We've talked a lot before about integrating psychiatry into palliative care (see here and here for two examples). Still, we haven't talked about integrating palliative care into psychiatry or in the care of those with severe mental illness. On this week's podcast, we talk with two experts about palliative psychiatry. We invited Dani Chammas, a palliative care physician and psychiatrist at UCSF (and a frequent guest to the GeriPal podcast), as well as Brent Kious, a psychiatrist at the Huntsman Mental Health Institute, focusing on the management of severe persistent mental illnesses. We discuss the following: What is Palliative Psychiatry (and how is it different from Palliative Care Psychiatry)? What does it look like to take a palliative approach to severe mental illness? Is "terminal" mental illness a thing? Is hospice appropriate for people with serious mental illness (and does hospice have the skills to meet their needs?) Controversy over Medical Aid in Dying for primary psychiatric illness (and for those with serious medical illness who have a comorbid psychiatric illness) The level of provider moral distress that can be created in a system not designed to meet the needs of specific populations... and when we are asked to meet a need we don't feel equipped to meet. That's what mental health is where you're able to engage in your life in a way that represents who you are and what you want to do. The transformative vision of the Huntsman Mental Health Institute. John Huntsman's legacy is council. The nine children looked around and they've had mental illness in the family and substance use disorder in their family, and they wanted to make this their legacy. To actually change mental health on a national level across everything, including, uh, looking at policy reform on a federal level, payment reform, access to care, uh, best practice, specialty, personalized care, all those kinds of things. a big part of that, so what they're funding right now is this Stop Stigma campaign, which is a national campaign that's intended to go out the next 10 years. What contributes to the mental health crisis?[21:18] Mental health is the number 1 disability in the world now. And so part of what contributes to all that, as you know, is the whole social environment, particularly kids. How do you interact with your peers? How many people get bullied, isolated from the rest of the pack, don't feel like they belong early on and how that carries through. So, creating an inclusive environment where people just get accepted for who they are, and they can be a part of the overall social contract without having to be the popular kids or whatever, you can be whoever you are and fit into that is a critical piece. And of course, we all know that there are these different benchmarks that really require a lot of going from elementary to junior high, junior high to high school, high school to college. It's a critical thing. She has her own practice, Bloom Therapy and Life Coaching and has been on staff at Huntsman Mental Health Institute for over 12 years. She began her working career in Architectural Design and continues to step into this arena periodically. She is the mother of 3 talented children, has a passion for connecting with people and has had the opportunity to have traveled over 65 countries, just recently having returned from trekking Patagonia. She enjoys skiing Utah snow, is an avid hiker and is currently learning to sail and kite board. This is Derek Miller Speaking on Business. May is Mental Health Awareness Month and a time for all of us to prioritize our mental health. Joining me today is Dr. Mark Rapaport, CEO of Huntsman Mental Health Institute to discuss how the newly dedicated institute will tackle complex mental health challenges facing Utahns. MARK RAPAPORT, M.D.. At the Huntsman Mental Health Institute, we are working to identify new ways to address suicide, trauma, substance use, and provide care to the underserved and rural communities of Utah. We are currently working across the community to develop a strategic plan for the institute. This plan will create a road map to address mental health stigmas, suicide, and the primary prevention of and the development of interventions for mental disorders in young people. We are also working on the implementation of new models for rural mental health care as well as the long-standing problem of workforce development. On May 26th we will break ground on a Mental Health Crisis Center on our new Campus of Hope in Salt Lake County. This unique partnership with the state, county and many others will be a vital community resource and a model for future mental health crisis care.
