Remembering all the different energy setting needed for synchronized cardioversion and defibrillation used to be confusing for a lot of people.Defibrillators can be broken down into three basic categories:1. Automated External Defibrillator (AED);2. Biphasic defibrillators; and3. Remembering all the different energy setting needed for synchronized cardioversion and defibrillation used to be confusing for a lot of people. Defibrillators can be broken down into three basic categories: 1. Automated External Defibrillator (AED);2. Biphasic defibrillators; and3. Monophasic defibrillators. Because AEDs are designed to be used by first responders, and lay people with only minimal medical training, the controls are kept simple and are pre-programmed into the machine. Use of an AED to rapidly deliver a shock. Biphasic defibrillators automatically measure the impedance between the defib pads and will adjust the energy to deliver the shock needed based on the patient. Biphasic defibrillator use and energy setting. For monophasic defibrillators, use 360J to defibrillate V-Fib or pulseless V-Tach. AEDs must not be used on patients with a pulse. Cardioversion of patients in unstable SVT or V-Tach with a pulse using biphasic vs monophasic monitor/defibrillators. Team safety when performing synchronized cardioversion. Abstract: Multiple biophysical mechanisms may generate non-negative extracellular waveforms during action potentials, but the origin and prevalence of positive spikes and biphasic spikes in the intact brain are unknown. Using extracellular recordings from densely-connected cortical networks in freely-moving mice, we find that a tenth of the waveforms are non-negative. Positive phases of non-negative spikes occur in synchrony or just before wider same-unit negative spikes. Narrow positive spikes occur in isolation in the white matter. Isolated biphasic spikes are narrower than negative spikes, occurring right after spikes of verified inhibitory units. In CA1, units with dominant non-negative spikes exhibit place fields, phase precession, and phase-locking to ripples. Thus, near-somatic narrow positive extracellular potentials correspond to return currents, and isolated non-negative spikes correspond to axonal potentials. Identifying non-negative extracellular waveforms that correspond to non-somatic compartments during spikes can enhance the understanding of physiological and pathological neural mechanisms in intact animals. Abstract: Background: Intermittent theta-burst stimulation (iTBS) is a transcranial magnetic stimulation (TMS) protocol which can temporarily modulate the corticospinal excitability. Conventionally, TBS is only applied using biphasic pulses due to hardware limitations. However, monophasic pulses are hypothesised to more selectively recruit cortical neurons than biphasic pulses which may improve the effect of iTBS. A custom-made pulse-width modulation-based TMS device with improved energy recovery and pulse-shaping capabilities allows the generation of monophasic iTBS. Objective: To compare the effects of monophasic and biphasic iTBS on motor corticospinal excitability in healthy volunteers. Methods: Thirty volunteers participated in three separate sessions, where monophasic and biphasic iTBS were applied to the primary motor cortex (active condition) or the vertex (control condition) using a custom-made TMS device. Changes in corticospinal excitability were quantified using the peak-to-peak amplitude of motor evoked potentials (MEP) with single-pulse TMS collected at baseline and at 5 - 60 minutes after the application of TBS. Results: Both monophasic and biphasic active iTBS led to significant increases in MEP amplitude compared to baseline, with monophasic iTBS showing significantly larger effects (linear mixed effect model analysis: ({chi}2(1) = 8.48, p = 0.004)), even after subtracting the control condition from the active conditions ({chi}2(1) = 6.21, p = 0.013). Conclusions: This study demonstrates that the pulse shape affects the extent to which TBS can modulate corticospinal excitability. Monophasic intermittent TBS has the potential to increase the efficacy of clinical treatment protocols in the future. Copy rights belong to original authors. Abstract: Alzheimer's disease (AD) is a multifactorial disorder driven by abnormal amyloid {beta}-peptide (A{beta}) levels. To identify new druggable pathways involved in the A{beta} cascade we here investigated the AD pathophysiological role of the presenilin-like intramembrane protease signal peptide peptidase-like 2b (SPPL2b). A{beta}42 induced a biphasic modulation of SPPL2b expression in human cell lines and ex vivo mouse brain slices. In addition, SPPL2b was elevated in AppNL-G-F knock-in AD mice as well as in human AD samples. Early high neuronal expression of SPPL2b was followed by a downregulation in late AD pathology in both AppNL-G-F mice and Braak stage V AD