Paul Krauss MA LPC speaks with John Bateson about his new book: Suicide: Understanding and Ending a National Tragedy which provides essential information about the current landscape surrounding suicide - debunking common myths, sharing demographic data, and identifying risk factors and warning signs. More than 49,000 people in the United States died by suicide in 2022 — that's one death every 11 minutes, and more than any other year dating to back at least 1941, according to the US Centers for Disease Control and Prevention. John Bateson has pragmatic suggestions for multiple segments of society regarding how we as a society can reduce the number of suicides in the United States. John Bateson was executive director of a nationally certified crisis intervention and suicide prevention center for 16 years. He also was executive director of three university counseling centers and deputy director of a multicounty social service agency. He served on the steering committee of the National Suicide Prevention Lifeline (now the 988 Suicide and Crisis Lifeline) and was appointed to a blue-ribbon committee that created the "California Strategic Plan on Suicide Prevention." He is the author of The Education of a Coroner, The Last and Greatest Battle, The Final Leap, and Building Hope. Dr. Alice Feller, MD speaks with Paul Krauss MA LPC about her experiences working as a psychiatrist both within the public mental health system and private practice. Dr. Feller discusses the barriers inside the current system that are diminishing care to helping those who are most afflicted by mental health concerns. Dr. Feller tells stories of those she has worked to help and how some healthcare policies, current laws, and ignorance of mental health conditions negatively affect those seeking care of severe mental illnesses such as Bipolar Disorder and Schizophrenia. Dr. Feller and Paul discuss the correlations between homelessness and those suffering from mental illnesses. According to Dr. Feller, author of American Madness: Fighting for Patients In a Broken Mental Health System, the deinstitutionalization movement (especially the obscure "16-bed law" regarding public federal healthcare funding) caused many of the homelessness issues we see today, and that mental illness and addiction don't necessarily cause homelessness — but that homelessness often triggers or exacerbates mental illness and addiction. What's more, most treatment for serious mental illness and addiction is doled out in American prisons, an issue which criminalizing homelessness would only worsen. Life Coaching: A New Dimension in Counseling Today we discuss the recent upsurge in life coaching, and feature one of the leaders in this emerging field, Angela Poch, and one of her wonderful students, Lorna Bird. Lorna Bird Angela Poch (see featured photo) is a Registered Professional Counsellor (RPC) with the Canadian Professional Counselling Association, certified Master Life Coach, and TEAM-CBT Level 4 Advanced Therapist and Trainer. She has been teaching health and wellness for over 30 years and was Vice President of Education with the Adventist Association of Health & Wellness Coaching. She has written several articles and books on health and wellness including, “The Truth Will Set You Free.” She has a YouTube channel (@talkingteamcbt) interviewing clinicians about their journeys with TEAM-CBT. She also has a channel on psychology and health (@bodymindhealth4u). Lorna Bird holds a Diploma of Counseling from Australia and is a certified Life and Health Coach. To get started, what IS coaching, and how does it differ from psychotherapy? I am aware that our podcast goes worldwide, so the answer may differ depending on the country or state where you live. According to AI, “In California, the term "psychotherapist" is defined in the Civil Code to include a number of mental health professionals, including psychologists, psychiatrists, and clinical social workers. In general, anyone who provides psychotherapy or counseling in California, whether in person, by phone, or online, must be licensed.in California.” Coaching is quite different. Again, according to AI, “In California, there are no specific state-mandated requirements for individuals to use the term "coach" or practice life coaching, meaning anyone can technically call themselves a coach without obtaining a specific license.” Angela Poch resides in Canada, and she has been a leading and beloved member of the TEAM-CBT community. She emphasized several differences between a “coach” and a licensed mental health professional: Diagnosis: A coach does not diagnose clients into the familiar DSM categories of “mental disorders,” such as “Major Depressive Illness,” “Bipolar Disorder,” “Schizophrenia,” or any of the hundreds of “mental disorders” listed in the DSM. In the same vein, licensed mental health professionals will typically screen for suicidal thoughts and urges, and will treat suicidal individuals, but this is forbidden territory for coaches. Purpose / aims of coaching: