Chris had this as an ex-VHS rental back in the day and has chosen the neo-noir black comedy crime drama, Miami Blues (1990). Miami Blues was an adaptation of Charles Willeford's novel, directed by George Armitage and produced by Jonathan Demme. The film starred Alec Baldwin as the charming yet psychopathic criminal Frederick J. Frenger Jr., Jennifer Jason Leigh as his naive love interest Susie Waggoner, and Fred Ward as the weary, veteran detective Hoke Moseley. While authorities have confirmed that the note exists, its contents have not disclosed what it contains. According to NBC News, on Tuesday authorities released a list of items discovered at the home following the shooting. Among them were several journals, a shotgun, cellphones, laptops, and a suicide note. Old photos and yearbooks from The Covenant School, where Hale was a former student, were also recovered. The MNPD declined to provide further details regarding the contents of the note, a document which many have suggested could shine some light on the potential motive behind the mass shooting, which left six people dead. Officers did reveal, however, that many of the othe documents they found included notes about firearms courses and school shootings of the past. On Monday, police revealed that Hale planned the attack "over a period of months," and that she fired 152 rounds from the time she shot her way into the school until being killed by police. "The writings remain under careful review by the MNPD and the FBI's Behavioral Analysis Unit based in Quantico, Virginia. The motive for Hale’s actions has not been established and remains under investigation by the Homicide Unit in consultation with the FBI’s Behavioral Analysis Unit," MNPD wrote. The Department of Health and Human Services (HHS) forced Head Start workers, and some volunteers, to get a COVID-19 vaccine starting in early 2022. Head Start provides childcare to children from low-income families. HHS cited the Head Start Act, which says the health secretary can add “administrative and financial management standards,” “standards relating to the condition and location of facilities (including indoor air quality assessment standards, where appropriate),” and “such other standards as the secretary finds to be appropriate.” Texas Attorney General Ken Paxton argued that the mandate was outside of the power granted by the act. The law “does not mention vaccinations,” he and other plaintiffs said in a brief. U.S. District Judge James Wesley Hendrix agreed. “The wisdom of Head Start’s rule is not before the Court—only its legality. If the people, through Congress, wish to impose a vaccine mandate on Head Start programs, they may do so by passing a law. But an agency can do only what Congress authorizes it to do,” Hendrix, a Trump appointee, said in his ruling. “Regardless of how well intentioned, HHS’s attempt here to shoehorn the vaccine mandate into statutory language authorizing modification of Head Start’s administrative, financial, and facility-management standards goes too far. Equally fatal to the rule is the agency’s decision to implement it without the necessary public notice and comment, consultation with stakeholders, and reasonable explanation,” he added. The ruling strikes down the mandate, which Biden’s administration had refused to rescind. The mandate had already been struck down in 24 states, per a 2022 ruling by a different judge. HHS and the U.S. Department of Justice did not respond to requests for comment. “I’m proud to announce that the Biden Administration’s attempt to use the Head Start program to mask toddlers and force the vaccine on staff and volunteers has now been permanently defeated,” Texas Attorney General Ken Paxton, a Republican and one of the plaintiffs, said in an emailed statement to The Epoch Times. Officials said the settlements will resolve claims from more than 75 plaintiffs, who alleged "the Air Force was negligent when it failed to transmit to the National Instant Criminal Background Check System information about the shooter that would have prevented him from purchasing guns from a federally licensed firearms dealer." The gunman in the November 2017 attack, Devin Patrick Kelly, served in the Air Force. He died of an apparent self-inflicted gunshot wound. "No words or amount of money can diminish the immense tragedy of the mass shooting in Sutherland Springs," said Associate Attorney General Vanita Gupta. "Today’s announcement brings the litigation to a close, ending a painful chapter for the victims of this unthinkable crime." The Justice Department said Wednesday that a "federal district court in Texas concluded that the United States was liable for damages caused by the shooting." "The agreement in principle would settle all claims for a total of $144.5 million. The settlement agreement has been approved, subject to the plaintiffs’ securing the required court approvals," it added. "Under applicable law, a court must approve some aspects of the settlements." The Justice Department also said the "NICS plays a critical role in combatting gun violence, and the federal government is always striving to improve the functioning of that system." Kelley had served nearly five years in the Air Force before being discharged in 2014 for bad conduct, after he was convicted of assaulting a former wife and stepson, cracking the child’s skull. The Air Force has publicly acknowledged that the felony conviction for domestic violence, had it been put into the FBI database, could have prevented Kelley from buying guns from licensed firearms dealers, and also from possessing body armor. U.S. District Judge Xavier Rodriguez said in 2021 that had the government done its job and entered Kelley’s history into the database, "it is more likely than not that Kelley would have been deterred from carrying out the Church shooting." Kelley was shot at by two churchgoers who later pursued him. Officials said an autopsy determined Kelley was shot by Willeford twice. On Monday, March 20th, a Liberal Democratic councillor, Katherine Miles, submitted a motion to the council to have the artworks either removed or sold. The two paintings she demands be removed or sold are the Rape of the Sabine Women and Salome and Head