Mark DeJong - Off Grid MedicDeJong retired Nationally-Registered Paramedic and founder of Off Grid Medic LLC, has over 30 years of experience in treating victims of trauma and sudden illness. Based out of South Carolina, Mark has a degree in Emergency Medical Services and holds an Advanced Wilderness EMT certification. As part of a military assignment, he spent two years providing remote medical care to the indigenous people of the Philippines, where his passions for medicine and primitive living fused together. He serves on the Advisory Council at the Appalachian Center for Wilderness Medicine, and has trained alongside elite members of the US Army and the US Marshals Service. Mark has served in several roles including field Paramedic, Field Training Officer, Director of Emergency Medical Services for a rural government EMS system, faculty at the Technical College System of Georgia, and Deputy Director of Emergency Management for a government agency. He considers himself a lifelong learner and, when not immersed in medical material, Mark continues to hone his outdoor skills. He is a proud graduate of the Pathfinder Advanced program and an original member of the Flint and Steel Primitive Skills Group. Mark has appeared on The Weather Channel, where he was featured on the series Uncharted Adventure. She is also the Director of the PosSUM Space Medicine Group and Chief Instructor of the PoSSUM Operational Space Medicine Group. Shawna holds degrees in neuroscience (BSc Hons. Neuroscience, University of Alberta), space (MSc Space Studies, International Space University), entrepreneurship (Graduate Studies Program, Singularity University) and medicine (MD, University of Alberta), and is currently completing a fellowship in Wilderness Medicine (Academy of Wilderness Medicine).Today, women constitute approximately 12% of all astronauts. Dr. Patrick Fink joins us on Episode 11 of The High Route Podcast. Fink is an ER physician in Bend, Oregon, involved with the local and broader Wilderness Medicine community. He hosts Wilderness Medicine Updates—"a podcast for those interested in wilderness medicine, search and rescue, disaster and austere medicine, and ski patrol medicine." Fink also has a site titled Mountain Lessons, where he shares valuable information about his travels and best practices. All this is to say that Fink comes to the podcasts with some expertise. One aspect of backcountry travel he has thought a lot about is avalanche airbags. Fink presented some research at the 2023 ISSW, which got us talking about airbags, airbag adoption, and efficacy. A solid podcast on Avalanche Airbags comes from the Utah Avalanche Center. On this month's episode, we chat with Matt Smith about wilderness medicine.  As we shift from a high resource to a low resource setting, there are 3 main concepts to consider: - Ideal to real - High risk to low risk - Stable to unstable These concepts are important as we shift our practice and prepare ourselves.  For this patient context, we need to focus on the things we can't replace, and learn to manage that. Our approach to these patients need to be algorithmic, and focus on doing the basics well. Two mnemonics (H-E-M-P; A-B-C-D-E) are presented to help with our approach. Lastly, hypothermia assessment and management is discussed.

ABOUT OUR GUEST: Katie Wightman worked for the Sequoia Parks Conservancy in Sequoia + Kings Canyon National Parks for 11 years and loved every minute. She had the privilege to work underground in Crystal Cave, organize the Dark Sky Festival, organize Science and Nature Camps for youth in the Central Valley, the backcountry Pear Lake Winter Ski Hut, and also Directed the Education Program for the SPC where she worked to provide as many opportunities as possible for people to connect with their public lands. Currently she is working on her Bachelor's of science in Nursing and is hoping to work in Emergency and Critical Care as well as continue in Wilderness Medicine. He practiced emergency medicine for thirty-five years and was a freelance medical writer for many of those years. His articles appeared in many outdoor magazines, and his books, The Ragged Mountain Press Guide to Wilderness Medicine & First Aid and The Onboard Medical Handbook were published by McGraw-Hill. He retired from medical practice in 2013, and now spends his time building boats and furniture, sailing, and hiking in the United States and Europe with his wife, Mary.

