Sarah Bishop is busier than most people. She is an ambitious runner, she is a police detective, and she loves the outdoors in New Zealand. But she started having trouble doing even normal things. Mary Norine Walsh, MD, MACC, JACC: Case Reports Deputy Editor, is joined by author Dena Hayes, MD, discussing this study from Hayes et al presented at ACC.25 and published in JACC: Case Reports. The diagnosis of cardiac sarcoidosis (CS) is often challenging, particularly in atypical cases. The authors describe a case of a previously healthy 33-year-old woman who was found to have a biatrial mass and evidence of a diffuse inflammatory or neoplastic process on multimodality imaging. Percutaneous biopsy of the cardiac mass was performed and histopathology revealed granulomas consistent with CS. This case adds to the growing number of reports of CS presenting as an intracardiac mass.
Updates from the 2024 ESC and AHA Scientific Statements on Cardiac Sarcoidosis Guest: Leslie T. Cooper Jr., M.D. Host: Kyle W. Klarich, M.D. Cardiac sarcoidosis is a systemic, chronic inflammatory disorder characterized by non-caseating, epithelioid cell granulomas that may involve many tissues and organs. The Heart Rhythm Society criteria for diagnosis of cardiac sarcoidosis require a biopsy-proven diagnosis of extra-cardiac sarcoidosis. If extra-cardiac sarcoidosis is confirmed, CS is highly probable when there is evidence of the following: (a) rhythm abnormalities in either ECG or Holter monitoring such as advanced AVB (Mobitz type II second-degree or third-degree AVB), sustained VT, and (b) LV dysfunction on echocardiography or CMR (LVEF

CardioNerds (Dr. Rick Ferraro and Dr. Dan Ambinder) join Dr. Sri Mandava, Dr. David Meister, and Dr. Marissa Donatelle from the Columbia University Division of Cardiology at Mount Sinai Medical Center in Miami. Expert commentary is provided by Dr. Pranav Venkataraman. They discuss the following case involving a patient with cardiac sarcoidosis presenting as STEMI: A 57-year-old man with a history of hyperlipidemia presented with sudden onset chest pain. On admission, he was vitally stable with a normal cardiorespiratory exam but appeared in acute distress and was diffusely diaphoretic. His ECG revealed sinus rhythm, a right bundle branch block (RBBB), and ST elevation in the inferior-posterior leads. He was promptly taken for emergent cardiac catheterization, which identified a complete thrombotic occlusion of the mid-left circumflex artery (LCX) and large obtuse marginal (OM) branch, with no underlying coronary atherosclerotic disease. Aspiration thrombectomy and percutaneous coronary intervention (PCI) were performed, with one drug-eluting stent placed. An echocardiogram showed a left ventricular ejection fraction (EF) of 31%, hypokinesis of the inferior, lateral, and apical regions, and an apical left ventricular thrombus. The patient was started on triple therapy. A hypercoagulable workup was negative. A cardiac MRI was obtained to further evaluate non-ischemic cardiomyopathy. In conjunction with a subsequent CT chest, the results raised suspicion for cardiac sarcoidosis with systemic involvement. In view of a reduced EF and significant late-gadolinium enhancement, electrophysiology was consulted to evaluate for ICD candidacy. A decision was made to delay ICD implantation until a definitive diagnosis of cardiac sarcoidosis could be established by tissue biopsy. The patient was started on HF-GDMT and discharged with a LifeVest. Close outpatient follow-up with cardiology and electrophysiology was arranged. Pearls - Cardiac Sarcoidosis Presenting as STEMI Cardiac sarcoidosis can present with a variety of symptoms, including arrhythmias, heart block, heart failure, or sudden cardiac death. Symptoms can be subtle or mimic other cardiac conditions. Conduction abnormalities, particularly AV block or ventricular arrhythmias, are common and may be the initial indication of cardiac involvement with sarcoidosis. The additive value of Echocardiography, FDG-PET, and cardiac MR is indispensable in the diagnostic workup of suspected cardiac sarcoidosis. Specific role of MRI/PET: Both cardiac MRI and FDG-PET provide a complementary role in the diagnosis of cardiac sarcoidosis. Cardiac MRI is an effective diagnostic screening tool with fairly high sensitivity but is limited by its inability to decipher inflammatory (“active” disease) versus fibrotic myocardium. FDG-PT helps to make this discrimination, refine the diagnosis, and guide clinical management. Ultimately, these studies are most useful when interpreted in the context of other clinical information. Primary prevention of sudden cardiac death in cardiac sarcoidosis focuses on risk stratification, with ICD placement for high-risk patients. For patients awaiting definitive diagnosis, a LifeVest may be used as a temporary measure to protect from sudden arrhythmic events until an ICD is placed. After finding success with Bone Thugs-n-Harmony, Krayzie Bone recorded his solo debut, Thug Mentality 1999, in 1999. The album was released as a double disc set, featuring a large selection of guest appearances, including Bone Thugs-n-Harmony, Mo Thugs, Mariah Carey, ThugLine, The Marley Brothers, Big Pun, Fat Joe, Cuban Link, Gangsta Boo, E-40, 8 Ball & MJG, Kurupt, Naughty By Nature and Snoop Dogg. In 2001, Krayzie Bone released Thug On Da Line, which received generally positive reviews from music critics and went gold. In 2005 Krayzie Bone was featured in the song "Ridin'" by Chamillionaire. "Ridin'" was awarded "Best Rap Performance by a Duo or Group" at the 49th Annual Grammy Awards. It was also nominated for Best Rap Song. It became number one in December on the Pop Chart 3 months after its physical release. The song also topped the Billboard Hot 100 and peaked at number 2 on the UK Singles Chart when it was released there. The song ranked #3 on Rolling Stone's "100 Best Songs of 2006" and #91 on VH1's "100 Greatest Songs of Hip Hop". It was also awarded the last Best Rap Video at the MTV Video Music Awards, which was awarded in 2006. Krayzie Bone comes from a fourth generation family of Jehovah's Witnesses, and as such does not celebrate Christmas. He believes many rappers are unintentionally following the agenda of the Illuminati.