On this episode of the Real Life Pharmacology podcast, I discuss Miralax (polyethylene glycol) pharmacology, adverse effects, and much more. MiraLAX is an osmotic laxative commonly used to treat occasional constipation by retaining water in the stool to increase stool frequency and soften consistency. The standard adult dose of MiraLAX is 17 grams (approximately one heaping tablespoon) dissolved in 4 to 8 ounces of liquid once daily. Electrolyte imbalances are rare but possible with prolonged or excessive use of Miralax, particularly in elderly or renal-impaired patients. Advise patients to maintain adequate hydration while using MiraLAX to support its mechanism and reduce side effects.

It all started with a desire to: – Lose a little weight – Clear up my skin – Have regular bowel movements Back in high school, I was breaking out regularly, trying to lose ten pounds, and relying on laxatives just to go to the bathroom. That's what led me down the rabbit hole of conventional medicine, where I experienced things like: -Appointments with a gastroenterologist who told me to eat more Cheerios and take Miralax daily – Being prescribed birth control by my PCP for acne – Eating 1,200 calories a day and running 1–2 miles daily to drop weight And honestly, those things kind of worked. Yes, I lost weight—but my digestive issues got worse. My skin was only clear as long as I stayed on the pill. And overall, I just felt awful. So I stopped birth control cold turkey. That's when everything fell apart. My face broke out into full-blown, inflamed cystic acne. My cycles never regulated themselves. And I was still dependent on Miralax just to go to the bathroom. After two more years of struggling, I was finally diagnosed with PCOS (polycystic ovarian syndrome) and given three options: – Go back on birth control – Start spironolactone – Try a keto diet I was overwhelmed and frustrated. I didn't want to go back on medication, and I certainly didn't want to keep treating symptoms without getting to the root. I thought to myself, There has to be a better way. And it turns out—there was. That's when I discovered food as medicine. I won't get into every diet or supplement I experimented with, but from the start, I focused on real, whole foods. Slowly but surely, my symptoms began to improve. That progress ignited my deep, burning passion for nutrition and women's health. Some major turning points from there: – Reading Dr. Jolene Brighten's Beyond the Pill Learning about cycle charting through FEMM—and eventually working for them as the lead nurse in their endocrinology and fertility telehealth clinic, where I also became a certified Fertility Awareness Instructor. On this episode, we go over a pharmacotherapy plan for a patient with sickle cell disease, HFrEF, DM2, dyslipidemia, hypertension, POAG, dry eye disease, constipation, GERD, and OSA. Episode 227 Patient Case Information 57-year-old male with sickle cell disease, HFrEF, DM2, dyslipidemia, hypertension, POAG, dry eye disease, constipation, GERD, and OSA comes to your clinic to establish care. His primary concern is the number of sickle cell crisis he has been experiencing (3 ER visits in the last 5 months). He is having significant constipation daily due to his maintenance opioid regimen.  He also reports difficulty controlling his blood glucose. He has experienced 6 hypoglycemic events (BG range between 65 and as low as 42) and is confused as to what he is doing wrong with managing his diabetes. He is also concerned because he is having vivid nightmares almost every night. He also asks about the best artificial tears to get OTC because his Refresh Optive (carboxymethylcellulose) doesn't seem to be working anymore. Medication List: Losartan 100 mg daily, atenolol 50 mg daily, torsemide 20mg – 4 tabs daily, metolazone 2.5 mg 2 times per week, amlodipine 5 mg daily, simvastatin 40 mg daily, fenofibrate 160 mg daily, lantus 40 units twice daily, humalog 5 units with meals if his pre-meal BG is > 150 mg/dL, metformin ER 500 mg twice daily, hydroxyurea 500 mg – two capsules daily (admits to limited adherence), latanoprost 0.005% nightly, esomeprazole 40 mg twice daily, famotidine 40 mg daily, MiraLax 17 grams daily, and bisacodyl 5 mg daily He has a CPAP at home but admits to using it very infrequently. BMP BP – 165/89 HR – 82 Na – 137 K – 3.6 Cl – 102 CO2 – 26 Glucose – 253 eGFR – 95 mL/min Ca – 8.9 Mg – 1.9 Lipid Profile Chol – 162 LDL-C – 99 HDL-C – 49 Triglycerides  - 242 Echo with EF – 32% B-Type Natriuretic Peptide – 56.2 Vitamin B12 – 367 Hemoglobin – 7.2 Hematocrit – 20.8 MCV – 98.6 Ferritin – 1491.6 (received blood transfusion 2 weeks ago) Thanks for listening! My question is about sodium. I get really dry skin mostly on my face when I have a very small amount of salt. Is this a gut issue like candida or yeast overgrowth, why would this be happening so much later in life? I'm a male, 43 years of age and I'm in pretty good health overall. I never had an issue with salt when I was younger, but now my nose, cheeks and sometimes hands get really really dry. I predominantly have salt only on the weekends, and whether its celtic or himalayan and maybe a cheat meal from a restaurant, it just seems to overloaded my body. I always keep up with my water throughout the day so I am hydrated as well. They're highly informative and I'm learning new things everyday!! My question is about bcaa's. I know you've talked about them before but I was just wondering if when working out or lifting weights, a person can have 3-5 grams in water during the session to keep the body anabolic and not tap into muscle as a fuel source? Ive heard that the leucine in bcaa's, if your having it every 3-4 hours throughout the day (especially if your not having any fish or meat with your meal) will basically flip an internal switch to keep you in a anabolic state so you don't slip into catabolism and you can keep building muscle. I know we are supposed to have half our body weight in protein a day, so would this go against that and up your protein intake too much? I have done multiple surgeries to remove them, but it keeps coming back. My last surgery was 2 years ago to remove two very large scars from my back and shoulder. After the surgery we followed up with radiation, compression, and steroid shots. I've been battling this since I was in 4th grade and have tried everything modern medicine has to offer. Yesterday I went to my doctor to get more steroid shots and he said I can't get them anymore because the skin is too thin. The scar is getting wider because of the tension in the shoulder. I've asked Naturopathic doctors before, but they didn't know how to help me. I've been following your work for the last few months and I'm getting ready to start the 21 day detox this weekend. Modern medicine claims that keloid scars are genetic, even though I'm the only one in my family who has it. I've always asked why I've only start developing them after age 10. The health and wellness industry talk a lot about exercising and restricting foods in order to lose weight, but there are some other key components to being healthy.    In this episode, I was a guest on my close friend's podcast, the Shine With Frannie Show, and we get very real to discuss some topics that most people are very embarrassed about but have issues with: digestion, bowel movements and emotional/mental pain. We are a three part being: spirit, soul and body. We have to address each of these areas if we want to experience Life the way God intends for us!   Frannie shares some of her weight loss journey, but specifically how after she lost her first 70lbs, her weight plateaued and as a result, she stopped having regular bowel movements. Child Constipation [9:36] Katie says: Hi Robb and Nikki! As always, thank you for all you do. My 7 year old daughter started complaining of stomach pain a few months ago. She's always been a good eater and has been gluten free since age 3 (not celiac, but noticed many health benefits for the whole family from cutting it out), eats minimal amounts of processed garbage and her diet is full of high quality protein, healthy fats and real, whole food. Her stomach pain increased in both severity and frequency over the summer. We cut back on dairy, with moderate success. We had blood tests done—all normal with no apparent food sensitivities or any other blood marker abnormalities. Finally her doctor did imaging that showed significant constipation. I have tended towards constipation my whole life, and my mom has IBS-C, so there's definitely some familial tendencies. Her pediatrician suggested miralax to get things going, which we have been doing for the past several days, and she's certainly pooping now, but still has daily tummy aches. Her pediatrician said daily miralax wouldn't become habit forming and was perfectly safe for long term use, but this obviously doesn't seem like the ideal answer. Do you have any suggestions on what I should try to bring her some relief, both in the short term but also for her digestive system as a whole? Why would a 7 year old who eats a healthy diet develop such severe constipation? How concerned should I be about using something like Miralax and are there better alternatives for keeping things moving? All my best, Katie 2. Anaerobic Ketolysis? [14:57] Howdy, Wolfs! Question... From what I understand, the body can only metabolize fatty acids aerobically, but it can metabolize glucose both aerobically and anaerobically. What about ketone bodies? Can the body anaerobically metabolize ketone bodies? I've scoured the internet periodically for probably a couple of years looking for a definitive answer to this nagging question, but I always come up empty handed. I have no functional reason to ask this question - I'm just a nerdy personal trainer with a penchant for delving deep and an evolutionary biology/ancestral health bent. 3. This is from an old post and the references don't work anymore - do you have any info to back up or refute the fasting /fast-twitch muscle claim? Would eating fat prohibit the muscle catabolism or would protein be needed?  thanks! 