About 5.4 million Americans live with some form of paralysis. Sometimes that's just a temporary loss of mobility, but for the Americans whose paralysis is caused by a spinal cord injury, that loss of movement is often permanent, as there's no biological way to heal an injured spinal cord. There are efforts to see if technology might be able to help these individuals regain use of their limbs, and that's the focus of this episode of Stats+Stories with guest Dr. David Friedenberg. Dr. Friedenberg is a Principal Data Science and Neurotechnology and the Team Lead for Machine Learning/AI in the Health Analytics group at Battelle. He's the Principal Investigator on several neurotechnology efforts developing new AI-powered technologies to help improve the lives of people living with motor impairments due to neurological injuries like spinal cord injuries and stroke. An experienced data scientist with consulting experience across several disciplines he is passionate about developing AI/ML-driven solutions to challenging problems for the betterment of humanity.

In this second episode of a mini-series based on the 2022 Employee Health Trends report, we talk with co-author Janet Young, M.D., Lead Clinical Scientist at Springbuk, about the impacts COVID-19 has had on employee healthcare and employer benefit plans. Armed with data from more than 30,000 COVID patients, Janet and her fellow data analysts split the research into individuals who had COVID into hospitalized and non-hospitalized to get a better understanding of what was happening within each group.Their findings were stunning: during a six-month period after an initial COVID diagnosis, hospitalized patients cost about 70% more than the researchers expected. Even non-hospitalized patients saw an increase of about 25%.What's this mean for employee benefit plans? Trying to capture the value open source software generates can be a bit chaotic. The CHAOSS project may lend a helping hand. By now, the tension between commercial interests and open source projects is well known.  Trying to balance building a sustainable business and a community around open source software, in a cloud-first world is not the easiest thing in the world. In the latest episodes of a long-winding saga, two more commercial open source vendors, Elastic and Grafana, changed their licenses. Their rationale was clearly communicated as trying to protect the business they have built around the respective open source projects from cloud vendors that they feel compete unfairly with them, without contributing as much as they do. Interestingly, all parties involved refer to "the community" as being front and center in what they do. While obviously important, however, what constitutes an open source community, how it's faring, and what value it generates all seem rather vaguely defined. The people working on the CHAOSS project under the auspices of the Linux Foundation want to change that. We caught up with Georg J.P. Link, CHAOSS project co-founder, to find out more. Guest Dale Sanders, CTO of Health Catalyst, joins hosts Gary Austin and Ken Kleinberg to discuss the future of interoperability and health analytics and how healthcare can be positively impacted.  Gary begins the discussion with patient identification. He asks Dale why patients do not yet have a single healthcare identifier for their healthcare records. What is going to change this situation? Dale explains that the answer is a combination of a voluntary system for those in the commercial healthcare space combined with a mandatory system for those that are benefitting from Medicaid and Medicare. We need to commercialize the management of those patient identifiers, just like we've done with internet domains. Ken states that TEFCAis looking at an infrastructure for a nation-wide health information exchange with a lot of these interoperability initiatives. He thinks we could do the same with patient identifiers if we could agree upon a dozen or so fields to try to do a better job capturing core data that could aid with patient identification.Gary asks Dale why we can't have data normalized across the board. Why is this such a difficult process? Dale explains that it boils down to economics. He suspects we've normalized enough to enable reimbursements but that's it. If you think about it, does it matter to a single healthcare system to normalize to a national or international standard? Dale explains that it is probably not that valuable for individual institutions. They are normalizing to their own vocabulary, which works for them. If we're interested in analyzing data beyond the boundaries of healthcare systems – as we need nationally for public and population health – we need to incentivize or mandate broader normalization of vocabulary and go way beyond LOINC codes.  Gary asks Dale if he sees the CDC mandating some of this for public health purposes post-COVID. Dale strongly believes it is time to mandate normalization. Ken notes that lab mapping is strikingly ineffective. There are physicians ordering tests by names that they know, but they don't really know what tests they are actually asking for because what's behind the scenes isn't visible to them. There is no convention for how those things are named, which can be a problematic situation.Gary asks Dale why there is not a standardized vocabulary. Why can't the power of the computer be used to translate all of this in a normalized fashion? Is the power just not there? Dale points out that human language alone is a very difficult thing to make sense of, especially the English language. If you layer on the complexities of clinical vocabulary, then it gets even more complex to turn that into computable, discrete elements. We can progress toward passive dictation, but there will always be a need for humans to make edits.  