The purpose or aims of coaching do not, as a rule, involve delving into your past to search for the “cause(s)” of your problems, such as adverse childhood experiences or traumas. Instead, the focus of coaching is primarily on making changes in the here-and-now in how you think, feel, and relate to others. This might involve learning to challenge distorted negative thoughts so you will think and feel more positively about your life, as well as how to relate to others more skillfully. Coaching is goal-oriented and forward-moving rather than dwelling on the past. Of course, good counsellors and licensed mental health professionals may also provide tools to move forward as well, so there can be overlap. Training / credentialing: Because coaching is so new, there are not yet any widely accepted standards or requirements for calling yourself a “coach.” There is a varying degree of training to be “certified,” which might just consist of watching a couple videos to 100's of hours of supervision with the ICF (International Coaching Federation), and everything in between. Here's a generalized diagram Angela created to help with further clarification. NOTE: many TEAM-CBT therapists also are client/goal focused as well as and will often use coaching-style tools. Angela described a 20-hour “Feeling Great Coach” certification program she has developed. Her program is based almost entirely on TEAM-CBT and includes a final exam you must pass to get certified. She also offers a TEAM-CBT Masterclass & Mentoring Program for both coaches as well as therapists that includes live training twice a month, online practice groups, case consultation in small groups, one-on-one mentoring sessions, and personal work as well. She said this integrates seamlessly with FGI's Fast Track program, any of David's intensives, or the Tuesday group for those who want more individualized support and training. Lorna enthusiastically described her experiences learning from many of Angela's training programs. I (David) have had the pleasure of knowing and working with Angela for many years now, starting with her attendance at a number of my intensives and two-day TEAM-CBT training programs in Canada. I am, for the most part, enthusiastic, but with a few reservations. I would have to confess that I spent 5 years in medical school, four more years in psychiatric residency, plus two years in post-doctoral research training at top institutions, but did not learn much at all, if anything, that has really helped me treat human beings who are hurting. Of course, I did learn how to prescribe drugs for those with severe difficulties, like Bipolar I Disorder, or Paranoid Schizophrenia, but that definitely did not require years and years of intensive medically oriented training. All I ever really wanted to do was psychotherapy. In my last year or so of post-doctoral psychiatric training, I discovered cognitive therapy, although it was not a part of the required curriculum, and learned quite a lot that has been really helpful in working with patients. But for the most part, all of the extensive medical training I received had very little, at best, to do with how I now work with patients, and an awful lot of what I do was things I had to figure out for myself. I do have great respect for psychology training (either clinical psychology or PsyD training), because of the emphasis on research and critical thinking, but I do think there is lots of room for new approaches such as coaching to emerge and evolve, and it seems to be the case that more people than ever are still struggling with depression, anxiety, addictions, rage, and more. Like any field, I think coaching is vulnerable to misuse by narcissistic individuals who wish to deceive or exploit the general public, and individuals who are naïve or unaware of the background or training of their therapists are perhaps at great risk of abuse. But I would also say that this problem is in no way limited to coaching, since a great many licensed mental health professionals have been charged and convicted of all manner of unethical conduct and as well as malpractice over the years as well. That's why I'm enthusiastic about responsible leaders who, like Angela Poch, are blazing new trails and setting the bar high for those who wish to enter the field of life coaching. Trauma can affect anyone at any point in life, including you. Not only difficult experiences during early childhood, but also events later in life can trigger significant trauma in individuals. The American Psychology Association defines trauma as any experience that results in significant fear, helplessness, confusion, or other disruptive feelings. Patricia Roos, Retired Professor Emerita of Sociology at Rutgers University speaks with Paul Krauss MA LPC about her personal story of losing her son to addiction as well as the systemic failures that contributed to his death. Dr. Connie McReynolds, PhD has caused quite the stir in the fields of psychology, psychiatry, and counseling as she has approached