#319 Last week I saw a patient who had been on a low FODMAP diet for 12 years. Self-prescribed. No reintroduction. No professional guidance. His story is far more common than most people realise — and it's exactly why I recorded this episode. Low FODMAP, keto, and carnivore diets all have legitimate therapeutic applications. But when they're self-prescribed, followed indefinitely, and used without ever investigating the underlying cause of symptoms, they can cause their own harm — quietly, progressively, and in ways that are very difficult to reverse. In this episode, I'm drawing on my extensive experience working with constipation and gut health to talk specifically about what these diets do to your bowel — the microbiome changes, the impact on butyrate and serotonin production, the loss of mechanical stimulation, and why constipation is one of the most commonly reported and least discussed consequences of long-term dietary restriction. I also cover the conditions — including SIBO, where a low FODMAP approach may be appropriate for longer than the standard timeframe, and why even in those cases, reintroduction is always the goal. Farnoush Djavaheripour is one of the only women who teaches the ancient Persian sport and practice of Zoorkhaneh (also known as Pahlavāni). Restricted to men within Iran, Djavaheripour is extending the practice to both diasporic and women and gender expansive communities alongside the University of Washington graduate student Sara Modjib Shirazi, who has founded a new non-profit called “We are Zoorkhaneh.” They share both their vision and the obstacles they face in conversation with Ron Krabill, Director of the Global Sport Lab. The Global Sport Lab, based in the UW's Henry M. Jackson School, is supported by over a dozen UW departments and schools and was founded in 2024. Limited exposure can hurt sellers if a home is not seen by the full pool of potential buyers. Unauthorized showings are a serious professionalism issue because they damage seller trust. Even vacant homes require proper showing procedures because there may be alarms, repairs, tests, occupants, pets, or other access concerns. Strong day-one offers can work, but they need to give the seller a real reason to stop showings. Being the first offer can help because sellers often give that first serious offer the most attention. Video showing instructions could help agents understand complicated listings, especially acreage, luxury homes, shops, gates, alarms, and hidden features. Key transfers should be simple, secure, and convenient, with the key ideally available at the property after recording. Open house scheduling can create confusion when private showings overlap with public open houses. No Children for Joseph Smith? Joseph Smith’s plural wives One of the most frequent arguments raised by skeptics of Joseph Smith’s polygamy is the “fertility paradox.” While Joseph Smith fathered several biological children with his first wife, Emma, there are zero verified offspring from his estimated 30 to 40 plural marriages. DNA testing on suspected plural children has continuously ruled out Joseph as the father in every known case. Does this lack of physical evidence prove his plural marriages were non-sexual? According to a statistical analysis by Rick Bennett and a research team, the absence of children is not a biological impossibility, but rather a predictable outcome of history, biology, and law. https://youtu.be/Zz6BpX2wZd0 Statistical Models of Probability Bennett, alongside a team that included OBGYN Dr. Joseph Stanford, built four statistical models to determine the mathematical probability of Joseph Smith having zero children with his plural wives. These models factored in female ovulation cycles and historical documentation of Smith’s whereabouts, assuming no birth control was used: Restricted & Low Models: Assuming very infrequent relations and excluding teenagers and already-married women, the probability of zero children sits between 22% and 47%. Bennett equates this to a coin flip or rolling a double in Monopoly, making zero children a statistically unsurprising outcome. Medium (Consensus) Model: Assuming one or two encounters around the time of the sealing for a broader group of wives, the probability of zero children drops to roughly 9% to 13%. High Model: Only when assuming large possible historical encounters across 37 women does the probability of zero children become statistically microscopic (around 0.03%). Biological Factors and 19th-Century Demographics A significant reason for the lack of children stems from the demographic makeup of Joseph’s plural wives. At least five women were post-menopausal (over age 47) and 11 were “polyandrous” wives already married to other men. Furthermore, several brides were teenagers. Interestingly, poorer nutrition and rigorous physical labor in the 19th century delayed a woman’s first period (menarche) until an average age of 17, meaning many of the youngest wives were likely prepubescent and physically infertile at the time of their sealings (See Dan Vogel’s presentation.) Contraception and the Concept of “Quickening” If the High Model is accurate and sexual relations were frequent, Bennett notes that the Nauvoo community had widespread access to birth control. Popular texts from the 1830s heavily circulated knowledge about methods like the sponge, withdrawal, and highly acidic douching to prevent pregnancies. Additionally, 19th-century medical and Mormon theological consensus believed that a soul did not enter a fetus until “quickening” (around 20 weeks.) Because of this, early herbal interventions used to “restore the menses” were viewed simply as regulating the female body, rather than as abortion or sin. Legal and Theological Survival Ultimately, concealing pregnancies was an absolute necessity for survival. In Illinois, bigamy and adultery were felonies heavily penalized by fines, whipping, and imprisonment. Under the law, a child would serve as incontrovertible physical proof of illicit cohabitation, inviting immediate legal prosecution and mob violence. To