In part 1 we talked about a Medscape Today article that reader Antoni sent me about weight stigma in healthcare called “Weight Stigma is Bad Medicine.” The author, Mengyi (Zed) Zha, MD made important points about weight stigma, some of which aren't often aren't made. So there's a lot of good in the article, which I appreciate.What Dr. Zha has written is brave (and she will undoubtedly get criticism from those who implicit or explicit anti-fat bias drives their thoughts about and interactions with higher-weight people.) When Zed Zha came to the United States for her education, she faced barriers to medical school admission and financial aid, and to establishing a sense of belonging. Then unexpectedly, she found the feeling of “home” in rural New Hampshire. Dr. Zha joins us to share her blog post Where is Home? and to take us through her journey to her life now as a doctor in rural America, focused on underserved patients. Our guest is: Dr. Zed Zha, who is a writer and a family doctor, a feminist and a fierce patient advocate. Her current work-in-progress is a non-fiction book project on medical misogyny. Dr. Zha is a board-certified family physician. She graduated from the Geisel School of Medicine at Dartmouth, where she received the Gold Humanism Honor Society Award and the John F. Radebaugh Community Service Award. She completed her residency at the Mayo Clinic - La Crosse Family Medicine Residency, where she was the recipient of the Residency Research Award. Zed practiced full-scope primary care with obstetrics and hospital medicine in rural Washington state from 2019-2022, where she served as her organization's COVID-19 Task Force Physician Lead. Zha is a Sichuan-born China-commentator who had a long-term professional sojourn in southern California, before settling in Bali, Indonesia. He hosts the Silk and Steel podcast, which covers China, the Silk Road, and more general history, culture and geopolitics. Zha and Razib have known each other since the 2010's, and often circle back to discussions of China, its history, politics and culture. The course of their conversations has spanned both the close of the “Chimerica” period of trade and political relations, and the more adversarial status that obtains in both the US and in the People's Republic of China under Xi Jinping. But first, Razib and Zha discuss what it's like to live as an ethnic Han Chinese in Indonesia, albeit one who resides in Hindu-majority Bali, where Zha settled after marrying a local woman and becoming a father. Though Indonesia has an economically and politically powerful Chinese minority, it was also the scene of ethnic riots in the 1990's and a genocide of Han Chinese in the 1960's. Until recently, the state did not recognize Confucianism as a religion and discouraged Chinese names and Chinese-language schools. Nevertheless, Zha presents a relatively positive picture of relations on the island of Bali, where the Hindu population seems to have had an easier time integrating Han in a more syncretistic culture than in the Muslim-majority islands. Then they discuss the pivot in US-China relations in the last half a decade, and the possibilities presented by great power rivalry. Razib and Zha address the thorny reality that though China and the US are now embarking on more explicitly adversarial geopolitics, their economic ties remain strong, with Chinese supply chains essential for American firms like Apple and the US consumer demand essential to propping up China's vast export sector. Zha also offers a defense of Xi Jinping's rule and the prospects for China as it turns inward from the world, focusing on its domestic market and shoring up its geopolitical positions. The discussion turns to the range of likely outcomes in a world where the 21st century is both the American and Chinese century, and the two great powers remain both economically and geopolitically entangled through trade and numerous bilateral relationships with other nations.

