As we learn more and more about the brain, researchers are developing new neuroscientific methods that can help diagnose patients with traumatic brain injury. For example, some of these methods might even be able to tell us that patients who otherwise appear unresponsive are actually still "alive inside". That's an amazing idea, but the story doesn't stop there. As such technology develops, it raises a number of ethical questions about how it works and how to use. In this paper, Andrew and his coauthors investigate the benefits, harms, and costs of using neuroimaging to detect human consciousness. Links and Resources * Andrew Peterson (https://philosophy.gmu.edu/people/apeter31) * The paper (https://onlinelibrary.wiley.com/doi/full/10.1111/bioe.12678) * Experiences of family of individuals in a locked in, minimally conscious state, or vegetative state with the health care system (https://bmcmedethics.biomedcentral.com/articles/10.1186/s12910-021-00674-8) * Ethical issues in neuroimaging after serious brain injury with Charles Weijer (https://www.charlesweijer.com/neuroimaging-project) * Practice guideline update recommendations summary: Disorders of consciousness (https://n.neurology.org/content/91/10/450) * Jason Karlawish (https://www.med.upenn.edu/apps/faculty/index.php/g275/p7909) * Adrian Owen (https://owenlab.uwo.ca/) Paper Quotes The practice guideline update is a milestone in the history of neurology. Recommendations to use investigational neuroimaging methods are but one aspect of the guideline, and there is a need for further normative analysis of its rich content. We encourage continued debate on these issues. Bringing clarity to the underlying ethics of caring for brain‐injured patients can assist clinicians and health care institutions as they incorporate the guideline in clinical practice. We think that investigational neuroimaging could facilitate access to opportunity for DoC patients. As the guideline highlights, investigational neuroimaging could function as a gatekeeper for continued rehabilitation, and it might also be used as a neural prosthetic, based on future technical improvements. Neuroimaging assessment could also inform clinical decisions that best reflect a patient's values, even if pursuing those values are inconsistent with standard notions of quality of life. Opportunity‐based frameworks for healthcare justice still require conceptual refinement, and further work needs to be done to thoroughly apply such a framework to the DoC context. However, we believe that this is a promising avenue of future research to explicate the justice claims that DoC patients (or other disabled populations) have to investigational neuroimaging and other novel therapies. Special Guest: Andrew Peterson.

Cognition – specifically, cognitive decline – are at the heart of why The Snooze Button began in the first place. And it's front and center as we officially kick off Season Two of the show, with not only a continuation, but an evolution as well. Our Guests We're excited to welcome Dr. Adrian Owen, OBE back to the show – he was our first guest ever, and we talk not only about his massive study of sleep and cognition that led to conclusions that even the researchers weren't expecting, but get his “signoff” on our approach to the research into cognitive function and sleep that will be the cornerstone of Season Two. If you'd like to revisit the details of the study where Dr. Owen's team found “the sweet spot” for sleep for most people – and what happens to your cognitive function if you get too little or too much sleep compared to that sweet spot, you can find it here (don't worry, it'll open in a new tab). In addition, our friend Dr. Seema Khosla returns for a look at what COVID has done to our sleep, what it's done to sleep research, and what sleep does to COVID, too. Continuation and Evolution Season Two marks a slight shift in not only direction, but also presentation of our show. First, the direction of the show is shifting slightly toward research, in addition to continuing the learning. Season Two continues the conversations with sleep researchers and scientists from around the world, but now also includes Neil's research into his own sleep issues. We'll be talking regularly with celebrities, high achievers and newsmakers who have sleep challenges baked into their day-to-day experience. Each of them will offer Neil a sleep tip which he'll try out for a couple of weeks, testing his cognitive performance and sleep data along the way, and bringing you into the loop so you can see which things worked, and which things didn't. Second, there's the presentation of the show – we're now on YouTube! The YouTube version of the show is the exact same content as the full-length podcast, just with a video element added. We hope you like it. That, too, will evolve as we get better at playing with all the fun video tools. Linkapalooza https://www.cambridgebrainsciences.com/science/tasks (Cambridge Brain Sciences) is the website where you'll find the tests of cognition that Neil and Dr. Owen discussed. There's no charge to take the tests – try them for yourself, and let us know what you find out! On this episode of Going Underground, we firstly speak to Prof. Adrian Owen, world-renowned neuroscientist and CEO of Cambridge Brain Sciences. He discusses the findings of studies on recovered coronavirus patients which show the development of neurological problems such as worsened concentration ability and damage to senses such as smell. We ask how this could affect employment and the global economy, how medical emergencies such as cardiac arrests caused by Covid-19 could lead to the development of neurological problems, how similar viruses in the past such as SARS-2 have caused the same issues, whether the neurological complications are long-term or short-term, and more! Finally, we speak to former Hong Kong legislator and founder of Demosisto, Nathan Law, about the new Hong Kong National Security Law, implemented by Beijing. He discusses why he must conduct the interview with Afshin Rattansi from an undisclosed location, why he believes the national security law destroys Hong Kong’s freedom and autonomy, allegations of organizations in the Hong Kong protest movement being tied to Western intelligence agencies such as the CIA, the role of the US-backed National Endowment for Democracy in the Hong Kong protests, Beijing using the narrative of foreign interference to fool its own citizens, how the law arguably curbs freedom of speech and assembly, China’s policies in Xinjiang regarding Uyghur Muslims, and more!

