Today we'll be talking about some puppies and babies rescued from danger, upcoming celebrations as same-sex marriage in Thailand nears its debut, and a little later the assassination of a former Cambodian MP in Bangkok.

In this episode of "Sleepless in Singapore," I recount my first trip through Southeast Asia, covering Cambodia, Laos, Thailand, and Myanmar. The journey begins with a memorable flight to Luang Prabang, Laos, on a small prop plane with a unique laminated boarding pass. The landscapes below are a breathtaking mix of green mountains and the brown flow of the Mekong River. Once in Luang Prabang, I explore the local market, savoring street food, and visit the stunning Luang Prabang waterfalls, capturing their beauty through my camera lens despite some technical challenges. Moving on to Chiang Mai, Thailand, I reunite with Karen, a friend met earlier in Cambodia, and we embark on various adventures, including visiting an elephant sanctuary and making onsen eggs in hot springs. The journey continues to Chiang Rai, with its winding roads and unique charm, before heading to Myanmar. In Myanmar, we delve into the rich cultural tapestry, visiting ancient pagodas in Bagan and experiencing local customs like chewing betel nut. Each destination offers a distinct flavor of Southeast Asia, creating a tapestry of vivid memories and immersive experiences.

Since the generals' takeover in February, hundreds of people have died in Myanmar's cities after mass protests. Last weekend the diversion of a Ryanair flight to Minsk in Belarus – though it was meant to be going to Lithuania – caused generalised outrage. After an alleged bomb threat, the plane had to land straight away. But it seems the real target on board was a young critic of the Belarusian government, James Landale analyses the shock felt across Europe as other countries judge how to respond. Today on The Leaders' Brief - The USA, which remains the worst affected country due to the 2019 coronavirus, but continues to lead one of the most effective vaccination programmes in the world, is seeing a slow but steady dip in the number of weekly infections. The Centers of Disease Control and Prevention revealed last Monday that as of April 24, the seven-day average was 57,123 cases, a dip of almost 15% in comparison to the previous week. Most experts believe that, unlike previous dips, the slow down in the rate of infection will be permanent this time as the USA continues to immunise its population.  Myanmar witnessed its largest violence since February after the military clashed with the Karen National Union at its eastern border with Thailand days after Southeast Asian leaders said they had reached a consensus with the Junta on ending violence. Following reports of gunfire and smoke from a Tatmadaw camp at the Thailand-Myanmar border, the region's ethnic army the KNA said that they had captured the outpost. According to local media, over 240,000 people have been displaced by the conflict and the region has witnessed military airstrikes.  Apple's new software update is likely to hurt smaller firms dependent on digital advertising while providing its users with more privacy protection. The decision to allow iPhone and iPad users to choose how much information they wish to share with any third party application has reignited a tiff between Apple CEO Tim Cook and Facebook CEO Mark Zuckerberg. Anyie Li, MPH is an Epidemiologist and CDC Foundation Field Employee for the Vaccine Introduction Team, in the Immunization Systems Branch, Global Immunization Division, within CDC's Center for Global Health. Anyie is an alumna of the PHI/CDC Global Health Fellowship Program and served as a Fellow from 2016 to 2018 on the same team she currently works on in Atlanta, Georgia. She discusses her first exposure to public health working on the Thailand/Myanmar border with refugees, as well as her current work on the HPV vaccine and COVID-19 response.

The displaced community on the Thailand–Myanmar border has long provided for the basic educational needs of large numbers of children. Providing accredited education, however, remains a struggle.

