In this episode we feature 2 articles that explore hot topics in genetics as well as opportunities to improve patient care in honor of DNA Day on April 25. DNA day commemorates the completion of the Human Genome Project and the discovery of DNA's double helix. Both of these studies utilize qualitative methodologies to highlight people's experiences and share their stories. Segment 1: Not Parent Expected” results through direct-to-consumer genetic testing Julia Becker (she/her) is a board-certified genetic counselor and CSU Stanislaus Genetic Counseling Program graduate. She has a strong interest in the ethical, psychological, and social implications of genetic testing, particularly in the context of unexpected parentage discoveries. Julia is the first author of the article, "Experiences of Individuals Receiving ‘Not Parent Expected' Results Through Direct-to-Consumer Genetic Testing," published in the Journal of Genetic Counseling. She presented this research at the American College of Medical Genetics and Genomics (ACMG) Annual Conference in 2021, contributing to the ongoing dialogue on the impact of unexpected genetic findings. Her work focuses on supporting individuals navigating complex genetic discoveries and advancing awareness within the genetic counseling community. In this segment we discuss: - The rise in Not Parent Expected (NPE) discoveries through direct-to-consumer genetic testing and what it means to receive this result. - Key emotional themes from interviews with 25 participants, including identity disruption, grief without death, and shifting family dynamics. - How a background in genetic counseling informed a sensitive and in-depth interview approach. - The emotional motivations behind seeking out biological relatives and the varied outcomes of those efforts. - The need for improved informed consent and follow-up care from DTC companies.   Segment 2: Transgender and gender diverse patients' experiences with pregnancy-related genetics discussions: A qualitative study Jaime Schechner (she/her) works as a neurology genetic counselor at Boston Children's Hospital. She completed her Master of Science in Genetic Counseling at Boston University, and previously worked as a genetic counseling assistant at Beth Israel's Maternal Fetal Medicine Center. Darius  Haghighat (he/him) is a reproductive genetic counselor at Boston Medical Center and an Assistant Professor of Obstetrics and Gynecology at Boston University Chobanian & Avedisian School of Medicine. He has prior experience as a cancer genetic counselor as well. He completed his Master's in Genetic Counseling at Boston University. As a queer genetic counselor he is especially passionate about LGBTQIA+ health equity. In this segment we discuss: - The inspiration behind focusing the study on pregnancy-related genetic counseling experiences among trans and gender diverse (TGD) individuals. - Major gaps in reproductive healthcare for TGD patients, including misgendering, binary language, and lack of provider knowledge. - Participant stories about feeling unseen or misgendered, and discussed the emotional impact of these encounters. - Frustrations with terms like "maternal" and "advanced maternal age," and suggested inclusive alternatives for clinical language. - Moments of affirming care, showing how small gestures can have a powerful impact across the healthcare journey. - The need for systemic change, including inclusive policies, provider education, and future research that centers TGD voices. Les policiers arrivent, et sur place, ils échangent des coups de feu avec ce même homme. L'auteur des coups est blessé par balle, et décèdera dans la soirée.  Le téléphone grave danger, aussi appelé TGD, est un dispositif pour protéger les victimes de violences conjugales ou de viol. Il ne date pas d'hier, le TGD mis en place par le ministère de la Justice, a été expérimenté en Seine-Saint-Denis en 2009, puis a été étendu au reste de la France en 2014. Qu'est-ce que c'est ? The most commonly used equation that determines someone's kidney function includes a binary male-female sex coefficient. But what does this mean for the care of transgender, gender-diverse, and nonbinary people? Cameron Whitley, an associate professor with kidney failure experience, and researchers Keila Turino Miranda, a PhD student, and Dr. David Collister, a nephrologist, shed light on this important topic.   Dr. David Collister (he/him) is a Kidney Doctor, Clinician-Scientist and Assistant Professor at the University of Alberta. He has a PhD in Health Research Methodology from McMaster University. His research program is funded by the Canadian Institutes of Health Research and the Kidney Foundation of Canada and focuses on randomized controlled trials of interventions for uremic symptoms, cannabinoids, the responsiveness of uremic symptoms to the initiation of dialysis, metabolomics, proteomics and the intersection of gender diversity and kidney disease. Keila Turino Miranda (she/her) is a first year PhD Student in the Cardiovascular Health and Autonomic Regulation Laboratory at McGill University. Ms. Turino Miranda's work focuses on understanding and addressing the unique healthcare needs and disparities faced by transgender, gender-diverse, and non-binary (TGD) individuals in the realms of nephrology and cardiology using a patient-oriented approach. Cameron T. Whitley, Ph.D. (he/they) is an Associate Professor in the Department of Sociology at Western Washington University. He studies issues concerning the environment, human-animal relationships, and transgender-affirming medicine. He got involved with studying transgender-affirming medicine when he was diagnosed with kidney failure and received a kidney transplant while writing his dissertation. Through this process, he coauthored one of the first articles addressing the discrepancies in evaluating kidney function for transgender people. In our fourth episode, we delve into the importance of gender-inclusive language in genetic counseling and the specific challenges transgender and gender-diverse (TGD) individuals face in accessing hereditary cancer care. You can find the Journal of Genetic Counseling webpage via onlinelibrary.wiley.com or via the National Society of Genetic Counselors website.    