4. Finasteride for baldness? [22:43] Steven says: Hi Robb and Nikki, I recently visited the doctor for a checkup and she noticed that my hair is starting to thin up top. Sad day :( She recommended that I get on a medication called Finasteride, and said that if I start taking it now I can likely preserve my luscious locks. I did some digging online, and the testimonials I have found regarding side effects scare the bejesus out of me. Do you have any thoughts on this medication? And do you have any recommendations on the diet/lifestyle side of things to help keep my hair? For context, I am a 32-year-old male athlete (rock climber), and in excellent health. I follow a mostly paleo diet, with a few exceptions here and there. I eat plenty of protein (1 g per lb bodyweight) mostly from grass-fed beef, about 200-300 g of carbs per day to fuel my training and climbing, and plan to start taking more collagen supplements to support tendons, skin, and hopefully hair. You have spent your career as a Brand Manager focused on healthcare products. What drew you to this space?Yeah. You know, that’s it, it’s a great question. About 20 years ago, I was at a little bit of a inflection in my career and I had actually popped out of brand management for a couple of years and I was about to get back in and I actually was down in Atlanta and I had a unique opportunity where I was choosing between two different roles. One was in the healthcare space with Novartis consumer care, and the other one was for a company down in Atlanta that makes some really sweet bubbling drinks. And I thought to myself, why would I rather work on brands that help people and facilitate healthy lifestyles? Or would I rather not work in perhaps what might seem like a sexier waiting area of carbonated beverages and more recognized brands, but I said, you know what, let me, let me go in the healthcare space because that’s clearly going to be around for a long time and just, you know, not to sound corny or cheesy, but the, the idea of helping people improve their lives was, was it, you know, It’s amazing.  You’ve been with Kerasal for close to 3 years. What did the brand look like when you first joined compared to where it is at today? Yes. So it’s a really interesting brand. So carousel is in the foot care space and it, at the time it had two primary areas where it offered consumer products. One was for a post-op ring from nail fungus, and the other one was more for dry cracked feet. And they were nice little businesses, but one of the things that inspired me was the opportunity to try and take them to the next level. And they seemed a little bit under marketed as me. And I felt like if we got more marketing muscle and investment behind them, as well as innovated, I thought we could take it to the next level. And, and now we close to doubled over the past two and a half years. Through increasing our efforts, both in TV and traditional marketing vehicles like TV, but then also working with, with folks like yourself, the digital and social space. And then on top of that, we’ve also launched a really exciting new products that are extending the carousel brand name into other areas of foot care, such as what washes, what soaps. But masks. So really catering to consumers, desire for care. And it’s been really exciting because just over the past year or so, as much as the pandemic has been such a tremendous drain on a lot of businesses and vibes from a category perspective. But care has really and self care has really taken off as people have had more time to engage in, in taking care of themselves, but also because a lot of traditional means salons and pedicures and things like that. Haven’t been quite as, quite as accessible. So it’s been kind of an interesting intersection of. Of opportunity and growth to make the brain into an even higher level.  So you’re saying that a lot of this growth in the past year at least is just because people are at home because they’re not going to the nail salon. It's a funny thing, people get that they're stressed and, they get that it creates a physiological response in them, and then it makes them want to choke people, and kill people, and do stuff like that. But they're also, you know, it's like my heart's beating and I'm huffing and puffing and I have no energy, but there's not a connection. A lot of times with the fact that the stress is either triggering, causing or perpetuating their conditions. Not only is there not an exception for that with Hashimoto's thyroiditis or just Hashimoto's hypothyroidism, stress is probably the first thing I look at when a person walks in here and I do do functional neurology. I am involved in that and functional neurology. One of the big aspects of it is to assess people's stress responses and how it affects their entire physiology. For those of you who've watched us, and maybe you've seen our back to basics, we kind of have an idea as to how to attack auto-immunity and there's an organization to it. You've probably heard me talk about hierarchy, probably tired of hearing me talk about hierarchy in order and how you attack things. Stress is right at the top, like literally right at the top. It's like the three first things I look at, the five first things I look at is stress, sleep, relationships, blood sugar, and oxygen. I mean, I don't even go anywhere else until I look at those. It's because if you're deficient on all of that, I can't help you with your relationships. If you have a bad relationship with your husband, or your wife, or your boss, or your workers, or coworkers or wherever, and it's ongoing, that's something that has to be kind of handled before we get into the whole involvement and getting in, finding out all your triggers. That's like the biggest trigger that there is because it causes stress. Stress is number one, so it's stress, sleep and relationship, stress, relationships, sleep. They need to be addressed. Why? Because stress, when you go into stress, it alters your entire physiology. There's not that many things that affect every single one of the trillions of cells in you. Stress happens to be one of them and thyroid happens to be another one of them. Stress will trigger, basically what stress does, is it shuts down your whole gut system and it shuts down your bladder, your urinary bladder. Stress creates an issue where your stomach's not working. How many people do not know that stress causes ulcers? Well, long before you got ulcers, that's altered physiology in your stomach so that you're not digesting food, you're not sterilizing food, your bowels aren't moving. Stress is probably the number one cause of constipation in this country. Today’s guest has some great advice for how to go about this so that you don’t need to accept your pain as normality.   About Bridgitte Carroll, RD Although Bridgitte felt like absolute crap and knew her health was off, doctors still commended her on how healthy she was. She was on thyroid, ADHD, and migraine medication all at the same time. She was overweight and had to take Miralax to go to the bathroom. She was the healthiest eater she knew but she was exhausted, on a bunch of meds, and felt horrible. Her doctors didn’t have the answers. She didn’t have the tools to heal herself. But she was sick of constantly feeling “off”, and she was ready for a change. Constipation as a diagnosis can be dangerous, mainly because it is a powerful anchor in our medical decision-making. Chances are, you’d be right to chalk up the pain to functional constipation — 90% of pediatric constipation is functional, multifactorial, and mostly benign — as long as it is addressed. We’re not here for “chances are“; we’re here for “why isn’t it?“ Ask yourself, could it be: Anatomic malformations: anal stenosis, anterior displaced anus, sacral hematoma Metabolic: hypothyroidism, hypercalcemia, hypokalemia, cystic fibrosis, diabetes mellitus, gluten enteropathy Neuropathic: spinal cord abnormalities, trauma, tethered cord Neuromuscular: Hirschprung disease, intestinal neuronal dysplasia, myopathies, Down syndrome, prune belly syndrome Connective tissue disorders: scleroderma, SLE, Ehlers-Danlos syndrome Drugs: opioids, antacids, antihypertensives, anticholinergics, antidepressants, sympathomimetics Ingestions: heavy metals, vitamin D overload, botulism, cow’s milk protein intolerance   Red Flags Failure to thrive Abdominal distention Lack of lumbosacral curve Midline pigmentation abnormalities of the lower spine Tight, empty rectum in presence of a palpable fecal mass Gush of fluid or air from rectum on withdrawal of finger Absent anal wink       You gotta push the boat out of the mud before you pray for rain.   — Coach     Medications for disimpaction (do this first!) Polyethylene Glycol (PEG) 3350 (Miralax): 1 to 1.5 g/kg PO daily for 3 to 6 consecutive days.  Maximum daily dose: 100 g/day PO.  Follow-up with maintenance dose (below) for at least 2 months (usually 6 months) Lactulose: 1.33 g/kg/dose (2 mL/kg) PO twice daily for 7 days Mineral Oil (school-aged children): 3 mL/kg PO twice daily for 7 days   Medications for Maintenance (do this after disimpaction!) Polyethylene Glycol (PEG) 3350 (Miralax): 0.2 to 0.8 g/kg/day PO.  Maximum daily dose: 17 g/day. Maintenance dosing for Miralax may need to be tailored; up to 1 g/day maintenance.   Lactulose: 1 to 2 g/kg/day (1.5 to 3 mL/kg/day)PO divided once or twice daily.  Maximum daily dose: 60 mL/day in adults. Mineral Oil: 1 to 3 mL/kg/day PO divided in 1 to 2 doses; maximum daily dose: 90 mL/day Docusate (Colace): 5 mg/kg/day PO divided QD, BID, or TID (typical adult dose 100 mg BID) Senna, Bisocodyl — complicated regimens; use your local reference   Enemas Are you sure?  