A week ago I noticed one of those beautiful red toadstools in our garden – the classic red fungus with white dots all over the skin. Amanita muscaria or Fly agaric – there are a few different sub-species with different colourations (orange-red to yellow, and various colours of the “dots”). This is a Mycorrhizal fungus that is associated with a few common host trees: Birch, beech and pine trees. It's not very edible – in fact, it's better not to muck around with. Some young children have ended up being poisoned and some rather risky adults (trying to go on a Hallucinogenic journey) ended up in similar troubles. But they look great, and this was the first time I saw this species in our front garden, which surprised me. Of course, I never saw the 7-meter tall Betula which really need pruning away from electricity wires… Many species are doing a great job in recycling dead materials, fallen leaves, and dead branches, and also dead trunks in all shapes and sizes. These are some examples of fungi doing the recycling job in forests – small and large and colourful. Ear Fungus is often found on dead trunks of trees. This is a weird looking, feeling, and tasting mushroom that can hardly be misidentified: It looks like a human ear, it feels like an ear, and it even tastes like an ear! This edible fungus was the very first export article that was sent from New Zealand to China in the eighteen hundreds. The Chew Chong brothers in Taranaki were the first people to send container loads of these fungi by ship. Gardeners will encounter fungi that cause all sorts of problems in fruit (fruit rots), in roots (Phytophthora), and in stems and on leaves. Often preventative gardening will reduce the problems developing. Copper sprays tend to protect a plant from Spores settling on the developing fruit. Brown Rot on Apricot What I love to see is interaction between fungi and insects. Here is a stinkhorn fungus with a decent amount of smelly, brown liquid. Flies are keen to harvest that brown stinky stuff and in doing so, they get the brown spores on their body. Those spores are distributed through gardens and forests. Joe Tucker, CEO of Enveric Biosciences, is developing non-hallucinogenic psychedelic drugs that aim to induce neuroplasticity and beneficial changes in the brains of patients with mental health disorders. The FDA rejected the approval of MDMA for PTSD treatment due to concerns about the inability to run blind trials to separate the placebo effect from the effects of the drug. Enveric addresses the promise of neuroplastic drugs that stimulate the neural connectivity between the prefrontal cortex and amygdala without hallucinations. Joe explains, "And the problem, endemic to hallucinogenic psychedelics, is that there's no question in the patient's mind, in the doctor's mind, in everybody's mind whether or not they got an hallucinogenic agent. And so when everybody knows whether they got it or not, that's what you call functional unblinding. And so the FDA was very concerned about this. They said you must be able to separate the placebo from the actual drug. Until we see data that gives us confidence that you separated it, we can't approve it, even though it looks like a very positive impact for the patients." "We were looking precisely to see if we could remove the hallucination without removing the benefit. So that's the field we're in. What's it being called right now? It's a very, very new field being called the non-hallucinogenic neuroplastogen- neuroplastogen indicating that it induces neuroplasticity, in other words, rewiring of the brain, but does so without inducing hallucination. We saw two challenges coming: How do you separate placebo from non-placebo effect and the treatment?" "Then the idea would be, hopefully, this psychedelic treatment was so impactful, you didn't need to take any other treatment again for a long period of time. And that seems pretty aspirational, honestly, and likely to get in the way of real patient acceptance that you want something which is not so sort of rock your world. Most people don't want that. They like to be able to have a drug that is like, take an aspirin. You take it every day. You don't have a big impact. You can go about your life. Visit your primary care provider for additional medical advice.Definition Cannabinoid hyperemesis syndrome (CHS) is a syndrome of cyclic abdominal pain, vomiting, or nausea in older adolescents and adults who have chronic ϲаnոаbis use.The term “marijuana” is considered racist by some people. In the 1930s, American politicians popularized the term “marijuana” in the U.S. to portray the drug as a “Mexican vice” and to have a justification to persecute Mexican immigrants. Epidemiology The overall prevalence of cannabinoid hyperemesis syndrome is unknown due to a lack of definitive criteria or diagnostic tests. It occurs in a population that may not disclose substance use. One study conducted in 2015 in a United States urban emergency department not named, found one-third of patients with near-daily cannabis use met criteria for having had CНЅ in the prior six months.Why are rates of CHS increasing?Between 2005-2014 hospitalizations cyclic vomiting syndromes increased by 60 %. concurrent cannabis use in hospitalized patients increasing from 2 to 21 percent. 