of John the Baptist. The former depicts Roman mythology, whereas the latter is a biblical tale. Ms. Miles has been working on an initiative to increase the diversity and representation of the artwork in Oxford City Council since June 2021. She claims that the paintings depict “animal cruelty and gender-based violence”, as well as a lack of diversity. More specifically, she argues the artwork contains fox hunting, representations of death, and depictions of rape. In an interview with Oxford Mail, the newspaper that first reported the story, Ms. Miles reflected on the “lack of diversity,” stating, “[with] only three portraits in the town hall of women, it is high time the council redresses the existing lack of diversity within the town hall portraits to better reflect the gender and ethnic diversity of our city. Therefore, in her words, the collection has “no practical value to the city” nor a “clear link to Oxford”. Another Liberal Democrat councillor, Lawrence Fouweather, announced his support of the motion. He argues that the artwork could be sold to assist local residents in financial difficulties. “The Council,” he argues, “is running short of cash, and at the same time the cost of living crisis is thrusting many of our residents into tremendous hardship and poverty…this proposed one-off sale of select artworks from the city’s collection would help alleviate the dire plight of some of our most deprived residents”. Ms. Miles, on the other hand, has stated that “[s]elling any existing artwork not on display, and not becoming of a public institution, may be [sic] one way to fund the costs of any initiative to improve the decorations in the town hall”. One critic, Peter Whittle of the New Culture Forum in London, criticized the move, stating, “our culture continues to be dismantled by nobodies, morons and philistines”. No verdict has been made on the sale. A spokesman for Oxford City Council has publicly stated that, as for now, “there is no council position on this matter”. A 43-year-old man was attacked Tuesday at around 2:35 a.m., San Francisco Police said in a statement. Officers arrived at the scene on the 300 block of Main Street in response to the stabbing. The San Francisco medical examiner has not yet identified the victim, but Lee was later identified by his friends as the person stabbed, according to KPIX. Lee was rushed to the hospital to receive treatment for his life-threatening wounds, but he later succumbed to his injuries. No arrests have been made in connection with the stabbing as of Wednesday morning. The incident remains under investigation. U.S. District Judge James Wesley Hendrix agreed. “The wisdom of Head Start’s rule is not before the Court—only its legality. If the people, through Congress, wish to impose a vaccine mandate on Head Start programs, they may do so by passing a law. But an agency can do only what Congress authorizes it to do,” Hendrix, a Trump appointee, said in his ruling. “Regardless of how well intentioned, HHS’s attempt here to shoehorn the vaccine mandate into statutory language authorizing modification of Head Start’s administrative, financial, and facility-management standards goes too far. Equally fatal to the rule is the agency’s decision to implement it without the necessary public notice and comment, consultation with stakeholders, and reasonable explanation,” he added. The ruling strikes down the mandate, which Biden’s administration had refused to rescind. The mandate had already been struck down in 24 states, per a 2022 ruling by a different judge. HHS and the U.S. Department of Justice did not respond to requests for comment. “I’m proud to announce that the Biden Administration’s attempt to use the Head Start program to mask toddlers and force the vaccine on staff and volunteers has now been permanently defeated,” Texas Attorney General Ken Paxton, a Republican and one of the plaintiffs, said in an emailed statement to The Epoch Times. In court filings, defendants had urged the judge not to block the mandate. The COVID-19 vaccines do not prevent transmission or infection, according to data from the U.S. Centers for Disease Control and Prevention, though observational data have indicated they have a small effect on both that might turn negative over time. The National Head Start Association, a nonprofit, welcomed the judge’s ruling. Head Start previously rescinded its mask mandate after months of lobbying from the nonprofit and others. The gunman in the November 2017 attack, Devin Patrick Kelly, served in the Air Force. He died of an apparent self-inflicted gunshot wound. "No words or amount of money can diminish the immense tragedy of the mass shooting in Sutherland Springs," said Associate Attorney General Vanita Gupta. "Today’s announcement brings the litigation to a close, ending a painful chapter for the victims of this unthinkable crime." The Justice Department said Wednesday that a "federal district court in Texas concluded that the United States was liable for damages caused by the shooting." "The agreement in principle would settle all claims for a total of $144.5 million. The settlement agreement has been approved, subject to the plaintiffs’ securing the required court approvals," it added. "Under applicable law, a court must approve some aspects of the settlements." The Justice Department also said the "NICS plays a critical role in combatting gun violence, and the federal government is always striving to improve the functioning of that system." Kelley had served nearly five years in the Air Force before being discharged in 2014 for bad conduct, after he was convicted of assaulting a former wife and stepson, cracking the child’s skull. The Air Force has publicly acknowledged that the felony conviction for domestic violence, had it been put into the FBI database, could have prevented Kelley from buying guns from licensed firearms dealers, and also from possessing body armor. U.S. District Judge Xavier Rodriguez said in 2021 that had the government done its job and entered Kelley’s history into the database, "it is more likely than not that Kelley would have been deterred from carrying out the Church shooting." Kelley was shot at by two churchgoers who later pursued him. Officials