This Day in Legal History: Expansion of US House of RepresentativesOn August 8, 1911, President William Howard Taft signed into law a measure that permanently expanded the size of the U.S. House of Representatives from 391 to 433 members. This change followed the 1910 census, which revealed significant population growth and shifts in where Americans lived. Under the Constitution, House seats are apportioned among the states according to population, and each decade's census can lead to changes in representation. Prior to 1911, Congress often responded to new census data by simply adding seats rather than redistributing them among states. The 1911 legislation reflected both that tradition and the political realities of the time, as expanding the House allowed growing states to gain representation without forcing other states to lose seats. It also set the stage for the modern size of the House—just two years later, New Mexico and Arizona joined the Union, bringing the total to 435 members. That number has remained fixed by law since 1929, despite the nation's continued population growth. The 1911 increase carried implications beyond arithmetic: more members meant more voices, more local interests, and a larger scale for legislative negotiation. It also underscored Congress's role in adapting the machinery of government to the country's evolving demographics. In many ways, the expansion reflected Progressive Era concerns with fair representation and democratic responsiveness. While debates over House size have continued into the 21st century, the 1911 law remains a pivotal moment in the chamber's institutional development. By enlarging the House, Taft and Congress preserved proportionality between population and representation, even if only temporarily.After the 1911 increase under President Taft, the size of the House stayed at 435 members following Arizona and New Mexico's statehood in 1912. The idea at the time was that future census results would continue to trigger changes, either by adding more seats or by redistributing them among the states.But after the 1920 census, Congress ran into a political deadlock. Massive population growth in cities—and significant immigration—meant that urban states stood to gain seats while rural states would lose them. Rural lawmakers, who still held considerable power, resisted any reapportionment that would diminish their influence. For nearly a decade, Congress failed to pass a new apportionment plan, effectively ignoring the 1920 census results.To end the stalemate, Congress passed the Permanent Apportionment Act of 1929. This law capped the House at 435 seats and created an automatic formula for reapportionment after each census. Instead of adding seats to reflect population growth, the formula reassigns the fixed number of seats among states. This froze the size of the House even as the U.S. population more than tripled over the next century.Critics argue that the 1929 cap dilutes individual representation—today, each representative speaks for about 760,000 constituents on average, compared to roughly 200,000 in 1911. Supporters counter that a larger House would be unwieldy and harder to manage. The debate over whether to expand the House continues, but the 1929 law has held for nearly a hundred years, making Taft's 1911 expansion the last time the chamber permanently grew in size.A fourth federal court blocked President Donald Trump's order restricting birthright citizenship, halting its enforcement nationwide. The order, issued on Trump's first day back in office, sought to deny citizenship to children born in the U.S. unless at least one parent was a citizen or lawful permanent resident. Immigrant rights groups and 22 Democratic state attorneys general challenged the policy as a violation of the Fourteenth Amendment's Citizenship Clause, which has long been interpreted to grant citizenship to nearly everyone born on U.S. soil.U.S. District Judge Deborah Boardman in Maryland sided with the challengers, issuing the latest in a series of nationwide injunctions despite a recent Supreme Court ruling narrowing judges' power to block policies universally. That June decision left a key exception: courts could still halt policies nationwide in certified class actions. Advocates quickly filed two such cases, including the one before Boardman, who had previously ruled in February that Trump's interpretation of the Constitution was one “no court in the country has ever endorsed.”In July, Boardman signaled she would grant national relief once class status was approved, but waited for the Fourth Circuit to return the case after the administration's appeal was dismissed. Her new order covers all affected children born in the U.S., making it the first post–Supreme Court nationwide injunction issued via class action in the birthright fight. The case, Casa Inc. et al v. The Justice Department's emergency filing seeks to overturn a ruling by U.S. District Judge Maame Frimpong, who barred federal agents from stopping or detaining individuals based solely on race, ethnicity, language, or similar factors without “reasonable suspicion” of unlawful presence. Her temporary restraining order stemmed from a proposed class action brought by Latino plaintiffs—including U.S. citizens—who alleged they were wrongly targeted, detained, or roughed up during immigration raids in Los Angeles.The plaintiffs argued these tactics violated the