The discussion moved on to cover combinatorial data. Gary asks “How do you look at pulling this data together and rationalizing it across the two massive domains (administrative and clinical data) that are really driving healthcare in this country?” Dale explains the Health Catalyst data model. He says what you would see are domain and vocabulary-oriented data models that sit in between late binding and enterprise data modeling. There is this middle ground of curated data that is a manageable thing to keep up with and execute.Gary asks Ken who he thinks is going to win by pulling all this payer and clinical data together. Is it the analytics companies, the EMR companies, big tech or who? Ken says there is this concept of a converged platform between payers and providers. Registries are an example of how pulling information together that a lot of people can use. The EHR vendors have taken a run at this, the analytics companies have taken a run at this. We've got the population health management vendors, some of which have been more on the business side of value based care, but there are also some that are on the tech side. It's frankly a huge opportunity. The payers don't have the same reliance on vendors that the provider side has. We have a very defined market of electronic health records vendors for providers, but how do you identify who the vendors are for payers? They are, in a way, a fortress to penetrate. I think Health Catalyst has a huge opportunity to sit in the middle and bring these two worlds together. Dale suggests that in the future, payers will need to become providers and providers need to become payers. The payers are moving toward the provider space at warp speed, the providers are moving into the payer space at a snail's pace. Providers need to be capable of working in the claims space, risk management and risk prediction space better than they are right now. The payers have to do the same working in the other direction in the clinical space. Are the recent CMS ONC regulations around FHIR APIs going to make a real difference for healthcare moving forward? Dale believes the regulations are going to be transformative. However, for FHIR to be successful, he thinks we're going to have to see more transformative changes in the EMR vendors. He'd like to see some aggressive re-architecting with the major software vendors to see if we can't move that along faster. Gary asks Ken to give a profile on consumer applications. Where are these things going? Ken thinks there are a couple of intersecting categories to consider. We have those apps that come under more FDA certification or approvals that are more medically oriented, and then others that will be more generally available that didn't have to go through all those approvals. There is another category of apps associated very specifically with, for example, EHR vendors, health systems or payers. Then, there are other apps that are more independent. The interesting competitive angle here is that those independent apps that can potentially tap into provider systems and payer systems can put a picture together that any one provider or payer organization can't equal. So it becomes more of a cross industry view and we're talking about a longitudinal view and it's usually longitudinal to the provider and the payer, but what it used to be was longitudinal to the patient. Whether those independent apps will succeed or not because they have a battle with the large players in their space. What CMS and ONC have delivered here is in fact that competitive environment far beyond the competition we saw in the past. Gary asks Dale if he sees Health Catalyst getting into the consumer applications space. Dale says the data operating system they build has three missions of data, including analytics, workflow application development and interoperability. They can build applications on top of that data operating system. We have a care management platform that we call a data first application so it looks like an analytics engine but it is really about the workflow of care management. We'll eventually put some patient-facing app on that as soon as the market calms down. That touchstone repository that I mentioned earlier is the national repository that I would eventually like to expose to the public so you, I and ken can query that national repository to other patients like me to see other patients like me where they are being treated, how they are being treated and eventually allow them to socially interact with other patients who are like them. We have to create social interactions between patients like all of us that's what will keep people coming back. Dale gives a primer about healthcare from a battlefield perspective. In the nuclear decision space, a lot of time is spent on subjective and objective data fusion as well as its reliability. The military has formalized decision making and the data required to support decision making. There's a concept known as intelligent preparation for the battlefield, which can be overlapped conceptually onto healthcare. We can build the digital battlefield of healthcare that would support population health, public health, ambulatory care, the framework applies.What is the biggest change we are going to see in healthcare next year? Dale says we will see a relaxation in some of the rules for FDA clinical trials. Additionally, he would like to see an increased emphasis on what we need to do to make quality measures more effective, less burdensome for clinicians.