Have you tried oral disimpaction over days? No phosphate enemas for children less than 2. Check out today's questions:  Denise: I just finished reading 'The Rain Barrel Effect'. I couldn't put it down. So good! I am 51 years old and I have been on a wellness journey for over 15 years now so I do follow a lot of the principles in the book. I have been struggling with gut disbiosis for a long time. My most recent attempt to correct this was through Viome (microbiome stool testing). I am not seeing the results I was hoping for.My main issues right now are hormonal and digestive issues. I recently had a colonoscopy and was very surprised to find out I have diverticulosis in my entire colon. I have been following a very strict all organic, mostly plant based with high fiber for over 15 years so this puzzles me.I have looked into your tests and detox protocols. My thought was to do a 21 day detox to start. I do have some concerns with the shakes. I tend to not like the taste of protein shakes. Would you happen to have samples of your shakes that could be tried before committing to the 21 day protocol? I currently weigh 130 pounds and feel really good at 125. I would like to do the 21 day detox for detox purposes, but don't want to lose 15 pounds. Melissa Leah: Hello and thank you in advance for answering my question! I would like to know why do bodies produce polyps? I seem to be a super producer of them. I have had surgery to remove them from my nasal cavity so I can breathe easier, I have had 9 removed from my colon(pre-cancerous), and I was recently told that I have one growing on my cervix. Also, what are your thoughts on the surgical removal of polyps? I’ve been told that I need to do a yearly colonoscopy to remove any more that have grown (I’m 35 years old). Is there a way to shrink and get rid of polyps without surgically removing them? Katie: I ran the organic acids test last October of 2019 and everything came back great. Since then I have started having thick mucus in the back of my throat/mid esophagus along with stuffy nose congestion alternating with runny nose and slight left ear pain. I feel exhausted and completely run down. My primary care doctor ran CBC/thyroid/metabolic panel and that was fine as well. In preparation for the main spring planting season, I wanted to release this interview with resources on dog safety. Of course this year with the pandemic a lot has changed, but regardless most of what we cover is still relevant for having dogs on and off the block tree planting! Here is Céline's post on KKR from yesterday for ALL planters with dogs this season: For planters with dogs, here is a fairly exhaustive list of items to add to your first aid + emergency preparedness kits. We have all been warned to come to camp prepared for everything we will need for the duration of our contracts. Please think about what your animal may need in the event of an emergency while you're in the bush! -Veterinary-prescribed parasiticide for fleas/ticks/heartworm (such as Bravecto, Revolution, Nexguard Spectra etc.) -Deworming (such as Drontal, Strongid etc.) -Any prescription medication or pain killers (such as Metacam, Rheumocam, Meloxicam, Gabapentin etc.) -Probiotic (such as Fortiflora) -Benadryl *XYLITOL-FREE! She earned a pre-veterinary animal science Bachelors of Science in 2015 and has worked in several veterinary clinics as a veterinarian's assistant since. She strives to educate and support dog owners in forestry workplaces and remote communities throughout Canada. Céline constructed a project last spring for Brinkman's dog owners, specifically safety protocols while living in bush camp. I spent a fair amount of time researching online for any sort of comprehensive safety manual for dogs owners in the backcountry - whether for employment or leisure - and found very little information that was legitimate and complete. A version of her work was shared at the WFCA annual conference this year in Victoria, BC during the "Dogs in Forestry Workplace" session. Through nearly a decade of planting, she has witnessed numerous emergencies involving dogs in the bush- dog fights, musculoskeletal injuries, marijuana ingestion, insect sting anaphylaxis, hit-by-car, heat stroke/hypothermia, etc. etc. etc... Given most planting camps are a lengthy distance from any veterinary clinic, educating yourself on proper first aid treatment may reduce an animal's suffering and even save its life. Thank you for joining us for our 2nd Cabral HouseCall of the weekend! I’m looking forward to sharing with you some of our community’s questions that have come in over the past few weeks… Let’s get started!    Alex: Dr. Cabral, thank you so much for your work. The info and protocols you share have literally given me my life back. I listened to your podcast on lectins and need some clarity. The Organic Acids test confirmed some yeast and bacteria overgrowth, and I am currently on month two of the CBO protocol. I did not have any parasites, hpylori or clostridia. I do not want to eat poultry/meat for welfare standard issues, and seafood is not very appetizing to eat (except wild salmon). Thankfully I am not allergic to eggs, so this has been my main source of protein. I noticed the vegan proteins on the sensitive gut guide are mainly legumes. Hemp hearts are great but leave me very hungry in between meals. I have been trying lentils and chickpeas, but still have painful and explosive gas which goes away when I eliminate legumes. I did not have any IGG sensitivities to legumes, but have avoided them for years due to symptoms. Should I rerun the food sensitivity and eat legumes the day before the test? Also, once I complete the CBO protocol would it be possible to digest these vegan legumes? I heard from another source that it's because there is not the bacteria present to breakdown the legumes, but adding certain strains of bacteria can help with digestion. I am wondering if the CBO probiotics will repopulate the gut with these bacteria. Should I try again, is there anything I can do to add these vegan proteins back into my diet or am I just to avoid them for life and occasionally eat poultry/meat to keep protein intake at a decent level? Thanks again Chanel: Dear Dr. Cabral, I am a new listener and have loved every minute. I appreciate your analytical, but practical and caring approach. I also appreciate how you put yourself in the listener’s shoes. Every day, I try to learn more about the microbiome and natural health by reading, listening to podcasts, docuseries, etc. I am writing in to obtain your perspective and guidance. My family and I are on a health journey. We’ve made a lot of changes over the last 1 ½-2 years. I’ve suffered with some health conditions and have made many diet & lifestyle changes in order to improve my health. I still have some things to work on and my goal is to include as many natural/holistic methods as possible in order to regain my health and may write in about my health challenges at a later date. My husband is in the process of making changes too. Most importantly, our three daughters (ages 5, 8 and 12) have been the catalysts for our diet & lifestyle shifts. I am writing in about our girls, primarily my middle daughter. I apologize in advance for the length of this email but will try to condense everything. I’ve been able to look back in hindsight and realize some of the things that brought us to this point. In 2017-2018, we incorporated a lot of wheat in our diet. I thought that was the healthier thing to do so we traded white bread and pasta, etc. for wheat. This, along with an overall less than nutritiously dense diet, regular antibiotic prescriptions, hectic lifestyle, etc. more than likely, contributed to the decline of their gut and digestive health. Our children began to experience many health and digestive issues. Not long after this “wheat swap”, my oldest daughter began to randomly break out in hives. My youngest was diagnosed with GERD and then my middle was diagnosed with GERD along with other conditions. My middle daughter also experienced continuous reflux, stomach pain, bloating, nausea, headaches…you name it! These things heightened last spring; traditional medicine, doctors and specialists were not able to help us and may have set us back with acid blockers. This was a very hard time for our family and for me as a mother as I watched my children suffer. My middle daughter had a feeling of malaise, had low energy and was just not herself. She was also having moments of getting hot/flushed, with rapid heartbeat, headache, stomachache, dizziness, etc. We saw a naturopathic doctor in the spring 2019 and she was very helpful at the time. I also ordered a Diagnostic Solutions GI Map stool test from another practitioner for my middle daughter and the results showed high opportunistic bacteria, extremely low Sec IgA and low normal/”good” bacteria. We were put on a protocol and after treatment, things improved, but my younger two daughters take Miralax to prevent constipation. We removed gluten and dairy and embraced a whole-food, plant-based diet, incorporating organic, grass-fed & finished meats, etc. basically removed most candy & sugar, improved hydration and sleep, as well as slowed down our lifestyle. I thought that we were done with health issues, but my younger two daughters were diagnosed with lichen sclerosis (after many doctor & specialist visits) during the summer of 2019. We recently began seeing another holistic practitioner. She’s been helpful and we’ve incorporated supportive supplements to provide key nutrients that their bodies were missing, as well as some natural based prevention methods. This practitioner uses a Zyto energy scan. I’m skeptical of the scan and don’t fully understand it, but felt it had low risk and low cost so it was worth a try. My daughters and myself had the scan done and the results showed many different types of viruses (to include lyme and co-infections), bacteria, fungi, metals, parasites, microbes, mold, dysbiosis, etc. for all of us. However, my middle daughter is starting to experience some of the same symptoms as last spring, but to a lesser degree. I am very concerned to say the least. I feel that gut & digestive repair is a big piece to this puzzle. She’s had many other symptoms too, such as skin rashes, peeling and itching, an ulcer on her gum, and most recently, we’ve been referred to the children’s orthopedic department for unexplained ankle pain and knee pain that significantly impacts her mobility and to neurology for tingling on the bottom of her feet. I realize that this a lot of information, but I wanted to give you some background on my children. Another MRI was ordered, along with some labs. I’ve ordered your OAT for all of my girls, along with your stool, mold and toxicity tests for