In this episode, we discuss the role of interprofessional teams within pre-hospital care and examine the latest research published on this topic. Over the past three decades, the role of physicians in pre-hospital care has grown significantly. In this episode, we are joined by Dr Mike Christian. Beginning his career as a paramedic, Mike is now a critical care physician, researcher, and educator in PHEM. He specialises in interprofessional team care for critically ill patients in Canada. He has held leadership positions in PHEM and hospital systems in the UK and in Canada. Mike's research demonstrates the transformative impact of inter-professional teams, which bring together physicians, paramedics, and other specialists to enhance care delivery and improve patient outcomes. This episode explores the evidence behind these teams, shedding light on their ability to reduce mortality and improve survival for critically ill and injured patients. In this episode, we will examine the fundamentals of training and education of high-performing teams. We discuss the role of real-world scenarios and high-fidelity training simulations in better-preparing trainees. We'll also discuss the role of high-fidelity simulation in enhancing the learning experience and the metrics used to evaluate training effectiveness. We will share strategies for continuously improving training programs and offer take-home points on harnessing those crucial one percent improvements in training.   To do this we have Dr. Clare Richmond with us. Clare is a Retrieval Specialist and the Director of Emergency Medicine Training at Sydney HEMS. She also serves as a State Retrieval Consultant for NSW Ambulance and as an Emergency Physician VMO at Royal Prince Alfred and Canterbury Hospitals. A graduate of Sydney University, Dr. Richmond trained in Emergency Medicine at Royal Prince Alfred Hospital and completed a fellowship at the Sydney Clinical Skills and Simulation Centre. She has taught and worked in PHEM in diverse locations including Sydney, Alice Springs, and London. We also examine the diversity within the UK context and how this has slowly but surely grown and affected the workforce for the better within pre-hospital care. In this episode we will look at the recent changes to the newly revised second edition of ABC of Prehospital Emergency Medicine. We will examine some of the updates to PHEM covered in the book and the recent evidence-based practice and expert opinion that encompasses the PHEM training curriculum. We will examine the three main domains that the updated edition addresses which are operational, clinical and system considerations of PHEM. Other aspects of the conversation will cover updates to monitoring and ultrasound, recent changes to prehospital management of medical, trauma and psychiatric emergencies. We will also examine changes to the considerations in mass casualty and chemical, biological, radiation, and nuclear incidents. Clare Bosanko is a PHEM consultant with Devon's Air Ambulance and trained in the Midlands. Clare also works as an Emergency Medicine consultant at University Hospitals Plymouth. She is also a co-host of the PHEM-Cast podcast and a contributor to the new edition.

Rose is now advocating for people to get tested and also shared how practicing meditation helped to get through it. In this conversation, we will examine the utility and functions of ultrasound within air ambulance services. We look at how the aggregation of data and familiarity with ultrasound is narrowing down and assisting time to intervention and time to definitive care. We look at the scans of preference and some of the data around pneumothoraces, tamponade and positive findings under ultrasound in the critical care patient. Importantly we will get Dan's perspective on where ultrasound is affecting patient care and where he sees the future of ultrasound going within pre-hospital care.    Dan Nevin is a PHEM consultant with London's Air Ambulance and an Anaesthetist with Barts and the Royal London NHS Trust. Dan has years of experience working in pre-hospital care in both South Africa, where he originally trained, and in the UK. His special interests include trauma, critical care anaesthesia and PHEM. Dan is also the ultrasound lead for LAA and has been leading the use of POC ultrasound within the service. In the conversation we exam: Why ultrasound has become a useful tool in trauma. Data collected on scans and relationship with intervention (surgical or otherwise). In this episode I am speaking with Debs Swann on her recent deployment to the Turkey Earthquake disaster. It is the deadliest earthquake in what is present day Turkey since the 526 Antioch earthquake, making it the deadliest natural disaster in its modern history. As of 20 March 2023, more than 57,300 deaths were confirmed: more than 50,000 in Turkey, and more than 7,200 in Syria. In Turkey alone there were at least 50,096 deaths and 107,204 injured across 11 provinces of Turkey. By 23 February 2023, the Ministry of Environment, Urbanization and Climate Change conducted damage inspections for 1.25 million buildings; revealing 164,000 buildings were either destroyed or severely damaged. A further 150,000 commercial infrastructure were at least moderately damaged. At least 15.73 million people and 4 million buildings were affected. About 345,000 apartments were destroyed. More than 2 million residents in the affected provinces were evacuated to nearby provinces including Mersin, Antalya, Mardin, Niğde and Konya. More than 20 percent of Turkey's agriculture production was damaged. The United Nations said crops, livestock, fisheries, aquaculture and rural infrastructure were also heavily damaged. The international organisation for migration estimated about 2.7 million people were made homeless. In the conversation we examine her anecdotal experience of deploying with the UK international Search and Rescue Team near the cities of Nurdağı and Gaziantep in Gaziantep Province, just outside the regional capital. We examine the reflection on cases she witnessed, the pattern of injuries, the use of search and rescue dogs, the hot and cold brief of the group, the near death experiences that she had and much more. Debs Swann is an Advanced Clinical Practitioner (ACP) working in the Cambridge area, she is also a PHEM practitioner working with the Birmingham care teams. Debs is also a World Extreme Medicine faculty member and is an active member of the UKISAR team. In the episode she reflects on her time both within the deployment and her reflections afterward. She also reflects on her near death experiences and how that translates to her perspectives on time horizons. My thanks to Debs for this candid and honest interview and her overall insights.

