In this episode of the ICHE podcast, we explore the theme of respiratory culturing appropriateness in pediatric patients, emphasizing diagnostic stewardship's role in optimizing patient outcomes. Host David Calfee, MD, MS and guest experts, Kathleen Chiotos, MD, MSCE and Edward Lyon, DO, MA, share insights on how stewardship practices can refine respiratory testing in pediatric intensive care units (PICUs), where evidence suggests these tests are often overused without clinical benefit. They examine challenges in implementing stewardship, including barriers like resistance to change and logistical issues, and discuss practical strategies to overcome these obstacles. The conversation highlights two recent studies published in ICHE: Dr. Lyon's work on repeat tracheal aspirate cultures in PICUs, which analyzes their frequency, resistance patterns, and impact on antimicrobial use, and Dr. Chiotos's study on a tracheal aspirate culture stewardship intervention, examining its effect in a tertiary PICU. Both authors discuss their studies' methodologies, findings, limitations, and implications for future practice. To wrap up, each expert offers actionable recommendations for healthcare professionals interested in improving respiratory culturing practices at their institutions, providing practical steps to enhance safety and care efficiency. Full versions of the articles discussed in this episode are available in the September issue of ICHE.

We talk about redefining what “flattering” really means, dressing without worrying about age, and getting creative with color matching.Self-expression and confidence are super important to us, and we explore how our clothing choices impact our mindset. We discuss the need for intentionality in fashion, the limits of traditional rules, and how context can complicate what's deemed appropriate. Episode 128 The NTSB report of a 2010 Cessna 310R air crash in Palo Alto, California gets mixed reviews. Todd's impressed by a sound study used to recreate the flight path, and John finds lots of detail in the examination of the wreckage, engine and prop. The positive first impression falls apart when Greg highlights missing details. The report does not answer many questions, including: Appropriateness of unusual air traffic control instructions to “take off at your own risk” Why the pilot turned left rather than right shortly after takeoff How 200 knot speed was achieved just 14 seconds after takeoff The “ridiculous” probable cause statement relies on the obvious and doesn't reflect the facts documented in the report. The Flight Safety Detectives raise questions that need to be answered to find the aviation safety lessons to be learned from this accident. They raise questions about this aircraft crash that will continued to be explored in the next episode. Last summer, a pair of murals celebrating New Mexico's landscape, heritage, and diversity appeared in Albuquerque's historic Old Town district. The large outdoor pieces, by New Mexico artists Jodie Herrera and Reyes Padilla, brought life back to a once-abandoned shopping plaza and became instant fan favorites, endlessly photographed by locals and tourists alike. Now the commission charged with protecting Albuquerque landmarks says the murals are ahistorical and must be destroyed. Business owners and the arts community are fighting to keep the murals—and a decision is approaching soon. In a city with such a rich multicultural heritage, how did it come to this? Whose interests would really be served by the murals' erasure? Must communities make a binary choice between historical preservation and creative growth? Inside historic districts, which versions of history do we choose to preserve—and who gets to make these decisions? Those are the big questions at the heart of this week's episode. You'll hear from Herrera and Padilla, but also from small business owners trying to revitalize Old Town, and from a city official charged with trying to steer sensible enforcement of the city's historic preservation ordinances. The May 11 Albuquerque Landmarks Commission hearing will take place via Zoom at 3:00 pm Mountain Time. The public may attend at this address (https://cabq.zoom.us/j/2269592859) or by calling 1-301-715-8592 and entering Meeting ID: 226 959 2859.UPDATE: In a supplemental report posted May 6, 2022 (the day this episode was released), the staff of the Landmarks Commission recommended that the commission reverse its January ruling and approve the Plaza Don Luis owners' request for a Certificate of Appropriateness for the murals and the other changes to the property. The report said, in part: “Taking into consideration the amount of murals/signage that can be found throughout Old Town HPO-5, the public support received for the murals, and the somewhat ambiguous 1998 Design Guidelines, Staff is reversing its' original position where it required the applicant to remove the murals from Plaza Don Luis.” The report is advisory only, and the Landmarks Commission ex expected to vote on the matter at its May 11 hearing (see details above). For a full transcript, photographs of the murals, and more details please go to https://www.soonishpodcast.org/506-albuquerqueNotesA special thank you to Jodie Herrera, Reyes Padilla, Jasper Riddle, Laura Houghton, Rosie Dudley, and Ellen Petry Leanse for all their help with this episode. The Soonish opening theme is by Graham Gordon Ramsay. All additional music in this episode by Lee Rosevere and Tim Beek.If you enjoy Soonish, please rate and review the show on Apple Podcasts. Hello and welcome or welcome back to another episode of The Untitled Podcast *insert applause here* This week's episode is particularly personal to me. I'll be discussing my journey with my hair—both physically and mentally—how the standards for “appropriate” ways of styling black hair push black people out of certain spaces, and the gross distinctions made between “good” and “bad” hair. This episode was made for everybody—black and non-black people, people of all genders, and even those who believe “it's just hair.” Black is beautiful, and today you'll learn why. I hope you enjoy this episode and stick around for more in the future!! Appropriateness, values, and activism in weddings. Katrina Majkut, author of the book, The Adventures and Discoveries of a Feminist Bride; What No One Tells You Before You Say ‘I Do', joins us this week. Everyone in the world is bubbled to the top, and on edge. A couple must be conscious of the location of their wedding, especially if they are inviting a diverse group of people who may be affected by the venue. For instance, Christy's brother, who is a gay man, was invited to a wedding in Jamaica where people are still openly prosecuted for being queer. He decided to not attend as it wouldn't feel safe. A wedding isn't just about your day, it's also about being a good host! Revisit the options, make a list of priorities, and figure out what are the most important factors for you and your guests. As a guest, what is the most effective way to show your beliefs? Is it protesting outright, or is it showing up and making sure you are seen? Ultimately, though. only attend if you feel safe in that space. If you choose to politicize your wedding, it doesn't have to be huge and over the top. There are ways to do it in small, meaningful ways. You can change the outdated language to be more reflective of the current times. Removing man and wife, or kiss the bride, etc will be impactful. Modernize your wedding, get rid of the outdated traditions. You can ask for charitable donations instead of a gift, or replace the wedding favor with a donation. Remember that good change will trickle down, but understand that you are not going to save the world on your wedding day. Just be conscious and deliberate with your choices that speak to your passions. These choices can be so impactful. Small ripples make big changes down the road. Some states make it harder for men to change their name, so if you are in one of those states, contact your local ACLU. Mx. is the new gender neutral new prefix.