my middle daughter. We want to be through and comprehensive in our approach. We don’t want to return to this place of ill-health ever again. I know you can only give advice, but if this were you & your daughters, what would you do? We are desperate to rebalance the body and restore health to my child. We live in a rural community and there are not many practitioners that explore and practice natural-based medicine, along with comprehensive testing. Needless to say, our lives have been disrupted and we’re looking to find a place of peace. I realize you won’t be able to answer this immediately, but wanted your direct feedback.Concerning my middle daughter: *What else could/should we be evaluating or exploring-tests or labs? *What do we do in the meantime while we wait on test results? *Do some of these symptoms sound like an issue with the vagal nerve or gastroparesis? *What are your thoughts on the energy/Zyto Scan? *How would I know if we actually have all of the microbes, infections, bacteria and viruses, etc. reported from the energy scan and the best protocol to treat these things? Where do we start? Please help. Any guidance and/or direction is appreciated. She normally has difficulty sleeping however it has gotten worse over the past 6 months. Typically, it lasts around a week and is right before she gets her period. We both believe it is hormonal , common, but not normal. I searched previous podcasts and didn’t find anything. Anthony Rose: Does too much sugar in older mans diet effect his prostrate levels?? Anonymous: Dr. Cabral, I am writing to let you know that I would love to have you record a follow up to podcast #1306:Mystery Illness Solved:Client Case Study (TWT). My story is similar to your complicated client in that I was diagnosed with a Myeloproliferative cancer in January 2018. I have primary myelofibrosis with a component of Polycythemia Vera (needing an occasional phlebotomy due to hematocrit >45). I also have a history of Hashimotos Thyroiditis (currently on watch and wait) and reflux diagnosed a few years before. I have never had any other health problems in my life andI have been fairly active and healthy. November 2017 when my abnormal bloodwork was found I started an Anti-inflammatory diet and was ultimately able to bring my TPO antibodies from 150 to 70 and heal my reflux. Unfortunately as I continued trying for the next year to heal myself I was not able to improve my blood work with the myelofibrosis. In November 2018 I began working with a Functional Medicine practitioner and he found that I had high levels of mycotoxins, mercury, gut dysbiosis, and parasites. For the next year we worked to resolve and eradicate everything we found with the necessary protocols. I have been dry brushing, juicing, sauna sessions, coffee enemas, DCD, intermittent fasting, weekly 24 hour fast, supplementing, and on a anti-inflammatory diet. I even enrolled in IHP level 1 and 2 in January 2019 in order to further my education. I was trained in conventional medicine as a Physician Assistant and now I feel I could never practice the same way again due to what I have learned in experiencing this health journey. I feel better than I ever have in the past 20 years, the problem is, is that the Myelofibrosis continues to progress and we are at a point where my oncologist is suggesting I need to start on low dose chemotherapy. studied at Johns Hopkins for both depression and PTSD right so the stopper got brought up and there note people that have had near-death experiences that is a profound cause of PTSD and people can ruminate on death and it did definitely has in it I can say from my own experience and it stuck with me pretty well and there's there's a couple things my life they were kind of weird whenever I experienced in that the but this one in particular was probably the only time that I actually thought that I was more than likely going to die and what happened in the year 2000 exactly one week to the day before Anne-Marie and I were to get married we were down in Galveston were down some friends Dave and will and another guy named Cameron Cameron lived in Galveston and rain at the time lived just south Houston what we went down to the beach and Cameron had a new surfboard anyone is that you want to show us how how cool it was and how he could certainly gallows to visit Hawaii were now or it takes real skill to get up and that in those low energy wave is I was thinking of Cabo where the word the ocean to be very dangerous with the huge undertone sure you now there are jetties though and he's catching some waves and where you just casually drinking a beer to while he's out there doing it and that I do remember looking back that as we're watching Cameron serve that a wave came in it was probably bigger than the others not huge though Andy, wash upon us and then a few seconds goes by and then suddenly I'm not on my feet anymore and I'm underwater coaches and on base I can feel myself just kind of being I don't know's scurried underneath water and water go to my nose in such a huge undertow cam upper exactly what was I not terrifying how quickly something like that can happen it was out of nowhere maybe I had no expectation that there was even any element of danger of us standing there and then when I popped up notes so whenever we were standing on the 81 in the idea what will is and I keep trying to swim laterally that's really the only thing that I'm I knew away from the Jenny he was doing no good though I just did kidnap power the wave was pushing me and it ends up pushing right to the end of this Jenny rock right at the very very tip and I can remember as well I'm riding a wave and you hear crash the closer you get the louder the sound gets and then the wave pick up any of the crash will then I can remember distinctly that the water in the wave just before the storm to the rock crashes on the edge the very tip of the jetty and it just goes and is it goes down I could see all way down I could see him exhibiting muscles and whatever LOL although the rocks on my and in my mind since Marie now living in southeast and often times on the local news are talking about people who who died on the weekend just hanging out on the beach and I sent in my mind I remember thinking your to be on the knees and you not gonna make it to the wedding your wedding and but they should family together I guess you know and so then the head though the my my turn comes apparently so your watch so you know it's, I there is nothing you could do about it you look at all these rocks and muscles and sharp objects skip looking back on gigantic pieces or red granite stonelike – consider square cube but they deftly been cut right so they could build these jetties so they been stacked along the end and so then my wave comes in a seat I'm thinking it's it's can be head injury now because that you being hurled into jagged large wall so just a big wall of jammed my my leg between two of those gigantic stone dry and water is crashing up and basically coming over my head with each successive wave and had to get energy try to think about the timing it really could believe that I was still there and finally as I began to peace together I really haphazard but it worked strategy to get out of there I crested the top and the moment that I get to the top I thankfully I see Dave and then what he's doing is he's retrieving Marie I thought I was a bad spot they actually washed up on close to the the tip of the jetty but actually on the side and that is nothing like a underwater washing machine boat shoes and she every time she tried to step up Dave said she was being sucked you see video having that of tourists and in Hawaii there's a state of the get sucked up and then they can't get out of that little whirlpool all awful Gallic when the crime got crashing and that the big stones and Dr. of I got back a big cliff it's it's very very both went through together going through that was comes bombing expansive we made we had several conversations about just that one evening that just that one 30 minutes that said they kinda form the way that we both were able to Kelly Golding and reevaluate where you are in life and then didn't really occur to me until a few years ago day what was what was that coping mechanism what allowed these different things to happen and I'll enough as I began to look through it I learned that Marine are actually fortunate that we did it together because people often times it have near-death experiences alone or away from all of their significant others or close family members they actually end up being not perceived well by those who don't understand their experience I can actually build up a wall lots of times there are people who have near-death experiences that they have a significant other that didn't go through the same sequence of events are the same expense at all and they actually have a hard time relating until the other person has time to recoup from that and I found that fascinating that just by complete block it number one we survived it in number two that we did it together that that wasn't something that became a barrier wall action was a bonding experience and others actually lots of the dopaminergic activity or dopamine release it happens and the realization of the things that you did right you get past the trauma and so which of course not I don't I just think it's fascinating because that is the way that you said that you enumerated note through through life you're going through other things and you you go back you can reflect on that you we came out of that so we can we can fix we can basically get through anything else that's not a life-and-death situation I really think that that's a lot of our veterans feel yes which is why they find community amongst other veterans because you don't look at the Vietnam era look at the rock era were people come back and third misunderstood get the PTSD that got traumatic brain injury all that kind of stuff and if you don't find your sense of community were other people can get it it can be a very lonely an anxiety provoking time without question into make it even more lucky and to bring and they the veterans it is exactly what I was