High readmission rates can result in financial penalties and are often caused by the complexity of the patient's condition and social determinants of health. Puzzle is working with health systems and SNFs to implement a readmission prevention program that includes physiatry and care coordination 90 days after discharge. Ahzam explains, "Hospital readmissions have become a focal point for our healthcare systems for several reasons. First and foremost, they are a key indicator of the quality of care being provided by these facilities. High readmission rates often suggest that patients may not be receiving the comprehensive care they need during their hospital stay or the necessary support post-discharge. This could be due to inadequate discharge planning, insufficient patient education on their conditions, or a lack of effective follow-up care." Synopsis: Every second Monday of the month, The Straits Times takes a hard look at Singapore's social issues of the day with guests. Youth Corps Singapore, which is under the National Youth Council, turns 10 in 2024. It is a division under the council that champions youth volunteerism to ignite positive change in society through community service. Nazir Afzal Nazir Afzal OBE was Chief Crown Prosecutor for North West England, and most recently, he was Chief Executive of the country's Police & Crime Commissioners. During a 24 year career, he has prosecuted some of the most high-profile cases in the country, including the so-called Rochdale grooming gang, and led nationally on several legal topics, notably violence against women and girls, child sexual abuse, and honor-based violence. Nazir was recently appointed to the Independent Press Standards Organisation (IPSO) and is National Adviser on Gender-Based Violence to the Welsh Government. Nazir joins me today to discuss his experience of growing up in inner-city Birmingham and why it could be a frightening place for his family and community. He shares what he found so fulfilling about working in the Crown Prosecution Service and how a move to London allowed him to build his skill set, develop and grow. Nazir also describes his involvement in the Rochdale case, the bravery of the young women giving evidence, and why it subsequently triggered a change in guidelines for cross-examinations in sensitive cases. Research estimates 1 in 5 women will lose a pregnancy in their lifetime and 1 in 20 will go through it more than once, but no official record is kept of how many miscarriages happen each year. After losing four pregnancies in the space of two years, with no obvious cause, Jennie Agg set out to understand why miscarriage remains such a profoundly misunderstood, under researched and under acknowledged experience. She has written about it in Life, Almost, which documents her path to motherhood and her search for answers. Dorset Police are investigating allegations that firefighters at Dorset and Wiltshire Fire Service had taken photos of women who had died in car accidents and shared the images on a Whatsapp group. In the group, male firefighters are alleged to have made degrading comments about the victims. Several female firefighters also spoke of sexual harassment, including claims a male firefighter demanded sexual favours at the scene of a fire. Nazir Afzal, the former chief prosecutor for North West England who carried out an independent review into the London Fire Brigade last year and Zoe Billingham, former head of the Inspectorate of Constabulary and Fire and Rescue consider if the fire service has a problem with its culture, and in particular women. The London Fire Brigade is "institutionally” misogynist and racist, according a report into its culture. The review, conducted by the former Chief Crown Prosecutor for north-west England, Nazir Afzal, concludes that unless the "toxic culture" is tackled other firefighters will take their own lives. The independent review was established by the London Fire Commissioner after a trainee firefighter took his own life in August 2020. Today's Martha Kearney spoke to Mr Afzal, who told her that he believed the issues he uncovered are common in other fire services. He added he was approached by other fire brigades and organisations as he undertook the review. Martha also spoke to the Commissioner of the London Fire Brigade Andy Roe, who said there was no place for discrimination, harassment and bullying in the brigade.

Where do we place the blame? Steve invited our friend Aqdas Afzal back on the podcast to discuss his recent article, “Collapse of Civilizations.” The article's title is a cheeky play on Samuel Huntington's “Clash of Civilizations,” which predicted massive conflict between the world's non-white, non-Christian peoples and the global North. Coincidentally, the countries of the global North have profited quite well from their destruction of the environment, whereas those in the global South bear the brunt – like floods of biblical proportions. Pakistan's contribution to carbon emissions is less than 1% yet, when hit by climate catastrophe, the devastation is not only physical, it is economic, it is political. With an economy choked by foreign debt obligations, Pakistan, in the best of times, struggles to meet basic needs. “Steve, to give you an example, about 40% of Pakistan's federal budget - remember that figure - 40% is now spent on paying interest on external loans that Pakistan has taken over the last 75 years. And this situation not only eats up all the fiscal space that this country has, we cannot spend on health, we cannot spend on education, we don't have enough money to spend on climate mitigation adaptation, on clean drinking water. And the situation is becoming worse by the day. It's a completely unsustainable situation.” Aqdas and Steve talk about debt jubilee and reparations to address the immediate situation, but the overlapping crises are a direct result of capitalism's failure to deliver on its promises – not just to Pakistan, but to most of the world – causing strife and division. Are we proving Samuel Huntington right? Aqdas Afzal finished his undergraduate and first master's degree in Political Science from Ohio State University, then returned to his native Pakistan. After