MYCIN. This is the name of the very first AI program developed in the medical field at Stanford University in the 1970s. Its goal was to recommend treatments to physicians for certain infectious diseases. Although this program was only live for a couple of years, it opened the door to greater innovations and breakthroughs. Today, AI helps doctors make diagnoses, it aids surgeons in defining the “next best step” for their patients and it assists researchers when screening new drugs. But the road to a fully AI-driven healthcare system is still long and tortuous. The MYCIN program was dropped because of low acceptability by physicians and poor integration into their daily practice. Sound familiar? If it does, that's because these challenges still exist today, over 50 years later. The idea was for a human examiner to evaluate the discussion between a human and a machine without knowing which was which. Two main movements emerged after the creation of the test: those advocating for a strong artificial intelligence, capable of human thinking and those advocating for a weak artificial intelligence, capable of helping humans in their tasks. one of the first and most famous experiments regarding artificial intelligence in healthcare is SPHINX, a program designed to help doctors identify patients with jaundice, which is a yellow pigmentation of the skin in people suffering from liver conditions. There are a growing number of applications of AI in healthcare: algorithms capable of identifying tumors on X-Rays, AI to help doctors interpret ECGs. During Covid, AI was used by hospitals to predict the incoming flow of patients and adapt their activity. The same technology was used by governments to anticipate the number of masks or vaccines that should be ordered depending on the evolution of the situation. This week, we're still getting our heads around coronavirus. We're newly locked down in the UK, we're all getting used to remote working and as we heard on the last episode with Dr Aman Gupta, the front line is preparing in almost military style for what is going to be a war on coronavirus and a huge influx of patients. To talk more about the clinical and technology implications, I have Dr Lydia Yarlott with me. Lydia is a community paediatrician and founder of the communication app Pando - a communication tool for health and social care that essentially gives clinicians a replacement for Whatsapp with some extra functionality making it ideal for primary, secondary and tertiary care. This week, James is joined by Neil Laycock, Managing Director of technology giant Servelec's healthcare division - one of the largest providers of healthcare systems in the UK. Typically this is its Rio electronic patient record (EPR), but it also extends to bed management solutions, designing interfaces for other healthcare systems and mobile solutions for clinicians - but by far at the top of the agenda for Servelec is interoperability. The ability for healthcare systems to be able to share data, information and be able to 'talk' to each other, is one of the primary concerns for NHSX, and will greatly improve patient care and experience, yet it still hasn't been widely adopted - data protection makes it extremely difficult. Servelec are working hard to tackle this and it has an interoperability platform, but there is still much work to do for them and for the sector as a whole, in order for easy sharing of patient data across more than one system. Why do humans ignore their limitations to climb Everest, run multiple ultramarathons to their death? Why do people bring food to funerals? What happens when you measure heart rate variability of French bankers during Brexit? This week, James is joined by David Plans, cofounder and CEO at BioBeats, an artificial intelligence company dedicated to understanding and promoting human wellbeing through preventative mental health. David describes himself as a psychology researcher and entrepreneur - his initial doctoral used AI to create adaptive media systems that imitated and queried the nature of human creativity. He's given papers and performances at SXSW, International Computer Music Conference, the European Conference on Artificial Life, IRCAM, the Darwin Symposium, and the Computer Arts Society in London. David is also currently pursuing a DPhil in cognitive neuroscience at the Social Cognition Lab, University of Oxford, with Geoff Bird. Interestingly, David's early research focused on computational intelligence approaches to understand and classify emotion in music making, which forced him to examine the psychophysiology of play, flow and stress. Having built technology (apps+sensors) that attempts to measure stress and interoception in neurotypical adults, he is now investigating the influence of interoceptive awareness on stress, as well as aspects of organisational behaviour surrounding empathy and vulnerability. How do you build an AI company in healthcare? What can AI in radiology actually do? What should it be doing? Are clinicians ever going to be replaced? What does the future look like in healthcare AI? This week, James is joined by Dr. Hugh Harvey, who was a consultant radiologist before entering academia and completing an MD research degree in computational analysis of prostate MR, and winning science writer of the year twice in a row. How does a night at the pub give you an idea for the first ever AI drug discovery company? Can you code how humans perceive beauty? Do you want the job title ‘Drug Hunter' for your CV? Well, this week, James is joined by Prof. Andrew Hopkins, a leader in using AI in the hunt for medicines who will answer all of those questions. A bold claim, but one that can be backed up: Andrew invented the Artificial Intelligence approach to fix the very expensive, very slow, and very risky process of drug discovery, design and development. Can AI diagnose skin cancer as well as a dermatologist? The answer is yes and it's was proved this month by my guest this week, Neil Daly from Skin Analytics. It's a UK based healthtech company whose DERM product featured in JAMA, The Journal of the American Medical Association for its ability to catch melanomas as sensitive and specific as a dermatology specialist. His journey to entrepreneurship started with a misdiagnosis during his PhD in Lyon, meaning he underwent fertility testing thwart with problems and inaccuracies. Mojo applies artificial intelligence to semen analysis, which is used to determine a man's fertility. Today, diagnostic semen analysis is a very time consuming manual process requiring a highly trained professional.