He serves as the Special Assistant to the USUHS President for Operational Medicine.   In this episode, he discusses his pathway to becoming a certified nurse anesthetist, a remarkable story of determination.  He followed this with deployments to Iraq and Afghanistan in the surges of combat operations. He discusses how he applied that experience to training student nurse anesthetists preparing to deliver anesthetic medicine into austere military environments.  He also discusses his expertise in diving and dive medicine. LTC(R) Welder developed the operational readiness programs of study for Dive Medicine, Military Mountain Medicine Course, Cold Weather Medicine, and Avalanche 1 at the Uniformed Services University in 2016. He is a certified Divemaster and a Fellow of the Academy of Wilderness Medicine. He received the Diploma in Mountain Medicine and Dive and Marine Medicine diploma and was inducted into the prestigious Explorers Club in 2018.    During his military career, he served as the lead subject matter expert in developing and training the Expeditionary Resuscitative Surgical Team – Africa for Army and Navy medicine. Created by Mari Feher, Hosted by Ania WiktoroviczIn July of 2022 Jaden Stewart of  Gallatin County Sheriff SAR in Montana, Karel Kaiser and their partner set out to summit Mt Rainier in the state of Washington via the Ptarmigan Ridge route. As they started out from their high camp in 30+ miles /hour wind, they noticed a lone headlamp from the party ahead of them giving off SOS signals. At this point, the team knew there would be no summit bid in their day as the terrain became increasingly dire with truck size boulders coming loose as they happened  upon  a climber with a crushed leg. The saga of their 24 plus hour rescue complete with a Chinook and aided by their Inreach and medical training is fascinating to say the least.

Being at home with the kids all day gave Toki time to think. He knew he needed to practice his skills and wanted to help others. Late one night he decided to create the joint dislocation simulator. Toki, tell us about the first time you reduced a dislocation? Back in 2000, we were playing an hour away from San Antonio There was no ortho available for the game. Dr. Bud Curtis said, “Toki, you have to do the reduction.” Tell us your AT story 1992 I came to the US from Japan. My father had business in Houston so we decided on Texas and since there were almost no Japanese people in San Antonio I knew I would have to learn English with total emersion. Spent one year at the University of the Incarnate Word learning English Brackenridge High School in 2000 After 14 years I needed a change Stayed home with kids Discuss the origin of your company One night I started dreaming and then began collecting materials. CAATE added 3 major standards including Reduction Techniques I built it for ATs, but I am now getting requests from Emergency Medicine, Wilderness Medicine and a DOD contract. When you were discussing Dr. Approved, tell us more about that. Dr. Curtis was my team doctor and we worked with him through the whole process. We also connected with Dr. Green, a local hand doctor , and had him test it out and give feedback. How can Jay help you today? Jay and I are actually going to be at NATA together sharing a vendor space. If you are no longer in school (an ATEP) how can we get this practice? I am hosting workshops across the country. If you can pay for the travel expenses and the best sandwiches in town, I will be there. What are you using for materials? It started with an old anatomy teacher donating a broken down skeleton and me using orthoplast to replicate the bones and joints. Do you feel guilty searching for new providers? I do not have a problem with switching if the company can do the same thing for cheaper and better. In the life balance you are creating, where is your line? I do not have a lot of expectations of someone giving back. I do a lot of things for others and do not expect anything back. Toki, talk about Shark Tank. I applied and presented to the first round. If I was selected I would fly out to California, but I was not. For me, I am glad I did not give up money and take on a loan.  What is the feedback you are getting? My first workshop was in San Antonio ISD with former colleagues that had never reduced a dislocation before. A lot of clinical programs do not have someone to teach the skills even though they bought the devices. The world of emergency medicine is commonly known for its 2 AM chest pain visits, motor vehicle accidents and trauma admissions, and foreign objects that require removal from different orifices. However, there is actually so much more specialization within this specialty in itself! Through additional training from 1 to 2 years of fellowship, one can sub-specialize in realms that expand even beyond the four corners of the emergency room, such as Critical Care, EMS/Disaster, Wilderness Medicine, Pediatrics, Hyperbaric Medicine, Medical Toxicology, and even Ultrasound! The last is true for our episode guest today, Dr. Siri ("Hey Siri MD") Chamarti, an Emergency Medicine physician who is near-ending her Ultrasound fellowship!Receiving her bachelor's degree in Neuroscience from UCLA in 2013, attaining her medical degree from the Medical College of Georgia at August University in 2017, and having completed her emergency medicine residency at Maimonides Hospital in 2020, Dr. Siri now stands as a New York doctor who promotes evidenced-based education on her social media space by recalling her gruesome experiences as the first line of defense during the spike of the pandemic during the City's epicenter during Spring 2020.From her advice for pre-medical to juggling enjoyment with medical school to moving across the country to pursue medicine to finally becoming an attending doctor now teaching the next generation of physicians and to her love for good food and traveling, Dr. Siri is a testament to how much of life can truly be a fun adventure even as a doctor.