Nya företag är bäst på hälsa Sandra menar att det har skett en förändring, framförallt bland nystartade företag och inom techbranschen, där man ser värdet av att jobba med organisationen och hälsan. Hon jämför med hur utvecklingsteam jobbar agilt i så kallade sprintar, korta och väl definierade utvecklingssteg. I sådana miljöer är människor vana att testa sig fram och våga prova väldigt konkreta saker inom väldefinierade ramar. Efter det är det dags för utvärdering och beslut om vad teamet ska fortsätta med, sluta med och börja med. I traditionella företag, däremot, tycker Sandra ofta att det är mer trögrörligt. Empowerment skyddar mot utmattning Det svåra med hälsoarbete är också att det finns så mycket man skulle kunna göra. Sandra tycker att det är viktigt att skapa förutsättningar för en kreativ organisation genom att arbeta med det som kallas empowerment: att ge människor inflytande över sin egen situation. Det är dessutom något som skyddar oss mot utmattning. Ett konkret exempel på empowerment kan vara gränsöverskridande lärande där grupper och individer som inte jobbar ihop till vardags får lära sig nytt och dela erfarenheter tillsammans. Att våga testa nytt kräver en kultur som tillåter det och tillit till varandra. Att söka och stärka det friska Wellbefys eget verktyg för att ta tempen på organisationens hälsa, Health Analytics, bygger på KASAM (känsla av sammanhang). Det är en modell som söker och stärker det som håller oss friska - till skillnad från reaktivt arbete med att läka det som redan är sjukt. Med t ex KASAM som grund blir det lättare att börja jobba hälsofrämjande. Alla organisationer och grupper har styrkor och det är ofta bra att börja med att ringa in och lyfta fram dem. Health Analytics hjälper organisationer att hålla koll på en mängd nyckeltal som engagemang, ledarskap, medarbetarskap och personlig utveckling. Meningen är både att medarbetare ska kunna bli delaktiga i organisationshälsan med korta, återkommande mätningar, och att ledare ska få stöd för dialog kring sånt som feedback. Inled en dialog om hälsa - en enkel övning Men man måste inte alltid köpa in saker för att börja jobba med hälsan i en organisation. I många fall finns varken budget eller förståelse för frågan. Då kan det vara klokt att börja i det lilla och visa resten av organisationen vilket värde det ger. Sandras tips för dig som är ledare och vill börja göra något i liten skala är att helt enkelt bara starta en dialog om hälsan på jobbet. Bjud in till samtal där medarbetare själva får skatta hur de har det på jobbet enligt, till exempel med verktyget Menti. Det bygger på att alla skriver in sånt de själva tänker och tycker via sin telefon och sen visas det för alla - men anonymt. Här är ett förslag från Sandra på hur man kan lägga upp ett sånt samtal: Be deltagarna i gruppen göra en skattning av hur de mår på jobbet, kanske på en skala 1-10. Låt dem sedan skriva ner: Vad de tycker att de själva borde göra för att stärka det som är bra och förbättra det som inte är bra. Vad de skulle önska att kollegorna gjorde. Vad de skulle önska att organisationen gjorde. När dialogen är igång i en förtroendefull miljö har ni redan kommit långt!

Att få förtroende och frihet på jobbet mår vi bra av. Men det är inte alltid så lätt för ledarna att släppa kontrollen. Göran Nilsson är ekonomie doktor och universitetslektor vid Uppsala universitet. Han har studerat flera stora organisationer som tillämpat tillitsbaserad styrning och tillsammans med Lennart Francke har han gett ut boken Det agila företaget. Där jämför de företag med fiskstim och supertankers. De förra är flexibla och klarar sig bättre i dagens föränderliga klimat medan supertankers är för långsamma för att kunna anpassa sig och överleva. För att uppnå flexibilitet och snabbrörlighet krävs en mindre hierarkisk organisation där medarbetare tar större ansvar. Tillit eller kontroll Han beskriver tillitsstyrda organisationer som en motsats till granskningsstyrda. I verkligheten är det ett spektrum där många befinner sig någonstans mellan extremerna. Kontroll är ofta sprunget ur en rädsla för att saker och ting ska gå fel, en naturlig instinkt som ofta leder till kontraproduktiva resultat. Det blir lätt att man mäter det som går att mätas men inte det som faktiskt leder till produktivitet. Medarbetare gör det som ser bäst ut, inte det som är bäst. Vill människor ta ansvar? Om man som ledare är modig och ger tillit, kan man förutsätta att medarbetare långt ut i organisationen kan och vill ta ansvaret? Det kan man förstås inte veta säkert säger Göran Nilsson. Som doktorand på Handelshögskolan studerade han ett