Xue Li, Ph.D., Shalini Nair, and Tim Anderson, Ph.D. Malaria is worldwide scourge infecting 200 million people around the world and killing more than 400,000 of them. The parasite is carried by the Anopheles mosquito, particularly in tropical areas like Sub-Saharan Africa and Asia. Texas Biomed scientists are “particularly interested in the evolution of drug resistance,” said Scientist Tim Anderson, Ph.D., adding that it is “a recurring problem in controlling tropical diseases.” Artemisinin is a recently discovered drug that is the gold standard for treating malaria, considered instrumental in reducing the number of cases of the infectious disease over the last decade. However, more than 125 mutation variations of drug resistance have emerged in Southeast Asia. Those mutations impact the metabolism of the parasite itself – inhibiting growth rate, for instance. A recent NIH-funded study published in the journal Antimicrobial Agents and Chemotherapy by Anderson and his team looked at one particular drug resistant variant sequence (called an allele) that is outpacing the others. Anderson’s team wanted to test the idea that successful resistance alleles have fewer adverse consequences for the parasites and, thus, thrived and spread. Shalini Nair works with malaria parasites in Dr. Tim Anderson's lab. Senior Research Associate Shalini Nair, who has worked at Texas Biomed for 18 years, was a key part of this project. “I started out by figuring out how to use CRISPR-Cas9 to edit key mutations in the malaria parasite,” Nair explained. CRISPR is short for clustered regularly interspaced short palindromic repeats. It’s a system for genome editing, using the cell’s own DNA repair mechanism to add or delete pieces of genetic material. To maximize the relevance of their results, the team used “a recently isolated parasite clone from the Thailand-Myanmar border for manipulation rather than a laboratory adapted parasite clone,” the journal article stated. Malaria parasites grow in flasks of red blood cells. In a head-to-head comparison between two mutants – one strong and thriving in patients and one not as strong and waning – researchers grew the parasites in flasks of blood cells in the lab. Scientists measure how the mutations impacted the “fitness” of the parasites. In other words, they checked to see if the mutations impacted the metabolism of the more common and less common strains in the expected way. Scientists hypothesized the more common and thriving mutations would have less “fitness cost” to the parasite. The cultures were maintained for 60 days. Then they extracted DNA and amplified it, comparing frequencies of the different mutations. Xue Li, Ph.D., is a post-doctoral student who compiled the data from the study. “Our results were not the same as our hypothesis,” Li stated. The scientists want to see if there are other mutations in different areas of the genome impacting whether these drug-resistant parasites flourish or fail. The answer may be more complicated than first thought. Dr. Anderson emphasized that malaria researchers like his team need to understand the process by which parasites adapt to anti-malarials.” If we understand that,” he said, “we are in a far better position to then to develop more effective, evolution proof interventions.”

Some thoughts and reflections on my resent finished teaching trip to Thailand & Myanmar

Filmed in April 2013 at the Shoklo Malaria Research Unit (SMRU) in Mae Sot, and at the Wang Pa Free Clinic and the Mae La refugee camp, this video highlights SMRU's work among Karen and Myanmar refugees, living along the Thailand-Myanmar border. Dr Francois Nosten, SMRU founder and head, discusses SMRU's work, its objectives and challenges. Dr Nosten warns about the growing public health threat posed by parasite resistance to artemisinin-based combination therapies (ACT); the cornerstone of malaria treatment in all malaria-endemic countries.

During the last two decades human infections with Plasmodium knowlesi are increasingly diagnosed in South East Asia and have also been reported in travellers. A severe case of imported P. knowlesi infection in a 73-year old German is presented, who had been travelling through Myanmar and Thailand for three weeks. Microscopy showed a parasitaemia of 3% and different parasite stages including band-forms resembling Plasmodium malariae. Due to the clinical picture of severe malaria and the microscopical aspect (combination of parasites resembling P. malariae and Plasmodium falciparum), P. knowlesi was suspected. The patient was treated with intravenous quinine; he was put on mechanical ventilation and catecholamines due to cardiorespiratory failure. Parasitaemia was cleared rapidly but renal function deteriorated resulting in intermittent haemodialysis. The patient was hospitalized for six weeks but he recovered completely without any physical sequelae. Plasmodium knowlesi mono-infection was confirmed by molecular methods later on. Plasmodium knowlesi infection has to be taken into account in feverish travellers returning from Thailand/Myanmar. Moreover this species can cause life-threatening or even lethal complications. Accordingly severe P. knowlesi infection should be treated like severe P. falciparum infections.