[9] On September 24, 2023, Krayzie Bone was admitted to a hospital in Los Angeles after coughing up blood in his sleep, what was later attributed to sarcoidosis. Doctors were unsuccessful in an emergency surgery. On September 26, he was sedated due to a main artery in his lung leaking. However, on October 3, he announced he had recovered from the ailment. Despite high hopes and promise from the drug Namilumab, Kinevant Sciences is reporting that the drug was not effective in treating Sarcoidosis. A well executed clinical trail showed that the drug simply didn't make a difference for sarcoidosis patients. Sarcoidosis snuck up on Karen Coleman. A dance instructor, she couldn't figure out why she wasn't keeping up in her jazzercize class. After plenty of time with doctors and more CAT Scans than she would like, she discovered she was suffering from pulmonary Sarcoidosis. In this episode of the FSR Sarc Fighter podcast I share a new study from Japan that compared people with systemic cardiac sarcoidosis -- those people who have cardiac sarc as well as other organ involvement -- with those who have only cardiac involvement. The research found that if you have it only in your heart it may actually be more serious. Also, FSR just released information that will give you a sigh of relief if you want to participate in a clinical trial. I also riff a bit about election night 2024, covering a Donald Trump rally, and a speech on the difficulty of sharing scientific data with the viewing public on TV news. The combination of Sarcoidosis and Covid proved too much for Wade Tomlinson. His lungs were wracked with damage from sarcoidosis. So when he got covid it was the punch his body couldn't handle and he needed to take drastic action. Wade got on several waiting lists for a lung transplant and then waited for the call. In episode 121 of the FSr Sarc Fighter Podcast, Wade recounts the moment the call came in and the series of events that led to him becoming a recipient. Let's just say it wasn't clear-cut. Dr. Kamal Bharucha is a Pediatrician-Scientist, Clinical Development Leader and Biotech Startup Advisor in the rare disease space. And suddenly he found himself dealing with a rare disease - sarcoidosis. He says he was feeling fine and went for a routine check-up and discovered everything was far from fine. Robb Bruce, 53, renal cell carcinoma and sarcoidosis, Raynham, and Brad McGregor, MD, Director of Clinical Research for the Lank Center of Genitourinary Oncology, Dana-Farber · Before cancer touched Robb, it impacted his family when both his mother and father were diagnosed with the disease. In 2013, Robb's mother was diagnosed with T-Cell Lymphoma and treated at Dana-Farber, by Dr. Philippe Armand, before passing gracefully in 2017. Three months later, Robb's father was diagnosed with bladder cancer and began treatment with Dr. Brad McGregor at Dana-Farber as well. · Recently, when a large mass was discovered on Robb's kidney, he knew he wanted to receive treatment from the same doctor who cared for his father. Before coming in to see Dr. McGregor, Robb was told that initial CT scans of his abdominal and chest area showed signs of possible metastasis from the kidney to his upper respiratory system. Robb met with Dr. McGregor who felt Robb was showing signs of sarcoidosis, not metastasis. Sarcoidosis is an inflammatory disease causing the immune system to overreact and sarcoid-like reactions associated with kidney cancer are rare. · Based on Dr. McGregor's evaluation, a thoracic surgeon removed some of the lymph nodes from Robb's upper-chest area, which confirmed he had sarcoidosis and not metastatic disease. · A separate biopsy confirmed that Robb had Stage 3 Clear Cell Renal Cell Carcinoma, so his right kidney was removed. Robb is now waiting to begin immunotherapy and potentially join a clinical trial. · In his free time, Robb loves to golf, listen to music, play guitar and spend time on the lake in New Hampshire with his family. He's supported by his wife, Jennifer, and his 3 children, Ethan, Lily and Grace who are all currently college students.

When it comes to sarcoidosis, one of the rarest forms is neurosarcoidosis or sarcoidosis in the nervous system. That's what Jessica Soules is dealing with. A long-time fitness instructor, sarc has taken the bounce out of her step. In addition, Jessica is trying to spread the message of another rare disease that claimed the life of a friend. In this episode she talks about her own struggles with sarcoidosis and a little about her group called Jonnie's Goodguys -- named after her friend who suddenly dropped dead one day while at work. Zombie is the theme song for the FSR Sarc Fighter podcast. If you listen, you know. Shortly after I launched the podcast Mark Steier e-mailed me to say he had sarcoidosis and had written and recorded a song about it. I didn't expect much. But the song blew me away, and Mark quickly agreed to allow me to use it as the theme song. You've been hearing it ever since. Meanwhile Mark's health has been up and down, sometimes seriously down.
