Today, we're speaking to Dr Victoria Tzortziou Brown, a GP and Reader in Primary Healthcare and Health Policy at Queen Mary University of London, and Vice Chair for External Affairs at the Royal College of General Practitioners.Title of paper: Language of primary medical qualification and differential MRCGP exam attainment: an observational studyAvailable at: https://doi.org/10.3399/BJGP.2024.0296To the authors' knowledge, this is the first study on the association between the language of the primary medical qualification and attainment in the Membership of the Royal College of General Practitioners (MRCGP) examination. It shows that undertaking undergraduate clinical training in a country where the native language is not English can statistically significantly and negatively affect examination performance in MRCGP exams. The study also shows statistically significant positive correlations between Multi- Specialty Recruitment Assessment, International English Language Testing System, and Professional and Linguistic Assessments Board scores and the MRCGP exam scores; this suggests that past performance in these assessments can help with the identification of those international medical graduate registrars who may find tailored support beneficial.

It advocates for open access and support for those exploring low-carb diets as a therapeutic intervention, signaling a much-needed shift in mindset. Throughout the episode, Beth and Matt stress how this Position Paper will empower both patients and clinicians, offering a solid foundation for discussing low-carb options within healthcare teams and integrating evidence-backed guidelines into mainstream T1D care. They encourage families to share the Position Paper with their doctors, fostering more comprehensive and supportive care. The couple also reflects on their involvement in the January 2024 LowCarbUSA Boca Raton Symposium for Metabolic Health, which featured a dedicated Type 1 Day. As requested by GP Trainees, this week's episode is on tips to pass the Simulated Consultation Assessment exam; the final exam for GP trainees in the UK. We spoke to Dr Anne Hawkridge who was a GP trainer in Bolton for over 20 years and has been a MRCGP examiner since 2007 when the CSA (Clinical Skills Assessment) started, the predecessor to the pandemic RCA and now the SCA. She is also the co-founder of the successful exam support programme here in the North West; the SOX programme (Support on Exams) and co-author of the North West England Consultation Toolkit which is published in full on the MRCGP SCA preparation website. Questions we had were; what is the exam, what does it look like, how much does it cost, when to consider sitting in, how to approach revising for it and what pitfalls to avoid. The top tips fit into 4 themes; GP consulting skills, ensuring knowledge, good exam technique and timing. Muslims believe the Qur'an was authored by Allah, who keeps an original copy in heaven. The Qur'an is apparently perfect in every respect, unchanged and unchangeable, miraculous, superior in its Arabic language and eloquence. Finally, they claim the Qur'an is Allah's last message to humanity. In this talk, all these claims and more are scrutinised using logical, historical, geographical, arithmetic, and manuscript evidence. Emil Shehadeh was born in Israel and educated mainly in the UK, achieving BSc Hons (Medical Biology), MSc (Biochemistry), MB ChB (Medicine), and a Diploma in Dermatology and MRCGP. Growing up in Israel, he accompanied his parents all over Galilee, spreading the Gospel amongst all communities including Muslims. He also worked with Child Evangelism Fellowship and OM. In the early 1990s, he co-founded an Arabic Church in Cardiff, S Wales. He practiced as a GP (Family Physician and a trainer) until 2016. Upon retirement, he dedicated his time to writing and speaking on the subject of Islam and evangelising Muslims. He supports evangelistic work amongst Muslims in North Africa and Nigeria. Emil has a PhD in Islamic Studies from Logos University and is engaged in another piece of research at LST, Northwood, on the Causes and Consequences of Muslim Conversions to Christianity. In this episode, we talk to Professor Niro Siriwardena, Professor of Primary and Pre-hospital Health care at the School of Health and Social Care at the University of Lincoln. Title of paper: Academic performance of ethnic minority versus White doctors in the MRCGP assessment 2016-2021: cross sectional studyAvailable at: https://doi.org/10.3399/BJGP.2022.0474Differential attainment is widely found in undergraduate and postgraduate medical examinations. It has been suggested that subjective bias due to racial in discrimination clinical skills assessments may be a cause of examination failure for UK trained ethnic minority candidates and international medical graduates. No previous study has examined differential attainment in all components of GP licensing assessments including workplace-based assessment, taking into account scores at selection in GP specialty training. Ethnicity did not reduce the chance of passing GP licensing tests once Sex, place of primary medical qualification, declared disability and selection (multispecialty recruitment assessment) scores were taken into account. Doctors admitted to GP speciality training who are in the lowest MSRA score bands may need additional support during training to maximise their chances of achieving licensing regardless of their ethnicity or other demographic characteristics.

