How does the human brain develop an addiction? The extremely high prevalence rates for disorders such as alcohol use disorder or drug use disorder (1 in 3 AUD lifetime prevalence, 1 in 10 DUD lifetime prevalence) suggests that addiction is not simply a disease but is instead a natural brain process that each and every one of us is susceptible to. The fascinating thing that is unraveling in the research is that the brain processes that lead to addiction are not drug or alcohol specific but instead are involved in every form of addiction (gambling, sex, shopping, technology... etc).
What happens in the brain when we experience addiction? Do some people just have more of an addictive personality than others? We talk with Bret S. Stetka, MD, Medscape's Neurology and Psychiatry editorial director, about different chemical messengers, neurotransmitter networks, our brains' complex reward systems, and the role anticipation plays in addiction.
In this episode we talk to Dr Marc Lewis, a neuroscientist and former professor of developmental psychology. We explore what addiction is from a neuroscience perspective, including why Marc does not consider it a disease, despite changes to the brain. INSIGHTS In the rush to institute COVID lockdowns, Marc Lewis says “the need for social connection was overlooked for an almost obsessive attunement to the disease.” We tried to replace physical proximity to loved ones with social media and Zoom, but it didn't work. “The actual presence of other people—proximal, real flesh and blood people that you can feel and touch and hug and smell— is really important,” says Lewis. [02:21] Lewis' research indicates that it's not who you are, but what happens to you that determines how likely you are to become addicted to something. “There is no addiction gene cluster. It's how well you fit with your surround,” says Lewis. In his experience as a psychotherapist, Lewis estimates that more than 90 per cent of his patients with addiction problems have histories of trauma. [14:53] Addiction wasn't considered a problem until the 20th century, when addicts were labeled as “sinners,” “weak” and “self-indulgent.” The 1980s and 90s introduced the disease model of addiction, which removed the blame from the addict but still connected them inherently to their “disease.” “It's a way of thinking that makes the problem intrinsic to the person,” says Lewis. [19:09] In contrast, Lewis uses the Internal Family Systems (IFS) model of therapy when he treats addiction patients. IFS identifies the part of the addict that wants to get high as one component of a greater whole. “There's a particular part of me that when I get upset enough, scared enough, depressed enough, this part knows how to fix that with drugs, booze, or whatever else,” says Lewis. [21:35] When people become addicted, it is often because they are trying to cope with problems or stress in their lives. According to Lewis, we have become too preoccupied with treating our issues as disorders or diseases. “We don't all need to be running to the drugstore to deal with things that humans have suffered with forever,” says Lewis. Treating, and warding off, addictive behaviours is complex, but one thing that helps is developing a network of loving friends and family, with which one maintains frequent, physical contact. [27:35]

Because that’s the bottleneck, the linchpin, where human affairs are cast and crystallized. That’s where the brain shapes our lives and our lives shape the brain” Marc D. Lewis, Ph.D. I had the honor and pleasure of interviewing Marc. D. Lewis on TSC Talks to discuss his life and work with the focus of the conversation on how he developed his understanding of addiction, and how he debunked the more traditional theory that addiction is indeed a disease. Marc is a neuroscientist, recently retired full professor of developmental psychology, at the University of Toronto from 1989 to 2010, and at Radboud University in the Netherlands from 2010 to 2016. He is the author or co-author of over 50 journal publications in psychology, cognitive science, and neuroscience, editor of an academic book on developmental psychology, and co-author of a book for parents. More recently he has written two books on the science and experience of addiction. I heard Marc on a podcast several years ago after reading his blog, https://memoirsofanaddictedbrain.com. I was a few years into recovery from substance abuse addiction and struggling with many of the traditional modes of treatment. As a parent of children with some extra challenges case managing their TSC related care post-divorce, I was buoyed when I heard Marc discussing his 2nd book, The Biology of Desire, Why Addiction is Not a Disease (2015)which claims “the scientific facts don’t support the disease model of addiction. Rather, addiction, like romantic love and other emotionally loaded habits, develops through deep learning and limited alternatives. Combining scientific views with intimate biographies of addicts who recovered, the book also shows how addiction can be overcome, through self-directed change in one’s goals and perspectives.” Having felt more validated in my own personal experience knowing that there were multiple factors personally at play that led to my own demise, I went and started reading more of his blog and research. Viewing myself as victim of a disease while at the same time having to maintain a somewhat subservient stance to wrangle the services necessary to help our family recover from years of ongoing traumas, was almost too much to bear. I simply could not accept the idea that I had a disease and as I chose to navigate away from that model of understanding, including delving into cannabinoid medicine, I was finally able to intimately integrate past present and future in a way that I could truly climb out of many self-limiting beliefs reinforced by this notion. Clinging to the disease model of addiction was limiting and I have to say, Marc’s work has made a tangible impact on my own and so many others who have had less than typical experiences for which the confines of traditional recovery methods led to deeper despair and frustration. Beginning during his undergraduate years in Berkeley, California, Marc experimented with a large variety of drugs, eventually becoming addicted to opiates. After quitting drugs at age 30, he continued his graduate education in developmental psychology. Addiction has been recognized as a disease since 1877, and it has been recognized as a disease by the American Medical Association since 1955. Addiction is not a learned behavior, or simply a way of medicating unpleasant feelings. A person suffering from addiction will experience repeated, serious negative consequences as a result of using chemicals, but will continue to use chemicals regardless. In this Part 2 of 2 episode we welcome Nicolette back who shares her brave story about her terrifying slide into devastating drug addiction and her brave battle into sobriety with a devastating relapse. She is now a survivor and we are so proud of her as she just completed her full 1 year of sobriety.In this episode she tells the details of how she went from being a functioning addict to a non functioning one and how her grades started slipping and she started to party more and more.She talks about the time when her drug use escalated and she was playing Russian roulette with mixing all different drugs and alcohol and eventually snorting cocaine etc. She got severely depressed and as her drug use got worse so did her depression.She got put on the antidepressant Lexapro that made her feel worse than ever. She then spiraled further and further out of control and started cutting. Nicolette shares the story when she was rushed to the ER and put on a 5150 psychiatric hold and was placed on suicide watch. From there things really started spiraling out of control and she ended up Overdosing while injecting drugs.Thank God she survived and she then made a promise to change her life and becomes clean and sober for good.March 15th, 2018 marks her 1 year sobriety and we are all celebrating with her and we know she has a bright future ahead.She tells us that she is now studying Neuroscience at the University of Texas and she is President of her Sober House.Nicolette would like to thank http://www.enterhealth.com where she successfully completed her treatment. Judson Brewer MD PhD is the Director of Research at the Center for Mindfulness and associate professor in medicine and psychiatry at UMass Medical School. He also is adjunct faculty at Yale University, and a research affiliate at MIT. A psychiatrist and internationally known expert in mindfulness training for addictions, Brewer has developed and tested novel mindfulness programs for addictions, including both in-person and app-based treatments. He has also studied the underlying neural mechanisms of mindfulness using standard and real-time fMRI, and source-estimated EEG, and is currently translating these findings into clinical use. He has published numerous peer-reviewed articles and book chapters, presented to the US President’s Office of National Drug Control Policy, trained US Olympic coaches, been featured on 60 minutes, at TED, TEDMED, TEDx, in Time magazine (top 100 new health discoveries of 2013), Forbes, Businessweek, NPR and the BBC among others.
