The three stages of periop are preoperative, operative and postoperative (PACU). Our guest this week is here to prove that you can indeed do nursing school with young children.  Sariah is currently finishing her last two semesters of her BSN program and get this folks, she just had TWINS in December of 2020! These beauties bring her up to 4 children  ages 7 and under. And she is not only surviving her program, but thriving as well! How does she do it? Take a listen to this weeks podcast to find out!Show Notes:Sariah has known from a young age that she wanted to be a nurse. She started out getting her CNA when she was 16 to get experience in the medical field.She has had 6+ years working in a drug rehab facility with her CNA. She feels this work experience has been very beneficial and recommends becoming a CNA to others looking to get into a nursing program.Starting off on her journey she began working through a local university to complete her pre-requisite courses but soon felt that with her young daughter the timing was not right.Never loosing sight of her goal to become a nurse she continued working as a CNA until a friend talked her into checking out the nursing program through Provo College.In this college she found a program that would allow her the flexibility she needed so she could still be there for her very young family.She is able to do much of her course work online (especially since the pandemic started in 2020). And the program goes straight through to a BSN in 3 years.Sariah admits that self motivation for this program is key to success as there is not as much in person class time. There is a lot of reading, zoom instruction, and self learning. But, as a perk for her, she is able to be home with her kids more. COVID-19 caused her cohort to miss an entire semester of clinicals. To replace this time they had to clinical hours in the school labs.Clinicals are open now and she does 2-3 clinical shifts a month. This allows her to work out schedules with her husband so he can be home with their children. No babysitter required! This has given Sariah peace of mind. She encourages others looking into programs to check out the smaller colleges and other accredited programs, not just the universities as you may find they fit your needs better. A key thing she has gained from this experience? CONFIDENCE  in her ability to learn the subject material largely on her own. How does she thrive in such a challenging situation? Don't procrastinate! I wasn't sure if I would be able to finish my nurse's assistant training this summer because of Covid. The last part of the course was going to be 40 hours of work in a care center for the elderly in the little town of Cashmere. "How am I going to finish my course?" was really on my mind. A few weeks ago, however, I received an email from our teacher telling us that there was no more Covid in the center, and that we could go there to do our clinicals. We were assigned in pairs to work with one particular patient, to help in any and every way needed.  The first morning, when I arrived at 5:55am, I was nervous and excited. I am not from the medical field, so this was all going to be new for me. Our teacher showed us around, gave us as much advice as we needed, I think, and let us get to work. My partner and I were assigned to a lady I will call 'J'. She had several chronic illnesses and required total care. She could feed herself, but other than that, the rest was up to us. Our duties included: getting her up, toileted, washed, dressed, lifted into her wheelchair, fed, and back to bed. We worked on a tight schedule(1). Another thing that we had to do was reposition her in bed every two hours. This is essential for patients who cannot move themselves because if they don't change position, their circulation will get cut off at certain points of their bodies, which could lead to infection and death. Another thing that we had to think about all of the time was infection control, keeping germs away from our patients and ourselves.  So, needless to say(2), we were busy all day. It was exhausting but really rewarding because J was quite a character. She understood that we were there for her, and appreciated our help, but she was also very upfront, and quick to be sarcastic. I loved that! It's hard to sum up all of the learning experiences that you get in a clinical like ours. We had such close contact and communication with not only our patient, but also with many of the others who were in the longterm care ward. I actually feel privileged to have been there with these wonderful, fragile people. They each had so many stories to tell, but sadly most of them could either no longer speak or remember. So what now? I finished the course, but I still have to take the state exams in order to be registered as a nurse's assistant. Then, who knows? I definitely would like to work for a while in this capacity(3). It could very well lead to nursing, but as yet I'm not sure. Even if it doesn't, it was one of the most valuable courses I have taken, and has opened up a whole new area of human experience to me. 1. 'On a tight schedule' means that you have a lot to do in a given, limited amount of time. a. We need to feed all of the patients between 12 and 1pm; we're on a tight schedule. b. The builders of the cabin are on a tight schedule because it will snow soon. 2. 'Needless to say' is like saying 'this extra comment is obvious because of the context I have already given.' a. The dog got out, and needless to say, it chased the neighbor's cat. b. He was the most punctual and hardworking worker, needless to say, he was awarded 'employee of the month.' 3. 'Capacity' can mean one of three things: the potential for storage, a position/job, or an ability. a. The cinema was filled to capacity. b. He has the capacity to be a great doctor. c. She volunteered in her capacity as an interpreter, and really helped the project.

Interventional radiology involves doing procedures or minimally invasive surgery under the guidance of machinery like ultrasounds, MRIs, and CT scans. This specialization is suitable for students who are interested in tech, enjoy interacting with students, and are good at working with their hands. Although interventional radiology is present in clinical settings, it is also applicable in academia. Medical students should take the initiative to immerse themselves in different fields rather than relying on luck to give them direction. They should take the time to think about whether or not they are interested in patient contact, research, differential diagnoses, etc. as they choose their medical school rotations, electives, or specialties. Dr. Elsie Koh believes that the biggest struggle that medical students can face is themselves. Students need to take advantage of online resources and books to cultivate leadership, professionalism, and self-improvement in medicine. She shares an anecdote about Jim Carrey, who would park himself in bookstores to read self-improvement books. As a starving actor, Carrey even wrote himself a check for 10 million dollars and had a vision and belief that his first gig would be for 10 million dollars. Sure enough, three years later, he landed his first movie “Dumb and Dumber” for 10 million dollars. Although medical students do not need to write checks for 10 million dollars, Dr. Koh emphasizes the importance of having a vision, rather than just focusing on day-to-day work. Write your vision down and repeat it. Do not be afraid to speak out and take initiative to achieve it. Each person should have three types of goals: Immediately achievable goals, such as a daily to-do list. Goals achievable with small amounts of additional effort, like upgrading your Jeep model. Goals that are large, and difficult to achieve. Each day, we should try to accomplish three tasks related to these goals. She is also a proponent of finding strong mentors and peers. To be a great leader, you have to be a great follower. Think about someone whose life and career you admire, and emulate them, even if they are not directly mentoring you. As for peers, filter out your friend group to avoid naysayers. Surround yourself with people who attract you with their positive attitudes. The five people with whom you spend the most time will influence you the most. To be a great preceptor or physician leader, you need to be approachable and to listen to your students. Let your students come back to you with feedback, and even personal criticism — this is the best path to success and effectiveness. Be there for your students, and be attuned to what they need. When listener Caven wrote in asking why CCOM graduates don't include hardly any specialists and why they all seemed to be going into primary care, Dave was puzzled. While it's true that a state school like ours, serving a rural part of the country, emphasizes primary care, he knew that not 'everyone' goes into primary care. On further questioning, it turns out Caven's info came from the Medical School Application Requirements (MSAR) tool on the AAMC website! What was going on? Dave sought help from his friends in Admissions, and it turns out that MSAR doesn't tell the whole story...and aspiring med students have to dig deeper. Also, Dave asks his co-hosts Matt Wilson and Tony Mai, both rising M4s, to give their advice for those starting clinical rotations. And they help Aline Sandouk and LJ Agostinelli answer some of Yahoo! Answers most probing health questions. This Week in Medical News, there's good news in med school diversity--the number of students underrepresented in medicine is on the rise. A paper in Nature Microbiology says the authors have found an easy and economic way to convert A and B red blood cells to type O cells, the universal donor type. And a study in JAMA notes that patients of surgeons who behave unprofessionally around their colleagues have more complications. Plus, cell phone horns are probably not a thing.
