A leading aged care pharmacist discusses how pharmacy has changed over the years – and his passion welcoming those changes The AJP Podcast talks to the Pharmaceutical Society of Australia's inaugural Aged Care Pharmacist of the Year – Neil Petrie, who has been working primarily in aged care, undertaking medication reviews for the past 25 years. He has also owned his own community pharmacy in the past and semi-retired into aged care onsite pharmacy under the ACOP program. With a father and uncle who were pharmacists, he says he remembers being about six or seven years old, “and going to the pharmacy after hours with my dad to make up an APC mixture, if I remember rightly, which was aspirin, phenacetin, and caffeine in it”. “And I vividly remember my father saying, hold that needle and don't drop it. And what did I do? I dropped it onto the footpath, didn't I? “So pharmacy’s been in my blood for a long time and I did have my own pharmacy for about 13 years, but I realised I wanted to do something a little bit different and when I had my pharmacy, I started servicing some hostels at that point in time and I really enjoyed that work.” Petrie was one of Australia's first pharmacists accredited to conduct medication reviews in 1998 – and ever since, his motto has been change. He says ACOP will only increase in time, and “highlights our impact on direct resident care, medication safety, the quality use of medicines, and really system-wide improvements to medication procedures. So it really showcases us as pharmacists as a valued integral part of the care team.” This is also highlighted, he says, by the sheer need for pharmacists' services; discussing the most common medication-related problems in aged care facilities, given the amount of polypharmacy – and more importantly, inappropriate polypharmacy – underway. They discuss how Vinted, an online marketplace for secondhand products, modernised its data architecture to address new AI search use cases and the challenges faced with Elasticsearch. From the switch to Vespa and the advantages of supporting multiple languages and complex queries, the podcast offers insights on the trade-offs organisations must think about when updating their search systems, especially regarding AI and machine learning applications.Vinted Elasticsearch ChallengesVinted's search architecture was built on Elasticsearch before they switched to Vespa. Elasticsearch is a functional system that presents a few major challenges. With over 20 supported languages, the company's "index per language" approach created significant sharding problems, leading to infrastructure imbalances and constant adjustments."The index for the French language, the biggest language that we support, was more than three times bigger than the second biggest language, which created imbalances in the Elasticsearch data nodes' load," Jocas explained.In addition to these technical obstacles, organisational issues arose as teams responsible for different parts of the search process found themselves "pointing fingers at each other at an increasing rate." The need for a more integrated, effective solution became clear.The Solution: A New Platform for a New EraThe search for a better solution led Vinted to Vespa. The initial adoption was a "one success story" when a machine learning engineer, working on recommendations, discovered that Vespa was ten times faster than Elasticsearch for their use case. This initial benchmark, run on a single decommissioned server, was a "true testament to how efficient Vespa is when it comes to serving requests,” Jocas told Petrie.Vespa helped Vinted solve their language problem by allowing it to set a language per document. Thus, it eliminates the need for separate indexes and the associated sharding headaches. As Jocas put it, "We got out of the sharding problem once and for all."TakeawaysVinted faced challenges with its initial Elasticsearch architecture.The need for better integration between matching and ranking was identified.Vespa outperformed Elasticsearch in handling image search and recommendations.Transitioning to Vespa involved significant learning and support from developers.Vespa allows for language-specific document handling, simplifying architecture.Organisations must evaluate the complexity and volume of their data before transitioning.Vespa is optimised for query performance, while Elasticsearch excels in data writing.The learning curve for Vespa can be steep, but support is available.It's important to focus on optimising new systems rather than emulating old ones.Partial updates in Vespa are more efficient than in Elasticsearch.Chapters00:00 Introduction to Vinted and...

He argues that the UN often oversimplifies conflicts, failing to address their root causes, while powerful member states prioritize national interests over humanitarian principles. This bureaucratic approach has led to systemic failures, particularly in protecting vulnerable populations.Discussing Myanmar, Petrie highlights the courage and resilience of Myanmar's Generation Z activists, who have built self-sustaining governance structures in the face of brutal repression by the military regime, the SAC. He notes that many Western policymakers indulge in “fairy tale” thinking, overlooking the complex realities of Myanmar's situation while simultaneously withholding meaningful support. In his view, Myanmar's emerging grassroots governance—largely led by young, educated activists—proves the people's capacity for self-determination. Petrie's journey into Karenni State allowed him to witness both the hardships imposed by the SAC's violent tactics and the unbreakable resolve of Myanmar's revolutionary forces.Petrie critiques the West's inaction and the UN's failure to stand firmly for its foundational values. While he views the UN as a flawed, symbolic institution, he believes it must still be preserved. He sees the UN as a beacon of hope for future generations advocating for justice and a better world. Ultimately, Petrie's reflections emphasize the need for honest, courageous global engagement to genuinely support Myanmar's fight for freedom.

One San Antonio researcher thinks so.
Major George Petrie served in the U.S. Army Special Forces in Vietnam. He was a member of MACV-SOG, and was the first soldier to hit the ground during the Son Tay Raid. MACV or “Military Assistance Command, Vietnam”, was a highly classified, multi-service special operations unit consisting in part of US Army Special Forces, Navy SEALs, and CIA personnel in Vietnam. SOG, or “Studies and Observations Group”, was a subcomponent of MACV that provided on the ground intelligence to and conducted operations, including reconnaissance missions, evacuations, prisoner captures, and much more from control sites across Vietnam. In this interview, Petrie recounts Operation Ivory Coast (Son Tay Raid), a significant POW rescue operation. Petrie also served in Panama and the Dominican Republic.