Neuroscientist Adrian Owen has spent much of his career exploring what he calls ‘the grey zone’, a realm of consciousness inhabited by people with severe brain injuries, who are aware yet unable to respond to those around them. It's this inability to respond which has led doctors to conclude that they are unaware. In the late 1990's, Adrian started to question the assumption that they lacked awareness and a chance discovery set him on a novel path of enquiry - could some of these patients be conscious or aware even though they don’t appear to be? His research has revealed that some are, and he’s pioneered techniques to help them to communicate with the outside world. This emerging field of science has implications, not only for patients but, for philosophy and the law. A British scientist, Adrian now runs a research programme at the Brain and Mind Institute at Western University in Canada, dedicated to reaching people in this ‘grey zone’. Between life and death, the gray zone, we've all heard the stories, near death experiences, comatose patients come back to life. On this episode of The Halli Casser-Jayne Show we explore that mysterious place between life and death with two people who are experts on the subject, world renowned neuroscientist Dr. Adrian Owen the author of a riveting new book INTO THE GRAY ZONE: A NEUROSCIENTIST EXPLORES THE BORDER BETWEEN LIFE AND DEATH and Dr. Eban Alexander renowned academic neurosurgeon and author of PROOF OF HEAVEN, A NEUROSURGEON'S JOURNEY INTO THE AFTERLIFE.In one of the most profound books of the year thus far, INTO THE GRAY ZONE, Dr. Adrian Owen takes his readers on a vivid, emotional, and thought-provoking journey in which he explores the little understood boundary between life and death. No cold scientific treatise this. INTO THE GRAY ZONE is a dazzling glimpse at a new frontier in our scientific understanding of the brain and the newly-discovered zones in the twilight region between full consciousness and brain death. His true-life characters and their tales of conscious or barely-conscious lives living on the border of life and death – and the heartbroken families having to make decisions about their care is at once fascinating, titillating and provocative. When Dr. Owen pushes forward the boundaries of science, he shockingly discovers that a sizeable number of patients previously thought to be vegetative or non-responsive – often victims of traumatic brain injuries, strokes, or degenerative diseases like Alzheimer's and Parkinson's – are actually vibrantly alive, trapped in the gray zone.Dr. Eban Alexander is a renowned academic neurosurgeon who has spent 54 years honing his scientific worldview. Having practiced fifteen years at the Brigham & Women's and the Children's Hospitals and Harvard Medical School in Boston, he thought he knew how the brain and mind worked. And then the doctor became the patient when Dr. Alexander's own brain was attacked by a rare illness. The part of the brain that controls thought and emotion—and in essence makes us human—shut down completely. For seven days he lay in a coma. And just as his doctors considered stopping treatment, Dr. Alexander's eyes popped open. He had come back to life, but only after he had experienced a transcendental Near-Death Experience … an NDE. He told his story in the controversial New York Times bestseller PROOF OF HEAVEN, A NEUROSURGEON'S JOURNEY INTO THE AFTERLIFE, which remained a bestseller for over sixty seven weeks, Alexander asserting his belief that there is an afterlife, and that consciousness is independent of the brain.Between life and death, consciousness, life after death, medicine, the brain, ethics, religion, philosophy, science -- tune in to The Halli Casser-Jayne Show, the podcast posted at Halli Casser-Jayne dot com.
