Also known as the "darknet" (this latter term is, in fact, more accurate) and primarily accessed through the Tor network, the deep web basically allows access to the 21st-century underground market. It's also an anonymous "playground" for content that's unsuitable for the "clearnet" (the everyday Internet). It is largely untraceable (though not completely) and is perhaps most widely used for the sale of unmentionable goods and services. Beyond drugs, some use the deep web to buy and sell weapons. But perhaps most alarming of all, some use it to exchange or discuss photos and videos of children cryptically referred to as "cheese pizza." (The "C" and "P" heading each word explains this code word.) While these aspects of the deep web are unsettling in and of themselves, the deep web's reputation as a playground for hedonism, depravity, and nefarious or sinister acts is a bit unfounded. Josef Mengele was a great guy! Just very misunderstood. Wait, no. I'm thinking of someone else. Josef Mengele was a MONSTER. His complete and total lack of empathy for anyone not a member of Hitler's nonsensical racial purity dream boggles the mind. He saw his experimental patients at the Auschwitz  concentration camp as nothing more than lab rats who happened to kind of look human. His experiments were cruel and misguided. And mostly dealt with identical twins. Child twins. And saddest of all - his cruelty did not stand out amongst his peers. He was but one of many, many monsters. A product of Nazi brainwashing that had gone on for so long, reality had been inverted. Evil was good, and good was evil in many ways. We look at all of this today. And sorry for the timing! Dr. Caplan is currently the Drs. William F. and Virginia Connolly Mitty Professor and founding head of the Division of Medical Ethics at NYU Grossman School of Medicine in New York City. Prior to coming to NYU, Dr. Caplan was the Sidney D. Caplan Professor of Bioethics at the University of Pennsylvania Perelman School of Medicine in Philadelphia, where he created the Center for Bioethics and the Department of Medical Ethics. He has also taught at the University of Minnesota, where he founded the Center for Biomedical Ethics; the University of Pittsburgh; and Columbia University. He received his PhD from Columbia University. Dr. Caplan is the author or editor of 35 books and more than 800 papers in peer reviewed journals. His most recent books are Vaccination Ethics and Policy (MIT Press, 2017, with Jason Schwartz) and Getting to Good: Research Integrity in Biomedicine (Springer, 2018, with Barbara Redman). He has served on a number of national and international committees including as chair of the National Cancer Institute Biobanking Ethics Working Group; chair of the Advisory Committee to the United Nations on Human Cloning; and chair of the Advisory Committee to the Department of Health and Human Services on Blood Safety and Availability. He has also served on the Presidential Advisory Committee on Gulf War Illnesses; the Special Advisory Committee to the International Olympic Committee on Genetics and Gene Therapy; the Special Advisory Panel to the National Institutes of Mental Health on Human Experimentation on Vulnerable Subjects; the Wellcome Trust Advisory Panel on Research in Humanitarian Crises; and as the co-director of the Joint Council of Europe/United Nations Study on Trafficking in Organs and Body Parts. Dr. Caplan has served since 2015 as a chair of the Compassionate Use Advisory Committees (CompAC), independent groups of internationally recognized medical experts, bioethicists, and patient representatives that advise Johnson & Johnson's Janssen Pharmaceuticals on requests for compassionate use of its investigational medicines. Dr. Caplan is a regular commentator on bioethics and health care issues for WebMD/Medscape, WGBH radio in Boston, WOR radio in New York City, and CNN. He appears frequently as a guest and commentator on various other national and international media outlets. Dr. Caplan is the recipient of many awards and honors including the McGovern Medal of the American Medical Writers Association and the Franklin Award from the City of Philadelphia. He was a USA Today 2001 “Person of the Year” and was described as one of the ten most influential people in science by Discover magazine in 2008. He has also been honored as one of the fifty most influential people in American health care by Modern Health Care magazine, one of the ten most influential people in America in biotechnology by the National Journal, one of the ten most influential people in the ethics of biotechnology by the editors of Nature Biotechnology, and one of the 100 most influential people in biotechnology by Scientific American magazine. During the COVID-19 pandemic, he is co-directing an advisory group on sports and recreation for the U.S. Conference of Mayors, created a working group on coronavirus vaccine challenge studies, developed an ethical framework for distributing drugs and vaccines for J&J, and helped develop rationing policies for NYU Langone Health and many other health systems. He is a member of the WHO advisory committee on COVID-19, ethics, and experimental drugs/vaccines, and he helped set policy for WIRB/WCG for research studies. He was an adviser to Moderna, Inc., and he serves on the NCAA COVID-19 Medical Advisory Group. Dr. Caplan received the Patricia Price Browne Prize in Biomedical Ethics for 2011. In 2014, he was selected to receive the Public Service Award from the National Science Foundation/National Science Board, which honors individuals and groups that have made substantial contributions to increasing public understanding of science and engineering in the United States. In 2016, the National Organization for Rare Disorders (NORD) honored him with its Rare Impact Award; that year he also received the Food and Drug Law Institute's Distinguished Service Leadership Award and the American Society for Bioethics and Humanities' Lifetime Achievement Award. After decades of pursuing public health policies to reduce unintended pregnancies in South Carolina, New Morning President & CEO Bonnie Kapp had a bold idea. What if we made birth control available at little