About this episode: For some people with a high risk of ovarian cancer, a standard approach has been full removal of the reproductive organs. But new research points to a far less invasive procedure called a salpingectomy, or removal of the fallopian tubes, as a potential “game changer” in ovarian cancer. Rebecca Stone and Kara Long Roche to speak about opportunistic salpingectomy to prevent ovarian cancer, specifically serous carcinoma. --- EARN CME Reflect on how this Podcast applies to your day-to-day and earn free AMA PRA Category 1 CMEs: https://earnc.me/i7IZlO --- SHOW NOTES The episode begins discussing ovarian cancer as a whole and how it affects 1 in 78 women in their lifetime with high mortality rates, no screening tests, and hardly any symptoms before it becomes metastatic. Most ovarian cancers (80-90%) are epithelial, and of those, most are high grade serous carcinomas, which are very lethal. Drs. Stone and Long Roche go on to explain how ovarian cancer is actually disseminated tubal cancer, and that there is data showing dysplasia in the fallopian tube before it turns into ovarian cancer. In fact, the fimbriated ends of the fallopian tube are very high in p53 mutations, and there is data showing a decreased risk of ovarian cancer with tubal ligation. As a result, salpingectomy can be used as primary prevention for ovarian cancer. They have been doing opportunistic salpingectomies on women already in the OR for hysterectomies, Cesarean sections, ovarian cystectomies, etc. They would like to expand it to women receiving abdominal or urologic surgeries, including hernia repairs or cholecystectomies, which would require education, training, and communication across multidisciplinary teams. The physicians continue to discuss the impact of opportunistic salpingectomies, mentioning 2000 lives could be saved every year in addition to 0.5 billion health care dollars saved as a result. They compare it to the HPV vaccine with a number needed to treat around 1 in 300 to 1 in 500 and a risk reduction of 65% or greater. Technically speaking, the physicians mentioned the difficulties of ensuring adequate fimbriae removal of the ovary without causing more harm; overall, data shows that if done correctly, this procedure does not result in early menopause or impaired ovarian function. They also stated there is no specific procedure code for opportunistic salpingectomy along with conflicting state laws on reimbursability for the procedure, which could affect data collection and implementation. Finally, the episode ends with talk about the future of opportunistic salpingectomies. Data shows there is a decreased incidence of high grade serous ovarian carcinoma, but more data is needed on mortality effects. The physicians strive to educate providers and patients about the procedure while ensuring no woman becomes sterilized before she is ready. Rebecca Stone is a busy woman. She’s a midwife, a mum, and someone who loves racing her mountain bike for 24 hours - all while living with fibromyalgia. In 2010, after 9 months of severe pain which ultimately left her unable to work, drive or walk for more than 5m, Rebecca was diagnosed with fibromyalgia. A chronic condition, fibromyalgia causes widespread pain and sensitivity throughout the body. There’s no cure and it is difficult to treat. Getting back on her beloved mountain bike plan was part of Rebecca’s initial treatment plan primarily for the mental health benefits.  We recorded this conversation in May 2018. Later that year, as the mother of a 2-year-old toddler, Rebecca finished 2nd at the Australian 24 Hour Solo Championships in 2017 and 2018, and then finished 6th at the World Championships in Scotland in 2018. In this conversation, we discuss motherhood as well as living, training and racing with chronic pain. Plus she shares why she thinks she’s a better athlete because of it. Released tomorrow June 7th this is an arresting and Important British Film about a difficult and under-discussed subject - dealing with the aftermath of being the victim of an acid attack. First-time actor Vicky Knight shows tremendous nerve and courage playing an acid-attack survivor in Sacha Polak’s gritty, grounded London drama. In real-life, the 23-year-old was scarred in a fire as a child and has spoken movingly to the Guardian about the trauma of filming nude scenes and closeups of her burns (enhanced for the film by makeup). Well, not only is she brave, she’s good, too, inhabiting her funny, impulsive, often unlikable character utterly. 'I hid my scars for years ... I thought I was only good for horror movies' Dirty God’s script skips the attack, picking up with Jade (Knight) as she’s leaving hospital after months of operations and skin grafts. Back at home in the flat on a London estate where she lives with her mum and two-year-old daughter, everyone tells Jade that she’ll be able to get on with her life when her ex-boyfriend is sentenced for the crime. But how can she move on when every time she leaves the house people stare? Jade’s doctors are happy with her progress, but she trawls the internet in search of a miracle cure, finding a cheap plastic surgery clinic in Morocco that promises to correct her scarring. Dirty God is Dutch director Polak’s English-language debut, and it’s an authentic portrait of London. Polak shoots with in-the-moment energy: in hospital wards, at a grime club, in a drab call centre where Jade gets a job to pay for surgery. This is a film about a young woman made by young women (co-written by Polak and Susie Farrell), who aren’t shy of thinking about what Jade’s sex life is going to look like now. The guy she was flirting with before the attack (Bluey Robinson), has got together with her best mate (Rebecca Stone) – both are brilliantly cast. It’s intense but not unwatchably painful, and so much more than an issue film or portrait of a victim. About 5 years ago, Rebecca Stone joined LiveRamp and the CEO said, “I want you to be as sophisticated as possible in advertising, and how you use it.” At the time, the company had two sales development reps, a funnel that wasn’t necessarily feeding leads properly, and a young tech stack and tons of potential, so Rebecca got to work on the company’s digital transformation. Now, the company has a tech stack built from roughly 40 different tools, broken into different operating nodes, a marketing team that supports 20 sales development reps, an in-house data lake, and data analytics models that can accurately predict how marketing spend will translate to revenue—a sort of holy grail for marketing analytics. Welcome!  Today’s SHEro is Becky Walsh, who is a down-to-earth, funny teacher and Hay House author. She has written several books, including 'You Do Know: Learning to Act on Intuition Instantly'. She has her own TV show called “Becky’s Life Hacks,” and writes for multiple magazines. Today she shares her adventure of listening to her intuition that took her from selling up in London, so that she could move to San Fransisco.Join me for an inspiring conversation with Becky!   In this episode, Becky shares: How she faced burnout and had lost herself while living in the UK. Everywhere she looked, circumstances pointed her to San Francisco, so she decided to move there and enroll as a student. The difference between heart intuition and gut intuition. How she experienced mixed reactions from her family, who were mostly supportive, but she was determined not to let them hold her back. How she describes the “wonderful series of synchronicities that screamed San Francisco!” There is a difference between wishful thinking and universal insight. She experienced “the call” from the universe. She studied neuroscience and psychology, worked on a work permit, and then wrote You Do Know. She blended her American and British styles to write the book, and describes her time in San Francisco as “incredibly creative.” Her biggest success? “Becoming the ME that I am now.” She says she learned to bond with and trust other women and open her heart.
