In this episode of the Prolonged Field Care Podcast, Dennis sits down with Alex to dissect a controversial claim that calcium is useless for hyperkalemia during cardiac arrest. What started as a social media post that "set his hair on fire" led to a full investigation: reading the primary paper, digging into its references, and uncovering how conclusions can get twisted. They break down the study's methods, limitations, and real applicability to austere, prehospital, and operational medicine — including crush injuries, medical CPR on base, and why black-and-white social media takes often miss the gray reality of medicine. Topics include cardiac membrane stabilization, physiologic mechanisms, JTS CPGs, confounders in resuscitation research, and why you should always chase the references.Key Takeaways:Calcium doesn't lower potassium — it stabilizes the cardiac membrane and buys critical time. The referenced paper has significant limitations (small hyperkalemia subgroup, very sick patients, poor outcomes overall) and does not support abandoning calcium.Medicine is gray: knowledge translation lags, and even published papers can overreach conclusions. In austere/prolonged field care, if your teammate is in arrest and it's safe to act, calcium + bicarb is still worth using while addressing reversible causes (Hs & Ts).Always evaluate studies with PICO, look for confounders, and consider functional outcomes (e.g., Modified Rankin Scale) over simple survival.Peak T-waves are unreliable — treat based on mechanism of injury and clinical suspicion.Whether you're a tactical medic, flight medic, or austere provider, this episode sharpens your critical thinking and reinforces why calcium remains in the toolkit.Resources & Links:JTS Clinical Practice Guidelines on HyperkalemiaProlonged Field Care website: www.prolongedfieldcare.orgFresh PFC Coffee & free downloadsSubscribe, share with your team, and stay on the bleeding edge of austere medicine.Episode Chapters00:00 – Intro & Welcome00:45 – The Social Media Post That Started It: “Calcium is Useless”02:18 – Knowledge Translation Lag & Gray Areas in Medicine06:05 – Why Dennis Dug Into the References08:21 – Does Medical CPR Apply to Operational Medicine?11:06 – PICO Breakdown of the Study (Taiwan Retrospective Review)15:50 – Better Outcomes Than Just “Dead vs Alive” – Modified Rankin Scale17:39 – Study Results: ROSC, Survival, and Neurologic Outcomes21:08 – Physiology: How Calcium & Sodium Bicarb Actually Work in Hyperkalemia28:52 – Field Recognition of Hyperkalemia (Crush Injury, Relative Bradycardia)31:58 – JTS CPGs, Treatment Thresholds (K+ >6.5), and Why Labs Aren't Everything35:40 – Very High Potassium Levels & Prognosis in Prolonged Arrest39:20 – Final Thoughts: Don't Abandon Calcium Based on Weak EvidenceFor more content, go to www.prolongedfieldcare.orgConsider supporting us: patreon.com/ProlongedFieldCareCollective or www.lobocoffeeco.com/product-page/prolonged-field-care

We continue our series on norms in the early church with "The Word." The Word (or a synonym for the Word) is mentioned at least once in every chapter in Acts 1-20. The Word was increasing; the Word was preached boldly, broadly, and consistently; preaching the Word was the main work of sent-out-ones. Whatever we are doing, let's saturate it with the Word. Three Themes on the Word in Acts: The Word of God increased and multiplied – in and out of Jerusalem, Samaria, Ephesus, and surrounding regions. The Word is powerful and has a power of its own, almost like the HS. Examples: Acts 6:7: And the word of God continued to increase, and the number of the disciples multiplied greatly in Jerusalem . . . (Jerusalem) Acts 12:24: But the word of God increased and multiplied. (Samaria and beyond) Acts 19:20: So the word of the Lord continued to increase and prevail mightily. (Ephesus) This should encourage us and build up our faith in the inherent power of the Word, apart from our own abilities! The Word went out broadly and boldly, publicly, and consistently. Many examples of public, bold proclamation (4:13, 29, 31; 9:27, 28; 13:46; 14:3; 18:26; 19:8; 26:26; 28:31). Paul and his teammates (missionaries) had an extremely high level of commitment to preaching and explaining the Word. This is what Paul did in every place, day after day, for hours, in different contexts. It's very clear in Acts 13-20 – preaching the Word was the work that Paul did. He was persuading and proving that Christ was Lord. He explained the Word with the goal of people responding. 3 Suggestions for application: Make sure everything we do is saturated with the Word. Always make the Word of God your main content! Not your own experience. Yes, you need to connect with your audience and teach in a way that they understand, but you don't have to be super creative. Get them connected to God's Word. Tools: We have tools now that put God's Word into people's hands more easily than at any other time in history. Do a heart check. Paul's heart was very involved. Is my goal to just share the gospel so I can feel like I'm doing my job? Or is my heart's desire to see people believe? To convince them to completely change their minds? Am I content to just walk away after a 3-minute conversation? When I have the opportunity, am I willing to share with a nonbeliever daily? Or for a complete hour? How do we get the gospel out more? We must always be asking this question. This is not only or even mainly a pulpit ministry. How do we get the gospel out boldly, broadly, publicly, and consistently? How many people in my city are hearing the gospel today? How often am I preaching the Word? What opportunities are right in front of me? Spirit-filling results in bold proclamation. If you want Spirit-filling, then put yourself in positions of preaching the Word, and the Holy Spirit will be more