to no cost in every community, in every county, for every person with a uterus in South Carolina, regardless of health insurance coverage? What if we did this against a backdrop of relentless political attacks on reproductive rights and a weak healthcare infrastructure, where 30% of counties have no OB/GYN providers, the average distance to a family medicine practitioner is 37 miles, and 29 of 46 counties in the state are 100% medically underserved? Against these odds, Choose Well was established in 2017. Choose Well works across a network of 119 health centers to provide free or low-cost birth control across South Carolina. In just four years, it has become the largest state-based contraceptive access program in the nation. In Episode 2 of Series 6 Attorney Dominique Day, founder and Executive Director of the Daylight Collective which seeks to fill the space between the status quo and substantive justice with creativity, diverse voices, and multi-sector approaches and understandings talks to Todd about how COVID is negatively and unequally impacting the lives and human rights of Black Americans of African descent. 00.00 – 02.20 Todd begins by asking Dominique to comment on the dis-proportionate impact of the Covid19 pandemic on people of African descent. She points to significant racial disparities in terms of: Who becomes infected Who has access to health care Differences in outcomes in terms of severe illness and death This is seen as an outcome of policies, which exemplify systemic racism at a global and local level. 02.20 – 05.30 Todd asks Dominique which factors she sees as playing a key role in the impact of the Covid19 pandemic.  Whilst racism is not intentional she sees it as being ingrained into the presumptions and actions of individual decision makers In emergency departments this translates to medical bias when doctors are working under stress As evidence she points to research which suggests that medical bias disadvantages people of African descent (and which she discusses in a related webinar) Although the data is widely known her concern is that the issue of systemic racism is embedded in decision making even at the level of the individual clinician 05.30 – 12.40 Todd summarises and points out that the reality is that people of African descent in the USA have a markedly higher mortality rate, which is linked to a long history of systemic racism. Dominque points to “social conditioning” in deciding which lives matter. By way of example she points to the decision to withhold the distribution of the Pfizer vaccine on the African continent and argues that it suggests that this is a decision made along the lines of race In terms of the impact of the pandemic, there are parallels within the fields of education, the economy and health where individuals make decisions on the basis of a bias which reflects systemic racism within society She references an email circulated within NYU hospital in New York where the onus to make rapid life and death decisions was placed on doctors working in the emergency department, without supervision and review. Given the intense stress doctors were under, those decisions were more likely to be influenced by bias (unwitting or not)  Health care providers showed no willingness to discuss the research data, predicting the disproportionate impact on black and brown communities identifying systemic racial bias  individual doctors were prevented from commenting publicly Warnings of racial bias were ignored and continue to be ignored 12.40 – 20.50 Todd moves on to examine differences of outcomes for black and white communities in relation to encounters with the police and references the killing of Michael Brown in Ferguson a suburb of St Louis in the US in 2014. The Ferguson killing follows a common pattern of outcomes for the black community A parallel is suggested with respect to the security preparations made for the Black Lives Matter protests in 2020 in Washington Comparisons have been made between this protest and the insurrection staged by pro- Trump militants. Todd argues that any move to suggest the two events were similar creates a false equivalence Dominique points out that: In terms of policing there was a higher level of perceived threat and a heavier response during the Black Lives Matter protests than for the recent march on the Capitol in Washington Dominique argues that the former was a racialised response conditioned by acceptance of white supremacy and a long history (in the USA) rooted in slavery and exploitation. She references the origins of racial policing in the USA as being to protect property from the actions of slaves. She identifies a “legacy mindset”, a baseline of white supremacy, where white people expect to be treated differently (better) than black people, a mindset which is a major barrier to progressing racial justice and equality. 20.50 – 23.45 The conversation returns to the pandemic and vaccination programs in the USA. Dominique has a number of concerns. Distribution is a major issue More thought needs to be given in terms of prioritising who gets the vaccine first. The role of essential workers, drivers, home helpers who have been disproportionately infected needs to be acknowledged when prioritising vaccination programs. There is also a need to talk about racial equity in the delivery of vaccination programs 23.45 – 26.00 Todd asks why significant numbers of African Americans are resistant to taking the vaccine. In Dominique's view there is a distrust in black communities which in part dates back to the infamous Tuskegee experiment, where black people were exploited in the name of medical science In order to increase the uptake of the vaccine in black communities their must first be an understanding that there is a legitimated scepticism based on historical fact 26.00 - end Todd ends by asking Dominique what she is hoping to see from the new government administration in terms of the issues she has discussed in this episode. In terms of the response to the Covid19 pandemic, she would like to see a critical re-evaluation of responses to the pandemic, and in particular the role of systemic racism and its impact on African American communities.