She is an 'FY5', taking a slightly circuitous route to qualifying in Emergency Medicine via the CESR pathway, taking time out wherever possible to do what she loves undertaking expedition medicine roles which have included working as chief medical officer on worked as chief medical officer on board Tall Ship Pelican of London, sailing from Germany to Tenerife, and as a trail medic supporting ultramarathons in Wales and the Azores islands. When she's not working, she's out playing in the mountains hiking, running, climbing and kayaking, usually accompanied by her trusty welsh collie Foxy. She also volunteers with her local mountain rescue team, North East Wales Search and Rescue. She has completed her Mountain leader training and is also a qualified Elementary Paragliding Pilot.

In this interview we will examine the emerging research priorities within pre-hospital care from the perspective of priority and importance. I will be speaking with Sarah McLaughlin – a senior research fellow with affiliations with Essex & Herts Air Ambulance (EHAAT) and Anglia Ruskin University. I will also be speaking with Lisa Ramage, Lisa is a PHEM trainee in the East of England and the chair of PHOTON. PHOTON is a group of non-consultant grade, pre-hospital, doctors with an ambition to expand the quality and reach of research within pre-hospital care. In this conversation we are going to examine a national Delphi study which is defining the national research priorities within pre-hospital care in the contemporary climate. We will also: Examine what is a Delphi study Why is it an important time for empirical research and audit at the moment within pre-hospital care Sarah's EHAAT Delphi study as an analogue of the national PHOTON study The process of a Delphi study in terms of rounds and refinement of the questions. How does it achieve consensus? What are the advantages to a Delphi study? Intubation is a key part of advanced airway management. Although some of you out there may not intubate, we'll be covering aspects where the identification of the need for intubation and how contributing as a team to the process can make a real difference to patient outcomes. Intubation is subject of a considerable amount of evidence and debate. Increasing use of supraglottic airways both in theatre and in cardiac arrest creates a situation in which there are limited opportunities to train and learn the skill. This brings into question who should these limited opportunities to train go to, what defines competence, which patients now would benefit from intubation. In this episode we're going to cover these topics and more, including talking through how to fine your intubation technique as much as possible. We're coming at this episode with our collective neonatal, ED and PHEM practice which all involve advanced airway management and it's fair to say that we're all passionate about delivering intubation and advanced airway management to the highest level possible.  We hope this episode gives a further opportunity to consider the topic in great depth and reflect upon how we can all contribute to improving practice. Décrit comme un "géant de l'Islam". Il a été considéré comme l'un des plus grands muhaddith de l'ère moderne. Après avoir terminé sa mémorisation du Coran et ses études fondamentales, il a commencé ses études religieuses à plein temps sous la direction du cheikh Muhammad ibn 'Abd al-'Aziz al-Mutawwa' et du cheikh Ali al-Salihi à Unayzah. C'étaient les deux enseignants que le cheikh 'Abd al-Rahman al-Sa'di avait nommés pour instruire les étudiants débutants. Après un an d'étude sous ces deux professeurs, al-Uthaymeen a commencé à étudier sous cheikh 'Abd al-Rahman al-Sa'di en 1945 et a continué à être son élève jusqu'à la mort d'al-Sa'di en 1956. Together with American Songwriter and Sean Ulbs of The Eiffels, we had the pleasure of interviewing Phem over Zoom audio! For Phem, this self discovery has paid off. Over the past couple of years she has both been featured on and co-wrote songs with a slew of artists including G-Eazy, Machine Gun Kelly, Lil Tracy and Iann Dior. She has toured the world supporting artists such as Lil Xan and Grandson. Phem has also become a regular contributor in John Feldmann's camp, writing a song on The Used's Heartwork and most recently Jxdn's single 'So What'. It was her own VACUMHEAD EP released last year that racked millions of streams and gained her a cult following of teens inking her name on their limbs. And somehow she accomplished all this while remaining completely independent. We were kindly invited to the Faculty of Pre Hospital Care Annual Scientific Conference to give a talk on the top 10 papers over the last year. Sadly, but completely understandably, the conference had to be cancelled due to COVID-19. In this podcast we cover the talk which is based on 2 cases that are common to PHEM;a major trauma and a cardiac arrest, and discuss how we can optimise outcomes for our patients in view of the recent evidence on these areas. Are we Placing mCPR Devices too Early?