Abstract: Interleukin-1 receptor (IL-1R)-associated kinases (IRAKs) are core effectors of Toll-like receptor (TLR) and IL-1R signaling, with no reported roles outside of innate immunity. We find that vertebrate cells exposed to ionizing radiation (IR) sequentially activate IRAK4 and IRAK1 through a phosphorylation cascade mirroring that induced by TLR/IL-1R, resulting in a potent anti-apoptotic response. However, IR-induced IRAK1 activation does not require the receptors or the IRAK4/1 adaptor protein MyD88, and instead of remaining in the cytoplasm, the activated kinase is immediately transported to the nucleus via a conserved nuclear localization signal. We identify: double-strand DNA breaks (DSBs) as the biologic trigger for this pathway; the E3 ubiquitin ligase Pellino1 as the scaffold enabling IRAK4/1 activation in place of TLR/IL-1R-MyD88; and the pro-apoptotic PIDDosome (PIDD1-RAIDD-caspase-2) as a critical downstream target in the nucleus. The data delineate a non-canonical IRAK signaling pathway derived from, or ancestral to, TLR signaling. This DSB detection pathway, which is also activated by genotoxic chemotherapies, provides multiple actionable targets for overcoming tumor resistance to mainstay cancer treatments. Dr. Shenghua Zha, an assistant professor of instructional design and development at the University of South Alabama, discusses her National Science Foundation grant "Preparing Elementary Pre-service Teachers to Integrate Computing across the Curriculum." This project is a collaboration between Zha and colleagues from the College of Engineering and College of Education and Professional Studies. The assumptions driving this grant are that computing is an interdisciplinary skill that is often not taught in an interdisciplinary manner, and one that is now significantly supported by state curriculum mandates. These project leaders are working together to deliver and evaluate training for early career teachers to integrate principles of computing into the teaching of English language arts, social studies, mathematics and science. This is accomplished through the use of developmentally appropriate interfaces that expose young learners to principles of computing from basic concepts to coding practices. The larger goal of the grant is to encourage teachers to continue integrating computing into their instruction across the elementary curriculum as they move into their careers.

Abstract: Intestinal epithelia express two long MLCK splice variants, MLCK1 and MLCK2. We have previously shown that disruption of inflammation-induced MLCK1 recruitment to the perijunctional actomyosin ring prevents barrier loss and attenuates disease progression. Here we sought to define the domains responsible for distinct MLCK1 and MLCK2 behaviors. Quantitative analysis of human biopsies demonstrated specific increases in MLCK1 expression and perijunctional localization in Crohns disease. When expressed in cultured intestinal epithelial cells, we found, as expected, that MLCK1 is most concentrated at the perijunctional actomyosin ring. In contrast, MLCK2 is predominantly associated with basal F-actin stress fibers. Immunoglobulin-cell adhesion molecule domain 3 (IgCAM3) must be critical for MLCK1 recruitment, as that domain is incomplete in MLCK2. Consistent with this, truncation mutants consisting of N-terminal IgCAM domains 1-4, without C-terminal catalytic domains, localized similarly to full-length MLCK1 and MLCK2, respectively. Further mutagenesis allowed identification of IgCAM2 and IgCAM3 domains as the minimal region required for MLCK1 recruitment. Although IgCAM3 does not concentrate perijunctionally, it can act as a dominant negative effector that limits steady-state and TNF-induced MLCK1 recruitment and barrier loss. Together, the demonstration of selective MLCK1 upregulation and perijunctional recruitment in Crohns disease and identification of domains required for perijunctional MLCK1 recruitment provide a conceptual understanding and structural data needed for development of therapeutic means of blocking MLCK1-mediated barrier loss without the toxicity of enzymatic MLCK inhibition. SIGNIFICANCE STATEMENTRecent work has demonstrated that long myosin light chain kinase isoform 1 (MLCK1) recruitment to the perijunctional actomyosin ring is a critical component of inflammation-induced intestinal barrier loss. Chanez-Paredes et al. show that this occurs in Crohns disease and define the essential structural elements that direct MLCK1 recruitment, thereby creating a foundation for therapeutic interruption of MLCK1 recruitment in disease. Dr. Zha talks about building trust with her patients, who are mostly immigrants and/or undocumented workers. Her own experiences as an immigrant prepared her empathy toward and understanding of the population she serves.

