Kurt Bramer from Advanced CPR Solutions returns again this week to continue the series on Head-Up CPR and other promising sudden cardiac arrest treatments. There are three previous episodes in this series. Elevated CPR Series Episode One Elevated CPR Series Episode Two Elevated CPR Series Episode Three After the severe weather update from disaster meteorologist Dan DePodwin, the Disaster Podcast team focuses on the science and physiology of Head-Up CPR. They ask why this approach provides for better protection of neurological function during a cardiac arrest. Dr. Joe Holley does an excellent job of describing the specific concepts of pressure gradients in blood flow to and from the heart during CPR in various patient positions. Kurt outlines what he calls the Four Rules of Head-Up CPR that are essential components for successful resuscitation during cardiac arrest treatment. Jamie Davis, the host of the Disaster Podcast, introduced the episode, which would focus on the science and physiology of elevated CPR, while Sam and Dan briefly discussed the early winter weather conditions across the United States. Head-Up CPR Research Advances The discussion focused on head-up CPR, a novel research advancement in resuscitation aimed at improving neurologic survival. Kurt explained its origins, highlighting a case study involving a ski hill cardiac arrest patient who survived due to an elevated head position, which was later recognized as a critical factor. The conversation also touched on a study conducted in South Korea, where researchers developed a modified EMS stretcher to accommodate patients in high-rise buildings with small elevators, leading to the question of the optimal CPR position. Initial findings from animal studies suggested that a head-up position could reduce intracranial pressure, potentially improving outcomes during CPR. CPR Physiology and Blood Flow Joe explained the physiology of CPR, focusing on how differences in pressure inside the body create blood flow. He described how chest compressions increase intrathoracic pressure, pushing blood towards the brain and other areas of lower pressure. He emphasized that over-ventilation and incomplete chest recoil can be detrimental during CPR, as they prevent blood from flowing back into the heart. He also noted that patients who exhibit “guppy breathing” during resuscitation have a better chance of successful resuscitation. CPR Vacuum Technology and Physiology Joe explained the physiology of blood flow during CPR, focusing on the importance of preload and the manipulation of pressure differences between the thoracic cavity and the brain. He described how the ITD16 device creates a vacuum in the chest to enhance blood flow to the brain and reduce intracranial pressure. Joe also discussed the concept of “head-up CPR,” which combines enhanced vacuum with gravity assistance to improve blood flow and neuroprotective outcomes. Head-Up CPR Techniques and Safety Sam and Kurt discussed the importance of using proper equipment and techniques for head-up CPR, as outlined by Dr. Joanna Moore’s “four rules.” Kurt emphasized that suction cup CPR in conjunction with an ITD or rescue pod is crucial for effective and safe head-up CPR. He also stressed the need for a priming period, avoiding excessive head elevation, and slow head-up positioning to prevent blood from draining from the head faster than CPR can pump it back in. Kurt noted that studies claiming head-up CPR doesn’t work often violate these rules, highlighting the significance of adhering to the correct procedures. Head-Up CPR: A Novel Approach The team discussed the importance of proper technique in head-up CPR, emphasizing that it requires a bundled care approach with specific parameters to be effective. Joe and Kurt shared a case study of a retired fire chief who successfully underwent head-up CPR, highlighting the potential benefits of this novel approach. The discussion also touched on the concept of using a combination of therapies in resuscitation, similar to the drug cocktail approach used in treating HIV. Success Stories in Advanced CPR The meeting focused on sharing and discussing remarkable cases of successful CPR, particularly highlighting the concept of “head-up” CPR, which has shown improved outcomes. Kurt and Joe shared several stories of patients who experienced significant neurologic recoveries after cardiac arrests, with survival rates and recovery rates significantly higher than national averages. The discussion emphasized the potential of advanced CPR techniques to improve survival and neurologic outcomes, with Joe noting that two of his services have seen similar success rates. What if the thing you've been following your whole life isn't your guide - it's a shadow? In this episode I'm calling you to wake up, brother. We dig into the Matrix metaphor not as science fiction drama but as a clear map of how collective narratives, institutions, and systems (government, money, religion, AI) become second-hand authorities that keep you walking in circles with your head down. This episode is both a wake-up call and a practical guide: learn how to recognize those shadows, use them when useful, and - most importantly - stop letting them be your authority.I unpack why this matters by drawing from human design (my defined head center and the channels that make me a transmitter of meaning), spiritual teachers like Krishnamurti, and timeless wisdom (Rumi: “you are the entire ocean in a drop”). I explain the difference between information and experience - the red pill of remembering who you are - and why true change requires embodied knowing, not just more facts. We trace how the “matrix” is a creation of a creation: useful, derivative, and dependent on our energy. That dependence is why the matrix seeks to keep you distracted and compliant. But it is also why it can be repurposed: once you pick your head up you can use these tools (sundials, systems, institutions) intentionally - rather than be used by them.We move through Spiral Dynamics and the shift from the first tier (head-down, external authority, sustenance and survival memes) into the second tier (inner authority, being, and self-realization). I show