moves on the misers reading through this we didn't lose anybody in our party out of the five of us that were together we all five left had someone been hurt whatever did net the scenario recovery would have been far more different that the same thing plays out people who have automobile accidents they have a plane crash or whatever but when a band of people go through something together it's traumatic and life-threatening with and that's one thing if somebody survives and someone doesn't it can actually really really way and that's what gives way to the survivor's guilt and different things like that so I just had a patient that told me that her functional medicine psychiatrist was at the national of PTSD meeting and in the meeting were a bunch of gastroenterologists around the country no kidding – it's good I was like wow I didn't think and choose like yeah will first of all in the intake for now you know that was throwing some intake form you do smoke do whatever is commendable to address certain things so I think that one of the criteria Medicare now was asking his have you been a victim of any trauma that you start going on the PTSD route and it makes total sense as a gastroenterologist if you are not able to get past an event in your life that's how come so many G.I. issues are tied to childhood abuse no joke and we've learned that forever that when you start looking at these different things that that could really be tied to something like that because the serotonin in the gut there's more certainly got that in the brain and then anytime you have any type of dopaminergic depletion or any type of cortisol rise that affects your intestines so all these kinds of things it's interesting because I'm trying to think about wow if I had the perfect practice it would include both the addressing not just using proper diet not just using some some bio hacking tools right like sauna like CBD but then also offering the environment where meditation can be taught mindfulness meditation so in article just came out this fits right and I just read this were they published that and realize how far you have to go mindfulness meditation can actually help with rumination not food rumination rumination of thought thoughts yeah so rumination of thought you look that many of the way it's that thing where you just go man can't get that out of my head why do I keep thinking about the negative thing why does this happen why am I your brain to administer the right amount of dopamine to where you find the focus necessary to achieve the task and even though it may set expectations unrealistically high for what the outcome is when those achievements don't happened or don't occur the level of depression is is less it's basically like well we can do it again next time but the times of people are withheld from doing their superstitions they almost inevitably always blame the fact that they didn't do the routine there so this all tied back to some really basic stuff that Tim Ferris and several others have said find your morning routine to establish your mode of success and I could start with making your bed every day or your routine for meditation or your routine on how you make your breakfast or just your routine on how you greet your family members but your routine matters you set up your routine you take control of your day that way and then it is you've set it in motion a mean Arnold Schwarzenegger Tony Robbins talks about it superset dilemma super successful people all have his morning routine with a get up and they do the stuff I think that you start using this morning routine and augmenting it by getting out and earth in general being to get outside little bit special with the sun shining start your circadian rhythm going right by doing that you can drink a big glass water start your hydration routine and then if you're capable to be really cool to do a meditation every day before we do the show I was go to the Colleyville lifetime I have got a little routine where I ended it by being in the sauna and during 15 minutes of mindfulness meditation and I think that that is a way to control even if this show sometimes you know we just go okay hope the show goes well least I have control of ColoRectal  cancer (CRC) is the second most frequent cause of cancer DEATH in the United States.  The Most Frequent Cause of Cancer Death is…… Lung Cancer. This year, an estimated 145,600 adults in the United States will be diagnosed with colorectal cancer. According to Cancer.net…… an estimated 51,020 of the 145,600 adults will die this year…..due to ColoRectal Cancer. When colorectal cancer is found early, it can often be cured. CURED Nation! This is due to improvements in treatment and increased screening….. which finds colorectal changes before they turn cancerous and cancer at earlier stages. Medicare Part B offers TWO Preventative Screening Tests The First…. Is a Fecal Occult Blood Test “Fecal Occult” Blood Test is just a scary way of saying….. “ Looking for Blood in your Poop.”  The test ONLY detects the “presence” or “absence” of blood in your stool. The test does not indicate potential sources of bleeding and it does not “Diagnos” disease. “Fecal” means……“Stool” or “Poop”….and…. “Occult Blood” means you can’t see the blood in your stool with the “naked eye,” so….. the specimen is sent to the lab for a closer look. Blood in the stool may indicate polyps…. or it may indicate cancer in the intestine or rectum….though not all cancers or polyps bleed. If blood is detected through the “Fecal Occult Blood Test,” additional tests may be needed to determine the source of bleeding as well as “diagnosing” an ailment or disease. Blood in the stool could also mean Hemorrhoids….which are swollen veins in the lowest part of your rectum and anus. Sometimes the walls of these blood vessels stretch so thin….. that the veins bulge and get irritated, especially when you poop! Straining while pooping is a major factor in Hemorrhoids.  EAT more Fiber Nation! Eat More Vegetables….try Metamucil or Miralax. Straining to poop is not good. Drink more water! You should try to drink at least 96 oz. a day. I use a 24oz bottle I fill 4 x a day….. to get my 96 oz of water. You can do it. It’s important. Hemorrhoids can cause itching & pain.  Hemorrhoids can also bleed. There are several types of Fecal Occult Blood Tests, I’m going to discuss the “newer version,” which is called a “ Immunochemical Fecal Occult Blood Test,” (aka iFOBT or FIT) The IFOBT or FIT test is less of a mess and easy to administer. Typically, you have a “spoon-like” device to collect the sample of stool and you place the device into a collection container then seal it. You either return the collection container to your doctor’s office, or you mail it. There are no dietary restriction with the iFOB-IT and the test can be performed on any random sample of your stool. Your Doctor will review the results and there are just two options: Negative Result, which means no blood was detected in the stool sample you provided. OR…….. Positive Result, which means blood WAS detected in the stool sample you provided.   This type of test ISN’T ALWAYS accurate. Your fecal occult blood test could show a negative test result when cancer is present (false-negative result) if your cancer or polyps don't bleed. If you had the test to screen for colon cancer and you're at average risk — you have no colon cancer risk factors other than age — your doctor may recommend waiting one year and then repeating the test.  If you have a “positive result,” You may need additional testing — such as a colonoscopy — to locate the source of the bleeding. Under Medicare…… The Fecal Occult Blood Test…. can be given ONCE every 12 months if you’re 50 or older, at ZERO Cost to you.        Now….. let’s take a look at Mult-Target Stool DNA Tests. You will know the “Multi-Target Stool DNA Test” more commonly known as “ColoGuard.” ColoGuard …….   addresses several barriers to colorectal screening. Patient concerns with colonoscopy. Include…having to schedule a separate and lengthy appointment at the testing facility. The need to undergo a “Stay Close to my Bathroom” bowel preparation the exposure to sedation or anesthesia……and the discomfort associated with an invasive imaging process…. Of sticking either the “colono-scope” during a colonoscopy or a flexible sigmoud device up your butt.   By comparison, the “Multi Target Stool DNA” screening test is a noninvasive, “multi-marker”, stool-based ColoRectal Cancer screening test….. that detects altered De-oxyribo-nucleic Acid (DNA), , as well as a fecal immunochemical test (FIT)… for blood released from cancer and precancerous lesions of the colon. The presence of fecal hemoglobin….. even in the absence of elevated DNA markers…..can lead to a positive result given the weighted nature of the Multi Target Stool DNA algorithm. Patients may collect and mail stool specimens from their homes with no bowel preparations and no dietary or medication restrictions.   Medicare covers this at-home multi-target stool DNA lab test…. once every 3 years…if you meet ALL of these conditions:   You’re  between the age of  50-85. You show NO CURRENT symptoms of colorectal disease including, but not limited to one of these: Lower gastrointestinal pain Blood in stool Positive Guaiac fecal occult blood test….which is an older version of the Immunochemical Test… where you “smear” stool onto a TEST Card with a wooden applicator or brush. The Guaiac test has dietary restrictions and you are required to collect “TWO” or more samples from the same Stool Sample for the test. Much Messier than the Immunochemical Fecal Blood Occult Test. OR…… A Positive Result from a Fecal Blood Occult Test ALSO    YOU NEED TO BE….. at average risk for developing colorectal cancer, meaning: You have no personal history of  (adenomateous ) polyps”  which are …..  a common type of polyp. They are gland-like growths that develop on the mucous membrane that lines the large intestine. They are also called adenomas: You have no personal history of  … colorectal cancer, or inflammatory bowel disease, including Crohn’s Disease and ulcerative colitis. OR…… You have no family history of colorectal cancers or adenomatous polyps, familial adenomatous polyposis, or hereditary nonpolyposis colorectal cancer.   