brains. Importantly, SPPL2b overexpression or its genetic deletion significantly increased or reduced APP cleavage and A{beta} production, respectively. Thus, our results strongly support the involvement of SPPL2b in AD pathology. The early A{beta}-induced SPPL2b upregulation may enhance A{beta} production in a vicious cycle further aggravating the A{beta} pathology suggesting SPPL2b as a potential anti-A{beta} drug target. Remembering all the different energy setting needed for synchronized cardioversion and defibrillation used to be confusing for a lot of people. Defibrillators can be broken down into three basic categories: Automated External Defibrillator (AED); Biphasic defibrillators; and Monophasic defibrillators. Because AEDs are designed to be used by first responders, and lay people with only minimal medical training, the controls are kept simple and are pre-programmed into the machine. Use of an AED to rapidly deliver a shock. Biphasic defibrillators automatically measure the impedance between the defib pads and will adjust the energy to deliver the shock needed based on the patient. Biphasic defibrillator use and energy setting. For older, monophasic defibrillators, use 360J to defibrillate V-Fib or pulseless V-Tach. AEDs must not be used on patients with a pulse. Cardioversion of patients in unstable SVT or V-Tach with a pulse using biphasic vs monophasic monitor/defibrillators. Team safety when performing synchronized cardioversion. Anaphylaxis is a relatively common and potentially lethal emergency. Current definitions highlight the presence of allergic and allergic-like reactions with end organ damage. Diagnosis can be difficult, but present guidelines (FAAN) focus on sensitivity above specificity. The main aspects of management are early recognition and early epinephrine. First line treatment is intramuscular epinephrine, fluids and positioning. In refractory cases, increasing dose of epinephrine, norepinephrine, vasopressin, glucagon, methilene blue and ECMO are considerations. Patients with airway compromise require advanced management. Disposition depends on severity and access to follow-up. An epinephrine auto-injector is necessary. Biphasic reactions are rare but real, with unclear incidence.

Author: Sam Killian, M.D. Educational Pearls Anaphylaxis is common in the ED. These patients are treated and then usually watched for 4-6 hours. Biphasic happens in patients that have a complete resolution of their anaphylaxis  for at least an hour, but then have a recurrence that requires pharmacologic  intervention. Most of the time this happens 3-6 hours later, but can happen more than 10 hours later. More commonly happens in patients that have a delayed presentation,  a wide pulse pressure, need multiple doses of epi, require the use of  a beta agonist, or are ages 6-9 years old. Background: Ankle sprains often result in ankle instability, which is most likely caused by damage to passive structures and neuromuscular impairment. Whole body vibration (WBV) is a neuromuscular training method improving those impaired neurologic parameters. The aim of this study is to compare the current gold standard functional treatment to functional treatment plus WBV in patients with acute unilateral unstable inversion ankle sprains. Methods/Design: 60 patients, aged 18-40 years, presenting with an isolated, unilateral, acute unstable inversion ankle sprain will be included in this bicentric, biphasic, randomized controlled trial. Samples will be randomized by envelope drawing. All patients will be allowed early mobilization and pain-dependent weight bearing, limited functional immobilization by orthosis, PRICE, NSARDs as well as home and supervised physiotherapy. Supervised physical therapy will take place twice a week, for 30 minutes for a period of 6 weeks, following a standardized intervention protocol. During supervised physical therapy, the intervention group will perform exercises similar to those of the control group, on a side-alternating sinusoidal vibration platform. Two time-dependent primary outcome parameters will be assessed: short-term outcome after six weeks will be postural control quantified by the sway index; mid-term outcome after one year will be assessed by subjective instability, defined by the presence of giving-way attacks. Secondary outcome parameters include: return to pre-injury level of activities, residual pain, recurrence, objective instability, energy/coordination, Foot and Ankle Disability Index and EQ 5D. Discussion: This is the first trial investigating the effects of WBV in patients with acute soft tissue injury. Inversion ankle sprains often result in ankle instability, which is most likely due to damage of neurological structures. Due to its unique, frequency dependent, influence on various neuromuscular parameters, WBV is a promising treatment method for patients with acute unstable inversion ankle sprains.