the ADHD and ADD diagnoses much differently than most practitioners. Rather than debating the nuances of "does someone HAVE ADHD or not", Dr. Connie has researched and utilized sophisticated Neurofeedback Assessments that differentiate multiple different issues that could be contributing to the largest gestalt (or diagnosis of ADHD): Including multiple tiers of auditory processing disorders, visual processing disorders, general learning disorders, differential medical diagnoses, environmental factors, and overlapping clinical psychological diagnoses. If this sounds intriguing, this is an episode you need to hear and she has written a book that I would recommend reading. Dr. Connie McReynolds, PhD and her team have fearlessly pushed forward into researching the nuances of all types of people tagged with the label "ADHD/ ADD." In going deeper, with their neuropsychological assessments and sophisticated proprietary and targeted neurofeedback treatments, they have discovered that there are a myriad of issues that are contributing to what your average clinician may conclude is ADHD (referencing the DSM or ICD codes). Not only have they been able to figure out the etiology of the symptoms that are contributing to the clinical picture / diagnosis--they have worked on treating these symptoms WITHOUT medications or minimal medications, and also with minimal therapy (unless called for from a crossover of other behavioral, environmental, or psychological factors). Dr. Connie McReynolds' team have had incredible results from only 20 thirty minute targeted neurofeedback subjects who are coming in with a range of ADHD-related symptoms. And 6 and 12 month follow up assessments and subjective reports are just as stunning! However, the idea that you can actually reduce ADHD symptoms for the long-term without extensive medications and behavioral therapies goes against the status quo of most practitioners in the United States. Thus, this has not yet been popularized. It also doesn't cost that much money and the treatment is not life-long, so thus it has not yet been picked up by the typical profiteers. The best news is that the data speaks for itself. From multiple published articles to the examples in Dr. Connie McReynold's book, there is hope for those that suffer from ADHD, auditory processing issues, visual processing disorders, and related issues. Whiteside, PhD, LP is a Board-Certified Clinical Psychologist, Professor of Psychology in the Mayo Clinic College of Medicine and Science, and Director of the Pediatric Anxiety Disorders Program at Mayo Clinic in Rochester, MN. He received a BA in Psychology from Northwestern University and Ph.D. in Clinical Psychology from the University of Kentucky before completing a pre-doctoral internship in Pediatric Psychology at the Geisinger Medical Center and a post-doctoral fellowship in Child and Family Medical Psychology at the Mayo Clinic. At three, orphan life with my sisters wasn't bad--it was basically all I knew. Love filled the void left by our parents. But our separation shattered that love and we were lost in the system and separated for most of our lives. The pain lingered for 43 years until a miraculous discovery and reunion occurred. "Broken Water" tells my story - surviving foster care abuse while searching for my sisters. Neuroscience and Trauma therapists agree: It's something like a ‘Windows update' for "being a human.” We are living in a very strange time in history. We actually have the knowledge that we need to create more compassionate and science backed care and education of every member of society. Yet we are falling farther and farther behind in many real spaces. Much of it comes down to systems that were not designed with emotional safety in mind as being important for learning or healing physical health issues. Fear of change keeps broken systems in place. This means that trauma-informed providers and educators are out here in society trying to share and equip a community working from within systems that were not designed to work that way. Rather than become discouraged, Laurie Belanger LCSWR is talking about it. As citizens, the steps we can all take from within the outdated systems we are working within. We are talking about the possibilities of someday re-inventing the systems that no longer serve us. I recently read an article in Counseling Today entitled Six Steps for Addressing Behavioral Addictions in Clinical Work and knew immediately I needed to invite the author onto the podcast. Fast forward a few short weeks and I was able to sit down with Dr. Amanda Giordano and talk all about behavioral addiction. What is a behavioral addiction you may be wondering. Well most folks recognize gambling addiction as the behavioral addiction, but Amanda sheds light on this topic as we discuss gaming, social media, pornography, sex, food, and others as behaviors where addiction can manifest. We talk through the difference between high engagement and addiction, the importance of education and early intervention, how