If you meet the above criteria….. You pay nothing for this test if your doctor…. or other qualified health care provider accepts Medicare.   So that’s the difference between Multi Target Stool DNA Tests vs. Fecal Occult Blood Test.   If you haven’t had one of these preventative tests, speak to your primary doctor and get one. Thank you for joining us for our 2nd Cabral HouseCall of the weekend! I’m looking forward to sharing with you some of our community’s questions that have come in over the past few weeks… Let’s get started! Eliana: Hello, I would first like to say that I absolutely love your Podcasts and everything you have taught me thus far! I am a 21 year old personal trainer who is extremely interested in natural health and this podcast has changed my world! I have been struggling with a hiatal hernia for years. It feels like something is pressed up against my diaphragm or sometimes my left rib cage (it switches between those 2 places so sometimes I wonder if my diaphragm is out rather than it being a hiatal hernia). I have received chiropractic treatment for this and while the manual adjustment of pushing it back into place does help, it does not last. My mom, grandma, and twin sister also struggle with this. I had lymes disease 3-4 years ago (so did mom and sister) and was treated with a combination of homeopathy, Chinese herbs and glandulars by a lymes specialist in Maine. Also had H pylori and Epstein bar and was treated for those coinfections as well. Since then I feel great, had a feeling of relapse a year ago and cut out all food sensitivities. I feel great again but my hiatal hernia comes and goes in long phases. It is back now and I have not noticed any pattern to when it comes and goes. It causes me extreme stress and anxiety, especially at night. Also causes bloat. I have eaten extremely healthy (organic Whole Foods) since I was born thanks to my parents, and I pay attention to deficiencies, eat 3 meals a day, have a smoothie in the morning, fast for 12-13 hours, etc. I also exercise 5 days a week (lifting, cardio, mobility work, etc) and avoid crunching movements due to the hernia. I take zinc, B12, D3, and am a pescatarian/gluten, dairy, corn free, no processed foods, no antibiotics, don’t eat late at night, don’t overeat, eat tons of veggies and fruit. Please let me know what you know and suggest regarding hiatal hernias. Thank you so much! Christie: Good morning. Sorry for the long post but I’m hoping you can help. My mom started with diverticulosis in 2007 and in 2012 it became “moderate” diverticulitis. It got continually worse and then in 2017 she had a bowel resection done. They removed a foot of her colon. Ever since then she has dealt with horrible gas and constipation. She just had endoscopy done a few weeks ago because she felt like things weren’t passing through and the doctor said her stomach was red and inflamed but she does not have h. pylori. She also has Gastroparesis. The doctor then put her on Zantac two times a day for 4 weeks and Miralax at night. She also takes the Beachbody digestive boost. Her doctor has also mentioned putting her on Linzess. She can’t afford testing because my parents are retired and on a limited income so I’m wondering if you think doing a certain protocol would help her without testing first. Can you recommend anything? I would appreciate any suggestions. I’m not happy with how things are going for her and really want to see her healthy again.  Laura: Hi I have a couple of questions. I have only been listening for around a month so I may have just missed this. I haven’t heard dr Cabral talk much about beans and legumes, is the a reason why hey aren’t recommended or not at the top of the list! I’m primarily veggie and eat a lot of veg, however I do like to add lentils, beans or chickpeas to my food to add in protein and other nutrients! I was just interested to find out what your take is on this. Also, I’m going to start a naturopath nutritional therapy course in the UK next month so maybe I will find all this out for myself. However I have taken nutritional supplements before and I use to really be bought into juicing detoxing. However I have read various things that suggests that a supplements can actually shorten your life and aren’t as good as they seem. Also taking butrioanl shakes with condensed nutrients can actually be difficult for your body to digest all at once. For example you would eat a whole meal in a few minutes, therefore how is it ok for your body to process nutritional supplement shakes in such a short amount of time! Renae: Hello Dr Cabral, My Mum was diagnosed with hemolytic anaemia a couple of years ago and has been taking pharmaceutical medication daily to manage this. We both believe that there must be a way of finding the root cause and getting to the bottom of it so that she can feel well and be medication free again. Other symptoms she experiences separate from the anaemia are under active thyroid, osteo arthritis, IBS, foggy brain and really bad memory loss. She is only 62. She has tried to improve on things like diet to help herself and has seen a naturopath briefly but has not seen a change in her blood levels or other symptoms. I gratefully discovered your work a few months ago and I am very hopeful that with your advice on what labs she should run and where to start that she can finally find her path to wellness again. In the meantime I have given her a copy of your book. I did write in to your team who recommended the HTMA and OAT (thank you team :)), however given this is not a common auto immune condition I just wanted to be sure if there were any other recommendations you would make. Children the world over are fascinated with what can possibly “fit” in their orifices.  Diagnosis is often delayed.  Anxiety abounds before and during evaluation and management.     Most common objects:1,2 Food Coins Toys Insects Balls, marbles Balloons Magnets Crayon Hair accessories, bows Beads Pebbles Erasers Pen/marker caps Button batteries Plastic bags, packaging Non-pharmacologic techniques Set the scene and control the environment.  Limit the number of people in the room, the noise level, and minimize “cross-talk”.  The focus should be on engaging, calming, and distracting the child. Quiet room; calm parent; “burrito wrap”; guided imagery; have a willing parent restrain the child in his or her lap – an assistant can further restrain the head. Procedural Sedation Most foreign bodies in the ear, nose, and throat in children can be managed with non-pharmacologic techniques, topical aids, gentle patient protective restraint, and a quick hand.  Consider sedation in children with special health care needs who may not be able to cooperate and technically delicate extractions.  Ketamine is an excellent agent, as airway reflexes are maintained.3  Remember to plan, think ahead: where could the foreign body may be displaced if something goes wrong?  You may have taken away his protective gag reflex with sedation.  Position the child accordingly to prevent precipitous foreign body aspiration or occlusion. L’OREILLE – DAS OHR – вухо – THE EAR – LA OREJA – 耳 – L'ORECCHIO Essential anatomy: The external auditory canal. Foreign bodies may become lodged in the narrowing at the bony cartilaginous junction.4  The lateral 1/3 of the canal is flexible, while the medial 2/3 is fixed in the temporal bone – here is where many foreign bodies are lodged and/or where the clinician may find evidence of trauma.  Pearls: Ask yourself: is it graspable or non-graspable?5 Graspable: 64% success rate, 14% complication rate Non-graspable: 45% success rate, 70% complication rate5 If there is an insect in the external auditory canal, kill it first. They will fight for their lives if you try to dismember or take them out.  “In the heat of battle, the patient can become terrorized by the noise and pain and the instrument that you are using is likely to damage the ear canal.”5,6  Use lidocaine jelly (preferred), viscous lidocaine (2%), lidocaine solution (2 or 4%), isopropyl alcohol, or mineral oil. Vegetable matter? Don’t irrigate it – the organic material will swell against the fixed structure, and cause more pain, make it much harder to extract, and may increase the risk of infection. Pifalls: Failure to inspect after removal – is there something else in there? Failure to assess for abrasions, trauma, infection – if any break in skin, give prophylactic antibiotic ear drops Law of diminishing returns: probability of successful removal of ear foreign bodies declines dramatically after the first attempt   LE NEZ – DIE NASE – ніс – THE NOSE – LA NARIZ – 鼻 – IL NASO Essential anatomy: Nasopharyngeal and tracheal anatomy. Highlighted areas indicate points at which nasal foreign bodies may become lodged.4 Pearls: Consider using topical analgesics and vasoconstrictors to reduce pain and swelling – and improve tolerance of/cooperation with the procedure. Use 0.5% oxymetolazone (Afrin) spray and a few drops of 2 or 4%   Pros: as above.  Cons: possible posterior displacement of the foreign body.7 Be ready for the precipitous development of an airway foreign body Pitfalls: Beware of unilateral nasal discharge in a child – strongly consider retained foreign body.8 Do not push a foreign body down the back of a patient's throat, where it may be aspirated into the trachea. Be sure to inspect the palate for “vacuum effect”: small or flexible objects may be found on the roof of the mouth, just waiting to be aspirated.   LA GORGE – DER HALS – горло – THE THROAT – LA GARGANTA – 喉 – LA GOLA Before we go further – Remember that a foreign body in the mouth or throat can precipitously become a foreign body in the airway.  Foreign body inhalation is the most common cause of accidental death in children less than one year of age.9,10 Go to BLS maneuvers if the child decompensates. Infants under 1 year of age – back blows: head-down, 5 back-blows (between scapulae), 5 chest-thrusts (sternum).  