In the summer of 2024, the National Institute of Standards and Technology (NIST) released a Hemp Plant Reference Material to help labs accurately measure the amount of THC, CBD, toxic elements and other compounds in cannabis products. It will also help producers and state regulators ensure that cannabis products are safe to use and accurately labeled. Today, we're going to talk about NIST's Hemp Plant Reference Material and about improving cannabis science with leading experts in the field of cannabis medicine. Our guests are Dr. Bryon Adinoff, president of the Doctors for Drug Policy Reform, and Dr. Leslie Mendoza Temple, former Chair of the Medical Cannabis Advisory Board for the Illinois Department of Public Health. Dr. Adinoff is an addiction psychiatrist, neuroscientist, academician, and advocate. He was appointed Clinical Professor at the University of Colorado School of Medicine following his retirement as Distinguished Professor of Alcohol and Drug Abuse Research in the Department of Psychiatry at the University of Texas Southwestern Medical Center and as a psychiatrist for 30 years with the Department of Veterans Affairs. He has published over 200 papers and book chapters on the neurobiology and treatment of addiction and is Editor-in-Chief of The American Journal of Drug and Alcohol Abuse. Dr. Temple is a Clinical Associate Professor for NorthShore University HealthSystem's Department of Family Medicine and Medical Director of the NorthShore Integrative Medicine Program. In the area of Jewish medical ethics brain death is the topic which just will not die. With the advent of technologies and medical developments since the mid-twentieth century, questions about the halakhic definition of death have gone from the largely theoretical (and philosophical) to painfully practical for physicians, patients, and their families. Perhaps no other rabbinic figure's opinion has factored in quite so significantly on the subject as R. Moshe Feinstein zt”l, the preeminent posek of the last century. However, the contours and applications of R. Feinstein's position have been intensely debated. In a recent piece of research that surfaces some relevant new points of evidence, Dr. Noam Stadlan offers a re-understanding of R. Feinstein's ruling, arguing that he defined death as irreversible apneic coma, without regard for the continued presence of heart function. This has various significant implications for end-of-life care and organ donation. Dr. Michael S. Lewis, M.D., is a distinguished orthopedic surgeon and former consultant for the Chicago White Sox baseball team, Chicago Wolves hockey team, and the Chicago Bulls basketball team, with whom won two championship rings. Previously, he served as the chief of staff at NorthShore University HealthSystem's Skokie Hospital and received a teaching award from Rush University Medical College. Dr. Wes Fisher joins the show to continue highlighting ABIM recertification and the MOC program following a petition that has gathered over 12,000 signatures to end this program. Dr. Fisher is a clinical cardiac electrophysiologist at NorthShore University HealthSystem in Evanston, IL; he is also a clinical Associate Professor at the University of Chicago Pritzker School of Medicine. Dr. Fisher is triple board-certified in Internal Medicine (Grandfathered per the ABIM), cardiology, and cardiac electrophysiology. He is due for his FOURTH re-certification in cardiology after 12/31/2023 and FOURTH re-certification in EP after 12/31/2023. The process of re-certification and MOC became a core center in Dr. Fisher's advocacy on behalf of the physicians. In this episode, CardioNerds co-founder Amit Goyal joins Dr. Iva Minga, Dr. Kevin Lee, and Dr. Juan Pablo Salazar Adum from the University of Chicago - Northshore in Evanston, IL to discuss a case of primary cardiac diffuse large B-cell lymphoma. The ECPR for this episode is provided by Dr. Amit Pursnani (Advanced Cardiac Imaging, Fellowship program director, NorthShore University HealthSystem). Case synopsis: A 77-year-old man with no significant medical history presents to the emergency department with progressive shortness of breath for 1 week. He reports an unintentional 15-pound weight loss in the prior month as well as constipation and abdominal/flank pain. On examination he was found to be tachycardic with a regular rhythm and further evaluation with a chest X-ray and chest CT scan demonstrated a large pericardial effusion. This was further investigated with an urgent echocardiogram that revealed a large pericardial effusion with a large mass attached to the pericardial side of the RV free wall, as well as signs of early cardiac tamponade. A pericardiocentesis was performed and 550mL of bloody fluid was withdrawn. The fluid was sent for laboratory analysis and cytology. A cardiac MRI demonstrated a large invasive mass in the pericardium and RV wall