  Now that we are entering the last transition phase of COVID-19 in the United States where we are moving from an epidemic to endemic for the SARS-CoV-2 coronavirus, we need to focus on how safest to get there. Clearly, the introduction of a highly effective vaccine is abbreviating the time it would normally take to get our population immunity to this novel virus. What would have taken 2-5 years with multiple outbreaks throughout the country will now be less than 18 months before we get to a point where we no longer have massive spikes of cases, hospitalizations, and deaths from COVID-19. Who is Dr. Noorchashm? My guest is public health expert, Dr. Hooman Noorchashm of Philadelphia, PA. He is a retired cardiothoracic surgeon and PhD immunologist who gained prominence in the public eye over the battle he and his wife had with the FDA. Their story is both inspiring and sobering as his wife Amy, a critical care trained anesthesiologist, fell to cancer in 2017. She underwent a routine laparoscopic surgery for a stage I (least dangerous) uterine cancer. At the time, a morselator was used to grind up the uterus in the abdomen and avoid the need for a larger incision. Sadly, the morselization process itself would create a metastatic spread of the primarily benign cancer. Fortunately, the odds of this outcome were very low (~1/350-400 surgeries). However, when you look at how many of these procedures were done for this condition it amounted to hundreds of women each year who were significantly harmed. This led Dr. Noorchashm and his wife to advocate for all the women in the future who might be affected in the same way she was. Their argument was that there were other ways of accomplishing a hysterectomy that didn't involve the risk of spreading the cancer, however small, that could be used. Bringing risk to a small minority of people was simply unacceptable and violated one of the core tenets of medical ethics - First Do No Harm. Who Should Get the Vaccine? The most important question we have to ask ourselves is who should get the vaccine? Initially, the question was who had the highest priority to get the vaccine which was in a limited supply. Now, however, there is far more mRNA vaccine available than demand for the vaccination. So now the question comes down to who should get the vaccine, or rather, who shouldn't get vaccinated? This is where Dr. Noorchashm argues that anyone who has had COVID-19 and cleared the disease should not get vaccinated. There are no other cases where we vaccinate people against a disease after they have successfully fought it off (flu doesn't count since there are many different types of flu that are different). In fact, most recently, when the chicken pox vaccine was introduced, one of the first questions asked was whether the child had already had chicken pox. A near immunological certainty is that once you have cleared a viral infection, you have immunity that is at least if not more robust than immunity you can acquire from a vaccine. Vaccines clearly create immunity, but it is obviously not the only way to acquire immunity. People who have been infected with SARS-CoV-2  also have immunity and will receive no benefit from a vaccination. So if you receive no benefit from a medical intervention and can only be harmed (however unlikely it may be) then it is unethical to do that intervention. The FDA and CDC Guidance is Not Only Wrong, But Unethical. Strangely, the FDA and CDC have issued guidance that all people eligible for vaccination receive them whether they have had COVID-19 or not. But this guidance is flawed and only puts people at risk who will gain no benefit from the vaccine. This is bad science, a lack of understanding of basic immunology, and not looking at current studies which show definitively that people who have been infected get antibodies. By definition, if you are doing something that only provides risk, no matter how small, is unethical and wrong. Dr. Noorchashm states clearly that the American people can handle complicated instructions and don't need to be put at risk by our health experts unnecessarily. What Should We Do About Vaccinations Then? Instructions for vaccinations should be fairly straightforward. If you have had COVID-19 and cleared the infection do not get vaccinated. If you are unsure whether or not you had COVID-19, get an antibody titer drawn to see if you have immunity. If not, get vaccinated. If you do, don't get vaccinated. Get vaccinated if you haven't had COVID and don't suspect it or have no reason to suspect an asymptomatic infection. Dr. Hooman Noorchashm Physician-scientist Dr. Hooman Noorchashm is an advocate for patient safety, ethics and women’s health. He specializes in cardiothoracic surgery and has taught and practiced medicine for nearly two decades. When his wife, also a doctor, Dr. Amy Reed, tragically (and needlessly) passed away from complications of an FDA approved gynecologist procedure that is still in essence performed today, Dr. Noorchashm began his new incarnation of patient advocacy of do no harm. Dr. Noorchashm now brings this perspective to why some people may have adverse effects from the vaccine. And how this might translate into who should or should not get the vaccine. As well as how we might test this ourselves. Food shaming puts undue stress on parents, especially as kids go back to school.  Our guest, Amy Reed, is a pediatric dietitian who is teaching parents how to advocate for the needs of their kids.  Whether your child is labeled as a “picky eater” or has significant challenges receiving nutrition, parents who aren’t confident in what they feed their kids are susceptible to food shaming and bullying. As a pediatric dietitian, Amy evaluates how well a child is growing and how their nutritional status is helping or hurting their growth.  She works with kids with a wide range of eating issues and reminds us that eating is a learned skill.  