The sudden death of anyone is a tragic event, but even more so a child, particularly when it comes completely unexpectedly. We've all seen stories in the papers or even been involved in caring for these young people. In this podcast Harshil Dhutia talks about the common causes fo sudden cardiac death, and gives a roadmap for the investigation of young people with worrying symptoms and ongoing care for their families. He is an expert in sports cardiology and has extensive clinical and research interests in the management of competitive and recreational athletes with cardiovascular disease. He is the medical lead for the Joe Humphries Memorial Trust, a Leicestershire based charity that raises awareness of sudden cardiac death in young people and provides cardiovascular resuscitation and AED training in schools, sports clubs and to members of the community.

Sudden cardiac death due to ventricular rhythm disturbances are rare but highly public. It's strange and curious because you don't expect healthy athletes to suffer serious cardiac issues. Recently three prominent athletes have survived sudden cardiac death. Christian Erikson, a Danish soccer player, Damar Hamlin, an American football player and Bronny James, son of Lebron James. These high-profile cases have highlighted the issue of sudden cardiac death of athletes. Dr. Dorian has published extensively on this topic. We had a great conversation. Let's talk about how omega-3 fats help support the heart and how to decrease the risk of sudden cardiac death and arrhythmias. Health benefits of fish oil (omega-3 fatty acids): • They are antiarrhythmic • They are anti-inflammatory • They trigger genes that can help turn off the fibrosis effect of the heart • They are antithrombotic • They support the endothelial layer in the arteries • They can help make your cell membranes more fluid and less rigid Omega-6 fatty acids are common in oils like vegetable oils and seed oils. These oils invade cell membranes and make them more rigid. It's crucial to have the correct ratio of omega-3 fatty acids to omega-6 fatty acids, which should be 1:1. The average person in the US has a 15:1 ratio of omega-6 fatty acids to omega-3 fatty acids. Omega-6 fatty acids not only increase the risk of heart problems but many other health issues. Omega-6 fatty acids also compete with omega-3 fatty acids. So keep in mind that the more omega-6 fatty acids you have in your diet, the less of an effect you will have from omega-3 fatty acids. Foods high in omega-6 fatty acids: 1. Vegetable oils 2. Grain-fed animals 3. Grains 4. Nuts 5. Salad dressings 6. Mayonnaise 7. Hummus 8. Farm-raised fish 9. Restaurant food Start increasing fish oils, especially cod liver oil, in your diet, include salmon and fatty fish in your diet, and avoid omega-6 fats to help support a healthy heart.

Thousands of babies were born brain-damaged, after their mothers became infected while pregnant. CardioNerds (Daniel Ambinder) join Dr. Tomio Tran, Dr. Vid Yogeswaran, and Dr. Amanda Cai from the University of Washington for a break from the rain at the waterfront near Pike Place Market. They discuss the following case: A 46-year-old woman presents with cardiac arrest and was found to have cor triatriatum sinistrum (CTS). CTS is a rare congenital cardiac malformation in which the left atrium is divided by a fenestrated membrane, which can restrict blood flow and cause symptoms of congestive heart failure. Rarely, the condition can present in adulthood. To date, there have been no cases of sudden cardiac death attributed to CTS. Dr. Jill Steiner provides the E-CPR for this episode. Case Media - A Sinister Cause of Sudden Cardiac Death – University of Washington A 40-year-old woman with a history of recurrent exertional syncope had sudden loss of consciousness while kissing her partner. The patient received bystander CPR while 911 was called. EMS arrived within 10 minutes of the call and found the patient apneic and unresponsive. Initial rhythm check showed narrow complex tachycardia at a rate of 136 BPM. ROSC was eventually achieved. A 12-lead ECG showed that the patient was in atrial fibrillation with rapid ventricular rate. The patient was intubated and brought to the emergency department. The patient spontaneously converted to sinus rhythm en route to the hospital. In the emergency department, vital signs were remarkable for hypotension (76/64 mmHg) and sinus tachycardia (110 BPM). The physical exam was remarkable for an inability to follow commands. Laboratory data was remarkable for hypokalemia (2.5 mmol/L), transaminitis (AST 138 units/L, ALT 98 units/L), acidemia (pH 7.12), and hyperlactatemia (11.2 mmol/L). CT scan of the chest revealed a thin membrane within the left atrium. Transthoracic echocardiogram showed normal biventricular size and function, severe tricuspid regurgitation, pulmonary artery systolic pressure of 93 mmHg, and the presence of a membrane within the left atrium with a mean gradient of 25 mmHg between the proximal and distal left atrial chambers. Vasopressors and targeted temperature management were initiated. The patient was able to be re-warmed with eventual discontinuation of vasopressors, however she had ongoing encephalopathy and seizures concerning for hypoxic brain injury. There was discussion with the adult congenital heart disease team about next steps in management, however the patient was too sick to undergo any definitive treatment for the intracardiac membrane within the left atrium. The patient developed ventilator associated pneumonia and antibiotics were initiated. The patient ultimately developed bradycardia and pulseless electrical activity; ROSC was unable to be achieved, resulting in death. Autopsy was remarkable for the presence of a fenestrated intracardiac membrane within the left atrium and lack of other apparent congenital heart defects. There was right ventricular hypertrophy and pulmonary artery intimal thickening with interstitial fibrosis suggestive of pulmonary hypertension. In this VETgirl online veterinary continuing education podcast, we discuss atrial fibrillation (AF) in dogs. AF is a common arrhythmia in our canine patients, characterized by that classic “tennis shoes in a dryer” sound on auscultation. This arrhythmia is caused by disorganized, rapid electrical activity of the atria (Pedro). After identification of this abnormality on physical examination, it is important to confirm the arrhythmia with an electrocardiogram (ECG) and consider a full cardiac assessment to better understand the cause and severity of this finding. AF can itself lead to structural remodeling, and in humans with AF, an increased risk of sudden cardiac death (SCD) is observed (Waldmann). However, it is unclear whether this risk for SCD is also true in dogs. So, Borgeat et al wanted to evaluate this in a study entitled "Prevalence of sudden cardiac death in dogs with atrial fibrillation." In this study, the authors hypothesize that a higher proportion of dogs with AF suffer from SCD than those with sinus rhythm and that SCD would be associated with complex ventricular arrhythmias as identified on a Holter monitor.