flertal svenska industriföretag. Där och då trodde många i ledningsgrupperna att medarbetarna inte ville ha ett sådant ansvar. Men det fanns undantag, som Atlas Copco Tools i Tierp. Där noterade cheferna att många medarbetare hade ganska kvalificerade aktiviteter med stort ansvar på fritiden. Deras slutsats blev att det var fel på organisationen som inte tog tillvara på medarbetarnas fulla kompetens. Det ledde till en förändring till mer tillitsbaserad styrning, något som gynnade både företaget och dem som jobbade där. Personligheter och kultur För att bygga en organisation där man vågar lita på medarbetare krävs det ledare som är intresserade av medarbetarna. Ledarskap är i många avseenden individuellt och därför spelar det stor roll vilken typ av personlighet man utser till chefer i en organisation, något som påverkar hela företagskulturen. Göran Nilsson citerar Handelsbankens förre vd Jan Wallander som beskrev decentralisering som ett gummiband man ständigt måste hålla i båda ändarna för att behålla det sträckt. Släpper man taget återgår tillståndet till det "normala": en hierarkisk organisation med större tyngdpunkt på kontroll. Naturkraften i organisationen är att centralisera och detaljplanera och det med goda skäl: att få kontroll över verksamheten. Men ofta leder det till alltså negativa konsekvenser. Förändra - och håll i förändringen Förändring gör ont, brukar man säga och nya arbetsätt väcker ofta visst initialt motstånd. På Atlas Copco Tools i Tierp var det en ny chef som lyckades genomföra positiva förändringar. När medarbetarna insåg att de hade mycket att vinna på den nya ordningen eftersom de fick större möjligheter och inflytande började processen få egen kraft. När väl förändringen är genomförd och har fått momentum gäller det att hålla det nya arbetssättet levande i vardagen genom hela kedjan. Det är värt att se upp för sånt som skickar motstridiga signaler, som till exempel belöningssystem som belönar kortsiktighet. Om den tillitsbaserade styrningen är personberoende är det lätt att den försvinner ut genom dörrarna om vissa individer slutar. Då blir det viktigt med genomtänkta överlämningar. Intern chefsrekrytering kan vara en lösning, som man till exempel gjort i Handelsbanken. Göran Nilsson nämner flera andra företag som lyckats med tillitsbaserad styrning. Företagsgruppen Virgin med kände Richard Branson är ett sådant exempel. Där har man låtit grundarens entreprenöriella syn genomsyra ledningen och de enskilda bolagen har stor egen frihet, något som har visat sig ge stora framgångar. Andra exempel på företag som lyckats med tillitsbaserad styrning är Semco och svenska Handelsbanken. Du kan läsa mer om dem i den här artikeln av Göran Nilsson i Svensk Företagsekonomisk tidskrift. Fyra fokusområden för tillitsbaserad styrning Sätt tydliga långsiktiga mål orienterade mot kunder och andra intressenter som medarbetare och samhälle. Planera för det långsiktigt viktiga och fastna inte i detaljer. Överdriven detaljplanering som aldrig stämmer får negativa effekter och stjäl fokus från det som är långsiktigt viktigt. Jobba med anpassningsförmågan och se organisationen som ett fiskstim istället för en supertanker. Var sparsmakad med granskning och kontroll. Sikta mot det högre syftet och se till att alla medarbetare ser och förstår det, då vill de flesta bidra. Medarbetare vill bli sedda men inte synade. Gynna människors inre motivation. För mycket fokus på yttre individuella prestationer som kortsiktiga bonusar kan sänka den inre motivationen. Bygg belöningsmodeller efter de beteenden du vill se, gärna sådana som bidrar till de långsiktiga målen. En skada tvingade henne att avbryta extremsportkarriären. Genom att hitta sin egen inre kompass kom Malin Rapp vidare i livet - och nu hjälper hon andra att göra samma sak. Det kallas självledarskap. I korta drag betyder självledarskap att känna sina värderingar och att kunna använda dem som kompass. I sitt arbetsliv såväl som sitt övriga liv. Eftersom verkligheten är såpass snabbrörlig och osäker idag blir det allt viktigare att själv kunna hitta sina drivkrafter och följa dem. Dina värderingar - inte företagets - skapar engagemanget Den som har koll på sina egna värderingar är mer engagerad på jobbet - medan företagets värderingar inte spelar lika stor roll för engagemanget. Däremot behöver våra värderingar stämma med företagets för långsiktig framgång. Allt börjar alltså med oss själva. Det berättar Malin Rapp som arbetar med de här frågorna på organisationen Selfleaders. Hon var tidigare professionell kitesurfare men en skada gjorde att hon tvingades avbryta tävlingskarriären i