Each child develops the skill at their own pace. Amy encourages parents to feed their kids with confidence, they know their child run best. Key Points: All foods have their place. Stay away from fads that eliminate food groups. One thing is constant: fruits and veggies are important. Parents of children with feeding issues often blame themselves and feel blamed by others. Many parents feel guilty because they aren’t sure what to feed their kids.  Lack of confidence make them vulnerable to misinformation. Parents should rely on good quality information rather than fads or marketing, especially during the pandemic. Family events can be very stressful for parent dealing with feeding issues. Don’t buy into “in my day kids ate what was put in front of them” guilt. Bring something along just in case. If you are hosting, ask about what the kids can eat. Have a go-to response, “I appreciate your concern.  We are working with professionals and have a plan.” Remember it is only one meal.  If all the kid can eat is the cake, it doesn’t make you a bad parent. In my 5thpodcast, on Feb. 8, 2019, I introduced you to Amy Reed, Dietitian, MS, RDN, CSP, LD. Amy is a Pediatric Dietitian with over 18 years of experience at Cincinnati Children’s Hospital who is now in private practice as well as working part time at Northeast Cincinnati Pediatric Associates. In podcast 5 she discusses her passion for children’s health and the role we, as parents, have in setting the right foundation for their lifelong journey with nutrition and fitness. What we feed them, what we make available to them, how we perceive food, nutrition, appearance, fitness, etc, all leave a lasting impression.Amy reached out to me to discuss a hot nutrition topic. We start with some basic facts about children and nutrition.· Just because a child looks underweight or overweight doesn’t mean they are.· Each child has a different genetic makeup and will follow their own growth curve.· You should offer healthy food to all your kids, regardless of their size, and teach them how to make healthy choices.· Pointing out your child as having a weight issue can leave them with negative feelings about not pleasing their parents.· Size stigma is a real issue for children. It means judging them on their size alone before getting to know them.· Children are learning that calories are bad. They need to be taught that growing bodies need calories, calories are energy.· Parents on extreme diets teach children that they will need to worry about their weight as adults.· The earlier teens start to diet, the more likely they are to be in a weight cycle. They look at diet as a temporary thing. They lose some, gain more back then repeat that cycle.· When you see a drastic weight loss, in a school age child, you need to find out how they got there. If you applaud the weight loss, you are cheering for the outcome without knowing if the weight was lost in a healthy manner. You could be reinforcing an unhealthy method such as starvation to lose weight.· It is natural and okay for children to gain weight during puberty. Girls gain abdominal fat as their body prepares for eventual childbearing. Boys tend to gain more muscle mass. If you restrict calories at this time you could be hindering their growth or even their fertility.· Sports specialization is happening at young ages. It can be a double-edged sword. It keeps select, competitive, athletes actively involved in practicing their sport. On the other hand, the children who enjoy playing but are not good enough to get on the select team can feel defeated and quit the sport entirely. The activity and enjoyment they got from sports is often replaced by sedentary pursuits.· We need to encourage public access to recreational activities to reinforce the idea that activity is fun for everyone.Today’s hot topic;Kurbo, a new app launched by WW (formerly Weight Watchers)Kurbo is an app targeted at children ages 8-17 to track their food intake and set goals for weight loss.We all want our kids to eat healthy meals that fulfill the requirements for their growing bodies. At the same time, we want to teach them healthy habits about nutrition and exercise that will follow them into adulthood. On the surface, Kurbo may seem like a good idea and Weight Watchers, as a company, has helped many adults reach and maintain their desired weight. However, this app, designed for children as young as 8, has many aspects that could cause lifelong issues with weight and self-image. The program is based on a stoplight diet that was originally created in the late 90’s by Stanford University. It places food into three categories: Red for foods you should stop eating, Yellow for foods you should eat in moderation and Green for foods you can eat as much as you want.· Children are encouraged to set goals which can be helpful. While having more confidence or more energy are positive goals, they can also choose “make my parents happy” or “look better in my clothes”. These types of goals are not healthy and can lead to eating disorders· This app is based on weight loss not nutritious eating. Unless you teach your children nutritious eating habits, they will continually be on the weight loss cycle.· Many foods that are healthy and give you energy are placed in the red category.· Growing children need calcium but according to this diet plan, calcium rich foods are in the red category. Your kids would need to get all their calcium from things like kale or spinach and we all know how that would work.· The app can be punitive in that if you eat too many foods in the yellow category, they become the red category.· The app labels foods as good or bad. Children see things in black and white and will struggle their whole life if they feel like calories are their enemy and not the fuel they need for energy.· The “experts” who are coaching your children are not doctors, dietitians or people in the medical field. They are experts in communication, tourism management, political economy and business. They have no medical experience and do not have access to medical records to determine what may or may not be healthy for your child.Instead of using the Kurbo app:· Don’t make everything about food or weight.