In this episode, I had the pleasure of speaking to Dave Marshall, who recently retired after serving over 30 years in policing. Dave is the author of the book 'THE FALL OF POLICING: When Woke Cancelled The British Bobby' where he discusses the decline of British policing. According to Dave, over time, the British police have shifted their focus from keeping the law-abiding public safe to politically correct initiatives, resulting in a less safe society. Before Álvarez in May, the Mets hadn't had a two-out, game-tying homer in the ninth inning of a game since Michael Conforto took Kyle Barraclough deep on September 13, 2018, a blast followed immediately by Todd Frazier's game-winning dinger.Álvarez last night became the first Met to hit a game-tying homer with two outs in the ninth inning on the road since Curtis Granderson off Matt Albers in Washington on July 3, 2017 — a game the Mets then lost in the bottom of the ninth on a Ryan Raburn RBI single.Last time the Mets won a game on the road after one of these special homers? That was September 13, 2015, in Atlanta, when Daniel Murphy hit a three-run shot off Ryan Kelly and the Mets added three in the 10th for a 10-7 shocker.All by himself, Álvarez has done this twice this year. The Mets as a team haven't had a pair of down-to-their-last-regulation-out tying homers since Scott Hairston and Lucas Duda in 2011.That tied the team record for occurrences in one season. The Mets also enjoyed such clutch hitting twice in 1970 (Dave Marshall and Donn Clendenon), 1988 (Howard Johnson and Darryl Strawberry), 1997 (Todd Hundley and Carl Everett), 2000 (Derek Bell and Bubba Trammell), and 2004 (Mike Piazza and Victor Diaz).Individually, Álvarez is now tied for the team career record with Hundley, Marshall, and Piazza. Marshall was not another catcher, but an outfielder whom the Mets acquired after the 1969 season from the Giants, along with Ray Sadecki, for Jim Gosger and Bob Heise. The Mets traded Marshall, who hit a total of 13 home runs for them in 264 games, to the Padres before the 1973 season for Al Severinsen. The Mets assigned Severinsen to Triple-A, but he never pitched professionally again. This is a public episode. Shout out to Dr. Dave Marshall whose grand round presentation this was based on.  Key Points:  Try not to judge your patient based on the results of their drug screen  Drug screens are used a lot; they are not always accurate  Based on one study there was no significant change in outcome, treatment, disposition or psychiatric course when a drug screen was obtained  Specific good uses for drug screens include new onset of psychiatric symptoms  Educational Pearls:  1 in 8 ED visits are due to psychiatric complaints which is up 44% since mid 2000’s  63% of new psychiatric complaints had a medical cause  Physicians use drug screens commonly in the Emergency Department, but often do not understand their limitations and are often not using them in the most clinically effective manner  Urine is by far the most commonly used in the Emergency Department  UA drug screens are notorious for having false positives and false negatives  Length of clearance from the urine:  Alcohol 12 hours  Methamphetamines/Amphetamines 48 hours  Benzodiazepines 3 days  Opiates 2-3 days  Cocaine 2-4 days  Marijuana dependent on use: 3- 15 days  Phencyclidine (PCP) 8 days  Urine drug screens have a false positive rate of 5-10 % and a false negative rate of 10-15%  False Positive; urine drug screen positive, but drug is not in their body  False positives for amphetamines: bupropion, trazodone, metformin, promethazine, pseudoephedrine, phentermine and atomoxetine  False positives for benzodiazepines: sertraline False positives for cannabis: ibuprofen, naproxen, proton pump inhibitors  False positives for opiates: poppy seeds, antibiotics, Benadryl  False positives for PCP: venlafaxine, ibuprofen and Ambien  False positives for cocaine: amoxicillin  False Negative; urine drug screen negative, but drug is in the body  Benzodiazepines: such as alprazolam, clonazepam, lorazepam or triazolam Cannabinoids: CBD, synthetic marijuana such as K2 Opiates: oxycodone, fentanyl, methadone and tramadol  If you get a test that you don’t think is accurate you can always send off for a confirmatory test References Summarized by Emily Mack OMSIII | Edited by Mason Tuttle The Emergency Medical Minute is excited to announce that we are now offering AMA PRA Category 1 credits™ via online course modules. Der gebürtige Brite ist geboren und aufgewachsen in der Nähe von Leicester und lebt seit nunmehr fast 20 Jahren in der bayerischen Landeshauptstadt München. Inspiriert von einem Mixtape seines Cousins von der Loveparade 1997, kaufte er sich bereits im Alter von 14 Jahren seine ersten Plattenspieler, um sich fortan mit voller Hingabe seiner großen Leidenschaft dem Djing hinzugeben. Wer sich einmal mit ihm und seinem Sound beschäftigt hat, der wird ziemlich schnell feststellen können, dass es sich bei ihm um einen überzeugten und leidenschaftlichen Musikliebhaber handelt. Dazu kommt noch eine weitere sehr besondere Gabe, er verfügt über ein hervorragendes Näschen für die richtigen Sounds zum richtigen Augenblick. So verwundert es auch nicht, dass er sich in seiner aktuellen Homebase München in der Vergangenheit mit Gigs in den angesagtesten Clubs der Stadt, wie der Rote Sonne, dem Bahnwärter Thiel oder dem Harry Klein in die Herzen des Partyvolks gespielt hat.