Fourth Amendment's protections against unreasonable searches and seizures, describing indiscriminate stops by masked, armed agents. Judge Frimpong agreed, finding the operations likely unconstitutional and blocking the use of race, ethnicity, language, workplace type, or certain locations as stand-alone reasons for suspicion. The Ninth Circuit declined to lift her order earlier this month.The challenge comes amid a major escalation in Trump's immigration enforcement push, which includes aggressive deportation targets, mass raids, and even the deployment of National Guard troops and U.S. Marines in Los Angeles—a move sharply opposed by state officials. The administration contends the restrictions hinder operations in a heavily populated region central to its immigration agenda. Milbank, of course, is among the big firms that bent to Trump's strong-arm tactics, cutting a $100 million deal and dropping diversity-based hiring rather than risk becoming his next executive-order target. The New York-founded firm used the same bonus scale last summer, signaling optimism about high activity levels through the rest of the year. Milbank, known for setting the pace in Big Law compensation, is the first major corporate firm to roll out such bonuses this summer—a move that often pressures competitors to follow suit.Special bonuses are not standard annual payouts, and last year rival firms mostly waited until year's end to match Milbank's mid-year scale, adding those amounts to their regular year-end bonuses. Milbank also led the market in November 2024 with annual bonuses up to $115,000. 250 Years Ago (July 1775) Benjamin Franklin wrote to William Strahan in England: "You are a member of Parliament, and of that majority which has doomed my country to destruction. You have begun to burn our towns and murder our people. Look upon your hands! They are stained with the blood of your relations! You and I were long friends; You are now my enemy, and I am yours." The mayor of London and the Common Council petitioned King George to end military operations in the colonies. The Continental Congress requested that each colony establish minutemen units. Maj. Gen. Philip Schuyler, arriving at 10 p.m. on July 18 to take command at Lake George in the Adirondacks, found a single sentry on duty. On seeing the general, the soldier quickly tried to awaken the three other guards. Patriots in New York City raided royal stores in Turtle Bay (East 42nd Street) for supplies, which were sent to Boston and Lake Champlain. New York observed a day of fasting and devotion, as requested by the Continental Congress. 150 Years Ago (July 1875) After assaulting and robbing the elderly Levi Washburne in Carmel inside his home at 2 a.m., five "ruffians" harnessed a pair of horses to a wagon and drove to Cold Spring, where they abandoned the vehicle. Two men chasing them took the property back to Carmel. The editor of The Cold Spring Recorder noted that local law enforcement did not continue the pursuit, but that the deputy sheriff in Cold Spring and the sheriff likely knew it would be difficult to get reimbursement from the Village Board for their expenses or a reward. A horse left untied by a pineapple peddler at Moshier's fish market was found near the Pacific Hotel. The Cold Spring and Nelsonville rifle clubs held a friendly match, shooting at targets of 50, 75 and 100 yards with Hunter's Pet guns. Cold Spring won, 390-375. Frederick Osborn, 17, drowned while swimming off Mine Point. "His brother made a heroic attempt to save him, but finding that both were sinking, said goodbye and released his hold," according to The Recorder. A passenger on the Montreal Express who disembarked at Cold Spring forgot his pocketbook, with a large amount of cash. He rushed to the telegraph office. His message and the train reached Poughkeepsie at the same time, and the pocketbook was found on the seat, undisturbed. At 7 p.m. on a Tuesday, Miss Kellogg called a girl playing the violin on Main Street for change to her carriage, where they conversed in Italian. After a series of overnight burglaries and home invasions, the Village Board adopted a resolution "that the constables of the town and police officers of the village be specially requested to arrest and take into custody and detain all persons of suspicious character found roaming through the village at any unreasonable hour of the night, or found under suspicious circumstances at any time justifying their arrest and convent without delay to the police magistrate." Billy McCormick, jailed in Cold Spring for stealing a barrel of liquor, claimed he was paid $250 [about $7,300 today] for the job but would not say by whom. A tornado at West Point broke 140 gas hotel lights and moved artillery guns. After a shot was heard at 10 p.m. on a Tuesday, two Fair Street men took their guns to Sandy Landing to investigate. They found a dog belonging to Michael O'Brien, of Garden Street, had been shot dead and supposed it was causing trouble for thieves hiding their goods. Passengers aboard the Boardman and Cornwall steam yacht complained that young male swimmers waiting for the swell created by the boat would stand naked on the dock rather than jumping in the water in advance of her passing. Burglars visited B Street, where they attempted to break into the home of James Ball and took a watch from under a mattress in the house of John Butler. A child in Nelsonville was pushed into the road while he and a playmate jostled to catch a pear falling from a tree, and only skillful horse management by Mr. Mangham prevented ...