förtid. Man kan säga att hon fick en oväntad duvning  i självledarskap av livet självt. Hon valde att istället läsa kognitiv neurovetenskap och positiv psykologi för att kunna jobba med ett av sina största intressen: människors motivation. Allt självförverkligande blir en press I Sverige är vi väldigt sekulariserade och individualiserade. Det kan man se på den så kallade Kulturkartan från World Value Survey som visar att vi ligger i topp när man kombinerar parametrarna sekularisering och självförverkligande. Men det här gör oss inte nödvändigtvis så lyckliga. Enligt Malin Rapp klassificeras vår situation i Sverige som "kollektiv beslutsångest". Alla valmöjligheter kan kännas som en press att prestera och vara lycklig. Börja fråga om drivkrafter Ett första steg mot mer självledarskap på arbetsplatsen kan handla om en dialog om erfarenheter och upplevelser som format oss. Du som är chef, inled samtal med dina medarbetare där du tar reda på vad som driver och motiverar dem, inte bara inom ramen för deras nuvarande yrkesroll. Där har du nyckeln till engagemang och till att utveckla både företagets och individernas verkliga potential. Vi är alla människor som vill känna oss sedda och först då vill vi bidra. Är chefen vår viktigaste hälsofaktor på jobbet? Ja kanske är det du som ska intervjua chefen när du söker jobb. I en undersökning som Academic Work genomfört visar det sig att unga i karriären uppger att en bra chef och ett bra ledarskap är en avgörande faktor för vilken arbetsplats man väljer. I det här avsnittet pratar vi om att dina framtida chefer är en viktig faktor när du söker nytt jobb och även att det finns anledning att reflektera över det ledarskap du har just nu - inte minst för hälsan. Eller om du är chef, det ledarskap du är med om att forma. KASAM - en bra vägledning Ett sätt att se på relationen chef/medarbetare är att utgå från KASAM-modellen (som vi har ett helt avsnitt om här). Hur ska min chef stötta mig i  min vilja, mitt kunnande och så att jag klarar av det jag förväntas göra? Och vad av detta ska jag stå för själv? KASAM-modellen kan vara en bra vägledning att ha med sig om man vill intervjua sin chef, eller för alla som vill reflektera själva kring hur man har det på jobbet och i livet. Att intervjua chefen är inte att vara fräck eller överdrivet kravfylld. Du kan luta dig på forskningsstöd när du hävdar att din relation till chefen är avgörande både för dig själv och för organisationen du ska ingå i. Listen NowHealth care analytics has typically referred to modeling insurer or payer risk or to predict patient utilization or to segment patient populations.  However, because of advances in personal or wearable medical devices, supporting software applications and the increasing use of electronic health records, individuals or patients now have the opportunity to gather their own health and medical data and information and use it to better manage their health status and/or medical needs.  This opportunity is what's become termed the democratization of health care or alternatively the emancipation of the patient.  During this 25 minute conversation Dr. Dwight McNeill provides an overview of his recently published work, "Using Person-Centered Health Analytics to Live Longer," i.e., he unpacks four domains he identifies ("knowing me," "protecting health," "minding illness," and "managing data") that can empower, enable and equip an individual to manage their health and medical needs.  Dwight also discusses barriers to the adoption to "person-centered" analytics  and near future potential of these tools.          Dr. Dwight McNeill is Lead Faculty for the International Institute for Analytics.  He is also President of WayPoint Health Analytics which provides consultation to organizations on health and healthcare analytics.  During his 30-year career, he has worked in corporate settings, most recently as global leader for business analytics and optimization for the healthcare industry for IBM and previously as director of healthcare information at GTE Corporation (Verizon).  Earlier, Dwight worked for the federal Department of Health and Human Services and the Commonwealth of Massachusetts, for information companies, and in provider settings.  Dwight has published two related books on healthcare analytics in 2013: A Framework for Applying Analytics in Healthcare: What Can Be Learned from the Best Practices in Retail, Banking, Politics, and Sports; and, Analytics in Healthcare and the Life Sciences: Strategies, Implementation Methods, and Best Practices.   He has also published frequently in Health Affairs and other related journals.