Introduction: Eczematous reactions to type I allergy-inducing antigens are documented in a subgroup of patients with atopic eczema. Yet, the underlying immunological mechanisms are not well understood. Material and Methods: To delineate the effect of native pollen grains on human skin of healthy and atopic individuals we performed patch tests (atopy patch test with native pollen grains, PPT). Nickel patch tests (NPT) served as an established model of contact dermatitis. Skin site biopsies were taken 6 - 96 h after allergen application and investigated immunohistochemically. Results: Histology of positive patch tests showed an influx of mononuclear cells (predominantly CD4+, CD25+, CD45RO+). This influx was detected earlier in the PPT reaction than in the immune response to nickel. A biphasic cytokine response could be detected in the PPT: IL-5 dominated in the early, IFN-gamma in the late phase. The NPT was continuously dominated by IFN-gamma. Dendritic cell subpopulations imitated the earlier kinetics of the mononuclear infiltrate. Discussion: Thus, pollen grains induce eczematous reactions in susceptible individuals. This reaction appears clinically and immunohistochemically similar to the contact hypersensitivity reaction to nickel but follows a faster kinetic and a biphasic course: Th2 and IgE in the early (24 h) and Th1 predominance in the late (96 h) phase. Continuous chemotrophic growth and respiration of Chromatiaceae species at low oxygen concentrations

Endogenous and maximum respiration rates of nine purple sulfur bacterial strains were determined. Endogenous rates were below 10 nmol O2 · (mg protein · min)-1 for sulfur-free cells and 15–35 nmol O2 · (mg protein · min)-1 for cells containg intracellular sulfur globules. With sulfide as electron-donating substrate respiration rates were considerably higher than with thiosulfate. Maximum respiration rates of Thiocystis violacea 2711 and Thiorhodovibrio winogradskyi SSP1 (254.8 and 264.2 nmol O2 · (mg protein · min)-1, respectively) are similar to those of aerobic bacteria. Biphasic respiration curves were obtained for sulfur-free cells of Thiocystis violacea 2711 and Chromatium vinosum 2811. In Thiocystis violacea the rapid and incomplete oxidation of thiosulfate was five times faster than the oxidation of stored sulfur. A high affinity of the respiratoty system for oxygen (K m =0.3–0.9 M O2, V max=260 nmol O2 · (mg protein · min)-1 with sulfide as substrate, K m =0.6–2.4 M O2, V max=14–40 nmol O2 · (mg protein · min)-1 with thiosulfate as substrate), for sulfide (K m =0.47 M, V max=650 nmol H2S · (mg protein × min)-1, and for thiosulfate (K m =5–6 M, V max =24–72 nmol S2O 3 2- · (mg protein · min)-1 was obtained for different strains. Respiration of Thiocystis violacea was inhibited by very low concentrations of NaCN (K i =1.7 M) while CO concentrations of up to 300 M were not inhibitory. The capacity for chemotrophic growth of six species was studied in continuous culture at oxygen concentrations of 11 to 67 M. Thiocystis violacea 2711, Amoebobacter roseus 6611, Thiocapsa roseopersicina 6311 and Thiorhodovibrio winogradskyi SSP1 were able to grow chemotrophically with thiosulfate/acetate or sulfide/acetate. Chromatium vinosum 2811 and Amoebobacter purpureus ML1 failed to grow under these conditions. During shift from phototrophic to chemotrophic conditions intracellular sulfur and carbohydrate accumulated transiently inside the cells. During chemotrophic growth bacteriochlorophyll a was below the detection limit.

Long-term normalization of glucose metabolism is necessary to prevent or ameliorate diabetic complications. Although pancreatic grafting is able to restore normal blood glucose and glycated haemoglobin, the degree of normalization of the deranged diabetic metabolism after pancreas transplantation is still questionable. Consequently glucose, insulin, C-peptide, glucagon, and pancreatic polypeptide responses to oral glucose and i.v. arginine were measured in 36 Type 1 (insulin-dependent) diabetic recipients of pancreas and kidney allografts and compared to ten healthy control subjects. Despite normal HbA1 (7.2±0.2%; normal