Abstract: The taxonomical structure of microbial community sample is highly habitat-specific, making it possible for source tracking niches where samples are originated. Current methods face challenges when the number of samples and niches are magnitudes more than current in use, under which circumstances they are unable to accurately source track samples in a timely manner, rendering them difficult in knowledge discovery from sub-million heterogeneous samples. Here, we introduce a deep learning method based on Ontology-aware Neural Network approach, ONN4MST (https://github.com/HUST-NingKang-Lab/ONN4MST), which takes into consideration the ontology structure of niches and the relationship of samples from these ontologically-organized niches. ONN4MST's superiority in accuracy, speed and robustness have been proven, for example with an accuracy of 0.99 and AUC of 0.97 in a microbial source tracking experiment that 125,823 samples and 114 niches were involved. Moreover, ONN4MST has been utilized on several source tracking applications, showing that it could provide highly-interpretable results from samples with previously less-studied niches, detect microbial contaminants, and identify similar samples from ontologically-remote niches, with high fidelity. Abstract: Impulsivity and thought suppression are two psychological traits that have great variation in healthy population. In extreme cases, both are closely related to mental illness and play an important role in behavioral addiction. We have known the role of the top-down mechanism in impulsivity and thought suppression, but we do not know how the related neural nuclei are functionally connected and interact with each other. In the study, we selected excessive internet users (EIU) as our target population and investigated the relationship between thought suppression and impulsivity in the following aspects: their correlations to psychological symptoms; the associated neural networks; and the associated brain morphometric changes. We acquired data from 131 excessive internet users, with their psychological, resting-state fMRI and T1-MRI data collected. With the whole brain analysis, graph theory analysis, replication with additional brain atlas, replication with additional MRI data, and analysis of brain structure, we found that: (i) implusivity and thought suppression shared common neural connections in the top-down mechanism; (ii) thought suppression was associated with the neural network that connected to the occipital lobe in the resting-state brain but not the morphometric change of the occipital lobe. The study confirmed the overlap between impulsivity and thought suppression in terms of neural connectivity and suggested the role of thought suppression and the occipital network in behavioral addiction. Studying thought suppression provided a new insight into behavioral addiction research. Abstract: Without an effective prophylactic solution, infections from SARS-CoV-2 continue to rise worldwide with devastating health and economic costs. SARS-CoV-2 gains entry into host cells via an interaction between its Spike protein and the host cell receptor angiotensin converting enzyme 2 (ACE2). Disruption of this interaction confers potent neutralization of viral entry, providing an avenue for vaccine design and for therapeutic antibodies. Here, we develop single-domain antibodies (nanobodies) that potently disrupt the interaction between the SARS-CoV-2 Spike and ACE2. By screening a yeast surface-displayed library of synthetic nanobody sequences, we identified a panel of nanobodies that bind to multiple epitopes on Spike and block ACE2 interaction via two distinct mechanisms. Cryogenic electron microscopy (cryo-EM) revealed that one exceptionally stable nanobody, Nb6, binds Spike in a fully inactive conformation with its receptor binding domains (RBDs) locked into their inaccessible down-state, incapable of binding ACE2. Affinity maturation and structure-guided design of multivalency yielded a trivalent nanobody, mNb6-tri, with femtomolar affinity for SARS-CoV-2 Spike and picomolar neutralization of SARS-CoV-2 infection. mNb6-tri retains stability and function after aerosolization, lyophilization, and heat treatment. These properties may enable aerosol-mediated delivery of this potent neutralizer directly to the airway epithelia, promising to yield a widely deployable, patient-friendly prophylactic and/or early infection therapeutic agent to stem the worst pandemic in a century. In 1996 Hernan settled in Hong Kong working originally for The Hong Kong Brewing and Restaurants Co. before establishing Zanghellini & Holt Associates in 1998. Since then, ZHA has participated in the design and opening of over 650 restaurants throughout Asia.