· Save negative self-talk for private conversations away from kids.· Rather than restricting unhealthy foods, make healthy options more available.· Model healthy behavior.· Do not use food as a reward. Amy Reed is an Asheville, NC based writer. She got sober in Oakland, CA in 2008 in the rooms of Alcoholics Anonymous and dabbled a little in Buddhism via Kevin Griffin and Spirit Rock. When she moved to Asheville with her family in 2014, she found herself drifting away from 12-steps and struggled to find a recovery community in her new town. Then she saw a flyer for Refuge Recovery in a café, went to her first meeting in the summer of 2016, and found exactly what she was looking for. She soon started a dedicated meditation practice, became active in her local and regional Refuge community, and was elected Asheville’s inter-sangha treasurer. In 2018, she was invited to join the Refuge Recovery Literature Committee and became part of the team that co-wrote what would eventually become The Dharma of Recovery, the first literature offering of the Recovery Dharma program. In February 2019, she joined the Refuge Recovery Board of Directors. Being healthy, happy and fit is a goal that most of us would agree is desirable. The amount of effort we put towards reaching that goal for ourselves is entirely up to us. But what about our children. By what we feed them, what we make available to them, how we perceive food, nutrition, appearance, fitness, etc. we are instilling in them the basis for their lifelong journey with nutrition and health. How scary is that? Are we screwing up our kids by underfeeding, overfeeding, not the right foods, not enough balance, picky eaters, etc?Today I will interview Amy Reed, Dietitian, MS, RDN, CSP, LD. Amy is a Pediatric Dietitian who had 19 years of experience at Cincinnati Children’s Hospital before starting a private practice. She will walk us through all sorts of hard questions that we might be afraid to ask or not know who we should be asking (pediatrician, dietitian, therapist)?1. Picky eaters2. Overweight/underweight3. Family eating – making it fun4. Is there enough nutrition balance within the home?5. Could we be accidently attributing to future eating disorders?Determining if your child is healthy involves many factors of which weight is only one piece of the puzzle. You must also consider: Do they eat a variety of food that is good for them? Are they active? Do they drink plenty of water Are they happy? Do they have low screen time? Do you have family meals? It is important to follow up those questions with ones about yourself. Are you portraying a healthy body image. Are you using positive words about food choices and exercise. Children are not born with an instruction book about how to properly feed them. Parents do not need to feel guilty about or apologize their children’s weight. Guilt makes it hard to be successful in your efforts. Professional help is available, so do not hesitate to seek it out.Homework:1. Have at least one family meal this week. Make sure it is about togetherness and not what food is or is not eaten. Serve it family style where each person serves themselves. 2. Do a family activity such as taking a walk or bike ride if weather permits or have an indoor dance session or game of twister. Get moving as a family. 3. Assess your cupboards and fridge. Make sure fruit, veggies and other healthy choices are available. Pre-set snack plates for after school snacks. 4. If you have been considering consulting a professional, purity it on your to do list. My theory is always “Knowledge is Power” and today is about opening our minds to a professional’s opinion What you do with that information is up to you.Goal setting will always be part of your homework. Once a week write down your focus or goals for the week. Every Friday reassess them, tweak them or change them to fit your needs. It doesn't matter what night you tune in to Division III basketball, something will surprise you. There are teams leading conferences that weren't expected to be in the conversation, there are programs who continue their record-setting starts to the season, and others who seem to be defying expectations - well, at least those outside of the locker room. On Thursday's edition of Hoopsville, Dave chats with a few coaches who have programs currently excelling - even if they have hit some bumps in the road recently. One has his program off to the best start in a storied program's history, another is leading a conference that has it's traditional leaders off the pace a bit, a third who was never thought would be on top, and a fourth looking to try and change national minds. This week's WBCA Center Court guest is Stephanie Stevens of Gallaudet University, the national deaf and hard of hearing University, in our nation's capital. She will discuss what it is like to coach using a different language along with her involvement in the WBCA's Diversity and Inclusion committee. Stevens was a recent honoree of the WBCA Thirty Under 30. Hoopsville is presented by D3hoops.com and airs from the WBCA/NABC Studio. The season has started fast and for some teams they still haven't lost. Not unexpected for some, maybe completely unexpected for others. And there was a big battle between #1 and #2 in Division III men's basketball on tap! On Thursday's edition of Hoopsville, Dave was back in studio and talked to some of the teams who find themselves without a loss, but probably still with plenty of questions that remain unanswered. Many of the teams are nationally ranked, but one of the teams finds itself undefeated and receiving narry a point in any of the Division III polls. Also on Thursday's edition, Dave will talked LIVE with both Amherst and Babson head coaches after not only a rate No. 1 vs. No. 2 regular season battle, but another thrilling double-overtime game between the two. Both coaches took the time to chat after the game.
