What do you do when the most evidence-based intervention you have is also the one your patient is "not allowed" to receive? Constraint-induced movement therapy has decades of research behind it, yet its exclusion criteria — especially around cognitive impairment — rule out a huge share of the stroke survivors clinicians actually see. In this episode, hosts Erin Gallardo, PT, DPT, NCS, and Claire McLean, PT, DPT, NCS, talk with neuro OT Briana Elson, MS, OTR/L, BCPR, CBIS, about her decision to try modified CIMT with an 86-year-old stroke patient who had significant cognitive impairment and underlying dementia — a textbook exclusion — and why his profile, a left-handed man whose affected side was his dominant hand, made forced use feel natural and salient rather than confusing. Briana walks through how she built the protocol around safety and supervision in inpatient rehab: getting a physician order for the constraint mitt, educating and enlisting his wife, keeping a consistent daily schedule, breaking the three-to-six hours into non-consecutive blocks, and removing the mitt before late-day sundowning. She shares the outcomes that surprised even her skeptics — meeting the minimal detectable change on the Box and Block Test and a standardized motor scale within roughly thirteen days, plus real gains in grooming and feeding independence — alongside the honest caveats, including that his cognition never improved and that real-world delivery never looks as clean as the published article. It's a practical, encouraging conversation about reading the research, respecting its limits, and giving more patients a shot at an intervention they're usually told they can't have.

Tried to introduce evidence about suspects who practiced those rituals, were connected to the victim, and whose interviews were lost or destroyed. Excluded. The defense even tried to present evidence about the quality of the investigation itself. Excluded. What the jury did hear: a State phone expert who Googled whether water damage could mimic headphones during the trial and was allowed to testify about what he found on anonymous forums — over the defense's hearsay objection. According to the defense's appellate filings, every meaningful avenue for challenging the prosecution's narrative was closed by the trial court. Allen was convicted in November 2024 and sentenced to 130 years. The State's position on every exclusion is the same: harmless error. Evidence about alternative suspects connected to pagan rituals, the victim, and the crime scene symbolism — excluded. Evidence about the investigation's failures over five and a half years — excluded. The State countered phone data breaking its timeline with a witness who Googled the answer during trial. The defense objected to the hearsay. Overruled. The jury convicted on November 11, 2024, after hearing what the defense calls a fundamentally one-sided presentation. Allen was sentenced to 130 years. Every exclusion, every blocked expert, every piece of evidence the jury never saw — the State calls it all harmless error. The defense argues it was constitutional error that crippled Allen's ability to defend himself. This is the final chapter in a five-part examination of how Abby and Libby's case went from a complex crime scene to a conviction built, in my opinion, on a foundation the full truth would not support. The Colombian village of Briceño might, at first glimpse, look like many communities in the rural Global South. Many of the people living there rely on small-scale farming, even as a newly constructed hydroelectric dam threatens traditional livelihoods. Yet after decades where Briceño suffered from a bloody conflict, the village has more recently become central to the nation's hopes for peace. In Governing the Excluded, sociologist Alex Diamond shares a closer look at Briceño and offers unique insight not only into the contemporary Colombian state but to how people across the Global South are struggling to maintain rural livelihoods amid economic dispossession.Governing the Excluded describes a landmark peace process between the Colombian government and the radical FARC guerrillas from the perspective of Colombian farmers, drawing links between economic transformation, drug economies, and armed conflict. Exclusion from global markets for traditional crops like coffee first pushed farmers to grow coca, the raw material for cocaine. This ushered in an era of violent conflict for control of the illicit economy, while farmers continued to be priced out of legal markets. In exchange for peace and state protection, farmers ultimately agreed to sacrifice profitable coca. But with its disappearance, they now find themselves dependent on the state: for machinery to maintain the roads they need to get legal harvests to market, municipal jobs that are the only decent work available, and for public resources to subsidize food crops with razor-thin profit margins. Ongoing economic struggles in the legal sector make the state's newfound authority tenuous, as some villagers replant coca, abandon the village for uncertain urban futures, or join a rearmed guerrilla group.Informed by deep ethnographic research and firsthand stories from Briceño residents, Governing the Excluded shows that when it comes to the forces driving dispossession—be they international corporate megaprojects, global food prices, or national initiatives to replace coca cultivation—state authority goes only so far as its ability to sustain local livelihoods. Rabbi Shimon derives from the words "et hamincha" that many additional menachot require hagasha (bringing the offering to the altar). However, he uses three other specific exclusions from the verse to exempt certain offerings: The two loaves and showbread: Excluded from the word "m'eleh" (from these), as these are not burned on the altar. The mincha of libations: Excluded from "v'hikriva" (and he shall bring it), as these offerings accompany animal sacrifices. Voluntary offerings of kohanim and the kohen gadol's griddle cakes: Excluded from "v'higisha" (and he shall bring it near), as these are burned entirely and no portion is given to the priests. The Mishna continues by categorizing sacrifices: those that require waving (tenufa) but not hagasha, those that require both, and those that require neither. The Gemara brings the biblical sources for the various sacrifices that require waving. Notably, waving can be performed even on the east side of the Temple courtyard. The Gemara questions why this differs from hagasha, which must be performed specifically at the southwest corner of the altar. Rabbi Eliezer ben Yaakov and Rabbi Yehuda each utilize a different verse to derive the source for the waving of the bikkurim. Why was only Rabbi Eliezer ben Yaakov mentioned in the Mishna? Two answers are suggested. Waving is a collaborative act performed by both the kohen and the owner of the sacrifice. The Gemara identifies the source for this joint action. However, an exception exists: if the owner is a woman, waving is still required, but the woman herself does not perform the act. The Gemara derives the source for this specific exemption. Converts are freed Caananite slaves are obligated in waving, as is derived from "hamakriv," the one who offers the sacrifice.

USD-denominated foreign exchange reserves include US Treasury securities, US mortgage-backed securities (MBS), US agency securities, US corporate bonds, and other USD-denominated assets held by central banks other than the Fed. Excluded are any central bank's assets denominated in its own currency, such as the Fed's Treasury securities or the ECB's euro-denominated securities. It's not that foreign central banks dumped US-dollar-denominated assets, such as Treasury securities. They did not. They added a little to their holdings. But they added more assets denominated in other currencies, particularly a gaggle of smaller currencies whose combined share has surged, while central banks' holdings of USD-denominated assets haven't changed much for a decade, and so the percentage share of those USD assets continued to decline.