In this final episode on Zaha Hadid we discuss a small fraction of the huge number of projects that ZHA produced from the early noughties up to Zaha's untimely death in 2016. We attempt to reflect on Zaha's legacy as a designer, try to understand what concepts defined her design process, from Parametricism to pure sculptural form. There are so many projects from this period that we could have talked about, so we focus on discussing the most Projects discussed: Maxxi Museum in Rome, Ordrupgaard Museum Extension in Denmark, Phaeno Science Centre in Wolfsburg, the Kartal Masterplan proposed for Istanbul, Bergisel Ski Jump and the Nordpark railway stations in Innbruck, the London Aquatic Centre built for the 2012 Olympics, the Library at the University of Economics in Vienna, Dongdaemun Design Plaza in Seoul, the SOHO projects in Beijing and the King Abdullah Petroleum Studies and Research Center in Saudi Arabia. Zha provides a primer on what has unfolded with the protests in Hong Kong. He describes how they started, the role an extradition bill has played, the poverty and inequality fueling protests, and the protesters' demands. He also provides a thumbnail history of the colonial history around China and Hong Kong. He goes on to recall some of the incidents and flashpoints that have occurred, including violence that has been committed by Hong Kong protesters. The National Endowment for Democracy and the Oslo Freedom Forum have provided tactical support. Zha addresses the way in which the U.S. government and U.S. groups are aiding Hong Kong protesters and what that means for the protests. Later in the episode, Zha discusses the nativism inherent in the protests that is directed against mainland Chinese people, and he also highlights the controversy that erupted around the NBA in the past week.

Zaha Hadid Architects has always embraced technology to stay ahead of the game — even the late Zaha Hadid was herself using computers to design buildings back in the 90s. Today, the company makes use of software including Revit, Maya, V-Ray, Rhino and Grasshopper to create its iconic parametric designs, as well as lots of bespoke tools to create VR experiences and model human interactions. In West Philadelphia, born and raised is where the story of our next guest begins. On this episode, you’ll get to meet Zhane Dadson. It’s in Philly where she got her start in the performing arts field at the Creative and Performing Arts High School as a dance major. Dance has been an integral part of her life and she continued to do so when she attended Spelman College. She got her degree from there and eventually moved to New York to work at Ogilvy and then Anomaly NYC, where she’s currently a producer. Through all of this, she found running and got her start from the inspiration that comes from the NYC Marathon. It’s been less than a year since she’s got her start but she’s already garnering some attention as one of the more popular women of color in the New York City running community through the start of her Run with Zha initiative. The purpose of Run with Zha is to inspire people of all backgrounds and those within minority communities to pursue a healthier lifestyle, learn more about running and connect as many people throughout the community as possible. This week, Sinica is live from Fordham Law School in New York City! This episode features Zhā Jiànyīng 查建英, journalist and author of China Pop: How Soap Operas, Tabloids, and Bestsellers Are Transforming a Culture and Tide Players: The Movers and Shakers of a Rising China, who joined Jeremy and Kaiser at a Sinica Live Podcast event on January 14. The three discuss the experiences of Zha’s half-brother, Zhā Jiànguó 查建国, a democracy activist in China who was charged with subversion of state power and subsequently jailed for nine years. In addition, they pore over the political realities of contemporary China, the likelihood of reform, and the pressures that “moderate liberals” encounter in the face of rising suppression of political freedoms in the country. What to listen for on this week’s Sinica Podcast:  3:34: In the era of “stability maintenance” in China, netizens have coined unique nicknames for actions that censorship and security officials take to maintain order. “To be harmonized” (被和谐 bèihéxié), or to have speech censored, is the most well known, but there are many others. “To be touristed” (被旅游 bèi lǚyóu), or sent packing on a mandatory vacation accompanied by friendly police officers, is the subject of Zha’s writing, in this case. Zha elaborates: “I think this is a very eerie kind of symptom of the police state moving, in fact, you might say a little more sophisticated way of silencing or [getting] rid of those troublemakers in different spheres, right? Some of them are Party officials, others are critics like petitioners, NGO activists, or civil rights lawyers.” 