On this episode, we are joined by April O'Connell to discuss exercise dosing for patients who have sustained injuries of their upper extremity. She shares with us factors that should be considered when designing an exercise program for a patient and how to progress them through their plan of care to achieve their goals and beyond. Guest Bio: April O'Connell, OTR/L, CHT, ACSM, is an occupational therapist at Cedars Sinai Hospital in Beverly Hills, CA. She works with a variety of sports teams including the NFL, MLB and NBA and recently spoke at ASHT and AAHT on Rehabilitation of the Overhead Athlete. She is one of the main contributors to the fellowship program for hand and upper extremity rehabilitation as well as the co-director for Cedars Sinai Hand Symposium. April got her start as the Clinical Specialist in the Hand and Upper Extremity Therapy Department at NYU Langone Health. She has lectured to orthopedic surgeons and therapists nationally and internationally in current concepts of orthopedic rehabilitation of the upper extremity. Some of her most recent lectures include being a guest speaker for Dr. Radio on Sirius Satellite on topics for rehabilitation of the hand and upper extremity as well as injuries to the overhead athlete. In addition, she has presented on Flexor Tendon Repair Advancement at AAOS in 2015 and was later asked to speak at the ASSH national conference in 2016 and ASHT in 2017. She recently presented her latest research at the ASHT National Conference on flexor tendon rehabilitation in 2019. April is a founding member of the NYU Langone Golf and Pitching Labs and was the head hand therapist for the first successful bilateral hand and face allotransplantation. She earned her Occupational Therapy degree at Boston University, is a Certified Hand Therapist and is certified by the American Council of Sports Medicine (ACSM) as a Clinical Exercise Specialist.

In today's podcast episode, I speak with physical therapist, chiropractor, researcher and educator, Greg Lehman. Greg has been in the rehabilitation field for more than 20 years. Following his undergraduate degree in Kinesiology, he was awarded the Canadian Society for Exercise Physiology Gold Medal for highest academic performance in Kinesiology. This degree allowed him to obtain certification as a Strength and Conditioning Specialist and Certified Fitness Appraiser and to work as a Strength and Conditioning coach for Sir Wilfrid University's Men's Basketball team and for Queen's University Women's Varsity Hockey Team. Greg's success in university resulted in being awarded a graduate scholarship (NSERC) to the University of Waterloo to be one of only two Masters students per year studying at the Occupational Biomechanics Laboratory, a world leader in Spine Biomechanics, exercise prescription and athletic performance. As a faculty member at the Canadian Memorial Chiropractic College, Greg developed a research program that produced more than 20 publications on exercise biomechanics, golf fitness and the science of manual therapy. He has taught more than 25 graduate students in Spine Biomechanics and Research Instrumentation and supervised more than 50 students and 20 undegraduate research theses. He was subsequently awarded a Researcher of the Year award by the Ontario Chiropractic Association. These days, most of Greg's time with patients is spent one-on-one using exercise, load/stress management, manual therapy techniques and education.  Both pain, injury and performance are influenced by a number of factors in an individual's life.  Greg's approach addresses the multitude of these contributors and he primarily works with my patients to come up with strategies where they are actively involved in their recovery. Over the last decade, there's been an increase in the number of people, particularly young adults, who struggle with low moods, distractibility, and anxiety and consequent difficulties with getting their life on track and making progress in work, friendship, and romance.In addressing these difficulties, people are often given or adopt a mental health diagnosis, and look for a solution in therapy and/or medication.My guest isn't opposed to these remedies. She is herself a clinical psychologist who's maintained a practice for a quarter century that specializes in treating clients in their twenties. Physios have been lambasted awhile for not understanding the basics of strength and conditioning principles. I reject this idea.  There two areas we explore in this podcast. 1. The idea that strength and conditioning basics are relevant for rehabilitation 2. Whether physios actually know these basics. I make an attempt at trying to understand the arguments againsts physios and try to provide a competing view.   Here is how the AI summarized it (not really accurate but I don't want to piss off the robots):   Recovery Mechanisms in Rehabilitation Programs Greg discussed the importance of understanding what facilitates recovery in rehabilitation programs, emphasizing that it's not always about building strength. He argued against the judgment of programs based on arbitrary ideas of what needs to change, as the actual mechanism of recovery can vary. He also addressed the concept of tolerance, suggesting that improving a person's ability to tolerate loads can lead to recovery, even without necessarily building strength. Greg rejected the idea that physiotherapists don't understand the basics of strength and conditioning, stating that they do and that the judgement often comes from bias. He also highlighted the need for more research to better understand what mediates recovery. Strength Training Program Design Debate Greg discussed the flexibility and options in designing a strength and conditioning program. He highlighted that the basics are simple and the key is to progressively overload the muscles. Greg also addressed the debate around whether to push to failure during exercises and suggested checking in to see how close one is to their failure zone. He further stated that there's no need to obsess over specific exercises, as the principle of progressive overload applies to them all. Greg concluded by stating that he'd like the debate to shift towards understanding when specific attributes are important and how to achieve them, with a focus on patient-centered care.