Visit your primary care provider for additional medical advice.DefinitionGestational diabetes mellitus (GDM) is a condition that occurs to previously non-diabetic pregnant women, caused by glucose intolerance at around the 24th week of gestation. PathophysiologyGDM arises due to an underlying pancreatic beta cell dysfunction in the mother which leads to a decrease in the amount of insulin produced and thus leads to higher blood sugar levels during pregnancy. The placenta of the fetus will produce hPL (human placental lactogen) to ensure a steady supply of sugars to the fetus, creating an anti-insulin effect. However, hPL readily crosses the placental barrier causing the mothers insulin requirement to increase, when the mother's pancreas cannot increase production of insulin to that level needed to counter the effect of hPL they become diabetic, and this leads to gestational diabetes. So, basically the placenta is asking for more glucose for the baby and the mother's pancreas struggles to keep the glucose level within normal limits in the body of the mother. If left untreated, high levels of glucose in the mother can cause glucotoxicity in the mother.“Glucotoxicity” refers to the toxic effect of glucose. Glucose is the main fuel for cell functions, but when it is high in the bloodstream, it causes toxicity to organs. Prevalence of GDM.The CDC reports mean prevenance of GDM is 6.9%. In U.S. mothers the prevenance increased from 6.0% in 2016 to 8.3% in 2021. Many different factors have played a role in increasing gestational diabetes in American mothers, some of those being the ongoing obesity epidemic with excess body weight being a known risk factor for insulin resistance. Another being advanced maternal age (AMA) as more American women have children later in life their body becomes less sensitive to insulin and requires a higher insulin output on top of the insulin that is required for the fetus. The “American diet” is also something that has a big effect in diabetes development. With the increase of high-carb foods that are readily available, the diet of Americans has declined and is affecting the metabolic health of mothers as they carry and deliver their children. Despite ongoing awareness of GDM, 6% to 9% of pregnant women in the United States are diagnosed with gestational diabetes, and the prevalence continues to increase worldwide. It is estimated that in 2017 18.4 million pregnancies were affected by GDM in the world, which then continued to increase to 1 in 6 births to women with GDM in 2019. It was also found that women living in low-income communities were disproportionately affected due to limited healthcare access. Additionally, women with GDM had a 1.4-fold increase in likelihood of undergoing a c-section, with 15% increase in risk of requiring blood transfusion. Screening for GDMGestational diabetes is screened between the 24th to 28th week of gestation in all women without known pregestational diabetes. In women who have high-risk for GDM the screening occurs during the first trimester, these women usually have at least one of the following: BMI > 30, prior history of GDM, known impaired glucose metabolism, and/or a strong family history of diabetes. The screening during the first trimester is to detect “pregestational diabetes” because we have to keep a good glycemic control to improve outcomes of pregnancy. So, if it's positive, you start treatment immediately. If these women are found to have a normal glucose, they repeat the testing again as done normally, at 24-28 weeks of gestation. How do we screen?The screening itself consists of two types of approaches. The two-step approach includes a 50-gram oral glucose tolerance test (OGTT), where blood glucose is measured in an hour and if it is below 140 they are considered to not have GDM, however if the reading is greater than 140 they must then do a 3-hour, 100g oral glucose tolerance test. The 3-hour OGTT includes measuring the blood sugars at Fasting which should be less than 95, at 1 hour at less than 180, at 2 hours at less than 155, and at 3 hours at less than 140. If 2 or more of these values exceed the threshold the patient is diagnosed with gestational diabetes mellitus. The one-step approach includes 75g after an overnight fast. Blood glucose is measured while fasting which should be less than 92, at 1 hour less than 180 and at 2 hours less than 153. If any one of these values is exceeded, the patient is diagnosed with GDM.If the mother is found to be GDM positive during pregnancy she will also need continued screening post-partum to monitor for any development of overt diabetes. The testing is usually 75g 2-hour OGTT at 6-12 weeks postpartum. If this testing is normal, then they are tested using HbA1c every 3 years. If the post-partum testing shows pre-diabetes, annual testing is recommended using HbA1c measurements. Maternal complications Women with GDM are at an increased risk for future cardiovascular disease, T2DM, and chronic kidney disease. GDM is also associated with increased likelihood of developing pre-eclampsia following delivery. Pre-eclampsia is a complication seen in pregnancy characterized by high blood pressure, proteinuria, vision changes, and liver involvement (high LFTs). Pre-eclampsia can then