18:24: Jeremy asks if Zha has ever been concerned whether her work as a journalist could potentially put her brother in danger. She says no, but adds that she intentionally kept him in the dark when writing her 2007 piece “Enemy of the State,” which was featured in The New Yorker, to protect him. Zha: “Still, the one point I did insist on was to not have the famous New Yorker fact-checkers call him beforehand because I knew all his phones and everything was tapped and monitored. And so I didn’t tell him I was writing this.” 29:58: Zha and Kaiser talk about political dissidents and activists. According to Zha, some of them endorse unfortunate and dated ideologies: “I don’t know, I used to think of them as liberals. Now I think maybe they need a different hat or label, you know — they’re sexist, because some of them in more recent phenomena really had a lot of trouble with #MeToo. The movement had kind of a short play in China…and there’s lots of people who have trouble with Islamic culture as well.” 32:17: High-profile Chinese dissidents and activists on a growing number of “sensitive” dates are often “touristed” for weeks on end. However, there is one caveat: No cell phones are allowed. Zha elaborates: “Back then, there were just these certain anniversaries or Party congresses. But now, China has emerged into this global powerhouse. So all kinds of global forums that are held in Beijing or in Qingdao or in Shanghai have also become sensitive days. And so, in such locations, the police would usually take selective numbers of ‘troublemakers’ out of the site of that city.” 57:53: Kaiser asks Zha about the modern Chinese intelligentsia: What role do Chinese intellectuals play in the political life of a country? Is their role understood in circles outside of China scholars? She responds, “Basically, the intellectuals played a very particular, important role of advising the emperor then, and now the leaders about the direction of the country, or they also are seen as the spokespeople for the common people…so they’re given this special kind of status or platform to govern or change the society. So that’s why this whole crackdown, right now, this whole ruthless crackdown on the intellectuals by stripping or removing platforms for their voices is so disturbing and casts such a chilling effect.”   Recommendations: Jeremy: Red Moon, by Kim Stanley Robinson, an interstellar work of speculative fiction. ZHA’s first project in the U.K. for a private client, the towers will include a hotel, offices, retail, residential and public space. Critics have said the development will further congest an already busy interchange and fear the buildings’ 185-m height will contribute to the “canyonization” of the Thames bank. Proponents point out the project’s creation of new public space and affordable housing. Additional details will be released later this year. When president-elect Donald Trump nominated Ben Carson to lead the department of Housing and Urban Development, the response was resoundingly: huh? The neurosurgeon came onto the national political scene in 2015, during his run for the Republican nomination, but after Trump took the presidency and started throwing around the idea of offering a Cabinet position to Carson, a spokesperson said "Dr. Carson feels he has no government experience, he's never run a federal agency. The last thing he would want to do was take a position that could cripple the presidency." Despite all that, Carson is now (almost definitely officially) secretary of HUD (which he knows just enough about to seriously backtrack the agency's work as pushed by Obama). So here we are. Special guest Marc Miller joins us on the podcast to discuss the implications of Carson's inexperience for HUD, as well as chew on the latest Schumacher-induced controversy: when the architect promoted the privatization of public space and trashed social housing at a recent talk in Berlin, ZHA responded to his remarks in an open letter, distancing the firm from its principal's so-called 'urban policy manifesto'. Miller has degrees in landscape architecture, architecture and fine arts, and has practiced as an architect, urban designer, campus planner, and architectural lighting designer. He currently teaches in the landscape architecture department at Pennsylvania State University, and previously at Cornell.