avoid exposure, the theology of plural marriage subtly shifted. While originally grounded in an “Abrahamic” mandate to multiply and raise up seed on earth, the practice pivoted to a “Melchizedek” order. This new paradigm was focused on creating secret, dynastic priestly linkages for eternal salvation in the afterlife, entirely bypassing the legal dangers of earthly procreation. In short, the absence of children in Nauvoo polygamy is not a historical impossibility, but a reality forcefully shaped by 19th-century logistics, biology, and the heavy anvil of American law. The Defense Department and its federal partners issued a warning Friday to drone operators, threatening to impose massive fines, imprisonment and other measures on those who illegally fly unmanned aerial systems in restricted airspace. Drone incursions over stateside military bases and other restricted areas have been widespread in recent years as commercially available systems proliferate. Just this week, the head of U.S. Northern Command and North American Aerospace Defense Command revealed that American forces recently identified and defeated a small UAS threat over a “strategic” U.S. installation. Agencies operating near the southern border have also been using weapon systems, including high-energy lasers, against suspicious drones, raising safety concerns among agencies like the FAA. Two incidents in Texas last month led to temporary airspace closures. The federal government restricts who can fly UAS over certain areas, such as military facilities and civilian airports, to protect national security and public safety. In a press release issued Friday, the DOD, Department of Justice, Department of Homeland Security and the FAA touted the government's detection capabilities, declaring that Uncle Sam has a “zero-tolerance policy” for illegal drone operations and threatening rule violators with “severe consequences,” including potential fines upwards of $100,000, criminal charges, incarceration, and the confiscation of their systems. The White House registered two new government domains last week: alien.gov and aliens.gov, according to publicly available federal records. Their appearance comes about one month after President Donald Trump announced plans to direct the long-anticipated release of U.S. government records about unidentified anomalous phenomena (UAP) and extraterrestrial beings. Those new domains were not connected to websites as of Wednesday morning. But public data managed by the Cybersecurity and Infrastructure Security Agency (CISA) reveals that both sites were registered Tuesday evening and are hosted on Cloudflare servers. Shortly after Trump's disclosure order in February, Defense Secretary Pete Hegseth said the Pentagon was keen to comply and had started actively working on the initiative. Grounding Techniques in Counselling Practice [starts at 03:46 mins] In this section, Rory and Ken explore grounding techniques in counselling practice, highlighting why every counsellor should understand how to help clients return to the here and now when they become distressed or overwhelmed. Key points discussed include: Grounding techniques are essential when a client becomes emotionally overwhelmed, dissociates, or is pulled into traumatic memories. Therapy happens in the here and now – if a client is reliving the there and then, effective therapeutic work cannot take place. Key points from this conversation include: ARFID is not driven by body image concerns but by fear, anxiety, sensory sensitivities, and sometimes neurodivergence. Recent guideline changes mean weight alone should no longer determine access to support – behavioural signs and functional impact are key. Neurodivergent individuals, particularly autistic people, may be more vulnerable due to sensory processing differences and rigidity around routines. Restricted diets can lead to nutritional deficiencies that impact brain function, creating a cycle of anxiety and further restriction. Interventions should be gradual and non-threatening – introducing small changes, using food chaining, and keeping “safe foods” in place. Battles around food can increase anxiety and shame; curiosity, patience, and flexibility are more effective approaches. Finding Your First Supervisor [starts at 01:01:32 mins] In this section, Rory, Ken and Paul Cullen reflect on the importance of choosing your first supervisor carefully and what to consider when beginning your clinical practice journey. Key points include: Supervision is an ethical requirement and becomes central to your practice the moment you begin seeing clients. It's that time again. Even numbered years are book reviews, odd-numbered years are non-book reviews, so you're limited to books for now. Write a review of a book. There's no official word count requirement, but previous finalists and winners were often between 2,000 and 10,000 words. There's no official recommended style, but check the style of last time's finalists and winners or my ACX book reviews (1, 2, 3) if you need inspiration. Please limit yourself to one entry per person or team. Then send me your review through this Google Form. The form will ask for your name, email, the title of the book, and a link to a Google Doc. The Google Doc should have your review exactly as you want me to post it if you're a finalist. Don't include your name or any hint about your identity in the Google Doc itself, only in the form. I want to make this contest as blinded as possible, so I'm going to hide that column in the form immediately and try to judge your docs on their merit. (does this mean you can't say something like "This book about war reminded me of my own experiences as a soldier" because that gives a hint about your identity? My rule of thumb is that if I don't know who you are, and the average ACX reader doesn't know who you are, you're fine. I just want to prevent my friends or Internet semi-famous people from getting an advantage. If you're in one of those categories and think your personal experience would give it away, please don't write about your personal experience.)