consistent with cardiac lymphoma. Cytology confirmed diffuse large B-cell lymphoma. Subsequent CT and PET scans did not find any other site of malignancy, giving the patient a diagnosis of primary cardiac diffuse large B-cell lymphoma. The patient underwent R-CHOP chemotherapy and was followed closely with repeat cardiac MRI and PET scans which demonstrated resolution of the cardiac mass at his one-year surveillance follow-up. This case was published in US Cardiology Review, the official journal of CardioNerds. To learn more, access the case report article here. Pearls - A Mystery Mass in the Heart - Cardiac Lymphoma The most common cause of malignant cardiac masses is metastasis. Primary cardiac tumors are rare. Cardiac tumors are separated into 2 categories: benign and malignant. They are often differentiated based on their location and their degree of tissue invasion. Multimodality imaging is essential in the diagnosis, management, and surveillance of cardiac masses. A multidisciplinary team approach is invaluable for management of patients with cardiac tumors. Show Notes - A Mystery Mass in the Heart - Cardiac Lymphoma 1. What is the clinical presentation of cardiac masses? Cardiac masses can have a variable presentation. They can present with arrhythmias, angina, heart failure symptoms, or pericardial effusion. Patients can also be asymptomatic; the masses can be found incidentally on cardiac or chest imagining. 2. What is the differential diagnosis for cardiac masses? Cardiac masses are separated into benign and malignant. The most common malignant cardiac masses are metastases from a distant source. The location of the mass is important in narrowing the differential. 3. What imaging modalities are used to diagnose cardiac masses? In a brief statement emailed to The Epoch Times after Kness’s approval, NorthShore wrote, “We are pleased with the Court’s approval of a supportive resolution to this matter and continue to prioritize the health and safety of our patients and team members.” Harry Mihet, vice president of legal affairs for Liberty Counsel, said in a statement that the group was “pleased to finally get the court’s final approval of this classwide settlement for these health care workers who were unlawfully discriminated against and denied religious exemptions from the COVID shot mandate.” “This case should set a precedent for other employers who have violated the law by denying religious exemptions for their employees,” he said. Liberty Counsel, a legal group that brings cases of alleged religious discrimination, was representing the 13 named plaintiffs in the case. The group successfully won class certification for all workers who were denied religious exemptions, a group that was initially believed to be 499 former and current workers but swelled after the preliminary settlement agreement to at least 519. As of Dec. 12, 493 class members had submitted claims for a piece of the settlement. Each worker who was fired stands to receive $24,225. Each worker who remained at the company stands to receive $3,725. The named plaintiffs are in line to receive an extra $20,000. Those payments, described as service awards, will provide compensation for the plaintiffs helping advise on court filings, gathering documents, and serving as lead plaintiffs “in a sensitive case involving personal health choices and religious beliefs over a matter of intense public debate, even when it was uncertain whether they would have to disclose their identities to the public,” according to a recent filing. Three workers objected to the settlement, but both parties urged the judge to disregard the objections, which were largely based on pay the trio felt they were owed after being fired. Marzena Novak, one of the objectors, said her actual losses from being fired and losing pay approached $140,000. Like many health care systems, NorthShore imposed a vaccine mandate on employees in 2021. NorthShore told workers that they could file a request for a religious exemption using a form that said the worker in question needed to provide “a description of my sincerely held religious principle or practice that guides my objection to receiving the required vaccination.” Northshore explicitly instructed applicants to not fill out lengthy answers. NorthShore initially approved some of the exemption requests but then reversed the decisions and denied “all or virtually all of them,” according to filings from the plaintiffs. Officials said the employees failed to meet the standard for religious exemptions. Employees who wanted a second look were told to file an appeal that included their vaccination history since they were 18. NorthShore then said that any religious objections based on “aborted fetal cell lines, stem cells, tissue, or derivative materials” would result in denials because those products were “not in NorthShore administered vaccines.” All