families with kids and teens can address behavioral addictions, what recovery from a behavior addiction looks like, and frankly way more than I can possibly include in this intro. Amanda is an amazing guest, she is so passionate about her field, and wants to raise awareness and educate people about this critical issue. She recently published a book of encyclopedic information on the topic of behavioral addiction for professionals doing clinical work. In the process, she interviewed numerous experts in the various fields of behavioral addiction and has been sharing this knowledge through regular articles in Psychology Today. Borderline personality disorders make things appear as black and white with no integration between the two. It manifests as rejecting the love and support from a partner, the very thing that a person with this disorder actually needs in the first place. Gabrielle Usatynski, a licensed professional counselor and relationship expert, explains that borderline personality disorders cause radical misperceptions which cause confusion in relationship. Displaying compassion plays a central role in healing the very source of the trauma to result in more loving and fulfilling relationships. This has a significant impact on displays of empathy and intimacy in relationship. 13:15 "Splitting" in personality disorders means a person cannot integrate opposing thoughts and only sees things as one or another. There is a lack of integration between the two, something which normally develops very early in life. 18:57 There is a nurture aspect to borderline personality disorders which is attributed to trauma, abuse, and/or neglect. Any sabotage to a growing person's self-activation will manifest in relationships well into adulthood. 26:46 Borderline personality disorder affects the socio-emotional capacity to separate raw emotions and physiological experiences. It is a lack of neural integration that results in poor reality testing. 35:08 Clinicians can easily misdiagnose borderline personality disorders. Maria Davis-Pierre is a mother to an 8-year-old girl on the autism spectrum, a Licensed Mental Health Counselor, and the Founder and CEO of Autism in Black Inc., an organization dedicated to raising autism awareness and reducing the stigma within the Black community. Maria is also a coach, speaker, advocate, and author. She has been featured in Forbes, the New York Times, and Counseling Today, and her book The Self-Care Affirmation Journal was published in 2018. In today’s conversation, Maria talks about the challenges of receiving her daughter’s diagnosis and explains why she started Autism in Black. As a Parent-Child Interaction Therapy intern, she describes how PC-IT can benefit families who are dealing with behavioral challenges. We discuss how religion, misunderstanding of autism, and distrust of the health care system contribute to misdiagnosis. We also address the lack of research on autism in the Black community and why this problem leads to culturally-insensitive assessments and treatment plans. Instead, it inserts symptom-based reductionism into talk therapy, contradicts the helping relationship, and violates counselor identity. My guest this week is Gregg Krech, who specializes in Japanese psychology. We focused on the principles of Morita therapy, which emphasizes taking action that brings meaning to one's life. This approach contrasts with our tendency to dwell on how we feel and what we feel like doing, and to get lost in self-focused attention. By asking instead, "What needs to be done?" we can build a life defined by meaning and usefulness. A conversation with Stacey Diane A. Litam on the reality and nature of human sex trafficking in the United States and what counselors need to know. Stacey Diane A. Litam, M.A., LPC, NCC is a doctoral candidate in Kent State University’s Counselor Education and Supervision program and adjunct faculty at the Northeast Ohio Medical University. Stacey’s dissertation examines the influence of gender and discourse on counselors’ reported levels of empathy and acceptance of rape myths when counseling sex trafficked survivors. Stacey currently works as a licensed mental health counselor at a Northeast Ohio agency where she co-developed the Human Trafficking Docket with the Cleveland Municipal Court. Stacey currently provides counseling services for survivors of sex trafficking, persons with addictions, and LGBTQ clients. She is a researcher, educator, and social justice advocate for human trafficking, LGBTQ, and multicultural issues. Ms. Litam has facilitated over 30 state, regional, national, and international presentations on topics including human sex trafficking, integrating creativity with diverse populations, and the influence of internalized racism and intra-ethnic othering on Asian American adult identity development. In February 2016, Ms. Litam was a recipient of the Movers and Shakers Award from the Cleveland Professional 20/30 Club, which honored 25 leaders in Northeast Ohio under the age of 30.