said an autopsy determined Kelley was shot by Willeford twice. On Monday, March 20th, a Liberal Democratic councillor, Katherine Miles, submitted a motion to the council to have the artworks either removed or sold. The two paintings she demands be removed or sold are the Rape of the Sabine Women and Salome and Head of John the Baptist. The former depicts Roman mythology, whereas the latter is a biblical tale. Ms. Miles has been working on an initiative to increase the diversity and representation of the artwork in Oxford City Council since June 2021. She claims that the paintings depict “animal cruelty and gender-based violence”, as well as a lack of diversity. More specifically, she argues the artwork contains fox hunting, representations of death, and depictions of rape. In an interview with Oxford Mail, the newspaper that first reported the story, Ms. Miles reflected on the “lack of diversity,” stating, “[with] only three portraits in the town hall of women, it is high time the council redresses the existing lack of diversity within the town hall portraits to better reflect the gender and ethnic diversity of our city. Therefore, in her words, the collection has “no practical value to the city” nor a “clear link to Oxford”. Another Liberal Democrat councillor, Lawrence Fouweather, announced his support of the motion. He argues that the artwork could be sold to assist local residents in financial difficulties. “The Council,” he argues, “is running short of cash, and at the same time the cost of living crisis is thrusting many of our residents into tremendous hardship and poverty…this proposed one-off sale of select artworks from the city’s collection would help alleviate the dire plight of some of our most deprived residents”. Ms. Miles, on the other hand, has stated that “[s]elling any existing artwork not on display, and not becoming of a public institution, may be [sic] one way to fund the costs of any initiative to improve the decorations in the town hall”. One critic, Peter Whittle of the New Culture Forum in London, criticized the move, stating, “our culture continues to be dismantled by nobodies, morons and philistines”. A 43-year-old man was attacked Tuesday at around 2:35 a.m., San Francisco Police said in a statement. Officers arrived at the scene on the 300 block of Main Street in response to the stabbing. The San Francisco medical examiner has not yet identified the victim, but Lee was later identified by his friends as the person stabbed, according to KPIX. Lee was rushed to the hospital to receive treatment for his life-threatening wounds, but he later succumbed to his injuries. No arrests have been made in connection with the stabbing as of Wednesday morning. The incident remains under investigation. Lee was the chief product officer of San Francisco-based cryptocurrency company MobileCoin. He previously worked at Google, where he led Android's core library team and launched "the world’s most used operating system," his profile on MobileCoin’s official website reads. The tech mogul also founded the social network Present, and invested in and advised other companies, including Figma, Clubhouse, Beeper and Faire. On November 5th, 2017, a gunman opened fire on worshippers at the First Baptist Church in Sutherland Springs, TX. A churchgoer, Stephen Willeford, then shot him twice causing him to flee the scene but not before he killed 26 people and wounded another 22. Willeford chased him down causing him to crash and when the police arrived at the scene they discovered the shooter dead from a self inflicted gunshot wound. As usual, there were red flags on this guy before the shooting took place including a domestic violence charge while he was in the Air Force that was never reported the FBI's National Crime Information Center that would have prevented him from purchasing the weapon used in the killings. Many people will claim that there is never a “Good Guy With A Gun” to save the day in an Active Shooter Situation. While simply untrue, there is a point to be made here: 94% of Active Shooter Events take place in so-called “Gun Free Zones”. This means that law abiding citizens are rarely able to carry near the incident. We'll look at several cases today where the citizens had to go to their car to get their gun. We took a dive into the 2000-2018 FBI stats on “Active Shooter” cases and we found at least 47 cases that were stopped in part or in whole by citizens, some with guns, and some without. Some of these are off-duty law enforcement, but these people certainly qualify as “Good Guys With Guns” and they were not there because of a 911 call. Again, most people will not carry a firearm in a gun free zone, but off-duty LEOs may feel more free to do so than most because of their ID, training, and position. Many were un-armed and were injured or certainly risked it all to wrestle the attacker down with their bare hands. Some didn't make it, but saved countless others. Let me ask you a question: Which is more evil: The gunman, or disarming the people who now provide the gunman easy targets? We could philosophize on the merits of a good guy or we could just tell you there stories: Story 1: In the early morning of April 23, 2001 before any children had been picked up for school in San Jose, California, CP went to the Bus Barn where she had worked for the better part of a decade, and began murdering her co-workers. She successfully killed a driver-trainer and wounded three other ladies, before Gregory Allan Lee, another driver, caught her attention and ultimately restrained CP until the police could arrive, saving an untold number of lives. https://www.latimes.com/archives/la-xpm-2001-apr-24-mn-54869-story.html Story 2: Near lunch time on July 28th, 2003, a disgruntled employee, RB, of the Kanawha School Board attempted to set his supervisor ablaze with a bucket of gasoline 10 minutes into the school board's meeting. After his match failed to light, RB resorted to his rifle, but was restrained by onlookers before he could inflict fatal wounds. These onlookers had to restrain RB without the aid of a firearm as they were at a School Board Meeting. He killed the owner of Gold-Leaf Nursery and one of his wife's co-workers after he