First, there are two proposed mechanisms of CPR, brilliantly summarized in this paper: Cardiac Pump Mechanism: “blood is squeezed from the heart into the arterial and pulmonary circulations, with closure of the mitral and tricuspid valves, preventing retrograde blood flow, and opening of the aortic and pulmonary valves in response to forward blood flow. Air is thought to move freely in and out of the lungs, so that the intrathoracic pressures do not significantly rise and the pulmonary circulation is not adversely affected by chest compressions. With the relaxation of chest compression, the heart fills with blood and air passively returns to the lungs.” Thoracic Pump Mechanism: “With each chest compression the intrathoracic pressure rises because of the collapse of the airways; the thoracic pump theory. This theory presumes that the rise in the intrathoracic pressures results in collapse of the pulmonary airways, thereby reducing the movement of air out of the lungs and reducing the size of the intrathoracic structures, but not necessary equally. The collapse of venous structures at the thoracic inlet was postulated to prevent retrograde venous blood flow and with each relaxation of chest compressions, the intrathoracic pressure falls with return of venous blood.” It is likely that both mechanisms are at play: …In patients with an average chest configuration and those with so‐called “barrel chests,” secondary to emphysema or other causes, the lateral chest roentgenogram often shows a significant distance between the anterior chest‐wall and the heart. In such patients it is nearly inconceivable that sternal compressions of the chest during CPR could result in cardiac compression. Rather, the mechanism of blood flow from chest compressions is probably secondary to the rhythmic alterations of the intrathoracic pressure and releases, for example, the “thoracic pump” theory. Is there any evidence that M-CPR Devices improve outcomes – since they’re marketed as “life saving devices”? A Meta Analysis from Gates et. al. concluded: Existing studies do not suggest that mechanical chest compression devices are superior to manual chest compression, when used during resuscitation after out of hospital cardiac arrest. However, if there’s no difference in survival, and it’s not WORSE than manual CPR, why not use it to cognitively offload tasks? Because, it may be worse. Gonzales et. al. compared “pit crew” resuscitation with “scripted” mCPR implementation and found: In this EMS system with a standardized, “pit crew” approach to OHCA that prioritized initial high-quality initial resuscitative efforts and scripted the sequence for initiating mechanical CPR, use of mechanical CPR was associated with decreased ROSC and decreased survival to discharge. Why might this be the case? We know based off work by Hwang et. al. who showed that standard CPR (inter-nipple line) often results in compression or narrowing of the LVOT or the aortic root. In this study, the area of maximal compression was over the aorta in 59% of patients! In another study, anderson et. al. used transthoractic echo to mark the location of the aortic root and the left ventricle of animals, and randomized them to receive CPR at one of the two locations. As you can probably guess, aortic systolic and diastolic pressures as well as ETCO2 were higher in the LV group, and 9 of the LV group (69%) achieved ROSC and survived at least 60 minutes compared to none who received chest compressions over the aortic root.  All of these studies and more are explained in a wonderful video created by Felipe Teran: The folks at The Ultrasound Podcast also discuss using TEE to guide hand or device palcement for CPR: TEE to save lives, guide compressions, and guide interventions Pt 1. #FOAMED.  p.s. – checkout cabofest2018.com  But, how do we do we know that we’re compressing the LV without TEE? Well, we don’t exactly. However, Qvigstad E et. al. published a study in Resuscitation titled “Clinical pilot study of different hand positions during manual chest compressions monitored with capnography.” They compared how hand positioning at the inter-nipple line (INL), 2 cm below the INL, 2 cm below and to the left, and 2 cm below and to the right affect ETCO2. They found that there’s not “one superior hand position”, and that optimal positioning varies: How does this explain when we should place mCPR? It doesn’t really, but one argument against mCPR, specifically one based off of the cardiac pump mechanism, is that the device is consistent and doesn’t fatigue, yet this might be it’s downfall. It’s postulated, and demonstrated in the above videos that it may just be consistently compressing the aortic outflow tract, and not the left ventricle.  