Post-inflammatory hyperpigmentation (PIH), when the skin becomes discolored or darkened, is a common consequence for people of color receiving laser treatments. It's a major challenge for Dermatologists and one with no clear solutions. Dr Vincent Richer of Pacific Derm in Vancouver, joins Kirk to discuss the article he co-authored with Dr. Ian Wong in the Jan/Feb 2021 issue of the Journal of Cutaneous Medicine and Surgery, "Prophylaxis of Post-Inflammatory Hyperpigmentation From Energy-Based Device Treatments: A Review." As Dr Richer explains to Kirk, the article is an attempt to "look into the science behind what supports all the 'witchcraft and wizardry' that we do either before or after our treatments, to try and prevent (PIH) from coming."Dr. Vincent Richer is a medical and cosmetic Dermatologist and holds positions as Clinical Instructor and Director of Continuing Medical Education at the Department of Dermatology and Skin Science at the University of British Columbia. He is motivated by getting optimal outcomes for his medical and cosmetic patients through customized care. The article focuses on how East Asians exposed to the urate-lowering drug allopurinol have a predilection for severe cutaneous drug reactions such as drug-induced hypersensitivity syndrome or drug reaction with eosinophilia and systemic symptoms and Stevens-Johnson syndrome/toxic epidermal necrolysis. Dr Barber and Dr Dutz discuss why screening of patients of East Asian descent for the presence of HLA-B*58:01 prior to allopurinol initiation has been a Canadian missed opportunity. Dr. Jan Dutz speaks with JCMS Editor-in-chief Dr Kirk Barber about the article Dutz co-authored in the Nov/Dec 2019 edition of the Journal of Cutaneous Medicine and Surgery. The article focuses on how East Asians exposed to the urate-lowering drug allopurinol have a predilection for severe cutaneous drug reactions such as drug-induced hypersensitivity syndrome or drug reaction with eosinophilia and systemic symptoms and Stevens-Johnson syndrome/toxic epidermal necrolysis. Dr Barber and Dr Dutz discuss why screening of patients of East Asian descent for the presence of HLA-B*58:01 prior to allopurinol initiation has been a Canadian missed opportunity. And what more can be done to encourage wider use of this inexpensive screening test. Dr. Jan Dutz is Professor and Head of the Department of Dermatology and Skin Science at UBC. Amanda Gorecki is an advanced registered nurse practitioner and founder of Healing Waters and Pure MD® Skin Science. She has a BSN from Seattle Pacific University and an MSN from the University of Alabama at Birmingham. She trained at Harborview Medical Center and Duke University where she worked as an emergency room nurse and nurse practitioner for the Department of Neurosurgery before venturing into private business. Gorecki was named Enterprising Woman of the Year in 2014 by Enterprising Women Magazine; she is an Atlanta member of the WPO (Women’s Presidents Organization) Chapter 1, received the Champions of Respect Award from theYWCA Women’s Shelter, has been nominated for the Ernst & Young Entrepreneur of the Year, and was named one of the 40 Under 40 by the Wichita Business Journal. Gorecki founded Healing Waters in 2002, twice named the Best Medical Spa in the Country, and recipient of numerous local awards for Best Spa, Best Massage, and more. Healing Waters currently has locations in Wichita, Kansas; Durham, North Carolina; and Columbia, South Carolina. In 2007, Gorecki pursued one of her greatest passions by developing the proprietary skin care line, Pure MD® Skin Science. All Healing Waters spas offer customized Pure MD® facials and retail products. Gorecki is regarded as an expert on skin care ingredients and has lectured nationally on cosmeceuticals at various women’s conferences nationwide.