had ended his wife's life. AC had a hard time figuring out which man was his wife's lover, and never had the chance to do so. After escaping momentarily, Bartell was shot in the forehead by JM, who went on as if to continue the shooting spree. Mark Johnson, a PE Teacher, confronted JM who raised his gun to shoot Johnson. This time however, it was a .22 caliber that KB had brought to school. Upon being confronted, KB shot and killed an assistant principal and shot two other faculty. He was restrained by students and teachers until police took him away. He was later released from prison only to be in and out of prison with some charges including domestic abuse and allegedly killing his girlfriend's toddler. Fagan had the courage to confront JN and demand that he put the gun down and wait for authorities. JN heeded Fagan's prompting. https://en.wikipedia.org/wiki/Pine_Middle_School_shooting Story 9: Angered by the disciplinary actions meted out by Principal Klang at Weston Highschool, EH walked into school at 8:00 AM on September 29th, 2006 and aimed a shotgun at the Social Studies teacher. A janitorial staff member jumped EH and wrestled the shotgun out of EH's hands. Upon seeing the staff member struggling with the student, Principle Klang attempted to help the struggling janitor restrain EH. EH, however also had a .22 caliber pistol with which he shot Principle Klang multiple times. According to the manifesto DA wrote, he intended to shoot congregants until he was shot by police. God had other plans. 10 Stories of Good Guys with a Gun Story 11: On March 5, 2001, at 9:20 a.m., Off-Duty San Diego PD Officer Bob Clarke was enrolling his teenage daughter in her new High School Santana High. Upon hearing the commotion, Clarke ran into the highschool where he found and apprehended CW, the active shooter who had murdered two of his fellow students and injured 13 more. Clarke was joined by three other officers on his way into the building, but once again, this is a matter of good guys on scene with guns who were not part of the 911 response. PO had just been suspended for poor academic performance so he had come back to shoot Dean L Sutin, the one he blamed for his pain. Faculty member Thomas F. Blackwell was also present and therefore was also shot fatally. Former school employee and now student Angela Denis Dales, 33 was the last fatal victim. While three other students were shot, no one else died that day due to the quick action of other Criminal Law students, two of whom had been already serving in law enforcement. ES was attending the chapel that day to “kill at least 10 white people” in revenge for the “Charleston Shooting”. Upon confronting ES, Caleb Engle suffered several injuries, including a dislocated shoulder, broken nose, and a gash to his forehead, ES suffers a gunshot wound from ES's weapon. Caleb Engles then retrieves a firearm from his father's, James Engles, truck. Don Smith and Nathan Taylor, co-workers of Cordero and Smith, were legal concealed carriers and put two bullets into RB, who they held, alive, for police. AT entered the eatery from the front door wearing eye and ear protection and no other noted motivation. Nazario and Wittle fired on AT, killing him, saving an unknown amount of lives, but forever leaving AT's motivations a mystery. Both died. The carnage ended when two legally armed citizens engaged GB and GB fled the scene, only to later be captured by police. Willeford grabbed his AR-15 and ran out the door without his shoes when his daughter spotted a man across the way in black tactical gear outside of the Baptist Church. Almost like a western, Willeford called out the shooter, who had murdered 26 people and wounded 22 others from the street. At this moment the shooter turned and left the building. There can be a lot of pressure to take on the family business, especially when the family name becomes synonymous with the industry they work in. Imagine what it must be like to be a Hilton or a Disney or a Manning. Everybody copes with it differently. Some embrace it. Some make their own mark on it. Some leave it all behind and do something else entirely. Christiaan's guests on this edition of Out to Lunch Acadiana come from Acadiana's wide world of sports. Christian Willeford is the son of Beau Willeford, the legendary boxing coach and operator of the Ragin Cajun Boxing Club. Willeford's dad trained scores of boxers in Acadiana over the years and brought the prestigious boxing championship, the Golden Gloves, to Lafayette. Sadly, Beau passed away in 2019 after a brief bout with cancer. Christian recently announced that he will close the club after a nearly four-decade run but is keeping the Golden Gloves going. Leigh Hennesy Robson first made a name for herself as a world trampoline champion. Leigh was coached by her father, Jeff Hennesy, a prominent name in competitive trampolining. For the last two decades, however, Leigh has made a second name for herself, this time in the movie business. Leigh is a stunt double and stunt coordinator. She was Demi Moore's double in GI Jane and Lucy Liu's in Charlie Angels. Leigh is one of only a few hundred stunt women working in the film industry today. Riding Shotgun With Charlie #088 Stephen Willeford Last month, I was at the Rod of Iron Freedom Festival in Greeley, PA, and was able to get some time with one of the featured speakers, Stephen Willeford. We’ve all heard the story about what happened on November 5. 