront that when his bone mineral density came back very low in his lumbar spine. During this episode we explore topics including how to navigate self doubt, appraising readiness to perform, overcoming the fear of the pain that we put ourselves through leading up to an event, Dave's concept of old man strength, the idea of not looking at the training in isolation or in a vacuum but rather over the long term considering the bigger picture, the mistake Dave made in mistaking the stable weight has been an energy balance.
