CardioNerds kicks off its advanced therapies series with Chair of the CardioNerds Heart Failure Council, Dr. Jenna Skowronski, co-chair of the series, Dr. Shazli Khan, and Episode FIT lead, Dr. Jason Feinman. In this first episode, they discuss the process of advanced therapies evaluation with Dr. Michelle Kittleson, Professor of Medicine and Director of Education in Heart Failure and Transplantation at Cedars-Sinai. In this case-based discussion, they cover the signs and symptoms of end-stage heart failure, the initial management strategies, and the diagnostic workup required when considering advanced therapies. Importantly, they discuss the special considerations for pursuing left-ventricular assist device (LVAD) versus heart transplantation as well as the multidisciplinary, team-based approach needed when advanced therapies are indicated. Pearls Guideline-directed medical therapy (GDMT) is indicated in all heart failure patients and improves survival, but progressive symptoms and intolerance to GDMT can be warning signs of disease progression. The I-NEED-HELP mnemonic is an excellent reference when considering referral for advanced therapies (Figure). Management of acute decompensation includes diuretics and possible inotropic support. The inotropic agent used should be whichever best suits your specific patient. Milrinone may result in more hypotension, whereas dobutamine may result in more tachycardia. Tachycardic and normotensive patients may do better with milrinone, while hypotensive patients with normal heart rates may do better with dobutamine. Notably, DoReMi found no difference between milrinone and dobutamine for patients with cardiogenic shock. The initial diagnostic evaluation includes an echocardiogram, right heart catheterization (RHC), and often cardiopulmonary exercise testing (CPET) to objectively assess the status of the heart. Comprehensive labs, imaging and cancer screening are also needed to assess all other organs. When making the decision to pursue advanced therapies, always ask: Is the heart sick enough? Is the rest of the body well enough? These two questions provide a framework to guide if patients are optimal candidates for transplant versus LVAD. The advanced therapies evaluation is a team sport! Patients will meet not only with advanced heart failure cardiologists, but also cardiac surgeons, psychiatrists, social workers, nutritionists and pharmacists. All team members are of critical value in the process. Notes 1.) What are the key features of advanced cardiomyopathy, and when should providers consider referral for advanced therapies? Advanced cardiomyopathy may present as recurrent hospitalizations for decompensated heart failure, intolerance to GDMT with symptomatic orthostasis and hypotension, and progressive symptoms of heart failure despite medical therapy. The I-NEED-HELP mnemonic is a helpful tool to identify patients at risk of heart failure and is defined as follows: Need for Inotropic support, New York Heart Association (NYHA) Class IV symptoms, End-Organ Dysfunction, Ejection fraction

Und um das herauszufinden, lässt er vier Medizinstudenten sich gegenseitig kontrolliert umbringen und kurz darauf wieder auferstehen, mithilfe eines Defibrillators und nervenaufreibendem Geschrei. Ein fünfter in der Runde weigert sich zu sterben, um hinter den Vorhang des Todes zu schauen und darf anscheinend als Kontrollgruppe trotzdem dabei sein. Die Kommilitonen fangen an sich übertrumpfen zu wollen, wer länger tot sein kann bevor sie ihn zurück ins Leben holen. Als ob es sich um ein Wettessen handele in dem am Ende einer Stolz behaupten kann 20 Burger auf einmal im Mund gehabt zu haben. Der Kater danach allerdings hat es in sich: jede und jeder der ins Jenseits schauen durfte, bringt ein schlechtes Gewissen mit, eine Schuld, die er oder sie im Laufe ihres oder seines Lebens auf sich geladen hat. Man beachte, ich darf gendern! Allerdings nur einer weiblichen Protagonistin wegen, der Rest der fünf ist zeitgemäß männlich und leicht problematisch. Die Schuld, die sie auf sich geladen haben (oder glauben zu haben) verfolgt sie im Diesseits und zermürbt die Freunde. Nelson, der Anführer der Todes-Medizin-Bande, wird sogar regelmäßig von einem zehnjährigen Jungen vermöbelt und die Verzweiflung wächst. Erst als David sich bei der Frau entschuldigt, die er als Kind gemobbt hat, hört sein Martyrium auf, und er rät den anderen, sich ihrer Schuld zu stellen. Da der Junge den Nelson damals ausversehen umgebracht hat, im Diesseits nicht erreichbar ist, sieht Nelson sich gezwungen sich erneut ins Jenseits zu katapultieren, um dort um Vergebung zu bitten. Gerade noch so können seine Freunde ihn zurückholen und der Spuk hat endlich ein Ende. Aber immerhin wissen sie jetzt, was nach dem Tod passiert. Saving Lives Together installed the defib machine in the phone box in the Square in 2017 and proivde16 others in the area. If someone is in cardiac arrest - i.e. unconscious and not breathing - you call an ambulance and they will tell you where the nearest Defib machine is and give out a code. Saving Lives Together was set up by Aisa Fraser after close friends died in cardiac arrest. She wants to see survival rates increased by a better community response when it happens, with training about CPR. Scandinavia for instance trains CPR as part of driving tests. Aisa says one aim is to de-medicalise the terms and emphasise action for the public to take. They have held training sessions at Petersfield Football Club and the WI and ask organisations to take up their offer of free training. More community sport hubs will soon have access to life-saving defibrillators in NSW. The Minns Labor Government is injecting $500,000 into an AED grants and research program, including a new defibrillator mapping system. Nearly 200 clubs have already been accepted for grant funding - the majority, in regional areas. Cardiac incidents occur without much warning, if any, and time is of the essence to save a life. CPR is important, but a device called an AED - automated external defibrillator - can restart someone's heart quickly, before serious brain damage can set in. While many offices, retail outlets and apartment buildings have these devices, it's not always clear where they are, and that's where Cardiac Crusade comes in. They're rallying volunteers around the country, including here in Massachusetts, to help them map these devices, with hopes of eventually working with tech companies to make it easy for you to find them in times of need. Plotting the location of every defibrillator on Google Maps could help save lives - according to a Fine Gael senator. Manus Boyle is calling on the HSE to work with the tech company to allow people to search for their nearest A-E-D on its app. Between 2012 and 2023, there were 304 cases of defibrillation by members of the public, with an almost 30 per cent survival rate. Senator Boyle says most people don't know where their nearest defib is. For more on this, Alan Morrissey was joined by Carol Murphy, a first aid instructor who's volunteered with Cooraclare GAA club, previously aiming to make training available to clubs throughout West Clare and Tracy Murray, treasurer of Kilmurry Ibrickane Defibrillator Community Group.
