In this episode I'll take you on a trip through the historical components behind the idea that Vikings did mushrooms before battle. We will explore the context for why that claim emerged in the 1600s, when the Danish scholar Thomas Bartholin first put it out there, and why it's the kind story that's thoroughly a myth in the sense of something that's entirely made up. After that, I'll offer some comments on the idea that psychotropic agents are helpful for the purpose of reaching a higher self, communing with spirit, ritual, whatever you'd like to call that connection we humans can make with the world around us.
For today's topic, we will be discussing psychotropic medications as they relate to nursing considerations for behavior monitoring, side effect tracking, and care planning for the care of your residents taking psychotropic medications.

When I contacted Ryan Stoyer about coming on the podcast it was with the idea that we would talk about his unique lifestyle and how it allows for him to life a more fulfilled life. I still like that topic, but Ryan had other ideas. After the official BoD instagram invite was sent, his reply was an enthusiastic yes, followed by a suggestion, "how about we go bikerafting?". My response was equally enthusiastic and I gave no thought to the fact that I'd never been bikerafting, or had a raft, or new how to build a route, etc etc. But we had 5 weeks and that turned out to be the just enough to assemble the necessary gear and courage to tackle new adventure territory. The plan was to create a bikerafting route and share our experiences through a podcast. As I learned more about Bikerafting, I learned that there is a lot of interest in this re-emerging sport. The first recorded bikeraft trip was in 1987 by Roman Dial, but it seems to be growing in popularity in recent years. So the focus of the podcast shifted from being about Ryan and our trip, we also tried to provide some information that may help you prepare for your own bikerafting or packrafting trip! We discuss where to get packrafts, what to look for, how they handle, how to pack, how to plan a route, etc. It was a huge learning experience for me and I'm excited to share what I learned. I'm a complete newb and my tips reflect that, which is why it was good to have Ryan who brings more experience. It came in handy on the river and on the podcast. The route my friend Connor Thomas created in NW Arkansas is a real beauty. I will be publishing that route soon!! Unfortunately, you can't plan the weather and we got Cold and Wet every day we were there. So our trip had an extra element of Type 2 fun sprinkled in to keep it interesting. I will let the podcast speak for itself, but I can tell you that every person on that trip had a great time and the ones that didn't have packrafts are all now wanting some. Bikerafting was a little scary to me at first. It doesn't make sense that a 195 lb guy + 55 lbs of gear can float safely down a river on a 5 lb inflatable raft. Call me crazy! Any initial fears I first had were quickly squashed and I found bikerafting quite to my liking, and as Ryan says, "It's a great rest day after a long day on the bike". Not only that, but it opens up so many possibilities both with routes and with scenery. I'm glad that Ryan challenged me to do this. It was barely on my radar, but I'm not one to shy away from a new adventure. I equally enjoyed getting to meet and learn from Ryan. He is a wealth of information on a variety of topics that I find fascinating and meaningful. We cover a lot on this episode! The question of whether, when, and what psychoactive medications may be helpful is both big and ambiguous. Mental distress has always been strongly influenced by cultural filters and subjective perceptions. Whereas a person might once have sought to placate a god, sufferers today may turn to medical management rather than mining their psychological symptoms for meaning. In his autobiography Jung describes his years of mental turmoil—and that they became “the prima materia for a lifetime’s work”; his Red Book documents his encounters with the unconscious in compelling and artistic detail. There is much evidence of the potential suffering holds for self-awareness and psychological depth – and it is also important to acknowledge that judicious use of today’s medications can relieve unnecessary or pointless suffering. No matter where on the spectrum of severity emotional illness may lie, psychotherapy, medication, lifestyle, and relationships can all play a role in recovery and growth. Dream “I am walking toward a large concrete structure with an unbelievably fit, handsome and powerful man. He appears to be my friend although I am envious of his physical attributes. We climb into a small passage that leads to a clearing in which undulating hills descend into a body of fast-moving water. I am immediately struck that there is a goal or intention to swim across this water and scale a flat concrete wall which is about 100 feet up, facing us across the body of water. My companion says, “I've got this,” and “I do things like this all the time.” He jumps in the water at the exact moment I become aware of holes in the wall which then begin to fire cannonballs. I rush back to where we began to avoid the cannon barrage, but also (it seems) to “watch” the negotiation of the obstacle. I am now with another man who is soft and slightly overweight. I ask him where the best place to watch this endeavor is and he leads me up a stairwell to a room that contains two old CR TVs. One is large and a smaller one is on top of the larger. The room has a greenish-yellow carpet and it looks very much like the late ’70s or early ’80s. I feel sorry for the man because in that moment I realize this is all he can afford. Next, I am struck with an awareness that the fit man has successfully completed his endeavor – although I did not see it happen. I then become aware that the room adjacent to the one I am in is filled with two groups of women. The first group are sitting at a table conducting what appears to be an executive meeting. The second group are on the floor engaging in a yoga class. My wife is among the women doing yoga and her cousin is among the executives. I suddenly am struck by the realization that I am only wearing a t-shirt and I’m naked from the waist down, and I fear they will discover this.” References Hillman, James. Here is a summary of this episode: In patients with liver impairment, it is better to use drugs that are metabolized by phase II enzymes or drugs that are not metabolized by the liver at all. Patients with severe renal impairment may also have liver impairment. Use ⅔ of the recommended dose. Benzodiazepines and Z-drugs are contraindicated in CO2 retainers and those with severe sleep apnea. Opiates are better than benzodiazepines in patients with terminal breathlessness. In patients who have undergone gastric bypass, use immediate release formulations, or give liquid form of drugs. There is evidence to suggest that IBS-constipation responds better to SSRIs and IBS-diarrhea to TCAs. The risk of GIT bleeding with SSRIs is 1.5 to 2 times greater. Patients with a defect in platelet number or function and those who H. pylori infection are at greater risk.