Reassess, repeat as needed. Children 1 year and up, conscious – Heimlich maneuver: stand behind patient with arms positioned under the patient’s axilla and encircling the chest. The thumb side of one fist should be placed on the abdomen below the xiphoid process. The other hand should be placed over the fist, and 5 upward-inward thrusts should be performed. This maneuver should be repeated if the airway remains obstructed.  Alternatively, if patient is supine, open the airway, and if the object is readily visible, remove it.  Abdominal thrusts: place the heel of one hand below the xiphoid, interlace fingers, and use sharp, forceful thrusts to dislodge.  Be ready to perform CPR. Children 1 year and up, unconscious – CPR: start CPR with chest compressions (do not perform a pulse check). After 30 chest compressions, open the airway. If you see a foreign body, remove it but do not perform blind finger sweeps because they may push obstructing objects further into the pharynx and may damage the oropharynx.  Attempt to give 2 breaths and continue with cycles of chest compressions and ventilations until the object is expelled. Chest films are limited: 80% of airway foreign bodies are radiolucent.11  Look for unilateral hyperinflation on expiratory films: air trapping.  Essential anatomy: Most esophageal foreign bodies in children occur at the level of the thoracic inlet / cricopharyngeus muscle (upper esophageal sphincter).  Other anatomically narrow sites include the level of the aortic arch and the lower esophageal sphincter. Coin en face – in the esophagus – lodged at the thoracic inlet.12  The pliable esophagus accommodates the flat coin against the flat aspect of the vertebra.11 Beware the “double-ring” sign: this is a button battery13 This is an emergency: the electrolyte-rich mucosa conducts a focal current from the narrow negative terminal of the battery, rapidly causing burn, necrosis, and possibly perforation.  Emergent removal is required. Button batteries that have passed into the stomach do not require emergent intervention – they can be followed closely. Not a button battery, not a sharp object, not a long object? If there is no obstruction, consider revaluation the next day – may wait up to 24 hours for passage.14  Sharieff et al.15 found that coins found in the mid to distal esophagus within 24 hours all passed successfully. What conditions prompt urgent removal? Size Infants: objects smaller than 2 cm wide and 3 cm long will likely pass the pylorus and ileocecal valve10 Children and adults: objects smaller than 2 cm wide and 5 cm long will likely pass the pylorus and ileocecal valve9 Character Sharp objects have a high rate of perforation (35%)1 Pearls: History is essential. Believe the parents and assume there is an aspirated/ingested foreign body until proven otherwise. History of choking, has persistent symptoms and/or abnormal xray? Broncoscopy! Cohen et al.16 found that of 142 patients evaluated at a single site university hospital, 61 had a foreign body. Of the 61 patients, 42 had abnormal physical exams and radiographs and 17 had either abnormal physical exams or radiographs, and 2 had normal physical exams and radiographs, but both had a history of persistent cough.  Bottom line: history of choking PLUS abnormal exam, abnormal films, or persistent symptoms, evaluate with bronchoscopy. For patients with some residual suspicion of an aspirated foreign body (mild initial or improving symptoms; possibly abnormal chest x-ray; low but finite risk), consider chest CT with virtual bronchoscopy as a rule-out strategy.17,18 Outpatients who have passed a small and non-concerning object into the stomach or beyond: serial exams and observing stools – polyethylene glycol 3350 (MiraLAX) may be given for delayed passage19 Pifalls: A single household magnet will likely pass through the GI tract, with the aforementioned dimensional caveats. Two or more magnets, however, run the risk of attraction and trans-bowel wall pressure necrosis. Not all magnets are created equal. Neodymium magnet toys (“buckyballs”) were recalled in 2014 (but are still out there!) due to their highly attractive nature.  These magnets must be removed to avoid bowel wall ischemia. 19–21 Patients should avoid wearing belt buckles or metallic buttons in case of single magnet ingestion while waiting for the single magnet to pass DES OUTILS DU MÉTIER – DIE HANDWERKSZEUG – Знаряддя праці – TOOLS OF THE TRADE – LAS HERRAMIENTAS DEL OFICIO – GLI ATTREZZI DEL MESTIERE –  仕事のツール It’s best to keep your armamentarium as large as you can.   Curette A small foreign body in the lateral 1/3 of the auditory canal may be amenable to a simple curettage.  Hair beads (if the central hole is accessible) may be manipulated out with the angled tip of a rigid curette.  Steady the operating hand by placing your hypothenar eminence on the child’s zygoma or temporal scalp, to avoid jutting the instrument into the ear canal with sudden movement.  There is a large selection of disposable simple and lighted curettes on the market. Right-angle Hook Various eponymous hooks are available for this purpose; one in popular use is the Day hook, which may be passed behind the foreign body.22  An inexpensive and convenient alternative to the commercially available right-hooks is a home-made version: make your own by straightening out a paperclip and bending it to a right angle23 at 2-3 mm from the end (be sure not to use the type that have a friable shiny metallic finish, as the residue may be left behind in the ear canal).  If it is completely lodged, use of a right-angle hook will likely only cause trauma to the canal. Alligator forceps Alligator forceps are best for grasping soft objects like cotton or paper.  Smooth, round or oval objects are not amenable to extraction with alligator forceps.  When using them, be sure to get a firm, central grip on the object, to avoid tearing it into smaller, less manageable pieces.  Pro tip: Look before you grip! Be careful to visualize the area you are gripping, to avoid pulling on (and subsequently avulsing) soft tissue in the ear canal. Cyanoacrylate (Dermabond®, SurgiSeal®) Apply cyanoacrylate to either side of a long wooden cotton swab (the lecturer prefers the cotton tip side, for improved grip/molding around object).  Immediately apply the treated side to the object in the ear canal in a restrained patient.  Steady the hypothenar eminence on the child’s face to avoid dislodgement of the cotton swab with sudden movement.  Apply the treated swab to the foreign body for 30-60 seconds, to allow bonding.  Slowly pull out the foreign body.  Re-visualize the ear canal for other retained foreign bodies and abrasion or ear canal trauma. Did the cyanoacrylate drip?  Did the swab stick to the ear canal? No problem – use 3% hydrogen peroxide or acetone.24  Pour in a sufficient amount, allow to work for 10 minutes.  Both agents help to dissolve ear wax, the compound, or both.  Repeat if needed to debond the cyanoacrylate from the ear canal.24,25 Irrigation Irrigation is the default for non-organic foreign bodies that are not amenable to other extraction techniques.  Sometimes the object is encased in cerumen, and the only “instrument” that will fit behind the foreign body is the slowly growing trickle of water that builds enough pressure to expulse it.  Do not use if there is any organic material involved: the object will swell, causing much more pain, difficulty in extraction, and possibly setting up conditions for infection. Position the child either prone or supine, gently restrain (as above).  Let gravity work for you: don’t irrigate in decubitus position with the affected ear up.  It may be more accessible to you, but you may never get the foreign body out. To use a butterfly needle: use a small gage (22 or 24 g) butterfly set up, cut off the needle, connect the tubing to a 30 mL syringe filled with warm or room-temperature water. Insert the free end of the tubing gently, and “secure” the tubing with your pinched fingers while irrigating (think of holding an ETT in place just after intubation and before taping/securing the tube).  Gently and slowly increase the pressure you exert as you irrigate. To use an IV or angiocatheter: use a moderate size (18 or 20 g) IV, remove the needle and attach the plastic catheter to a 20 mL syringe, and irrigate as above. Acetone Acetone has been used successfully to remove chewing gum, Styrofoam, and superglue from the ear canal24,26,27  Use in cases where there is no suspicion of perforation of the tympanic membrane. Docusate Sodium (Colace®) Cerumen is composed of sebaceous ad ceruminous secretions and desquamated skin.  Genetic, environmental, and anatomical factors combine to trap a foreign body in the external canal.  Use of a ceruminolytic such as docusate sodium may help to loosen and liberate the foreign body.28  Caveat medicus: Adding docusate sodium will make the object more slippery – this may or may not be an issue given the nature of the foreign body. In the case where loosening the ear wax may aid extraction (and will not cause a slippery mess), consider filling the ear canal will docusate sodium (Colace), having the child lie with the affected side up, waiting 15 minutes, and proceeding.  This is especially helpful when planning for irrigation. Magnets Rare earth magnets (a misnomer, as their components are actually abundant) such as neodymium can be useful in retrieving metallic foreign bodies (e.g. button batteries in the nose or ears).29,30  Magnetic “pick-up tools” – used by mechanics, engineers, and do-it-yourselfers – are inexpensive and readily available in various sizes, shapes, and styles such as a telescoping extender.  Look for a small tip diameter (to fit in the ear canal as well as the nose) and a strong “hold” (at least a 3-lb hold). Alternatively, you may purchase a strong neodymium bar magnet (30- to 50-lb hold) to attach to an instrument such as an alligator forceps, pick-up forceps, or small hemostat (a pacemaker magnet may also work).  The magnetic bar, placed in your palm at the base of the instrument, will conduct the charge (depending on the instrument) and allow you to retrieve many metallic objects.31  Although stainless steel is often said to be “non-magnetic”, it depends on the alloy used, and some may actually respond to the strong rare earth magnet.  