of the COVID-19 vaccines available in the United States have links to aborted fetal cell lines. The plaintiffs said the treatment violated the Civil Rights Act, which requires employers to treat workers similarly, and the Illinois Health Care Right of Conscience Act, which forbids discrimination on the basis of “right of conscience.” NorthShore repeatedly denied that it violated the law. A judge will hold a hearing to review the payout by NorthShore University HealthSystem later this month. He recently told employees he’s creating a weird initiative called “Simplicity Sprint” to cut costs as Google suffers declines in profits and stock price. Employees are being asked how to make Google more productive because “it’s clear we are facing a challenging macro environment with more uncertainty ahead,” he said, according to CNBC. It doesn’t take much reading between the lines to conclude Pichai is basically asking employees how he and his team can weed out their unproductive colleagues. Can’t wait to hear their thoughts. CASEL is sponsored by the Allstate Foundation. The Queer Mathematics Teacher is an educational consulting firm that seeks to promote “queer mathematics” in American K-12 classrooms. The firm sells a book titled “Talking about LGBTQ+ Identity,” which includes chapters titled “The Gender Unicorn,” “How Do I Talk About LGBTQ+ Identity With My Students,” and “What if I Receive Pushback?” The firm also offers coaching services to teachers, which include curriculum design, district-wide audits, and professional development workshops. The site claims, “The ability to consider sexuality irrelevant in the mathematics context is a heteronormatively privileged position” and notes that some areas of expertise include “queer pedagogy,” “teaching mathematics for social justice,” “Fostering Students’ Growth Mindset (While Advocating for Systemic Change),” and “humanizing Mathematics Education for LGBTQ+ Students.” Waid confesses in her article that math used to be a source of anxiety for her, which she links with her sexuality. She writes, “I associated mathematics with profound anxiety, intricately tied to my fears of confronting my queer identity.” The left-wing activist even goes so far as to suggest that activist educators should teach critical race theory and gender theory, even if it means breaking the law. Senate Bill 57 by State Sen. Scott Wiener (D-San Francisco) passed in the Senate by a margin of 21-11. The bill had previously passed the Assembly in June on a 42-29 vote. Under the new legislation, the three cities would be allowed to establish the sites. The legislature has previously approved legislation allowing safe heroin injection sites in 2018, but the bill was vetoed by then-Gov. Jerry Brown, a Democrat. The bill would authorize cities and counties to establish "safe consumption sites" where addicts could use illegal narcotics under supervision. Those accessing the "hygienic space supervised by trained staff" could consume pre-obtained drugs. Program staff would be trained to administer an "opioid antagonist" in the event of an overdose. Under federal law, it is currently a felony to operate a space to distribute or use a controlled substance. In a written statement, Wiener claimed the sites are needed due to rising "dramatic and preventable" fatal overdoses in San Francisco and across the country. 711 people died from overdoses in San Francisco in 2020, followed by another 640 deaths in 2021. California saw over 5,500 deaths related to opioid overdoses in 2020, almost a 70 percent increase from 2019. Wiener said, "Safe consumption sites are a proven model to help people avoid overdose deaths, reduce HIV and hepatitis transmission, reduce syringe litter, and help people access treatment. This legislation isn't about whether we want people to use drugs. Rather, it's an acknowledgment that people are using drugs, and our choice is whether we want to make every effort to help them survive and get healthy." However, according to recent studies and data, these sites do the opposite. A 2020 study from the Alberta government on the province's Safe Consumption Sites (SCS) f0und sites result in increased calls to police, lack of response from police, increased needle debris on public and private property, increased crime, increased overdose deaths, and minimal amounts of users seeking treatment. Last December, former New York City Democrat Mayor Bill de Blasio and the New York City Department of Health ignored federal law and announced the opening of legalized heroin injection sites. In 2021, the Rhode Island General Assembly passed legislation aimed at opening the first heroin injection sites in the US. Weiner has backed legislation to put biological males in women's prisons, reduce penalties for some sex offenders, and create a "gender identity" sanctuary for underage children across the country to seek