Three years on from her shock death, a coronial inquest for Olympic sprint cyclist Olivia Podmore is set to get underway in Hamilton on Monday. Sports correspondent Dana Johannsen spoke to Corin Dann.
On August 9 2021, the day after the Tokyo Olympics ended, cyclist Olivia Podmore died in a suspected suicide. Podmore had represented New Zealand in the 2016 Olympics, but was not selected five years later. Her death sparked shock throughout the close knit cycling community and wider sporting network, and the culture at Cycling NZ was eventually investigated. Now, over three years later, a coronial inquiry into Podmore's death is due to begin today. Medical experts have expressed doubt as to whether some of the victims of the Christchurch mosque attacks could have survived their injuries had they been taken to hospital sooner. The coronial hearing into the 51 worshippers lost during the March 15th shootings has entered its final days. Yesterday, a panel of expert witnesses provided evidence around whether the deaths of some of them could have been avoided. Adam Burns was at the inquest.

A packed court in Christchurch has seen harrowing footage of the 2019 mosque attacks during an emotional first day of inquest into the killings. Described as one of Aotearoa's darkest days, 51 people were killed and 40 others injured after a gunman opened fire at the al-Noor mosque and the Linwood Islamic Centre on the 15th of March. An hour-long audio-visual timeline, providing an overview of the day's horrific events was met with silence and tears from families. The court also heard from a senior police officer about the emergency response. Adam Burns was there.
As the coronial inquest into the mosque shooting gets underway surely one of the ironies is the fact that something that it is hoped will provide answers needs to answer a couple of questions itself. Why does it take so long to get an inquiry underway? Is something this far down the track from the event able to give the answers people want? One of the stories yesterday suggested the great hope was this inquiry would lead to something like this never happening again. I'm not sure how this is possible. A mass shooting has not been part of New Zealand history so there is not really a pattern to be studied. The outworkings of an errant, crazy man is not something that an inquiry can prevent. It can potentially delve into some of the other issues like the emergency response, they are looking at an exit door and whether it failed to function. Some are looking for closure. I hope for those who are looking for closure they get it, but for that to happen surely it must be based on the mere functionality of the inquiry, not the specifics and outworkings because an inquiry is not a miracle. It's a series of questions and probes and recollections. I can't see how we are going to conclude mass, catastrophic, systematic failure, whereby a large series of recommendations are put forward and the fabric of society is changed as we implement them and go about our business in a completely different way. This was a tragic day, as a result of a crazed man with a gun, bent on madness and destruction. I may be proved wrong, but the emergency response appeared astonishing and the bravery involved was extraordinary. The things we have done already, the gun amnesty and register, seem as political as a response as they are practical. The Christchurch call that was set up has not stopped online hate. If an entire Government and its contribution is limited to that, what is a coronial inquiry going to achieve? Call me cynical, but this seems as much procedural as anything. It's four and a half years later, with findings not until at least five and a half years later, it's not what you would call urgent, is it?See omnystudio.com/listener for privacy information.

She will determine the cause and circumstances of the 51 deaths and make recommendations to prevent similar tragedies in the future. She discusses the 2017 death in custody of much-loved Yorta Yorta mother Tanya Day, and also her work over the past two years reviewing the culture of correctional services. A joint coronial inquest into a murder and a suspected suicide starts in Christchurch this morning. Nicole Tuxford was murdered by Paul Tainui in her Christchurch home in 2018, while he was on parole for a similar murder. The sudden death of the father of Tainui's earlier victim is also under investigation. Ōtautahi reporter Niva Chittock is covering the inquest.

Coronial inquest into death of Kumanjayi Walker starts in Alice springs
Former WA secretary of the Australian Nursing Federation Mark Olsen painted a grim picture of the hospital system and conditions for nurses since the pandemic at the coronial inquest into the death of seven-year-old Aishwarya Aswath at Perth Children's Hospital. He outlined details of letters from nurses, including one who wrote that two nurses were left to look after 40 patients in a ward. 6PR reporter Jake Battrick told Mornings host Liam Bartlett from the Coroner's Court of WA that Olsen's stark comments led to a extraordinary exchange between the coroner and state solicitor Carolyn Thatcher, who was worried Olsen's evidence was "heading down a troubling path". "[Thatcher] wanted the coroner to clarify if she was going to make any recommendations based on the horror stories Mr Olsen was giving," Battrick told Bartlett. The long-time family advocate of seven-year-old Aishwarya Aswath, who tragically died of a bacterial infection after waiting hours to see a doctor at Perth Children's Hospital, has raised concerns after the resignation of Perth Children's Hospital CEO Aresh Anwar just 12 days before an coronial inquest into the young girl's death. Suresh Rajan said the evidence of Dr Anwar, the bureaucrat in charge of WA's child health division when Aishwarya died, was critical to the coronial inquest but his resignation meant his participation in the inquest was not guaranteed. Rajan told Liam Bartlett on 6PR Mornings he hoped Dr Anwar had not been made a scapegoat. I have been working on this case for a while. I have found links between this case, the drug network today and to the alleged Outlaw gang to Kings Cross. I have talked to Michelle's mother over Facebook but she thinks I am wrong. I know the spirit keeps floating around with the research to put the clues together. Yesterday I wrote again to the NSW Police for the DNA to be tested on a Person of Interest that had not been interviewed by the Coronial Inquest. Why? The Police didn't think about the links at the time of the murder to the alleged security at Dawson Mall was never followed up. Within my investigation the name I asked around was this name known? The answer came back yes. So I knew then this person was allegedly still high up with in the same gang I was trying to expose with many others. Epilepsy New Zealand hopes this week's coronial inquest into six unexplained epilepsy deaths changes the way the disorder is treated.It comes after New Zealand's drug buying agency Pharmac faced criticism for switching the brand of the drug, known as Logem, affecting 11,000 patients.Chief executive Ross Smith says his organisation and other groups warned Pharmac the plan to change brands was dangerous."It's really important for the families, but also for the wider community to know whether the change in brand that Pharmac forced on people had anything to do with their loved ones deaths."He told Kate Hawkesby people taking medication for epilepsy should never have had their medication switched at the counter."It was unacceptable. People with epilepsy should never have been forced to change their drug at the pharmacy counter. That only should have ever taken place with their prescribing doctor, taking into account their circumstances and their seizure history."LISTEN ABOVE