The list of things they don't teach trainers – whether in a four-year degree or a certification is long. Consider that there are such a broad wealth of foundational skills a trainer or fitness instructor needs to know and there's generally a specific promise about the time and energy investment required to earn the degree or certification.    We've already got limits on what can be taught. A four-year degree isn't a trade school, it's not a specialty or an intensive training. Many of us know, it's an experience in learning how to learn and realizing there are many moving parts in learning about one topic.    With certifications, we're preparing the fitness instructor, trainer or coach with the minimum viable information to enter the field. That's the instruction provided to us as exam item writers and reviewers. So, with that, the weight you might place on, “Are you certified?” should be taken into consideration.    That said, there are a wide variety of trainings available. Some are a significant investment in time, money, and energy to study and to pass. Others are weekend investments and passing grades come easily. Those that are more rigorous are fewer and further between.    I recall someone once saying, who because a director for university fitness programming, “It's just exercise. It's not that serious.” And yet, to address a special need or specific health issue, it does become more than that. There's a reason chiropractors, functional doctors, and physical therapists now often have physical fitness training programs within their businesses. There's a need for closer supervision, a match for the dose response that often doesn't happen with random drop-ins to a gym or studio.    What We Don't Teach Trainers   1.  Exercise is Stress  a. Cortisol is the energy and the stress hormone. For a body in situational stress or a life phase where the negative effects of stress are going to change, understanding this relationship is key.  b. Adrenal stress can exist even before physical exercise is attempted and the quota and typical exercise recommendations based on minutes, steps, nor intensity should not override the individual's status.  c. The trainer or health coach will be the one to identify this when the client doesn't.  2. What Happens to the Body When Sleep Deprived  a. Reaction skills are similar to that of a mildly drunk individual after a night with 2-hour sleep deprivation. b. Reaction skills are similar to that of what would get you locked up for DUI with 4-hour sleep deprivation. 3. Menopause  Most 4-year degrees in physiology or kinesiology are void of any discussion about menopause. Don't get me wrong, there's a discussion about the side effects of menopause and of PMS for that matter. But when you consider that weight gain, bloat, and belly fat can all be signs for each, and a trainer without understanding why these things happened may go too quickly to a “fat burning exercise or protocol.” This can backfire. We have to know more than what is a sign of something not going right, and know why it is happening before we can determine an exercise protocol that will solve the problem.    Certifications for Trainers Aren't Better  Certifications are very similar. There's nothing – absolutely nothing – about working with women in perimenopause or training her around her cycle, or when it becomes erratic, or she is experiencing:  a.     Hot Flashes & Night Sweats  b.     Belly Fat  c.     Insomnia  d.     Weight gain she can't remove  e.     Pre-diabetes or insulin resistance  f.      libido g.     Or any of the other 34 symptoms directly associated with menopause that can begin to influence how she will most benefit and how she'll be most likely to sabotage herself with exercise.   4.  How to determine the exercise based on the collective of joints, muscles, bones, and hormones.   When you consider the components of fitness, the physiology, the anatomy and joint actions of the human body, and physical fitness it is far easier to understand why a basic degree or a single certification doesn't truly prepare a health & fitness coach to make all the decisions for every individual.    Can they begin to describe basic safety concerns and precautions with exercises, usually yes. That too can depend on the individual. You want to realize within exercises, a range of motion, a placement of elbows, and a stopping or starting point … can change the safety and effectiveness of movements.    Caution with Generic Trainer-Created Exercise Programs   We can't say, a deadlift is a deadlift and it's safe for everyone if it's done like “this.”  We can't say, here's how to do this machine row and be sure that an individual is taught to retract their scapula. Even machine weight use, though often thought of as “safer,” is not necessarily a guarantee one is doing a balanced working based on their body.    For instance, in recent months working with a long-time client we've removed chest press from her workouts. Why? When we think about the benefits of exercise, we tend to think of what it does for our body, making us leaner, stronger, and healthier. But my guest is out to emphasize the powerful effect physical activity has on our brains too, and just how much our bodies and minds are connected.Dr. Jennifer Heisz is a professor, the director of the NeuroFit Lab which studies the effects of exercise on brain health, and the author of Move the Body, Heal the Mind. Today on the show, Jennifer and I first discuss how physical activity can help treat mental disorders. She shares the way that low to moderate intensity exercise can mitigate anxiety, and how short bouts of intense exercise can be used as exposure therapy for treating panic disorders. We also talk about the phenomenon of inflammation-induced depression, and how exercise can alleviate it. And Jennifer shares how exercise can strengthen someone's attempt at sobriety, as well as prevent addiction in the first place.