Lois Fergusson (Dr Lois Morgan, MB BChir, MRCGP, DRCOG) qualified as a GP in 2013 having completed GP VTS in east London. From December 2013, she has worked for Medair, a Christian faith based international NGO carrying out humanitarian relief and recovery activities. This has taken her to South Sudan, Switzerland and Jordan and seen her do jobs from health manager to being in the global advisory team, covering at various times programming in Lebanon, Jordan, Syria, Yemen, Iraq and Sudan as well as, at times, holding staff health and training responsibilities. In our very first episode, I am joined by Dr Hashim, a newly qualified GP who has recently achieved her MRCGP through the Pennine GP training scheme in the North West of England.  This is a really interesting chat with a trainee who initially failed the AKT by a single mark and unlike many, she waited twelve months before retaking the exam! To then pass with flying colours. Dr Hashim is also an international medical graduate, and she shares her experiences on how the AKT poses its very own challenges for IMGs.

We have repeatedly called for his release on urgent medical grounds. Since then, medical experts who examined Julian Assange testified in court to the seriousness of his medical condition. They explained that he would not survive oppressive prison conditions, and his extradition to the United States was denied on those grounds. The High Court subsequently stood by the medical findings, and ruled that the medical evidence could not be challenged. Meanwhile, having won his case, Julian Assange remains in the very conditions that caused and perpetuated his precarious state of health in the first place. With appeals set to drag on for years, unless Julian Assange is released from prison, there is every reason to expect his condition to deteriorate, potentially dramatically so. Given the medical evidence that is openly on the table now, Doctors for Assange warns that Julian Assange may not survive the appeal process." I had the pleasure of speaking with Dr. Ann Donnelly MB MRCGP DRCOG DFP DYT Dippallmed LFhom. During our interview, she explained what all the letters after her name meant, and, among other things, spoke about life activation. She has worked in all aspects of medicine, including hospice and palliative care. She is an international teacher with the Modern Mystery School. After entering the dating world, she wrote "Once Upon a Tinder", which I will be ordering from Amazon shortly. Dr. Ann's biography is below. Dr. Ann, 'the love doctor', is a Family Doctor with over 25 years experience working in the NHS. She currently works in Palliative and Holistic Medicine. She is often consulted as a medical expert in the media, including Yahoo, Cosmopolitan and Glamour. Furthermore she is an International teacher in the Modern Mystery School of Metaphysics. Her passion for helping people understand their full potential shines through, in this, her first book. I previously believed that HRT (Hormonal Replacement Therapy) was off the table for me - don't ask me why. I guess I always thought it was for women that were really struggling with menopause symptoms and that being a qualified fitness educator, I should be able to battle on through with the knowledge that I had. SO glad I spoke with Dr Rebecca, if only to be able to make more informed decisions about how I intended to embrace longevity and stay feeling SEX-AY! Dr Rebecca Lewis (MBBS FRCA DRCOG MRCGP) qualified from Guy's Medical School in London in 1991 and initially pursued a career in Anaesthetics, obtaining FRCA in 1997. She then changed speciality and became a GP gaining MRCGP in 2000. She continues to practise as a GP and has developed an interest in the menopause, giving lectures and writing articles on the subject. She is determined to improve the understanding of the menopause in current and future generations through education and access to accurate evidence based medical information. She is passionate about making the menopause a much more positive experience and enabling women to get back to feeling themselves again. She has two young children and enjoys running and keeping fit. Andrew Papanikitas Primary Care Health Sciences and John Spicer Health Education England give a talk on their Knowledge Exchange research project on teaching ethics to medical students. Andrew Papanikitas, Primary Care Health Sciences Dr Andrew Papanikitas qualified as a general practitioner (MRCGP) in 2008. His PhD in medical education was awarded in June 2014 and is entitled, "From the classroom to the clinic: ethics education and general practice." The PhD is one element of a broad interest in professional ethics education as applied in medicine (and more specifically primary care) and in the notion that education represents the active translation of ideas between the academy and practice. Dr Papanikitas is part of an informal network of academics, educators, and clinicians with an interest in the study of ethics in, of, and for primary healthcare. He welcomes conversations on this topic, especially via the 'Primary Care Ethics' LinkedIn Group which is now approaching 300 members from the UK and internationally. John Spicer, Health Education England Dr John Spicer has been a GP in South London for 35 years, and is a leader of postgraduate primary care education for Health Education England. He has taught Clinical Law and Ethics at St George’s University of London for ten years, and contributed to the literature in this arena via various books and articles. He is engaged more generally in the medical humanities and is a Trustee director of the London Arts and Health Forum.