how community care and unity talk can still be head-down if they come from abdication rather than empowerment - and how the real aim is to teach people to remember themselves so institutions eventually serve the individual instead of the other way around.Expect practical analogies (shadow as sundial and compass), stories from coaching clients who reclaimed authority during times of pressure, and a firm yet compassionate call to action: pick your head up. This episode isn't about abandoning creating, building, or governing - it's about changing the relationship so that our creations serve our becoming. Scariff's Michael McNamara has been appointed Co-Chair of the European Parliament's AI Monitoring Group, which has responsibility for implementing the Artificial Intelligence Act, that became law in August. The group will aim to ensure systems used by businesses, schools, states, and the general public are fairly regulated based on the risk they pose to society. Paul was born in East London and played in the streets close to his beloved West Ham United. By the age of 8 he had already made his mind up that he was going to be in the military.His first choice was the Royal Marines but the recruiting office that he went to steered him towards the 9th /12th Lancers. He eventually trained in the Royal Tank Regiment and then transferred to the Blues and Royals. One of the officers in the Blues and Royals at the time was James Blunt. During his service Prince William, Prince Harry and Arthur Soames (Winston Churchills grandson) were also officers in the regiment.Paul saw active duty in Afghanistan and Iraq and saw war first hand. He was continually engaged with the Taliban and was subjected to one of the biggest ambushes in the Afghan conflict. It was during this fight that he worked alongside one of the UKs most decorated soldiers Mick Flynn. The military angels were looking down at Paul and his comrades. They were engaged in a fight and despite being small in number they successfully defended their position. Some of their colleagues were less fortunate.Mental trauma began to take hold and during an incident Paul had convinced himself that a man on the tube was carrying a bomb. Paul tackled the man to the ground and when police officers attended it was quickly discovered that the man was an innocent commuter and Paul was duly arrested and taken away in handcuffs. Despite the best efforts of some members of the military Paul remained in service but was eventually medically discharged..After losing several comrades to suicide, Paul founded Head Up to enable veterans and service personnel to access mental health support. If you're the kind of person that doesn't give a whole lotta' "you know what's" about fine jewelry or diamonds, or where the prices come from, I'm here to tell you, that's okay. In fact, I think it's perfectly normal. If diamonds, gemstones, precious metals and fine jewelry don't interest you…don't learn about them. I believe this is true for pretty much all things. If something doesn't interest you, find someone who knows their stuff, and let them do the work for you. This is a common theme in pretty much every podcast I've done so far, mostly because it's almost always a fail safe way to get things done quickly, and easily. (With one important footnote that I'll mention at the end.) I think the idea of finding an expert to make your life easy, particularly in the jewelry department, makes sense and saves time because there is a lot to learn, and quite frankly a lot of people think it's boring. And that's perfectly okay, because there are people like us…professional jewelry experts…all over the world…who DO take an interest in it, and learn about it, and study it and then arrange for these valuable items, and powerful symbols to be at the ready, when YOU'RE ready to take the plunge. I know people want you to care more, but if you don't, you don't. And there's no shame in that. What I've found over the years is that a lot of guys are cool with whatever they buy when they know just 2 things: How much does it cost? Is she going to love it? These are highly logical questions, and important ones. They may be the only 2 questions that need answering before you decide what to buy. This got me thinking. I used to know the late, great Chris Farley back in my college days at Marquette University, and that sometimes leads me to think of the movie Tommy Boy. (If you haven't seen it, you should. It's good old '90's humor, and Farley is a classic!) There is a scene the movie that illustrates my point about seeking out expert advice to make life easier as you get to your end game. It's near the beginning, and Tommy Boy's dad, "Big Tom," is talking “sales” at his wedding to a bunch of guys at and around his table, and he says, “Of course I could get a helluva' good look at a T-bone steak by sticking my head up a bulls ass…but I'd rather take the butcher's word for it.” I love that line because it clearly illustrates what I'm talking about when I tell people to seek out experts, and let them do their work on your behalf. Like when you're hunting for the right piece of jewelry. If you're not interested in knowing more about what you're buying besides what it costs and if she'll like it or not...no worries. Take the butcher's word for it...and hit the "go" button. But here's the footnote I promised: this only holds true if you're dealing with a reputable place. And how do you find that place? You ask around, read some reviews…use word-of-mouth…visit a website…or you could listen to Episode 36 called “Diamonds, Heroes, and A Simple Answer: The Best Advice for Making Jewelry Buying Easy” where I spell it out for you. Physically going to a jewelry store is a thousand times more effective than trying to purchase online, because you can actually talk to the butcher. (Of course I'm referring to a jewelry professional.) It's like having a human search engine standing right in front of you to answer your questions (if you have them) as they guide you to the perfect piece for your particular situation. To loosely quote Big Tom: "They're going to help you get that jewelry…and you won't need to stick your head up a bulls ass to do it.