medicalized gender treatment without parental consent. The site claims, “The ability to consider sexuality irrelevant in the mathematics context is a heteronormatively privileged position” and notes that some areas of expertise include “queer pedagogy,” “teaching mathematics for social justice,” “Fostering Students’ Growth Mindset (While Advocating for Systemic Change),” and “humanizing Mathematics Education for LGBTQ+ Students.” Waid confesses in her article that math used to be a source of anxiety for her, which she links with her sexuality. Senate Bill 57 by State Sen. Scott Wiener (D-San Francisco) passed in the Senate by a margin of 21-11. The bill had previously passed the Assembly in June on a 42-29 vote. Under the new legislation, the three cities would be allowed to establish the sites. The legislature has previously approved legislation allowing safe heroin injection sites in 2018, but the bill was vetoed by then-Gov. Jerry Brown, a Democrat. The bill would authorize cities and counties to establish "safe consumption sites" where addicts could use illegal narcotics under supervision. Those accessing the "hygienic space supervised by trained staff" could consume pre-obtained drugs. Program staff would be trained to administer an "opioid antagonist" in the event of an overdose. Under federal law, it is currently a felony to operate a space to distribute or use a controlled substance. California saw over 5,500 deaths related to opioid overdoses in 2020, almost a 70 percent increase from 2019. Wiener said, "Safe consumption sites are a proven model to help people avoid overdose deaths, reduce HIV and hepatitis transmission, reduce syringe litter, and help people access treatment. This legislation isn't about whether we want people to use drugs. Rather, it's an acknowledgment that people are using drugs, and our choice is whether we want to make every effort to help them survive and get healthy." However, according to recent studies and data, these sites do the opposite. A 2020 study from the Alberta government on the province's Safe Consumption Sites (SCS) f0und sites result in increased calls to police, lack of response from police, increased needle debris on public and private property, increased crime, increased overdose deaths, and minimal amounts of users seeking treatment. Last December, former New York City Democrat Mayor Bill de Blasio and the New York City Department of Health ignored federal law and announced the opening of legalized heroin injection sites. Lawyers for the plaintiffs claim this is the first class-action settlement over religious discrimination and the COVID-19 vaccines. A judge will hold a hearing to review the payout by NorthShore University HealthSystem later this month. More than a dozen employees sued in October, alleging a violation of civil rights after the company refused to give religious accommodations to those seeking an opt-out from its COVID-19 vaccine mandate issued last year. “Let this case be a warning to employers that violated Title VII. It is especially significant and gratifying that this first classwide COVID settlement protects health care workers. Health-care workers are heroes who daily give their lives to protect and treat their patients. Ouch. Google CEO Sundar Pichai was a bit more diplomatic, in a techie and creative kind of way. He recently told employees he’s creating a weird initiative called “Simplicity Sprint” to cut costs as Google suffers declines in profits and stock price. Employees are being asked how to make Google more productive because “it’s clear we are facing a challenging macro environment with more uncertainty ahead,” he said, according to CNBC. It doesn’t take much reading between the lines to conclude Pichai is basically asking employees how he and his team can weed out their unproductive colleagues. Can’t wait to hear their thoughts. Their coordinator, Mac Philips, said, “To preach the Gospel for us is to die, to be safe is to stop preaching the Gospel. It's a choice we all have to make – for us, we choose the former. The Gospel in Nigeria cannot be preached without casualties. Our lives are increasingly on the line, but nothing compares to the excellency of sharing the Gospel to a dying world without Christ.” According to Open Doors, Nigeria is the seventh most dangerous country in the world for Christians. Romans 8:36-37 says, “‘For Your sake, we are killed all day long; we are accounted as sheep for the slaughter.' Yet in all these things we are more than conquerors through Him who loved us.” Poland's abortions dropped by 90% last year Poland's Ministry of Health reports the number of abortions in the country fell by 90% last year. In 2021, 107 unborn babies died by abortion, down from 1,076 deaths in 2020. Last January, a ruling by the country's Constitutional Court came into effect, banning nearly all abortions. However, babies in the womb