2017 at a church in Sutherland Springs, TX. Mr Willeford was the good guy with a gun who heard the gunshots, grabbed a handful of ammo and his AR 15 and headed to confront the murder. Stephen Willeford grew up shooting and hunting on a dairy farm. He saved the money he earned to buy .22 ammo and eventually a .30-30 lever action rifle. As a parent, his church parishioners would have regular shooting competitions on Sundays, teaching the children gun safety while the mothers shot. Then the mothers would take the kids while the fathers shot. On the day that changed Stephen’s life, he was taking it easy on a Sunday afternoon while he was on call at a local hospital where he was a plumber. His daughter heard gunshots nearby and yelled to her dad. Stephen grabbed his rifle and pursued the murderer at the church down the road. Stephen was able to distract the murderer and exchanged gunfire with him. The murderer ran out of the church and into the getaway car he had ready. Stephen ran to a man sitting in a truck, who saw the whole gun right. Stephen hopped into the stranger’s car who was arealdy talking to 911 on the phone. Stephen and the driver, Johnny, got into a high speech chase for several miles. Stephen checked his rifle’s condition and found out he only had one round in the magazine and one in the chamber. He told the driver that they may have to crash into the car to stop it. But Johnny already knew that. The murderer ended up driving his car off the road and killed himself. But Stephen and Johnny didn’t know he was dead. They waited 5-7 minutes for the police to arrive. It was another 4 hours before Stephen could go home a hero. You can hear how Stephen sees that God was in control over the situation, even though 27 people were killed and 20 more were injured and lived. There are several things that happened that day that proved it. From the local hospital having an emergency training drill which meant several doctors and nurses on hand, to a church member who is a police officer who had several trauma kits in his car. This story is one that many of us heard and read about in the news. Now, you can listen to the whole story, the real story, as Stephen Willeford tells us exactly how things happened that day. Dr Carolyn Lam: Welcome to Circulation on The Run, your weekly podcast summary and backstage pass to the journal and it's editors. I'm Dr Carolyn Lam, Associate Editor from the National Heart Center and Duke National University of Singapore. We will be discussing accelerated diagnostic protocols for chest pain, a very, very important issue in Cardiology with very important new safety and effectiveness data on one such protocol provided in our feature paper this week. Coming right up after these summaries. Our first original paper this week identifies a new link between specific gut bacteria and atherosclerosis. Co-First authors, Dr Yoshida and Emoto, corresponding author, Dr Yamashita, from Kobe University Graduate School of Medicine, and colleagues recruited patients with coronary artery disease and controls without coronary artery disease but with coronary risk factors. They then compared gut microbial composition using 16S ribosomal RNA gene sequencing in fecal samples. Subsequently, they used atherosclerosis prone mice to study the mechanisms underlying the relationship between such species and atherosclerosis. Their analysis of gut microbial profile in patients with coronary artery disease showed a relative depletion of bacteroides vulgatus and bacteroides dorei compared to controls with coronary risk factors. Gavage with live bacteroides vulgatus and bacteroides dorei decreased fecal and plasma lipopolysaccharide levels and protected against atherosclerosis in apoE deficient mice. Fecal lipopolysaccharide levels in patients with coronary artery disease were significantly higher compared to controls. These findings suggest that bacteroides treatment may serve as a novel and effective therapeutic strategy for suppressing lipopolysaccharide-induced inflammatory response in coronary artery disease. The next paper identified a potential novel molecular target in the treatment of myocarditis. Co-First authors, Dr Chen and Zeng, Co-Corresponding authors, Dr Song from Fuwai Hospital in Beijing, and Dr Yang from Shenzhen University School of Medicine, and their colleagues aim to elucidate the role of BCL2 Like protein 12 in the pathogenesis of biased T Helper-2 response in myocarditis. Using a combination of mouse models of myocardial inflammation and human hearts from patients undergoing heart transplantation, the authors found that CD4 positive T-cells isolated from hearts in myocarditis at the end stage of heart failure expressed high levels of BCL2 Like protein 12, which was required for the development of aberrant T Helper 2 polarization in the heart. Thus, BCL2 Like protein 12 may be a novel target in the treatment of myocarditis, as well as other T Helper 2 biased inflammatory processes. Could vaccination against LDL be a way to prevent atherosclerosis? Well, the next paper brings us one step closer to this dream. First author, Dr Gisterå, corresponding author, Dr Hansson from Karolinska School University Hospital and colleagues developed T-cell receptor transgenic mice to study LDL autoimmunity in a humanized hypercholesterolemic mouse model of atherosclerosis. A strong T-cell dependent E-cell response was induced by ODL leading to production of anti-LDL IgG antibodies that enhanced LDL clearance and ameliorated atherosclerosis. Results show that anti-LDL immuno-reactivity evoked three atheroprotective mechanisms, namely 1) antibody-dependent LDL clearance, 2) increased cholesterol excretion, and 3) reduced vascular inflammation, thus targeting LDL-reactive T cells may enhance atheroprotective immunity, and vaccination against LDL components may be an attractive way to prevent atherosclerosis. MicroRNAs regulate nearly all biological pathways and dysregulation of MicroRNAs is known to lead to disease progression. However, are there cell type specific effects of MicroRNAs in the heart? Co-First authors, Drs Rogg and Abplanalp, corresponding author, Dr Dimmeler from Goethe University Frankfurt, and colleagues assessed MicroRNA target regulation using MicroRNA 92a3p as an example. Their data showed that MicroRNAs have cell type specific effects in vivo which would be overlooked in bulk RNA sequencing. Analysis of MicroRNA targets in cell subsets disclosed a novel function of MicroRNA 92a3p in endothelial cell autophagy and cardiomyocyte metabolism. These findings may have clinical applications for the fine tuning of autophagy and metabolism to mitigate tissue damage in patients with cardiac disease. The next paper establishes a mechanism by which cardiac inflammation may be initiated in response to hemodynamic stress, but in the absence of significant cardiomyocyte cell death. Co-First authors, Drs Suetomi and Willeford, Co-Corresponding authors, Drs Brown and Miyamoto from University of