Are we applying mCPR too early? Poole et. al discuss this in a paper titled: Mechanical CPR: Who? When? How? In their paper they discuss how the device is frequently deployed early, even before defibrillating the patient. Others In clinical practice, published literature reports marked variability in the hands-off time during device deployment, with pauses in excess of 1 minute being reported. This podcast is based on the Focused echocardiographic evaluation in life support and peri-resuscitation of emergency patients (FEEL) Study, and inferences made from it. The study was to determine the feasibility of prehospital ultrasound, but there were more astonishing results: 74.5% of patients in (pseudo) PEA had cardiac activity. 35% of patients in (suspected) asystole had cardiac activity.  We brought on EMS Physician Walt Lubbers to answer whether or not ultrasound would inform our decision making or alter how we manage a patient in cardiac arrest.   What we really wanted to know is 1) should we shock asystole "just in case" it could be very fine VF, and 2) would the presence of cardiac activity on ultrasound change how we manage a patient in cardiac arrest.  We discussed in detail some cardiac arrest physiology and referenced a video lecture and studies cited by Dr. Peter Kudenchuk from the Resuscitation Academy.

Terminology Wet, dry, or near drowning are not medically accepted terms and should not be used. There is nothing “near” about drowning. It happened or it didn’t. Drowning is: “the process of experiencing respiratory impairment due to submersion or immersion in liquid.”   Drowning has three outcomes. This is a uniform way of reporting data after a drowning event is using the Utstein template: 1) Morbidity 2) No morbidity 3) Mortality Submersion or immersion incident without evidence of respiratory impairment is considered a water rescue and not a drowning. Pathophysiology A person holds their breath until water enters the mouth. This water is voluntarily spat out or swallowed. The next conscious response is breath holding which lasts around one minute. The inspiratory drive becomes to strong and some water is aspirated into the airways (reflexive swallowing). Laryngospasm occurs, but is rapidly terminated by the onset of brain hypoxia. Aspiration continues, and hypoxemia leads to the loss of consciousness and apnea. The initial cardiac rhythm is tachycardia, then bradycardia, then asystole. - Adapted from Szpilman et. al.  On average, less than 30 mL of fluid or NO fluid enters the lungs.  Clinical presentation Water washes out surfactant = frothy pulmonary edema Cellular Injury – alveolar collapse leads to atelectasis Hypoxic vasoconstriction due to V/Q mismatch Bronchospasm Inflamation The main problem is NO OXYGEN TO THE BRAIN. This is a hypoxic cardiac arrest and should be treated as such.  In-water Rescue (If properly trained) “Reach, Throw, Row, Don’t GO” – Don’t become a victim If properly trained, and the patient HAS A PULSE, mouth to mouth resuscitation can result and a 3 times greater likelihood of surviving as compared with taking the person first to land.  CPR will be ineffective Cervical Spine Immobilization  Incidence of cervical spine injuries is 0.5%-5% Without obvious signs of trauma or a known fall or diving event, routine cervical spine immobilization is unnecessary and can delay providing oxygen to the brain. If the patient is awake, follow standard spinal clearance algorithms. Land Resuscitation  Airway, Airway, Airway  Start with basics: Mouth to mouth, mouth to mask, BVM, Etc. Delay placing an advanced airway – restoring oxygenation is more important. Cardiac arrest in drowning is due to hypoxia. There is no place for “hands only CPR” CPR only will only circulate blood with a severe oxygen debt that won’t be restored without positive pressure ventilation. Beware of Acute Pulmonary Edema  Water + Soap = Foam. Possibly lots of it. You don’t need to suction unless vomit or frank water is present. Suctioning foam will keep coming. Bag it down. Use PEEP The Heimlich maneuver No. Little water is aspirated into the lungs, and it only delays the administration of oxygen. If patient is awake and/or has a pulse and is intubated CPAP if not at risk for vomiting If managed via an advanced airway, use an ARDS approach Low tidal volumes: 6-8 cc/kg of ideal body weight Respiratory rate to maintain eucapnea: 16-18 Increase PEEP and FiO2 in tandem to achieve and adequate SpO2 Who should be resuscitated?  