In recognition of Alcohol Awareness Month, we are joined by George Koob, PhD, director of the National Institute on Alcohol Abuse and Alcoholism (NIAAA). Dr. Koob talks about his background and career journey, along with major insights made through his research, including regarding alcohol withdrawal. He addresses stigma related to alcohol use (or abstinence from) and offers advice for the next generation of researchers and clinicians in this exciting and dynamic space, with so much potential for expanding knowledge. Dr. Koob shares resources for individuals, family members, physicians, or anyone looking for information related to alcohol use and alcohol use disorder, including Rethinking Drinking and the NIAAA Treatment Navigator. He emphasizes the importance of motivation (e.g., motivational interviewing) rather than confrontation and seeing your doctor, or encouraging your loved one to see their doctor, for a physical as a good first step in addressing any issues related to alcohol use. Dr. Koob finally explains his motivation for continuing his work in this field. In addition to serving as director of NIAAA, Dr. Koob is also a senior investigator at the Intramural Research Program of the National Institute on Drug Abuse where he directs the Neurobiology of Addiction Laboratory in the Integrative Neurosciences Research Branch. As an authority on alcohol use disorder, drug addiction, and stress, he has contributed to our understanding of the neurocircuitry associated with the acute reinforcing effects of alcohol and drugs and the neuroadaptations of the reward and stress circuits associated with the transition to dependence. He has published over 800 peer reviewed papers; mentored 13 PhD students, 85 post-doctoral fellows, and 11 K99's (Pathway to Independence Award postdoctoral researchers); and authored several books including the Neurobiology of Addiction. He received his PhD in behavioral physiology from Johns Hopkins University in 1972. He did post-doctoral studies at Walter Reed Army Institute of Research and the University of Cambridge (England). He subsequently held positions at The Salk Institute and the Scripps Research Institute. Dr. Koob is the recipient of many honors, including membership in the National Academy of Medicine (USA) and award of the Legion of Honor (France). In this episode of Causes or Cures, Dr. Eeks chats with Dr. Tera Fazzino about her research on how the biggest US tobacco companies owned the leading food companies from 1988 to early 2000s and disproportionately produced hyper-palatable foods (HPFs). She explains what a hyper-palatable food is and how they are created, the theory for why some Americans get hooked on them, and how these foods may impact our health. Then she takes us back in history to describe when and why the tobacco industries owned and controlled leading food companies (Kraft, Nabisco, General Foods...) and how her and her team determined that they were making hyper-palatable foods. She also discusses what is happening with the current food industry, why this is a big public health issue, and what we can do to move toward a healthier food supply. As such, there is much neurobiological overlap in eating and drug addiction. Dr. David Wiss, Ph.D., advocates for change and improvement in the emerging fields of nutrition and behavioral health. Being a dietitian, he welcomes new ideas and encourages critical thinking when collaborating with other professionals, colleagues, and students. As a leading Health Nutritionist in Los Angeles, Dr. Wiss hosted several annual non-profit conferences in Los Angeles that focused on controversial issues in nutrition. This led the California Academy of Nutrition and Dietetics to award him the “Emerging Dietetic Leader Award” in 2020. Dr. Wiss has spent over 10,000 hours conducting one-on-one nutrition counseling to clients and families both in-person and virtually. Clarissa Kennedy, RSW, is a Clinical Social Worker and the founder of Reinvent Your Bliss Point. She has a diploma in Professional Addiction Studies and is a registered member of the Ontario Association of Mental Health Professionals. She has over 14 years of experience working with adolescents, adults and families in a wide range of treatment settings, including residential treatment, outpatient services, and private practice. Molly Painschab, LCPC, LAC, is a dual-licensed mental health and addiction counselor and has worked in the field since 2005. She is co-chair of the Sugar & Food Addiction Professional Network. Michael Walsh speaks with Dr. Antonello Bonci on today's episode. Dr. Bonci is the President, Chief Scientific Officer & Partner at GIA Chicago. Before moving to GIA Miami, he served for nearly a decade at the National Institute of Health (NIH) as Scientific Director of the National Institute on Drug Abuse (NIDA). Between 1999 and 2010, he was Professor-in-Residence in the Department of Neurology at the University of California, San Francisco (UCSF), and held the Howard J. Weinberg Endowed Chair in Addiction Research while also serving as Associate Director for Extramural Affairs at the Ernest Gallo Clinic and Research Center. Over the last 5 years, Dr. Bonci and his collaborators worldwide have successfully pioneered the translation of non-invasive brain stimulation such as Transcranial Magnetic Stimulation (TMS), from the preclinical stage to the clinic, to treat cocaine use disorders and other types of addiction.

The discussion revolves around ibogaine, alkaloids, and addiction therapy solutions.   Show Notes: Dr. Benjamin Malcolm discusses psychedelic alkaloids that have potential to treat                addictions. When conducting human subjects research, it’s a good idea to at least    run it past an IRB. There are risks involved in taking in ibogaine that can be used to treat addiction. For people age 18-24, opiates are a major cause of death. Holistic House teaches addicts heath habits for treatment. Ibogaine is still an unregulated area. 2CB haven’t had that many research studies. Surveys tend to have a bit of bias, often given to supportive subjects to begin with. Mescaline, San Pedro, and peyote appears to lack research. There is going to be a need to potentially or switch between traditional therapeutic   modalities and psychedelic-assisted psychotherapies.

Adron Harris, director of the Waggoner Center for Alcoho and Addiction Research at The University of Texas at Austin, and his team mapped the differences in gene expression between an alcoholic's brain and a non-alcoholic's brain. They found that, as a person becomes dependent on alcohol, thousands of genes in their brains are turned up or down, like a dimmer switch on a lightbulb, compared to the same genes in a healthy person's brain. The scientists are now using an innovative technique to find drugs that can, in a sense, turn those switches back to their original settings and, they hope, revert an alcoholic's brain into a non-alcoholic brain. The work might help the millions of people who suffer with the emotional, financial and health consequences of alcoholism.