California San Diego, and their colleagues used conditional cardiomyocyte-specific calcium calmodulin-regulated kinase Delta all CaM kinase II Delta knockout mice to demonstrate that cardiomyocytes generate inflammatory chemokines and cytokines and are the initial site of NLRP3 inflammasome activation. They further identified a causal role for CaM-Kinase II Delta-mediated activation of NLRP3 inflammasome and inflammatory responses in macrophage recruitment, cardiac fibrosis, and development of heart failure induced by pressure overload. Their elegant mouse experiments revealed sites and mechanisms of proinflammatory gene and inflammasome activation within cardiomyocytes which could serve as targets for early intervention or disease prevention. Are there different metabolomic effects between PCSK9 inhibitors and statins? First author, Dr Sliz, Corresponding Author, Dr Würtz from Nightingale Health Limited in Helsinki, Finland, and their colleagues quantify 228 circulating metabolic measures by Nuclear Magnetic Resonance Spectroscopy for over 5300 individuals in the PROSPER Trial at six months post randomization. The corresponding metabolic measures were also analyzed in eight population cohorts, including more than 72,000 individuals using a specific PCSK9 inhibitor SNP as an unfounded proxy to mimic the therapeutic effects of PCSK9 inhibitors. Scaled to an equivalent lowering of LDL cholesterol the effects of genetic inhibition of PCSK9 on these 228 metabolic markers were generally consistent with those of statin therapy. Alterations of lipoprotein lipid composition and fatty acid distributions were also similar. However, discrepancies were observed for very low-density lipoprotein or VLDL lipid measures where genetic inhibition of PCSK9 had weaker effects on lowering VLDL cholesterol compared with statin therapy. Genetic inhibition of PCSK9 showed no significant effects on amino acids, ketones, or a marker of inflammation, where a statin treatment weekly lowered this marker of inflammation. Thus, if VLDL lipids have an independent causal effect on cardiovascular disease risk, the observed discrepancy on VLDL lipid lowering could contribute to differences in cardiovascular risk reduction between statins and PCSK9 inhibitors for an equivalent reduction in LDL cholesterol. Moreover, these results exemplify the utility of large-scale metabolomic profiling with genetics and randomized trial data to uncover potential molecular differences between related therapeutics. The final original paper this week demonstrates a novel biomarker discovery paradigm to identify candidate biomarkers of cardiovascular and other diseases. Co-First authors, Dr Mosley and Benson, co-corresponding authors, Dr Wang from Vanderbilt University Medical Center and Gerszten from Beth Israel Deaconess Medical Center, and their colleagues employed a virtual proteomic approach linking genetically-predicted protein levels to clinical diagnosis in more than 40,000 individuals. They used genome-wide association data from the Framingham Heart Study to construct genetic predictors for more than 1100 plasma protein levels. They validated the genetic predictors for 268 proteins and used them to compute predicted protein levels in more than 41,000 genotyped individuals in the eMerge Cohort. They tested associations for each predicted protein with more than 1100 clinical phenotypes. These associations were validated using directly-measured protein levels and either LDL cholesterol or subclinical atherosclerosis in the Malmo Diet and Cancer study. Using this virtual biomarker strategy the authors identified CLC1B and PDGFR Beta as potential circulating biomarkers of atherosclerosis and validated them in an epidemiologic cohort. Thus, these results demonstrate that a virtual biomarker study may efficiently identify potential biomarker disease associations, and that wraps it up for our summaries. Now for our feature discussion. Accelerated diagnostic protocols for testing are used everywhere. They're designed to improve the quality and value of chest pain risk stratification. However, many of them lack sufficient prospective safety and effectiveness data. We're so pleased to have a paper today that provides such important data on one of these accelerated diagnostic protocols for chest pain, and it's the HEART Pathway. To discuss this, I've got the corresponding author of today's featured paper, Dr Simon Mahler from Wake Forest School of Medicine, as well as our Associate Editor, Dr Deb Diercks from UT Southwestern. Simon, could you start by just telling us, what is the HEART Pathway? Dr Simon Mahler: Sure. Yeah, it's an accelerated diagnostic protocol. It's based on an accelerated diagnostic protocol called the HEART Score. We use a modified version of the Heart Score. We actually use a HEAR score, and that stands for the history, EKG, Age, and risk factors. That is combined with two troponin measures at 0 and 3 hours. We also factor in whether or not the patient has had prior coronary artery disease or has an acute ischemic EKG. So, to be low-risk you have to have a HEAR score of 0-3. HEAR is an acronym. You get points for each of those categories. If you have less than 3 points that's a low score. You have to have a low score, a non-ischemic EKG, no history of prior coronary disease, and two troponins less than a 99th percentile at 0 and 3 hours to be considered low risk and recommended for early discharge. If you don't meet any of those criteria then you are considered non-low risk and appropriate for further in-hospital evaluation. Dr Carolyn Lam: That's great. Could you just tell us what you did to give us some real-world safety and effectiveness data on this. Dr Simon Mahler: Yeah, so we had done a single-site randomized controlled trial. That was published in 2015 in Circulation: Quality and Outcomes, and really showed some promising results. We received some funding to do an implementation trial. So, this is the results of our implementation study. It's a before and after study. What we did was we sought to implement a HEART Pathway as a clinical decision support tool, integrated fully into our electronic medical record so that when providers see the patient with chest pain and