Episode 2432 - BEST OF: On this Friday's BEST OF show, Vinnie Tortorich speaks to Dr. Nadir Ali and they discuss low-carb events, lipotoxicity and cholesterol, dopamine and hijacking your brain, and more. (2:35) They also discuss how several doctors' or writers' careers have been limited, or even stopped, when they have tried to educate with the truth. (5:45) Maryanne Demasi from Australia is an example; Vinnie explains who she is. Also, both Gary Fettke and Tim Noakes have been targeted previously. The limitations on certain freedoms and access to information are concerning. The conversation leads to the use of statins. (12:07) Vinnie relays info from a consult he had and why he advised the person to find a different doctor. Dr. Ali goes into detail about when things like statins and CAC scans can be useful, and when they are unnecessary. For a doctor to just give you a statin and tell you it's good for you is not great; there are instances where a statin helps, and when they do not. He also goes into detail about genetic factors like PCSK-9, and how it makes a difference in cholesterol levels. (20:30) Vinnie asks about the carnivore diet and whether it's a good idea for cholesterol. (23:00) There are plenty of good reasons to eat meat. There is potential for lipotoxicity in some people; Dr. Ali explains. (25:10) There needs to be an individualized approach; as a result, it may be better to transition in stages. HIJACKING YOUR BRAIN They switch to talking about how to get people to change their habits, as everyone always wants a "magic bullet". (31:00) Dr. Ali goes into how he and his colleagues have come up with tricks and hacks to help people improve their habits. (31:30) Dr. Ali goes into detail about the "dopamine spike effect." (32:10) He describes studies that show the anticipation of the reward is what spikes the dopamine, NOT the reward itself. He also talks about how food engineering has manipulated our brains to want to eat more, especially things like sugars. Vinnie and Dr. Ali discuss addiction, and willpower vs. changing your environment. (37:00) They discuss how to get someone who is less disciplined to be able to change habits. (47:10) Dr. Ali describes a study that shows how random rewards affect dopamine, anticipation, and behavior. (50:50) The same behavior modification is used in Vegas on slot machines. The food industry uses a similar effect to hijack your brain by modifying food to trigger addiction and purchasing patterns. (4:00) Gary changed his approach based on observations in his practice. He became targeted by Big Food and others because of his messaging. He protested the ridiculous things recommend to diabetics: They were told it was ok to have ice cream and cookies 3 times a day! Journalists were targeted as well. (11:00) All they were doing is talking about biochemical science. The health system didn't want to admit that it got it all wrong. (12:45) The last time the medical community admitted fault was thalidomide in the late 1950s and early 1960s. Thousands of babies were affected worldwide, born with multiple physical deformities. The difference is now the number of people affected by bad medical advice is in the millions. DIETARY GUIDELINES AND MYTHS Vinnie talks to Gary about his recent Instagram post about avoiding oatmeal and the negative responses he received because people just can't believe oatmeal is bad for you. (14:00) Gary explains how just 4 g of carbs–ONE TEASPOON—can hurt your body. (17:00) Gary defines what the recommended dietary guidelines should be: Eat fish, local, seasonal, whole foods based on your culture and environment, avoiding added sugar and processed foods. Orthopaedic Surgeon Dr Gary Fettke frustrated by the increasing number of diabetic patients' limbs he was having to amputate, started prescibing low sugar diets to these patients. But then found himself sanctioned by the Australian medical board for giving nutritional advice. Fettke was told: "there's nothing associated with your medical training that makes you an expert, or authority on the field of nutrition, diabetes or cancer, you're not allowed to make any recommendations to your patients on nutrition". It would take nearly five years for the decision to be reversed. Fettke's belief in the importance of preventative medicine began his lifelong research into the science of nutrition, dietary guidelines and exactly what makes a healthy diet. And he has become convinced that for all of us - not just those with diabetes - sugar, refined carbs and polyunsaturated fats come together to create a perfect storm in our bodie. That is why he believes we are seeing a higher level of metabolic illnesses such as diabetes type 2, heart disease and cancer, than ever before. Dr Gary Fettke, is an orthopaedic surgeon, based in Tasmania, Australia. He has a particularly