Most stainless steel has a minimum of 10.5% chromium, which gives the steel its 'stainless' properties (essentially corrosion resistance).  A basic stainless steel has a “ferritic” structure and is magnetic.  Higher-end stainless steel such as in kitchen cutlery have an “austenitic” structure, with more chromium added, and so less magnetic quality.  (Ironically, the more “economical” instruments in the typical ED suture kit have less chromium, and so are more magnetic – use these with your neodymium bar magnet to conduct the magnetic charge and extract the metallic foreign body.) Bottom line: if it’s metal, it’s worth a try to use a magnet.  Even if the metal is very weakly magnetic, the strong neodymium magnet may still be able to exert a pull on it and aid in extraction. Snare Technique A relatively new method, described by Fundakowski et al.32 consists of using a small length of 24-gauge (or similar) wire (available inexpensively online, and kept in your personal “toolkit”; see Appendix B below) to make a loop that is secured by a hemostat (the 24-gauge wire is easily cut into strips with standard trauma scissors).  After treatment with oxymetolazone (0.05%) and lidocaine (1 or 2%) topically, the loop is passed behind the foreign body (in the case report, a button battery).  Pull the loop toward you until you feel that it is sitting up against the button battery.  Now, turn the hemostat 90° to improve your purchase on the foreign body.  Pull gently out.  This technique is especially useful when the foreign body has created marked edema, either creating a vacuum effect or making it difficult for other instruments to pass. Balloon Catheters (Katz extractor®, Fogarty embolectomy catheter) Small-caliber devices (5, 6, or 8 F) originally designed for use with intravascular or bladder catheterization may be used to extract foreign bodies from the ear or nose.7,33  A catheter designed specifically for foreign body use is the Katz extractor.  Inspect the ear or nose for potential trauma and to anticipate bleeding after manipulation (especially the nose).  Deflate the catheter and apply surgical lubricant or 2% lidocaine jelly. Insert the deflated catheter and gently pass the device past the foreign body.  Inflate the balloon and slowly pull the balloon and foreign body out.  Re-inspect after extraction. NB, from the manufacturer of the Katz extractor, InHealth: “the Katz Extractor oto-rhino foreign body remover is intended principally for extraction of impacted foreign bodies in the nasal passages. This device may also be used in the external ear canal, based upon clinical judgment.” Mother’s kiss The mother’s kiss was first described in 1965 by Vladimir Ctibor, a general practitioner from New Jersey.34 “The mother, or other trusted adult, places her mouth over the child’s open mouth, forming a firm seal as if about to perform mouth-to-mouth resuscitation. While occluding the unaffected nostril with a finger, the adult then blows until feeling resistance caused by closure of the child’s glottis, at which point the adult gives a sharp exhalation to deliver a short puff of air into the child’s mouth. This puff of air passes through the nasopharynx, out through the non-occluded nostril and, if successful, results in the expulsion of the foreign body. The procedure is fully explained to the adult before starting, and the child is told that the parent will give him or her a “big kiss” so that minimal distress is caused to the child. The procedure can be repeated a number of times if not initially successful.”34 Positive Pressure Ventilation with Bag Valve Mask This technique is an approximation of the above mother’s kiss technique – useful for unwilling parents or unsuccessful tries.10,25  The author prefers to position the child sitting up.  A self-inflating bag-mask device is fitted with a very small mask: use an abnormally small mask (otherwise inappropriately small for usual resuscitative bag-mask ventilation) to fit over the mouth only.  Choose an infant mask that will cover the child’s mouth only.  Occlude the opposite nostril with your finger while you form a tight seal with the mask around the mouth. Deliver short, abrupt bursts of ventilation through the mouth – be sure to maintain good seals with the mask and with your finger to the child’s nostril – until the foreign body is expulsed through the affected nostril. Beamsley Blaster (Continuous Positive Pressure) Technique For the very uncooperative child with a nasal foreign body amenable to positive pressure ventilation who fails the mother’s kiss and bag-mask technique, a continuous positive pressure method may be used. Connect one end of suction tubing to the male adaptor (“Christmas tree”) of an air or oxygen source.  Connect the other end of the suction tubing to a male-to-male adaptor (commonly used for chest tube connections or connecting / extending suction tubes).  Insert the end of the device into the child’s unaffected nostril.  The air flow will deliver positive pressure ventilation continuously. With this technique there is a theoretical risk of barotrauma to the lungs or tympanic membranes.  However, there is only one case reported in the literature of periorbital subcutaneous emphysema. To minimize this risk, some authors recommend limiting to a maximum of four attempts using any positive pressure method.10 Nasal speculum Optimize your visualization with a nasal speculum.  The nostrils, luckily, will accommodate a fair amount of distention without damage.  Hold the speculum vertically to avoid pressure on and damage to the vessel-and-nerve-rich nasal septum.  Hold the handle of the speculum in the palm of your hand comfortably and while placing your index finger on the patient’s ala.  This will help to control the speculum and your angle of sight. Your other hand is then free to use a hook or other tool for extraction. Lighting is especially important when using the nasal speculum: a focused procedure light or head lamp is very helpful.  The author keeps a common camping LED headlamp in his bag for easy access. Suction tips / catheters Various commercial and non-commercial suction devices are on the market for removal of foreign bodies.  All connect to wall suction, and vary by style, caliber of suction, and tip end interface.  A commonly available suction catheter is the Frazier suction tip (right), a single-use device used in the operating room. A modification to suction can be made with the Schuknecht foreign body remover (left; not to be confused with the suction catheter of the same name): a plastic cone-shaped tip placed on the end of the suction catheter to increase vacuum surface area and seal.  Laryngoscope and Magill Forceps If a child aspirates and occludes his airway, return to BLS maneuvers (as above).  If the child becomes obtunded, use direct laryngoscopy to visualize the foreign body and remove with the Magill forceps.  Hold the laryngoscope in your left hand as per usual.  Hold the Magill forceps in your right hand – palm side down – to grasp and remove the foreign body.  Take-home Points  Beware the “vacuum palate”: a flat (especially clear plastic) foreign body hiding on the palate Take seriously the complaint of foreign body without obvious evidence on initial inspection – believe that something is in there until proven otherwise Control the environment, address analgesia and anxiolysis, have a back-up plan Motto Like a difficult airway: plan through the steps MERCI – DANKE – Дякую – THANK YOU – GRACIAS –  ありがとう— GRAZIE Appendix A: Prevention At the end of the visit, after some rapport has been established, counsel the caregivers about age-appropriate foods and “child-proofing” the home.  This is a teachable moment – and only you can have this golden opportunity! Age-appropriate foods 0-6 months: breastmilk or formula 6-9 months: introduce solid puree-consistency foods 9-12 months: small minced solids that require no chewing (well cooked, soft, chopped foods) Although molars (required for chewing) erupt around 18 months, toddlers need to develop coordination, awareness to eat hard foods that require considerable chewing. Not until 4 years of age (anything that requires chewing to swallow):             Hot dogs             Nuts and seeds             Chunks of meat or cheese             Whole grapes             Hard or sticky candy             Popcorn             Chunks of peanut butter             Chunks of raw vegetables             Chewing gum Child-proofing the home Refer parents to the helpful multi-lingual site from the American Academy of Pediatrics: http://www.healthychildren.org/English/safety-prevention/at-home/Pages/Childproofing-Your-Home.aspx An abbreviated list: use age-appropriate toys and “test” them out before giving them to your child to verify that there are no small, missing, or loose parts.  Secure medications, lock up cabinets (especially with chemicals), do not store chemicals in food containers, secure the toilet bowl, and unplug appliances. Parents should understand that “watching” their child alone cannot prevent foreign body aspiration: a recent review found that in 84.2% of cases, incidents resulting in an airway foreign body occurred in the presence of an adult.35 Best overall tip: get down on all fours and inspect your living area from the child’s perspective.  It is amazing what you will find when you are least expecting it. Appendix B: The Playbook's ENT Foreign Body Toolkit Although your institution should supply you with what you need to deal with routine problems, we all struggle with having just what we need when we need it.  High-volume disposable items such as cyanoacrylate (Dermabond), curettes, supplies for irrigation, alligator forceps, and the like certainly should be supplied by the institution.  However, some things come in very handy as our back-up tools. NB: we should be cognizant of the fact that tools that must be sterilized or autoclaved are not good candidates for our personal re-usable toolkits. One of the most problematic matters relating to urination is urge incontinence. The rewiring solution involves working out the perineal muscles by doing Kegel exercises. Constipation has many possible treatments. I favor the simplest one using and OTC product, Miralax.