order a troponin they interact with a HEART Pathway tool that guides them through the HEART Pathway risk assessment and then provides real-time decision support regarding their treatment and disposition decisions based on whether or not the patient has a low-risk assessment or a non-low-risk assessment. The design of the study was we collected data on all patients with chest pain and troponin order for one year while we worked on how we were gonna build this tool and embed it, and then we had three month watching period where we built the tool into the electronic health record across our three sites. Then, we had one year where we were post implementation where we collected data and looking at the difference in outcomes, particularly looking at both safety and utilization outcomes before and after use of the HEART Pathway. Dr Carolyn Lam: That's just such a clever design. Just give us a summary of the results before I ask Deb to chime in here. Dr Simon Mahler: There's a few really important things that we found. Probably the most important thing was the safety data that came out of this study. We had some good safety signals on prior studies. They didn't have enough sample size to really have a good precision around the safety point estimate, so in this study we had over 4000 patients in our post-implementation cohort, and about 31%, 30.7%, of those patients were classified as low-risk by the HEART Pathway. Among those patients that were classified by low-risk, the rate of death and MI, the composite outcome at 30 days, was 0.4%. Typically for these accelerated diagnostic protocols we want them to have an adverse cardiac event rate less than 1%, so a finding of 0.4% with a confidence in our role that doesn't extend beyond 1% that was a really important finding that really confirms the safety of this strategy. The other thing that we found which was interesting was that the use of the HEART Pathway was actually associated with detecting more myocardial infarctions during the index visit, which means that possibly the HEART Pathway use improved the recognition of those patients that were presenting with MIs. It's possible that without using the HEART Pathway some of those cases may have been missed. Finally, we were able to demonstrate that use of the HEART Pathway as a clinical decision support tool was able to decrease hospitalizations and some other utilization metrics such as stress testing and possible length of stay. Dr Carolyn Lam: Oh, that's awesome, Simon. I said it earlier. I'm gonna say it again. Thank you so much for publishing this wonderful work with Circulation. I really think that implementation, science, and decision support tools you've got that all in this paper, just beyond even the actual topic. Deb, take us behind the scenes a little bit with how we reacted as editors to this paper, please. Dr Deb Diercks: Well, I think that overall, we were really excited about this paper. It really does add a real, real context to something we were really discussing and wondering about. I think one of the great things about the implementation, and Simon, please comment on this, is the diversity of the places that you actually used this in. I mean, most of us when we look at papers there's always a fear that it won't be able to be generalized to real-world practices. Correct me if I'm wrong, but you really applied it to just a wide variety of Emergency Departments that really support that this could be used anywhere. Dr Simon Mahler: Yeah, I think that's a really important point, that we did this across our system so that included a large academic busy Emergency Department that sees over 100,000 patients per year, all the way, basically to a smaller 12,000 per year, essentially almost a free-standing Emergency Department at the time that we started our study; it now has inpatient bed capacity, and then a suburban/rural hospital, as well, with about 30,000 patient visits per year. We extended beyond kind of the typical kind of comfort zone of large academic centers and into smaller community Emergency Departments as well. Dr Deb Diercks: One of the things that this manuscript nicely articulated is that you kind of break it into the HEAR and then the troponin. Dr Simon Mahler: Right. Dr Deb Diercks: Things change in the US with troponin. How do you think that's gonna impact how you guys apply this Pathway in the future? Dr Simon Mahler: It's a big topic of discussion right now, what to do with these Pathways. Are these Pathways still needed with the availability now of high-sensitivity troponins in the United States? I think that for many years as we've kind of followed data coming out of Europe we've been anxiously awaiting the arrival of these tests in the U.S., and there's a lot we can learn from the European data so far. Most of that data suggests that the high-density troponins are best used still in the context of a Pathway or an accelerated diagnostic protocol. I think that this particular study was conducted just using contemporary troponins, particularly given the time frame of the study in which we were accruing patients from 2013 through 2016, but I think it's still gonna be highly relevant, because I think that best practices are gonna still require us to use some sort of structured framework with high-sensitivity troponins. Now, it does remain to be seen a little bit what the best Pathway is gonna be to incorporate that. My take on this is that I believe that clinical decisions support tools or decision aids integrated with high-sensitivity troponins is going to be the best way to go. I'm a little bit skeptical about troponin-only approaches. Dr Deb Diercks: That's a great summary. I don't think it's time to throw out all the value of that risk stratification tool, and I think your study showed that how it can easily be incorporated into what we do in a manner that doesn't really negatively impact the work flow, which I think is so important. Dr Simon Mahler: You know, we did a smaller study where we looked at the performance of the HEART Pathway with high-sensitivity assays. We studied it with both the Roche troponin high-sensitivity troponin T and the Abbott high-sensitivity I, and at the 99th percentile it actually made