working there for five years he won the Fulbright scholarship for his second master's and PhD in Economics from UMKC. The Black Spy Podcast A sit down with Nazir Afzal - The first Muslim CPS (Crown Prosecution Service) Chief Prosecutor in England & Wales This week Carlton King explores the life and times of Nazir Afzal, the renowned first Muslim Chief Prosecutor of the Crown Prosecution Service (CPS) in England & Wales' (North West Region). The nearest equivalent body to the CPS in the United States is the District Attorney's office. We also learn about Nazir's tough up-bring in inner city Birmingham in the 1970's, his distrust of the police and other English institutions of state in the era and the depth of feeling this created within the young Nazir. We are made to understand why he decided to become a lawyer, rather than the doctor or engineer his parents hoped he'd become. And we learn about Nazir's journey into prosecuting on behalf of the British state. We also, discuss the all encompassing racism Nazir was subjected to from his very early life onward, to his somewhat more comfortable rise to the top of his profession. Finally, the men discuss a plethora of current and systemic problems suffered by the contemporary British state. Carlton points out that the CPS very rarely takes responsibility for its decisions and cites the Stephen Lawrence case as a prime example of this, noting that after Stephen Lawrence's murder the Met police within ten days had arrested four suspects, however, the crown prosecution service told police that in their opinion, their was not enough evidence to gain a conviction at court therefore police would have to release them! The suspects would all remain free for several years thereafter. The Met police would suffer greatly for not having arrested Stephen's murderers and were taunted as incompetent and institutionally racist. However, the CPS who prevented the four suspects taken to trial, took none of the blame or derision for the several year hiatus in bringing the very same suspects to trial of whom three would be found guilty. However, solely the Met Police have taken the blame for not closing the case quickly, when those at fault were the CPS. Carlton challenges Nazir to explain this anomaly! Nazir Afzal Nazir Afzal OBE was Chief Crown Prosecutor for North West England, and most recently, he was Chief Executive of the country's Police & Crime Commissioners. During a 24 year career, he has prosecuted some of the most high-profile cases in the country, including the so-called Rochdale grooming gang, and led nationally on several legal topics, notably violence against women and girls, child sexual abuse, and honor-based violence. Nazir was recently appointed to the Independent Press Standards Organisation (IPSO) and is National Adviser on Gender-Based Violence to the Welsh Government. First-hand accounts from two families who were caught up in the chaos of Kabul airport as they had to flee Afghanistan. Yusef had travelled to the UK in advance and was in quarantine when the Taliban seized control of the capital. His wife Sherbano was in shock when she saw them seize the police station and she was forced to leave their home and take their daughter Malalai and teenage nephew to the airport. Reports of crowd surges and deaths had already spread and she was terrified of what lay ahead. Nine-year-old Malalai also shares how they struggled to get through and saw Western military and Taliban forces face to face at the airport. Afzal had not believed NATO forces would really withdraw from Afghanistan and was trying to get his passport on the day the capital was seized. His family were separated in the airport grounds and he shares his desperate struggle to unite with his pregnant wife and three children in the crowds and get them onto a flight before the deadline passed. A false dawn is a promising situation which comes to nothing. This is how Aqdas Afzal describes the situation in his native Pakistan and India at the end of the Raj. “The point to remember here, Steve, is that the British were in India not to govern. They were in India to extract surplus and to maintain what they called law and order. And so the British left without giving the local people any taste or mechanism for bringing about accountability or democracy. But they did leave behind these two very, almost draconian institutions for keeping law and order. And because of these two institutions - these two state institutions that the British left behind - in the case of Pakistan, the first 25 years of Pakistan's history was complete chaos.” Aqdas talks to Steve about the chaos of partition – a humanitarian crisis. Remember, Pakistan was not only separated from India, but it was also cleaved from its own Eastern wing, now known as Bangladesh. The generation that sacrificed and struggled to gain independence was hoping for a bright future. That was the false dawn. Pakistan fell into the lap of neoliberal thinking because of the Cold War, as Aqdas explains it. When the Soviets entered Afghanistan, the military government took over in Pakistan, cozying up with the US defense establishment. Pakistani policy makers began to sound like the godparents of the neoliberal project, Thatcher and Reagan. The interview covers the destructive role of the IMF, World Bank, and WTO – what Steve refers to as the evil trinity. No matter how many of our guests talk about them, there is always more outrage to be uncovered in their manipulation of the economies of the global South. Steve and Aqdas discuss Francis Fukuyama's concept of “the end of history.” With the collapse of the USSR, liberal democracy and capitalism were expected to be the final stage of human evolution, leaving no other pathway for developing nations. Aqdas counters with the notion that history is not linear. “Russia is a country that went through shock therapy, that was undertaken by experts coming from the World Bank and the IMF. These experts are basically telling Russia how to open its economy, how to change over from socialism to a market-based economy. The same Russia today is challenging the might of capitalist countries like Britain, United States, Germany.” He calls this the beginning of history.