likely to show up! What does it really mean to become an expert in resuscitation and critical care? It is tempting to think that expertise comes from accumulating enough facts, passing enough exams or simply spending 10,000 hours at work. In this episode, Iain Beardsell is joined by emergency physician, intensivist and medical educator Sara Crager to explore why expertise is less about how much we know and more about how we think. Sara explains how experts develop high-quality mental models that allow them to organise information, recognise patterns and approach difficult clinical problems. Crucially, these mental models do not have to remain hidden inside the heads of experienced clinicians: they can be identified, explained and deliberately taught. The conversation moves from the limitations of mnemonics and assessment-driven education to the value of deliberate practice, feedback and safe failure. Sara describes how an expert might organise the differential diagnosis of cardiac arrest into respiratory, haemodynamic and metabolic problems, rather than relying solely on a memorised list of Hs and Ts. Iain and Sara then discuss Rapid Sequence, the gamified clinical-learning platform Sara created with emergency physician Ryan Ernst. Learners work through realistic cases in a simulated clinical environment, managing several patients while dealing with interruptions, competing priorities and the consequences of their decisions. After each block, Sara and Ryan deconstruct the cases, make their clinical reasoning explicit and introduce mental models that learners can immediately apply when they try again. It is a cycle of practice, failure, teaching and repetition—without putting a real patient at risk. They also explore why attention, storytelling and visual design matter in medical education; how “multitasking” may be better understood as rapid task switching; and what Sara has learned from turning an educational passion project into a working product. In this episode Why expertise is about cognitive strategies and mental models—not simply knowledge Why experts are made rather than born The limitations of the “10,000-hour rule” How deliberate practice differs from repetition When learners are ready to be taught expert ways of thinking Foundational knowledge versus clinically useful organisation Moving beyond mnemonics such as the Hs and Ts How experts can make their implicit reasoning explicit Why acquiring a new mental model can produce a sudden leap in performance The importance of inspiration—and giving learners an achievable pathway How Rapid Sequence creates a safe place to make mistakes Managing several patients, interruptions and cognitive load Teaching shock, respiratory failure and acid–base physiology Why engaging design is part of the educational method The role of games alongside podcasts, lectures and clinical experience Reframing multitasking as rapid task switching The “pause and bookmark” technique for managing interruptions The realities of building an independent medical-education project Why partnership, persistence and a genuine belief in the project matter Learning from podcasts? Send us Fan Mail16-year veteran math teacher, Noah started his journey in education through ACE, The Alliance for Catholic Education, at the University of Notre Dame. During this program, he moved from Indiana to Florida...making less than 25k as a catholic school teacher. After completion, he moved home to suburban Minneapolis to teach HS math at his Alma mater. While teaching, Noah also began side hustling as a black jack dealer at a local casino, and as a DJ... all while keeping rent low, living with 5 buddies in a multi-family housing unit. Home was great, but adventure called. Soon, Noah was off to teach internationally in Vietnam, where he met his now wife who is also an international educator. Low cost of living in Vietnam and the lessons learned living off of less than 25k in Florida enabled Noah to save and invest thousands. Expecting their first child this summer, Noah and his wife have realized a lifelong dream to life in Europe and are both teaching in Prague. The Church PT1.What comes to mind when you hear the word "church"? Many people think of a building, a denomination, or a Sunday event. But the Bible teaches that the church is much more than a place we attend—it is a people God has called together for His glory. In this first lesson, we discover that the church was not man's idea but God's eternal plan. Before Bethlehem, Calvary, or Pentecost, God already envisioned a redeemed people who would belong to Him. The church was established by Jesus Christ, purchased by His blood, and empowered by the Holy Spirit to carry the Gospel to the world.Acts Chapter 1 reminds us that Christ left His followers with a mission: to be His witnesses everywhere. The church exists to worship God, fellowship with believers, grow through discipleship, serve others, and proclaim the Good News of Jesus Christ. We learn that the church is the "Ekklesia"—the called-out assembly of believers. It is both the universal body of Christ and the local congregation where believers worship, pray, learn, and serve together. Through the power of the Holy Spirit, people from every background become one family in Christ. This lesson also highlights the importance of Christian fellowship (koinonia), where believers share life together through love, prayer, service, giving, encouragement, and even suffering. The church is not a spectator event but a spiritual family on mission for God.The ordinances of Baptism and the Lord's Supper remind us of Christ's death, burial, resurrection, and our commitment to follow Him. They are visible testimonies of the grace we have received through Jesus Christ. The church is God's people, established by Jesus Christ, empowered by HS.

