Guest Anna Lembke is a psychiatrist and a specialist in the behavioral sciences who studies addiction. While there is tremendous variety in the things people can be addicted to, all forms are tied to dopamine, a biochemical that is key to human senses of pleasure, reward, and motivation. She says that new treatments are combining traditional abstinence with programs that help patients reenergize dopamine centers in the brain through physical exercise, which is a known producer of dopamine. There are a lot of negative consequences that can come from behavioral addictions. They include everything from financial problems to legal repercussions to health issues. Whether it's gambling, gaming, sex, or pornography, it's important to understand why some people are susceptible to certain behaviors. In fact, the same mechanism that influences chemical addiction could be responsible for behavioral addiction as well. On this episode, Duane speaks with Amanda Giordano, a licensed professional counselor and associate professor at the University of Georgia, who specializes in addiction counseling both in chemical addiction and behavioral addiction. Amanda is the author of a clinical reference book titled A Clinical Guide to Treating Behavioral Addictions.  Today, she imparts her expertise in this field, explaining how the changes in the brain associated with behavioral addictions are actually very similar to the changes associated with chemical addictions.  In other words, instead of becoming addicted to a chemical that originates outside of the body, the body is addicted to the release of its own neurochemicals (dopamine in particular). Once the brain's reward system is activated, it releases neurotransmitters which can become very difficult for some people to abstain from as they become their primary means of regulating their emotions. Hence, for some susceptible individuals, a rewarding behavior can become an addiction where there's a loss of control over that behavior. The behavior is continued despite negative consequences and it becomes compulsive for that individual. They experience cravings or a mental preoccupation with the behavior when they're not engaging in it.  When we understand the neuroscience behind it, we can have a lot more empathy for people with addiction because we can now understand that their experience might be very different from the experience of someone who is not genetically predisposed to addiction. While a medical student at NYU in the early 1980's, he was first exposed to a stress related disorder-- his own! After a residency in Family Medicine at UCLA/Santa Monica Hospital, he received a Certificate of Additional Qualifications in Sports Medicine. Dr. Schechter has been treating chronic pain and other functional disorders with a psycho-educational approach for over 25 years in private practice. Dr. Schechter taught at the USC/California Hospital Family Medicine Residency, part-time, for over a decade and received two Outstanding teacher awards. In addition, Dr. Schechter is the author of Think Away Your Pain, The MindBody Workbook, and the MindBody Workbook for Teens. The first two are home educational materials for stress-pain and functional disorders. The last is an educational tool for adolescents to help deal with and prevent these disorders. He is a co-author of A Diagnostic Guide For Psychophysiologic Disorders. This new book is designed for professionals and all proceeds go the non-profit PPDA below. Dr. Schechter is Treasurer, Board of Directors, Psychophysiologic Disorders Association (PPDassociation.org), a non-profit dedicated to educating the public and health care professionals about PPD. He was the Principal Investigator of the Seligman Medical Institute between 2003-2007, and, with that part-time grant, he published several papers, the final one an outcome study published in Alternative Therapies. I recently read an article in Counseling Today entitled Six Steps for Addressing Behavioral Addictions in Clinical Work and knew immediately I needed to invite the author onto the podcast. Fast forward a few short weeks and I was able to sit down with Dr. Amanda Giordano and talk all about behavioral addiction. What is a behavioral addiction you may be wondering. Well most folks recognize gambling addiction as the behavioral addiction, but Amanda sheds light on this topic as we discuss gaming, social media, pornography, sex, food, and others as behaviors where addiction can manifest. We talk through the difference between high engagement and addiction, the importance of education and early intervention, how families with kids and teens can address behavioral addictions, what recovery from a behavior addiction looks like, and frankly way more than I can possibly include in this intro. Amanda is an amazing guest, she is so passionate about her field, and wants to raise awareness and educate people about this critical issue. Abstract: Impulsivity and thought suppression are two psychological traits that have great variation in healthy population. In extreme cases, both are closely related to mental illness and play an important role in behavioral addiction. We have known the role of the top-down mechanism in impulsivity and thought suppression, but we do not know how the related neural nuclei are functionally connected and interact with each other. In the study, we selected excessive internet users (EIU) as our target population and investigated the relationship between thought suppression and impulsivity in the following aspects: their correlations to psychological symptoms; the associated neural networks; and the associated brain morphometric changes. We acquired data from 131 excessive internet users, with their psychological, resting-state fMRI and T1-MRI data collected. With the whole brain analysis, graph theory analysis, replication with additional brain atlas, replication with additional MRI data, and analysis of brain structure, we found that: (i) implusivity and thought suppression shared common neural connections in the top-down mechanism; (ii) thought suppression was associated with the neural network that connected to the occipital lobe in the resting-state brain but not the morphometric change of the occipital lobe. The study confirmed the overlap between impulsivity and thought suppression in terms of neural connectivity and suggested the role of thought suppression and the occipital network in behavioral addiction. Studying thought suppression provided a new insight into behavioral addiction research. While growing up in a small town in Texas, Jose was terrified to come out as gay to his family—particularly his physically and mentally abusive father. After coming out, Jose's family was not accepting. The rejection he faced from his parents played a role in leading Jose to rely on other ill-intentioned adults. Unfortunately, what happened next included a pattern of sexual abuse, manipulation, and sex trafficking at the hands of his abusers who, at the time, disguised themselves as men wanting to help Jose. Jose’s story of being sex trafficked as a male who is a member of the LGBTQ+ community is not one that is often told—even though those in the LGBTQ+ community regularly experience exploitation. Jose is now a public speaker, author, advocate, activist, and sex trafficking survivor who is sharing his story to help shed a light on these issues. There are a lot of negative consequences that can come from behavioral addictions. They include everything from financial problems to legal repercussions to health issues. Whether it's gambling, gaming, sex, or pornography, it's important to understand why some people are susceptible to certain behaviors. In fact, the same mechanism that influences chemical addiction could be responsible for behavioral addiction as well.On this episode, Duane speaks with Amanda Giordano, a licensed professional counselor and associate professor at the University of Georgia, who specializes in addiction counseling both in chemical addiction and behavioral addiction. Amanda is the author of a clinical reference book titled A Clinical Guide to Treating Behavioral Addictions. Today, she imparts her expertise in this field, explaining how the changes in the brain associated with behavioral addictions are actually very similar to the changes associated with chemical addictions. In other words, instead of becoming addicted to a chemical that originates outside of the body, the body is addicted to the release of its own neurochemicals (dopamine in particular). Once the brain's reward system is activated, it releases neurotransmitters which can become very difficult for some people to abstain from as they become their primary means of regulating their emotions.Hence, for some susceptible individuals, a rewarding behavior can become an addiction where there's a loss of control over that behavior. The behavior is continued despite negative consequences and it becomes compulsive for that individual.