7 years after the commercialization of cannabis in Canada, the Canadian health services found a 13-fold increase in cyclic vomiting syndromesPotential correlations for the increase in CHS are increased legalization and commercialization of cannabis, higher tetrahydrocannabinol concentrations in cannabis products, and increased recognition of the syndrome.Legal status of Cannabis in the USCannabis is legal in 24 states: Alaska, Arizona, California, Colorado, Connecticut, Delaware, Illinois, Maine, Maryland, Massachusetts, Michigan, Minnesota, Missouri, Montana, Nevada, New Jersey, New Mexico, New York, Ohio, Oregon, Rhode Island, Vermont, Virginia, and Washington. It is also legal in Washington, D.C. Cannabis is approved for medical use in 38 states.Federal level: Cannabis is a Schedule I drug, under the Controlled Substance Act (added in 1970) in the group of Hallucinogenic or psychedelic substances. Tetra-hydro-cannabinol (THC, a “mind-altering substance in cannabis”) is on the same list. However, cannabidiol (CBD, derived from hemp or non-hemp plants) was removed from the Controlled Substances Act in 2018. CBD is FDA-approved (under the name of Epidiolex®) to treat rare seizure disorders. CBD is still on the list of controlled substances in some states. I see THC as a problem.THC increased concentration As recreational Cannabis becomes more normalized, innovators look to find new ways to differentiate their product and increasing THC has become a common way to perform this similar to alcohol content in the beer, wine, and liquor industry. An article by Yale School of Medicine titled “Marijuana: Rising THC Concentrations in Cannabis Can Pose Health Risks” states, “In 1995, the average THC content in cannabis seized by the Drug Enforcement Administration was about 4%. By 2017, it had risen to 17% and continues to increase. Beyond the plant, a staggering array of other cannabis products with an even higher THC content like dabs, oils, and edibles are readily available—some as high as 90%.”Recently, cannabis-infused water started to be sold in some grocery stores.Pathophysiology of CHSIt is not entirely understood. Some suggest multifactorial involving cannabinoid metabolism, exposure dose and tolerance modifying receptor regulation, complex pharmacodynamics at Cannabinoid receptors, and even changes in genetics and cannabinoid variation in plants. CB1 receptors are involved in gastric secretion, sensation, motility, inflammation, and lipogenesis. The activation of CB1 and CB2 receptors has been suggested as the possible cause of CHS.Risk FactorsCHS can occur after acute or acute on chronic use but many report daily 3-5x cannabis use cannabis use over one year and many over at least two years. Median age 24 years. Interesting factsMedical visits for inhaled cannabis are more likely associated with CHS while edibles are more likely for acute psychiatric reactions.Also, CHS is a paradoxical effect since cannabis and cannabinoid receptor agonists are known antiemetics (as seen in nabilone and dronabinol (synthetic analogs of THC)) and prescribed by some physicians to combat chemotherapy effects.Clinical Features of CHSCyclical pattern with abdominal pain, severe nausea, and vomiting up to 30 episodes daily. Pain is intense and even referred to as “scromiting” due to its intense nature, causing patients to scream and vomit concurrently.Typically, it presents with 2 or more episodes over a 6-month period with no symptoms in between. It starts within 24 hours of last cannabis use (differentiating from cannabis withdrawal) and occurs at day or night. There is a gradual symptom resolution of nausea and vomiting after several days of cannabis cessation. Some patients had symptoms 2 days to 2 weeks after cessation. Diagnosis of CHSClinical diagnosisRule out neurological symptoms such as migraine headaches, acute abdomen, motion sickness, and medications, such as recent antibiotics and chemotherapy.Often the diagnosis is discovered with a thorough history reporting a decrease in symptoms with hot showers/baths.Management of CHS AcuteRehydrate with Fluids Dopamine Antagonists– Droperidol (0.625 or 1.25mg) /Haloperidol (0.05 to 0.1mg/kg with max dose of 5mg initially) favored over typical antiemetics like Zofran or Reglan.If needed, combine with an antiemetic like metoclopramide IM or ondansetron IV and consider patients' dehydration status likely requiring US-guided IV.Topical capsaicin cream 0.025 – 0.1% on the abdomen. Long term97% resolution of symptoms completely in a systematic review of patients who stopped cannabis use.Reinforce it may take several weeks of abstinence for symptoms to resolve and symptoms can worsen if cannabis is resumed. It is unknown if a reduction in use can prevent recurrence.Approaches in the clinicEducate patients on the etiology of their symptoms with complete cessation of cannabis use.Consider referral to counseling for cannabis use disorder and abstinence support for