Per WMS Practice Guidelines: “Minimal chance of neurologically intact survival with submersion time greater than 30 minutes in water greater than 43 degrees F.” “Minimal chance of Minimal chance of neurologically intact survival with submersion time greater than 90 minutes in water less than 43 degrees F.” When should resuscitative efforts cease?  WMS Recommendation: After 25 minutes continuous CPR What about cold water drowning? A Dutch prospective of children who drowned in cold water showed that no child in asystole, who was resuscitated for more than 30 min survived without being neurologically devastated. Another large study conducted by Quan et. al. involving adults who drowned in cold or very cold waters showed that of 1094 victims, the majority had bad outcomes. Those with good outcomes were likely to be submerged for less than 11 minutes. Brown et. al found that there is a neuroprotective effect in avalanche victims, but not in those who asphyxiated first, then became hypothermic. The mantra “they’re not dead until they’re warm and dead” appears to be overhyped, and more and more data suggests there’s not as much of a neuroprotective effect of cold water drowning as once thought, and that mantra is probably only applicable to the patient who becomes hypothermic before going into cardiac arrest or becoming hypoxic. Disposition of an awake patient Most patients who experienced a drowning incident requiring the need for EMS to be contacted should be transported for evaluation. Depending on where you practice, you may have to make a mission critical decision of whether or not to evacuate or continue with close monitoring of the patient.  Statistically, a person without a severe cough, frothy sputum or a foamy airway, and a normal cardiac examination has a mortality rate of 0%.  The more severe the symptoms, the higher the mortality:

For this episode we’ve been lucky enough to catch a number of the speakers from the traumacare conference. First up, conference organiser Caroline Leech (EM + PHEM consultant) gave us a few minutes of her time to talk about the latest major trauma key performance indicators from NICE. Nicola Curry (Consultant Haematologist) spoke about transfusion in trauma and the use of massive haemorrhage protocols. Importantly, she covers the evidence behind the current strategies and where future research opportunities exist. Stuart Reid (EM + PHEM consultant) covered the ways of optimising timely transfer of major trauma patients. This had an inter-hospital focus, but there were certainly some elements which can be applied to a primary patient transfer. David Raven (EM consultant) provided an update to the ongoing work with the HECTOR project. We’ve previously heard about their amazing course but this time he was able to let us know about the “silver trauma safety net” which is being used by the ambulance service in the West Midlands. This aims to provide appropriate recognition and triage of trauma in the elderly population. Finally, Elspeth Hulse (anaesthetic SpR) gave us a timely reminder about the identification and management of organophosphate poisoning - really useful from both and EM and PHEM perspective. Thanks again to Caroline for the invite to the conference and keep and eye out for a special podcast in the next few weeks where Simon Carley will be running through his top 10 trauma papers of 2017/18 (we were going to try and condense it, but there was way too much good stuff!) phem Talks Growing Up in the Valley, Being an Accomplished Drummer, the Lowest Lows that Led to Major Self Discovery and Performs “Blinders” Live In Studiophem is the latest and most permanent iteration of Liv Marsico's creative alias'. She's been making music for years in the LA scene. Cutting her teeth in the Low End Theory world, I first heard her on Nocando‘s “Little Green Monsters.” At the time she was making waves with her band Liphemra. She also as a hired gun drumming for studio sessions and touring acts. An accomplished jazz drummer, phem went to a creative arts high school where the emphasis was on musicianship.But while working on Liphemra, Liv felt uncomfortable in her own skin, and living space. Renting a dingey recording space, and living illegally in it, her life hit an uncomfortable low. Thankfully a strong support group helped her out of the situation, and she emerged stronger. Now more comfortable with herself, her art, and her sexual identity (“I just like people, man”), phem is ready for her moment. “Blinders” is a fantastic summary and, in turn, elevator pitch of her music and personality.