interest in preventative medicine. Although his speciality is surgery, Gary believes it is much better to help patients avoid surgery if at all possible, by making dietary changes. (2:35) They also discuss how several doctors' or writers' careers have been limited, or even stopped, when they have tried to educate with the truth. (5:45) Maryanne Demasi from Australia is an example; Vinnie explains who she is. Also, both Gary Fettke and Tim Noakes have been targeted previously. The limitations on certain freedoms and access to information are concerning. The conversation leads to the use of statins. (12:07) Vinnie relays info from a consult he had and why he advised the person to find a different doctor. Dr. Ali goes into detail about when things like statins and CAC scans can be useful, and when they are unnecessary. For a doctor to just give you a statin and tell you it's good for you is not great; there are instances where a statin helps, and when they do not. He also goes into detail about genetic factors like PCSK-9, and how it makes a difference in cholesterol levels. (20:30) Vinnie asks about the carnivore diet and whether it's a good idea for cholesterol. (23:00) There are plenty of good reasons to eat meat. There is potential for lipotoxicity in some people; Dr. Ali explains. (25:10) There needs to be an individualized approach; as a result, it may be better to transition in stages. HIJACKING YOUR BRAIN They switch to talking about how to get people to change their habits, as everyone always wants a "magic bullet". (31:00) Dr. Ali goes into how he and his colleagues have come up with tricks and hacks to help people improve their habits. (31:30) Dr. Ali goes into detail about the "dopamine spike effect." (32:10) He describes studies that show the anticipation of the reward is what spikes the dopamine, NOT the reward itself. He also talks about how food engineering has manipulated our brains to want to eat more, especially things like sugars. Vinnie and Dr. Ali discuss addiction, and willpower vs. changing your environment. (37:00) They discuss how to get someone who is less disciplined to be able to change habits. (47:10) Dr. Ali describes a study that shows how random rewards affect dopamine, anticipation, and behavior. (50:50) The same behavior modification is used in Vegas on slot machines. The food industry uses a similar effect to hijack your brain by modifying food to trigger addiction and purchasing patterns. This leads the conversation to today's tech, algorithms, and the challenge of addiction in that area. (54:00) Vinnie tells Dr. Ali about what he observed with a young man at the gym. Boyd Myers rarely says ‘no'. He is the type of person who runs a marathon and drinks a case of beer while doing it (yes, he did this). Boyd lives with a limitless perspective, saying ‘yes' to new challenges and getting every drop out of life. The cool thing about Boyd is he hasn't always had this perspective. After leaving the armed services and joining corporate America, Boyd packed on some extra weight and became the unhealthiest he's ever been. He decided he needed a change. After running 500 feet (yes, feet), he decided he needed something to train for - an Ironman. From there, the rest is history. Boyd has been training for, and demolishing different endurance events ever since from 100 milers to his most recent pursuit - a full-distance Ironman followed by a half-Ironman the next day (or a 210.9 mile swim, bike, and run). Boyd has a contagious mindset and zest for life and we hope you enjoy this episode!

(5:50) She mentions what happens to your bowels when too much sugar is consumed, including fructose and both natural and artificial sweeteners. (6:30) Vinnie mentions how they are poison to the system, similar to how alcohol works; he explains. (8:00) Vinnie also tells Gina how the consumption of sugar since the 1800s has increased. (9:00) They also chat about how this has led to more recent developments of NAFLD (non-alcoholic fatty liver disease). He explains what A1C is, how it works, and how the numbers have ultimately been altered, which is basically lowering the bar. He gives reasons why this doesn't work, and how it allows for "stacking" medications. Gina asks what a high A1C for an extended period of time will do. Although he has retired from his Orthopedic practice, he remains very vocal in public health advocacy and is currently working with a group to have therapeutic carbohydrate restriction included as a safe option in the management (and prevention) of Type 2 diabetes in Australia. He has a major interest in the preventative medicine and encourages his patients to lose weight before undergoing surgery. He is an advocate of a low carbohydrate, healthy fat (LCHF) lifestyle for the various health benefits, including the reversal of type 2 diabetes.