very little difference in terms of the performance of the HEART Pathway. What the potential advantages of incorporating high-sensitivity assays is that you probably no longer need a 0 and 3 hours, evaluation can be condensed. I think there's a lot of really interesting questions that availability of high-sensitivity troponins has created, and I think that there's gonna be a lot of emerging evidence over the next few years about new Pathways, and what are the best ways to fully take advantage of these higher-sensitive assays because, frankly, most of the decision aids that are currently in use they were developed using contemporary troponins, and they may not fully take advantage of high-sensitivity troponins. We may see modifications of our Pathway, and it will interesting to see kind of how things evolve as we study the impact of high-sensitivity troponin. Dr Carolyn Lam: Wow, exciting work ahead. Just one last question regarding the future. So, you followed up the patients in your study for 30 days. Am I wrong? Any plans to follow them up longer, and do you think such data are needed? Dr Simon Mahler: Yeah, we actually followed them for a year. Our primary analysis was through 30 days, and so we do have one-year data on all of our patients, and so we'll be doing a secondary analysis looking out to a year. Yeah, you can look forward to that coming up hopefully in the next six months or so. Dr Carolyn Lam: That is awesome. Thank you so much, Simon. Thank you so much, Deb. The Incident & The Suspect * Below are elements of the official story as I know them, not necessarily what I believe. The massacre inside the First Baptist Church in Sutherland Texas was caught on camera. For seven minutes, a man wearing all black body armor, a protective helmet and possibly a grim reaper mask moved from aisle to aisle shooting churchgoers and their children execution style, in the head. When he left, 25 of the 50 or so member congregation were dead and 21 more victims were wounded, 1 of which would later die in the hospital. I say possibly wearing a grim reaper mask because there has been some inconsistencies in the reporting on this. That man who investigators say is responsible for the worst mass shooting in Texas history is twenty-six year old Devin Patrick Kelley, a former Air Force member who court-martialed in 2012 for beating his wife and cracking his 2-year old step-son's skull. Prior to his court martial, according to the El Paso Police Department, Kelley had escaped from Peak Behavioral Health systems in Santa Teresa, New Mexico. Kelley had been committed after having been caught sneaking firearms onto the Air Force base in what authorities believe was an attempt to carry out death threats he had previously made towards his chain of command. The following year, in 2013, he was investigated for sexual assault. In 2014, he plead guilty to a cruelty to animal charge after being accused of repeatedly punching a dog in the face. And in the months approaching the shooting, he preached atheism on Facebook and mocked those who believed in religion. Oh, and it was initially reported that he once taught 4 to 6 year old children bible studies. So there's your suspect in a nutshell. I'm sure I'm leaving a few things out. The Alleged Motive Investigators have suggested that the shooter's motive was related to a domestic issue, and that his mother-in-law, who often attended the church, might have been the target. She was not present on that day however. This raises questions. If he was after his mother-in-law, why was he in full SWAT gear? Why did he need an assault rifle capable of holding more than 400 rounds of ammunition? Why did he need multiple extra weapons which were found inside his vehicle? Why wouldn't he just go after her? While his mother-in-law may have been one of his targets, it's hard to believe the simple domestic related motives that have been suggested. The Good Samaritan Heroes Upon leaving the church, the suspect was met with gunfire from a local who is being hailed a hero. Stephen Willeford was in relaxing in his home a few blocks away when his daughter alerted him to the gunfire. Willeford, a former NRA instructor, grabbed his rifle, a few magazines and was out the door before even putting his shoes on. (Reminiscent of John McClane in the original Die Hard movie) Taking cover behind a neighbors truck, Willeford exchanges gunfire with Kelley from across the street. Kelley is shot twice. (Willeford was aiming for the gaps in the body armor). Kelley then drops his rifle, flees to his SUV and speeds off. I don't know how long this gunfight lasted. The likely assumption is that it was brief, just not sure how brief. After Kelley speeds away, Willeford notices a truck sitting in the intersection. He approaches the truck and enlists the help of the driver, Johnnie Langendorff, the other hero of the story. Willeford hops in Langendorff's truck and the two good samaritans race after the suspect at speeds reaching 95 mph. During the chase, Langerdorff spoke with police as he drove and Willeford fired at the suspect's SUV, shattering its back window. After about a 11 mile chase, Kelley's vehicle hits a road sign, speeds up and then overturns in a ditch nearby. The two locals then pull up to the scene. Willeford exits the truck with his rifle and calls for the suspect to get out of the overturned vehicle. There is no response. Willeford keeps his gun fixed on the overturned SUV until police arrive. Authorities later allege that prior to driving into the ditch, the shooter called his father and told him that he wasn't going to survive his injuries. Then he shot himself in the head. This is quite a story, one that raises many questions. Meaning, he helps dental consultants help their dentists with their numbers. He has the formulas and the calculations to figure out the health of dental practices. He does not normally work directly with dentists and so hearing his story is interesting from our perspective. This episode serves as the intro to the series of interview coming over the next few months concentrating on numbers in the dental practice. We discuss general numbers and concepts but will be diving much deeper into them in future episodes.