Xiaohua: If you want to break up with me, you have to return the ten roasted chickens I sent you. This is not said by me. It’s made by a young man in Henan province. After a policeman mediates, the girl agrees to return five roasted chickens, instead of ten. 焦作街头一对情侣闹分手,男友称“分手可以,得赔我十只烧鸡!”在民警调解下,姑娘答应赔五只。Hilarious it may be, but this actually touches upon a sensitive topic among couples, that is should people ask gifts--if we can call food a gift—back, when they break up with their partner? Liu Yan: No, I would say very simply no. Because by definition, when something is considered a gift and when you give that gift to somebody else, then that person is the owner of this specific thing. Xiaohua: Unless it’s an engagement ring; then that’s different. Liu Yan: So if this thing is already that person’s, then you have no right to say I want it back, because it’s no longer yours. John: Yeah, I don’t know. I think it’s really difficult to say what is appropriate and what is not appropriate. I think that break-ups are always difficult; especially if it’s a passionate relationship. In English, we have a phrase. It’s very politically incorrect but we still use it quite often. It’s used to describe someone who gives the gift but then asks for it back. We call them Indian giver. It’s kind of referring to the way the Indians were treated usually by the British and the subsequent American governments. Ostensibly, they gave the Indians their land. Only a generation later or ten years later, they just take it back by force. Xiaohua: Shouldn’t that be an American giver or British giver? John: Yeah, you are right. But this is the phrase that we use. I think it’s difficult. Because for me, it just comes down to sentimental value, right? Like if I was in what is understood to be a committed relationship and we have been together two or three years, I give you something of my mother’s-a necklace. My mother has died and I give you one of her pearl necklace or something like that, which has enormous sentimental value for me. I think it would be appropriate, in fact. I think it would be justifiable at least to ask for it back after we broke up. Xiaohua: So it’s not material value that matters the most just more of the sentimental value, if you attach sentiment to it. Liu Yan: I can go along with that, but my question is if it is that important to you. Shouldn’t you have thought this through better? John: No, that’s the point, because at the time I cared for you so deeply, right? And maybe this man cared so deeply about the chickens he gave to this woman. Xiaohua: What sentimental value does he attach to chicken? John: I don’t know. Maybe he raised them himself and treated them like family. Xiaohua: I do agree with what John said in that there’s big sentimental value attached to that item, and then it’s OK. But if it’s food and if it’s perishable items… John: Apparently some people are making jokes, like a man asks for his sperm back… Xiaohua: I don’t know if it’s a joke. It’s probably a real thing. John: or a man asks his ex to cover half of the condom money. That’s obviously perishable. Liu Yan: This is getting weird. John: I am just reporting what I see. Xiaohua: My concern is there are numerous more stories about this. We call it Zha’nan. It’s the really horrible guy story on the Internet. My question is maybe the girls like to complain a lot, but I’m sure there are girls like that as well. John: I think also we have to remember even in western culture, there still is an implied financial transaction between the man and the woman. While the man pays for the dinner and the man pays for the movie tickets, and there is expectation of certain reciprocity after the man has paid for that, whether that is in physical intimacy, other types of romance, emotional intimacy or something like that. So perhaps that’s kind of what we are seeing here, it’s just like there was an expected reciprocity that is since been broken or something like that. Xiaohua: I guess if you are a good girl and you don’t want to be in a tangled relationship after break up, you should probably not accept very expensive gifts. John: or just give the chickens back.

China watchers and writers Ian Buruma, Yan Lianke, Linda Polman, David Rieff, and Zha Jianying spoke at the PEN World Voices Festival of International Literature about human rights in China at the Great Hall at Cooper Union. Bon mots: Zha Jianying, author of "Tide Players: The Movers and Shakers of a Rising China," on human rights: "The questions of values and human rights lies not outside China but in China. And with the Chinese people and the Chinese leaders. This is about their life and their future. Nowhere else have these issues been debated and fought with as much passion and with a wider array of positions; the views as polarized and complicated as the situation. And the characters involved are four dimensional, not black and white."  Zha on humor: "I do know the party is not known for having a sense of humor. They wouldn't appreciate someone like Oscar Wilde who says, 'Life is too important to be taken seriously.'" Yan Lianke, who got the 2000 Lu Xun for "The Year, The Month, The Day" and the 2004 Lao She for "Pleasure," on Ai Weiwei: "An academic from Beijing told me something that shocked me. He said, 'What does all this have to do with our lives?' For example, when we see that Ai Weiwei is arrested, we see that he has a long list of crimes. And one of these crimes is fraud, and when people read about how much money he deceived from the people they think he deserves to be arrested and locked up. For all those who are struggling and fighting, 99 percent of the people in China don't really care about what they're doing. They care about their lives, they care about money, and their basic need to survive." Yan on censorship: "I think that people like Liu Xiaobo and Ai Weiwei are true warriors, where as someone like me, I'm a coward. I can't fight out loud like they do, all I can do is silently write. So, to be a lonely writer in China, is perhaps one of the luckiest things to do."