treatment-seeking cannabis users. Approach topics such as changing one's environment, seeking social support, and using self-help techniques to non-treatment-seeking individuals.Consider referring patients with polysubstance use and significant comorbidities to a supervised withdrawal management setting. Conclusion: Cannabis use is increasing with legalization and commercialization across the United States. With increased use, Cannabinoid hyperemesis syndrome incidence increases. Often it can be diagnosed with a thorough history including chronic cannabis consumption and symptomatic relief by showers. Physicians will need to develop counseling approaches to better understand CHS patients and how to approach an often-difficult topic.Even without trying, every night you go to bed a little wiser. Off to Scotland this week. Four secondary school students (two delinquents, one street-tough poseur rapper wanna-be, and a diligent hardworker) are set off in the wilderness on the Duke of Edinburgh Award program. One of them wants to do a good job to pad his CV, the other three want to screw around and get high. You may be familiar with the setup. The four soon find themselves in the crosshairs of an upper crust psycho whose annual sport is to kill the ne'er-do'wells who are sent out on this expedition. Hallucinogenic rabbit feces. Wackiness ensues. Social commentaries are made. The movie has a hard time mixing the "psychological thriller" genre with the "goofball comedy" genre and end up offering a fairly inoffensive product. It is entertaining, though. Get Duked Final Grades Steve: Another middle of the road movie. It's a reasonable movie to watch, so don't avoid it, but there's nothing particularly noteworthy about it. Relatively family friendly. Calling this a "horror" movie is a stretch. 3/5 Brandon One of a handful of movies that don't stir a strong reaction for or against. Production quality is good. If the sense of humor in this movie suits you, you'll enjoy it. 3/5 Cocktail of the Week: Rusty Nail 2 2/3 oz Scotch Whisky 1 1/3 oz Drambuie Cocktail Grade: Essentially a Scottish Old Fashioned. Whiskey base and the Drambuie serves as a sort of Old Fashioned Mix. There are different theories on how to proportion the cocktail, so do what you want. The Drambuie is pretty honey-heavy and very sweet, so perhaps go with a stronger whisky pour than we did. Still a 5/5. Joe explains, "This whole neuroplastogen space was recently discovered, and it was discovered through academic research that we're looking at the effects of these agents we call psychedelics, psilocybin being the active ingredient in magic mushrooms and a very well-known psychedelic agent. It was in looking at psilocybin and other molecules known to be psychedelic agents like ayahuasca and LSD and others that scientists realized that what's going on is you're activating a particular receptor in the brain - something called the serotonin 5-HT2A receptor. So, at Enveric we thought this was a great opportunity because of some of the early work that others had done in using the actual magic mushroom." "This is a whole new area of research and science. It is not known what shape of a molecule will induce a hallucination versus not inducing hallucination, for example, and what shape will strongly induce the neuroplasticity that we're looking for so that the brain can essentially be repaired versus not induce it. So, we had to develop our own internal artificial intelligence tool that was trained by the other molecules that had been designed previously or were naturally occurring. McKenna received his masters in botany at the University of Hawaii in 1979, followed by his doctorate in the same field at the University of British Columbia in 1984. Dennis is the brother of Terrence McKenna, a cultural figure and proponent for the exploration of psychedelics. Together they co-authored The Invisible Landscape. Much later Dennis would write a memoir, Brotherhood of the Screaming Abyss,  detailing he and his brothers exploits in the field. LSD is a powerful, mind-bending drug. Only a crazy person would try and tell a coherent and compelling two-hour-plus historical narrative while frying their brain on the equivalent of four hits of acid. And well... I am that crazy person. Today, to celebrate the 300th episode of Timesuck, I try and pull this story off immediately after dropping two double-hits of LSD. And things get.... weird. Also, what an amazing story! Bass Reeves' name and story are finally getting more exposure and it's about damn time! There's a good reason some people think Bass is the real-life inspiration for the Lone Ranger. This guy was a borderline superhero. Quick on the draw, fearless, and possession a relentless sense of justice, if you were and outlaw and Bass had ahold of your warrant.... you were as good as caught. Or as good as dead. The man was like a Wild West Boba Fett. He was the best at one of the hardest jobs in the Wild West  - a Deputy Marshal tracking down wanted men - and the occasional wanted woman - in Indian Territory, an especially lawless land that sat where present day Oklahoma now stands.