progress to eclampsia or HELLP syndrome, both of which can include end organ damage. Additionally, she can develop polyhydramnios which leads to overstretching of the uterus and can induce pre-term labor, placental abruption, and or uterine atony, all of which additionally put the mother at increased risk for c-section. All of these maternal complications that stem from GDM lead to complications and extended hospitalization. Child's complications Although there is an increased set of risks for the mother, the neonate can also develop a variety of risks due to the increased glucose while in utero. While the fetus is growing, the placenta is the source of nutrition for the fetus. As the levels of glucose in the mother increase so does the amount of glucose filtered through the placenta and into the fetal circulation. Over time the glucose leads to oxidative stress and inflammation with activation of TGF-b which leads to fibroblast activation and fibrosis of the placenta. This fibrosis decreases the nutrient and oxygen exchange for the fetus. As the fetus attempts to grow in this restrictive environment its development is affected. The fetus can develop IUGR (intrauterine growth restriction) leading to a small for gestation age newborn which can then lead to another set of complications. The low oxygen environment can lead to increased EPO production and polycythemia at birth which can then lead to increased clotting that can travel to the newborn brain. Newborns can also be born with fetal acidosis due to the anerobic metabolism and lactic acid buildup in fetal tissues which can cause fetal encephalopathy leading to cerebral palsy and developmental delay. And the most severe of newborn complications to gestational diabetes can lead to fetal demise. Furthermore, the increase of glucose can also lead to macrosomia in the infant which can often lead to a traumatic delivery and delivery complications such as shoulder dystocia and brachial plexus injury. Brachial plexus injury sometimes resolves without sequela, but other times can lead to permanent weakness or paralysis of the affected arm. The baby can be born too small or too big.Additionally, once the fetus is born the cutting of the umbilical cord leads to a rapid deceleration in blood glucose in the fetal circulation and hypoglycemic episodes can occur, that often lead to NICU admission. The insulin that is created by the fetus in utero to accommodate the large quantities of glucose is known to affect lung maturation as well. The insulin produced inhibits surfactant production in the fetus. Upon birth some of the newborns also have to be placed on PEEP for ventilation and some children require treatment with surfactant to prevent alveolar collapse and/or progression to NRDS created by the low surfactant levels. Additionally, neonates who are macrosomic, which is usually seen in GDM mothers, are larger and stronger and when put on PEEP to help increase ventilation the newborn's stronger respiratory effort can lead to higher pulmonary pressures and barotrauma such as neonatal pneumothorax.Long term complications to the child of a mother with GDM also occur. As the child grows, they are also at an increased risk for developing early onset obesity because of the increased adipose storage triggered by the increase in insulin in response to the high glucose in utero. This then can lead to a higher chance of developing type 2 diabetes mellitus in the child. With diabetes, also comes an increase in cardiovascular risk as the child ages and becomes an adult. The effects of GDM go beyond the fetal life but continue through adulthood.What can be done?Gestational Diabetes Mellitus has many severe and lifelong consequences for both the mother and the child and prevention of GDM would help enhance the quality of life of both. Many of the ways to prevent GDM complications include patient education and dietary modifications with a diet rich in whole grains, fruits, vegetables and lean proteins. Benefits of some vegetables in the Mexican cuisine that may be beneficial: Nopales, Chayote, and Jicama. Those are good alternatives for highly processed carbs.Mothers are usually offered nutritional counseling to help them develop a tailored eating plan. This and 30 minutes of moderate exercise daily is recommended to increase insulin sensitivity and lower the post-prandial glucose levels. If within 2 weeks of implementing lifestyle changes alone the glucose measurements remain high, then medications like insulin can be put onboard to manage the GDM. If they require insulin, I think it is time to refer to a higher level of care, if available, high risk OB clinic.Conclusion: Now we conclude episode number ###, “[TITLE].” [summary here]. _____________________References:Eades CE, Burrows KA, Andreeva R, Stansfield DR, Evans JM. Today, I am joined by Mark Huberman, President of the National Health Association and Editor of Health Science magazine.  He  has adhered to a whole food, plant-based vegan diet since birth in 1951 and credits his excellent health to the pioneering vision of his parents, Max and Ruth Huberman, who had the foresight to recognize the profound benefits of this lifestyle long before most others.  