Josh Berman was an Ivy Leaguer on his way to becoming a professional skier when a catastrophic knee injury derailed his chances of becoming pro. But Josh's passion for skiing was as strong as ever and pre-surgery, Josh strapped up his leg, headed up the mountain and went out and shot for his first Level 1 Production. What was supposed to be a 3-year adventure in filmmaking turned into an award-winning 25+ year career in snow. In this Episode of the Viral Volley Podcast, ascending pro volleyball athlete Alaina Chacon jumps after she and partner Maddie Anderson go 2-0 at the opening weekend of the AVP League in Belmar, NJ. Before that weekend, Alaina has always been one of those "watchlist" athletes from her days in HS indoor, to Florida St., and now the Beach Pro Tour and the AVP. One of three tallented sisters, Alaina has been a switch-blocker on the beach but has also been a lightning quick defender with a full toolbox of attacks-- that include a very heavy hammer. This week she reflects on her volleyball beginnings, getting discovered by coaches Hector Gutierrez of TCU and Brooke Niles at Florida St. After finding success at the collegiate beach game she ventured into coaching and eventually her pro career where she has continued to find success at both the BPT and AVP! How she had to find a partner rather quickly at the start of the 2026 season being that sister, Morgan was carrying her first baby for Auntie Alaina! What did she do? Did she "...slip into Maddie's (Anderson) DMs?" Nope. She just texted her :). Listen in!
In this episode of Derms and Conditions, host James Q. Del Rosso, DO, welcomes back Michael Payette, MD, a dermatologist in private practice in Connecticut, for a wide-ranging discussion on the realities of managing hidradenitis suppurativa (HS) in clinical practice. Drawing from his experience running a dedicated HS clinic, Dr Payette reflects on what surprised him most after launching the practice, including the number of early-stage referrals and the growing recognition that even patients with relatively mild disease may benefit from more proactive care. The conversation highlights the importance of identifying HS earlier in its course, before patients develop irreversible scarring, sinus tracts, and other long-term sequelae. Drs Del Rosso and Payette discuss the logistical and emotional complexity of caring for patients with HS, emphasizing that meaningful visits cannot be compressed into a few minutes. Dr Payette shares how he structures conversations around realistic goal-setting, long-term therapeutic partnerships, and individualized treatment “buckets” ranging from topical and systemic therapies to lifestyle considerations, clinical trials, and surgery. The discussion also explores the psychological burden of HS, including how issues such as intimacy, work absenteeism, and social isolation often shape the patient experience as much as the physical disease itself. They also examines evolving treatment strategies for HS, including the challenges of managing patients in the mild-to-moderate range, the rationale for early intervention, and the growing role of combination therapy. Dr Payette shares insights on combining biologic and JAK inhibitor therapies in select patients, integrating GLP-1 agents into treatment plans, and balancing medical and surgical management to optimize outcomes. Throughout the discussion, both physicians emphasize the importance of empathy, collaboration, and helping dermatologists feel more comfortable initiating treatment for HS, even if they ultimately refer more advanced cases to specialized colleagues. Tune in to the episode to hear insights on building therapeutic relationships with patients with HS, approaching multifactorial disease management, recognizing opportunities for earlier intervention, and navigating the therapeutic landscape for this historically underserved disease state.

I found myself in tears looking at the signatures on the paintings of Helene Schjerfbeck at her show at The Met. There was one where her name loomed very large in the corner. It was "H. Schjerfbeck" but still, it was bold and it was her name on the painting. Most of her work was signed with a discrete "HS" or not at all. So I was moved to see this a woman sign her name with verve, her signature hung right at eye level. To read Sign Your Name visit the Songs for the Struggling Artist blog.