Segment 1 “Use of gender-inclusive language in genetic counseling to optimize patient care”   Heather Motiff graduated with a B.S. in Psychology from the University of Wisconsin-Whitewater in 2006. She has extensive experience working as a crisis response advocate and co-facilitating support groups for survivors of intimate partner violence. Heather discovered her interest in genetic counseling during her first pregnancy in 2010. She has served as a Community Resource Specialist and contributed significantly to gender-affirming care initiatives during her graduate studies at UW-Madison. Heather is now an oncology genetic counselor at SSM Health Cancer Care in Madison, WI, and is dedicated to providing inclusive, quality healthcare and genetic services.   In this segment we discuss: Specific examples and terms used in gender-inclusive language. Comfort levels of genetic counselors with using gender-inclusive language. Findings from thematic analyses on the use of gendered language and its impact on patient care. Suggestions for additional training resources for healthcare professionals. Segment 2 “Experiences of hereditary cancer care among transgender and gender diverse people: “It's gender. It's cancer risk…it's everything”   Sarah Roth is a genetic counselor and a PhD candidate in Anthropology at Johns Hopkins University. She is a BRCA1 carrier whose research focuses on the experiences of patients, communities, and providers in cancer care and genomic medicine. Sarah has been a founding editor of Tendon at JHU's Center for Medical Humanities & Social Medicine, a contributing writer at Synapsis: A Health Humanities Journal, and a recent predoctoral fellow in Bioethics at the National Institutes of Health.   In this segment, we discuss: Challenges faced by TGD individuals in accessing hereditary cancer care. Participants' perspectives on gendered language in healthcare. March 31 is International Transgender Day of Visibility (TDOV), which commemorates the achievements and resilience of transgender and gender-diverse (TGD) people around the world, including our own staff, patients, and supporters.  For Fenway Health and our Trans Health Program, it is also a time to reaffirm our commitment to providing comprehensive, compassionate healthcare and services to all members of the TGD community.  In honor of TDOV, we will be posting two new episodes. In the first one, we interview Steph DeNormand (They/Them), the Manager of the Trans Health Program at Fenway Health and a chapter author for Transgender and Gender Diverse Health Care: The Fenway Guide. Steph's focus throughout their education and academic work has been on queer and trans theory and intersections with feminist pedagogy and medical ethics. They have been working with trans and gender diverse communities their entire professional career, and continue their community and academic work through writing, conferences, and community-driven events. In their position at Fenway Health, Steph has had the privilege of working directly with hundreds of trans and gender-diverse community members in navigating healthcare systems, while simultaneously engaging in the creation and management of educational and support structures for care teams working in the field. Mercredi 7 février 2024 dans la soirée à Noisy-le-Grand, une femme a activé son téléphone grave danger. Son ex-compagnon violent et armé s'est présenté devant chez elle. Les policiers arrivent, et sur place, ils échangent des coups de feu avec ce même homme. L'auteur des coups est blessé par balle, et décèdera dans la soirée.  Le téléphone grave danger, aussi appelé TGD, est un dispositif pour protéger les victimes de violences conjugales ou de viol. Il ne date pas d'hier, le TGD mis en place par le ministère de la Justice, a été expérimenté en Seine-Saint-Denis en 2009, puis a été étendu au reste de la France en 2014. Qu'est-ce que c'est ? The Transcreative Network - An Australian online digital platform designed to foster creativity and connections among TGDNB (Transgender, Gender Diverse, and Non-Binary) individuals in Australia. A secure collective gathering space for TGDNB writers and creators in Melbourne and Regional Victoria; a mycelium of trans/gender/queer creative expression and culture making. They de-compose the offcuts and debris of mainstream culture, synthesising new forms. The network is currently held in place by Avery, Madison, and Alex, disabled settler-colonial trans humans. They currently meet every month via Zoom and physical locations at both Bendigo and Melbourne.In this episode on Queering The Air, Sasja were joined by Madison Moffat and Alex Lee to explore the essence of The Transcreative Network. If there was one thing you think society should talk more about, what would it be?“The conflict that's occurring between gender roles at the moment. As a trans gender male, I have a different perspective on gender to many and see the world through multiple lenses.”________ Today I'm talking with a lovely friend of mine, Andrew Guy who is a transgender male that educates others on Trans issues & advocacy. When I first met Andy, more than 20 years ago, he was known as Anna. We both worked in Tourism and whilst I knew him reasonably well, I had no idea the internal battle he was going through. Andrew shared quite openly his journey of the emotional, physical and often psychological challenges that he faced through his transition. I was in awe of the openness and bravery of someone to share something so personal but it gave me such a deeper understanding of gender dysphoria.Andrew is an actor and presenter and was recently in Still Point Turning at Sydney Theatre Company. Andrew is a script adviser and advocate for trans representation in film, TV and radio and was the first transgender presenter on Channel 10's The Project. Andrew has spoken at many corporate functions on gender identity, diversity and inclusion. Having received two successive nominations for Australia's LGBTI awards, this year Andrew was invited to be a judge. Andy is incredibly approachable and a highly experienced speaker advocate and I was thrilled when he agreed to come chat to me about a topic many of us need to understand more fully. As I was taking an afternoon walk I began to reflect on my time in the Pacific Northwest and realized in two months I will be coming up on 10 years living in Oregon. This is one of the longer shows I have recorded and is basically a series of short episodes I have had in this area since I've been here. In September of 2018 the American Academy of Pediatrics published a policy statement calling for gender-affirming health care for all transgender and gender-diverse (TGD) youth. The policy is aimed at pediatricians trying to promote the health and positive development of young people who identify as TGD. uh-PARENT-ly cohosts Tracy Weiner and Anne Johnsos admit this topic is unchartered territory for them, so they asked two parents of trans and gender-fluid kids to help them learn.