His parents operated a legendary health food store in Boardman, Ohio for more than 33 years where they influenced countless individuals to change their diet and lifestyle and regain and preserve their health. For nearly all of Mark's adult life, his greatest passion has been supporting the work of the National Health Association, the oldest organization in the Country advocating the benefits of a whole-food, plant-based diet and lifestyle, something it has been championing since its founding in 1948. After serving his community as an attorney for more than 30 years, Mark retired and moved his focus fully to the NHA. Cardiovascular disease is the leading cause of death in women and this risk increases after the menopause, and a woman's risk of heart attack is around five times higher after the menopause than before*. Joining Louise on this week's podcast is Dr Jeremy London, a board-certified cardiothoracic surgeon based in the US, to discuss heart health, hormones and menopause. They discuss the role of oestrogen in reducing inflammation in the body, why women typically present with different heart attack symptoms compared to men – and the signs to look out for – and the crucial role of nutrition and exercise in maintaining good heart health. Finally, Dr London shares his top three tips on what women (and men!) can do to help their future cardiac health: Don't smoke: it is the single worst thing you can do for your health in general. From a cardiac standpoint, from a blood vessel standpoint, for the risk of lung cancer. Nutrition: avoid processed foods and eat real, whole foods. Drew will share his top three carp fishing tips, including the legendary drag-and-drop technique you don't want to miss. Plus, he'll dive into the must-have gear for the season and give us the scoop on spotting carp in those tricky river flats. And if that's not enough, get ready to be hooked by tales of fish camps and exotic fishing trips to places like Belize! Episode Chapters with Drew Evans on the Carp Clave 3:11 - Drew describes the Carp Clave event as possibly the first of its kind, aiming to educate and introduce more people to carp fishing, which traditional trout fishermen often overlook. It was held in Boardman, Oregon, a location chosen for its abundant flats and carp. 7:18 - Drew explains that the Carp Clave highlights the parallels between flats fishing, typically a saltwater technique, and carp fishing in shallow waters across the United States and even globally. 8:55 - He walks us through the event. He mentions Dick Sagara and a few more of the huge supporters of the event and the vendors and brands that were there. 18:20 - Drew explains that House of Fly is where he works. They offer a comprehensive range of fly fishing gear, from saltwater equipment to small creek necessities, carrying well-known brands like Echo, Sage, and Winston. 20:51 - We talk about his home river. For steelhead, he goes to Clearwater, Idaho. He also targets salmon and smallmouth bass. 25:01 - Drew explains that spring brings challenges like weather fronts and strong winds, which can affect water clarity essential for flat-style fishing. Despite potential visibility issues, carp can still be targeted due to their bright appearance, especially against sandy bottoms. He emphasizes the importance of identifying the fish's orientation to place the fly correctly. 28:04 - We dig into the gear for fishing in the Boardman area of the Columbia River for successful fishing during April. He emphasizes the importance of having a rod with a good backbone, suggesting an eight-weight rod as ideal. This rod size is versatile for different types of fish, including redfish, bonefish, permit, pike, smallmouth bass, and carp. 29:06 - He highlights the importance of selecting lines that can handle wind and varying temperatures. He recommends saltwater tapers, especially the Rio Outbound short for cold conditions and the SA Grand Slam or redfish taper for warmer conditions. 31:18 - Drew typically uses a long leader, around 12 to 13 feet, particularly when dealing with spooky fish. To counterbalance the long leader and ensure the fly sinks faster, he uses a heavier fly and often opts for a fluorocarbon tippet, ranging from 12 to 16 pounds. He mentions specific products like the redfish leaders from SA or Rio, which he finds particularly effective. 32:15 - He notes that anything from sizes 8 to 4 can be effective for the flies. He mentions the importance of having a variety of flies with different weights, from light bead chain eyes to medium dumbbell eyes, to adapt to different water depths. He highlights a particular fly, the hybrid carp fly created by John Bartlett, which has proven highly effective in carp fishing. 37:28 - Drew gives us three reasons to try carp fishing. He points out the accessibility of it, as it doesn't require long drives like trout fishing. He also mentions the practice it provides for saltwater fishing and the conservation benefits, as carp can be fished in hot weather when other fish are less active. It also offers a thrilling, challenging experience. 41:16 - He talks about the strategies for finding and not spooking carp while fishing. He emphasizes the importance of locating shallow water areas that can accommodate multiple fish. Drew also points out the value of patience, mentioning that if carp have been seen in a particular flat before, they are likely to return. 47:09 - Drew gives some strategies for catching carp, particularly for those who may not have had much success. He advises anglers to lengthen their leader and practice the "drag and drop" technique to avoid spooking the fish.
