Red Flags: Look for indicators of spinal infection, spinal bleed, or space-occupying lesions (e.g., tumors, large herniated discs). Assessment: A thorough history and neurological exam (strength testing, gait) is essential. Additional Tools: Use bedside ultrasound for post-void residual assessment in suspected cauda equina syndrome Imaging Guidelines: Routine Imaging: Generally not indicated for young, healthy patients without red flags. ACEP Recommendations: Avoid lumbar X-rays in patients under 50 without risk factors, as they do not change management and may increase costs and ED time. Advanced Imaging: Reserve MRI for patients with red flags, neurological deficits, or suspected cauda equina syndrome; CRP may be a part of your calculus when evaluating for infectious causes of back pain Treatment Options: Evidence-Based First-Line: NSAIDs offer modest benefit.

The importance of patient history and reevaluation after initial treatment. Recognizing the unique presentation of cluster headaches and their management implications. Effective Acute Migraine Treatments: First-line treatments including anti-dopaminergic medications like metoclopramide (Reglan) and prochlorperazine (Compazine), and parenteral NSAIDs like ketorolac (Toradol). The limited role of triptans in the ED due to side effects and less efficacy compared to anti-dopaminergics. The use of nerve blocks (greater occipital nerve block and sphenopalatine ganglion block) as effective treatments without systemic side effects. Treatments to Avoid or Use with Caution: Diphenhydramine (Benadryl): Studies show it does not prevent akathisia from anti-dopaminergics nor improve migraine outcomes. IV Fluids: Routine use is not supported unless the patient shows signs of dehydration. Magnesium: Conflicting evidence with some studies showing no benefit or even harm. Managing Refractory Migraines: Second-line treatments including additional doses of metoclopramide combined with NSAIDs or dihydroergotamine (DHE). Considering opioids as a last resort when other treatments fail.

“We will continue to support Ukraine on its irreversible path to NATO membership”. It was this statement by NATO members at a recent summit in Washington that sent shockwaves through the foreign policy community and elicited strong reactions from both sides of this foreign policy debate. Those in favour argue that bringing Ukraine into NATO would solidify the West's long-term commitment to Ukraine's security, force Putin's hand to end this war, and deter Russia from invading Ukraine in the future. Critics argue that admitting Ukraine to NATO would be a huge mistake. The US and its allies would be signalling a commitment to fight Russia should they invade Ukraine again which would lead to a showdown between the world's two leading nuclear powers. And allowing Ukraine to join the alliance only at the conclusion of this war will motivate Putin continue the war indefinitely, the prospect from which Ukraine will never recover. Arguing in favour of the resolution is John Herbst, a former US ambassador to Ukraine and the Senior Director of the Atlantic Council's Eurasia Center. Arguing against the resolution is Benjamin Friedman, the policy director at Defense Policies, a foreign policy think tank. KEY TAKEAWAYS At graduate school I'd studied nonprofit management and worked at a nonprofit for a few years and found it was very process driven, we kept talking about what to do but never actually trying things. I'm much more results oriented, so I moved into an e-commerce company which doubled every year and it was an amazing ride. I became addicted to the startup journey. A lot of startups have a unique journey, every founder is an individual, but there are some common themes. Successful startups have founders that are clear on who they are and what they're trying to accomplish. That might seem easy, but it's actually very hard work to regularly check in with yourself. The other application is around vision and values; those will remain constant even as the business pivots, evolves, changes and challenges occurring. The final piece is feedback loops, successful founders are constantly seeking advice from advisors, investors, other founders and their team, they're constantly seeking feedback – they may not act on it every time – new ideas and how to get things done. Align outcomes, results and activities. A lot of people get worked up by working really hard and spending a lot of time and effort but activity alone won't generate the outcomes that you're looking for, you have to see what results are coming from that activity and then you have to see if those are affecting the outcomes that you desire. Are you looking at it through industry metrics, internal metrics? You need to be constantly running experiments to see if ‘x' improves your quality, your efficiency for you or your customers. You have to constantly evaluate your process but aligning your activity results and outcomes is really critical. Bringing in fractional experts can do a number of things; they can help you look holistically across the business and make connections between different functions, how do sales, marketing, finance and product all connect and what are the ways to optimise them collaboratively to grow your business. How have things been done in the past and how can they be applied to your business in order to grow as fast as possible? Cost and risk are considerations as well, by looking at fractional resources you're mitigating both; you pay people who come in much less than someone doing the job full-time, especially if you don't know how they're going to work and how they'll fit with your team. In this episode, we focus on Religion and the Rise of Capitalism. We get first into a bit of the early history of economics. We focus a lot on David Hume and Adam Smith, and also in how economic thinking changed after them. We talk about some of the most influential schools of thinking in economics during the 19th and 20th centuries. We discuss the political influence of the evangelical protestants in the US. We also talk about the relationship between capitalism and political ideology, like liberalism and conservatism.