are still in danger. Pro-life activists warn that tens of thousands of Polish babies are still killed each year with abortion pills. England shut down transgender clinic for kids England's National Health Service is shutting down the only facility in the U.K. that offers children “Gender Identity Development Service.” Doctors and patients have accused the facility of rushing children into puberty blockers. Meanwhile, in the U.S., the Food and Drug Administration warned last month that puberty blockers can have serious side affects. Puberty blockers, known as gonadotropin-releasing hormone agonists in children, can cause brain swelling and loss of vision. The FDA identified six cases in girls between the ages of 5 and 12 years old. Illinois hospital forced to pay $10 million over COVID-shot firings Liberty Counsel announced yesterday it achieved the first class action settlement in the U.S. over illegal COVID-shot mandates. Illinois' NorthShore University HealthSystem must pay $10.3 million for discriminating against 500 current and former health care workers. The company denied hundreds of religious exemptions from the COVID-shot mandate. As part of the settlement, NorthShore must change its policy of “no religious accommodations.” Mat Staver with Liberty Counsel said, “This settlement is the first in the nation and should serve as a strong warning to employers that thought they could violate longstanding Title VII religious freedom laws.” Arkansas Methodist Church leaves denomination over homosexuality A large Methodist church in Arkansas is leaving The United Methodist Church over the debate on homosexuality. Like many other Methodist congregations, First United Methodist Church of Jonesboro is joining the theologically conservative Global Methodist Church. In a letter, First Church stated, “Our congregation has been consistently clear that the shifts in theological teaching and the failures of the UMC to enforce the doctrinal discipline do not reflect the beliefs of the strong majority of our membership.” As of June, 35 Methodist churches in Arkansas were considering leaving the United Methodist Church. California declares Monkeypox emergency California declared a state of emergency on Monday over the Monkeypox outbreak. It's the third state to do so after Illinois and New York. The U.S. has reported 6,000 cases of Monkeypox across 48 states. Forty-seven percent of the cases are in California, Illinois, and New York. The vast majority of cases are spread through sodomy. He then matched into the combined Orthopaedic Surgery Residency Program at RUSH University Medical Center in Chicago, IL--a program consistently ranked in the top 10 nationwide. Upon graduation, he was awarded the Gunnar Andersson, MD, PhD Award for best Chief Resident by his co-residents. He spent a year in Rochester, MN at Mayo Clinic completing a Fellowship in Hand and Microvascular Surgery. There, he was trained in all aspects of general hand surgery, peripheral nerve surgery including brachial plexus reconstruction, and microsurgery including vascularized bone grafting. After completing training, he practiced and taught hand surgery at University of Miami's Miller School of Medicine in Miami, FL. There, he focused primarily on complex trauma, reconstruction, and limb salvage and operated at Bascom Palmer hospital as well as Jackson Memorial Hospital. He was also selected as best Attending Surgeon Teacher by the Orthopaedic Residents. As Sandra Joe (LinkedIn), Senior Compliance Analyst at NorthShore University HealthSystem explains in this podcast, surprise bills had typically arisen in three cases: A patient visits an out of network provider in an emergency While at an in-network facility, charges are incurred by an out-of-network provider When self-pay patients undergo a procedure and only find out what it costs after the fact The No Surprises Act established new Federal protections to prevent these costly occurrences. It bans out of network cost-sharing and balance billing. It also requires that health care providers and facilities provide easy-to-read and understandable notices explaining their billing protections and providing information on who to contact for patients who are concerned that their protections were violated. For the uninsured and those choosing to pay for their own procedures, the law provides that they receive a good faith estimate, at least one day in advance, for a scheduled procedure. If the charges exceed the estimate by $400 or more, patients have the right to dispute the cost. So what should compliance teams do? Be sure to document the steps you take to put proper controls in place, including the training that is provided to the workforce. Familiarize employees with how to update good faith estimates if there are changes, and make sure the necessary disclosures are posted in both provider settings and on the website.