NASGP member Eva Kalmus describes how she became co-chair of the new GeriGP group of the British Geriatrics Society (BGS), and why being a portfolio GP has never been boring. When I did my GP training, about half of the GP trainees undertook a self-made rotation of approved jobs, followed by a year in general practice to qualify. Some studied for MRCGP towards the end, and many transferred from other medical specialities. If I had stayed in hospital practice it would have been geriatrics for me. I was a geriatric SHO for an extra nine months with some wonderful, knowledgeable geriatricians and later held a clinical assistantship on long-stay wards, the NHS’s equivalent to today’s nursing homes. During my first FY1 job I had been inspired by a compassionate and wise consultant geriatrician, appreciated and admired by patients and staff alike. Years later, as a salaried GP, my practice manager forwarded me an advertisement seeking medical cover for community hospital beds; he said that the practice was not interested, but what about me? I continued with some GP sessions but had a wonderful 10 years as part of a multi-disciplinary team looking after mostly older people and learnt so much about rehabilitation, pragmatic discharge planning and cross-organisational working. I also worked in virtual and community wards, providing enhanced care in their own homes to patients at risk of admission to hospital and even dipped my toe in commissioning waters - discovering that I definitely got more satisfaction from patients than meetings! We can spend more time, and focus on these complex patients as well as developing services offering more proactive and holistic care, than is possible in traditional general practice. The community hospital moved onto the acute hospital site for a few months’ rebuilding work and I rediscovered my curiosity about the best that specialist medicine could offer to older people. The clinical director offered me an unfilled post for a community geriatrician. So my job title of Interface Medicine GP was invented, and I have had a rollercoaster four years managing moderate and severe frailty around an acute setting. These patients de-compensate significantly in response to a minor stressor and so are admitted and treated aggressively, but then take a long time to recover - if they do at all - and are often confused, de-conditioned and institutionalised by their prolonged hospital stay. For some, a team - of which I was part - could prevent the admission, but much of the work is smoothing the path to a rapid and effective discharge when hospital no longer offers a net benefit. It was from these experiences that I felt the need for mutual professional support. Some geriatricians expressed unease that a “mere GP” can undertake some of their role successfully, albeit differently. And some GPs are not aware of the complex issues involved for people with greater degrees of frailty who particularly need continuity of care, advanced care planning and the necessity of good communication with them, their carers and other professionals coming into contact with them. Over time I’ve probably learnt more than the average GP about the geriatric giants summed up recently as the 5Ms—mind, medication, mobility, multi-complexity and matters most. My knowledge of acute medicine has been updated, but I am not keeping entirely up to date with the latest guidance on contraception, child health etc because I do not think there is enough space in my brain or time to cover it all thoroughly. Dr Susan Oldfield hails originally from England and completed her medical training in the UK at UCL medical school, obtaining MBBS. After this she went on to complete a Diploma in Family Planning as well as a Diploma in Obstetrics and Gynaecology, whilst completing her GP training and gaining the qualification MRCGP in the UK. Her family and her decided to move to NZ in 2004. She then obtained GP qualifications in NZ, FRCGP. Since then Susan has been working as a GP in a busy urban practice in New Plymouth. She decided to pursue her interest in Integrative Medicine as she feels that combining a conventional approach with a more holistic approach provides clients with the best of both worlds. Because GP consultations are so time-limited, it is difficult to investigate problems in depth. With more time and an integrative approach, Susan is able to add much more to a consultation.   In this interview we discuss why the conventional medicine model wasn't working for her anymore, what an "Integrated Practitioner" means and what she sees coming in the future in the world of medicine, health and wellbeing.
