Anna Kerry went from yoga fan and enthusiast to Yoga teacher. Then the pandemic hit. A   year later, at age 35, she had a stroke due to as PFO. In this episode she shares her story. She tells us how yoga got her through stroke recovery and how it informs her work today. Anna talks about the relationship between trauma and yoga, and she talks about the impact stroke has had on her life with her husband. As Anna has gone through this journey and continued both her studies and her teaching, she developed a yoga program specifically for stroke survivors. In Anna's own words: I had a stroke aged 35 in March 2021. I've had a regular yoga practice for about 10 years and decided I loved the practice so much that I wanted to learn how to teach and share my love of yoga. I qualified in Aug 2020 and had only been teaching for around 7 months when I had a stroke. As the stroke came out of the blue I had to dig deep into my yoga practice to help me through and believe that my yoga practice has helped my mindset and my mental health during this traumatic time. I'm now in a position where I want to help other stroke survivors through their recovery so I designed The Life After Stroke Programme -- a 6 week programme designed to help stroke survivors regain their life and confidence through a holistic and embodied approach to recovery. What is a PFO? Anna's stroke was caused by a PFO. She found that out a month after her stroke, and she will likely get it fixed eventually. A PFO is a hole in the heart. Roughly 25% of the population has one. I have one. Guests Misha Montana and Christine Lee both had PFOs that led to their strokes. After we are born, our blood follows a path through the heart. It comes in the right side. When the heart beats, the blood on the right side heads out of the heart to the lungs. There, it drops of CO2, picks up oxygen, filters out clots, and heads to the left side of the heart. It will pour into the left side and when the heart beats, it sends that oxygen-rich blood on to the brain and other parts of the body. Then that blood drops off its oxygen, picks up CO2, and heads back to the right side of the heart to start the whole cycle over. Before we are born, though, the process is different. While we are developing in our mothers' uteruses, we don't breath air. All the oxygen and nutrients we need to build fingers and toes and kidneys and hearts and brains comes from the umbilical cord. Since we're not breathing air, there's no point in sending blood to the lungs. Instead, in utero it goes straight from the right side of the heart to the left side of the heart through a hole in the middle. That hole is called a Patent Foramen Ovale, or a PFO. It normally closes on its own shortly after we are born. A quarter of the time it doesn't close after birth, and that allows unoxygenated, unfiltered blood to sneak across the heart, skip the lungs and drag a blood clot to the brain. So, if you've had a stroke, and you have a PFO, should you have surgery to close that hole? Maybe. Christine and Misha had their PFOs closed. I did not. Anna is waiting to get her PFO closed. A Place of Her Own Anna Kerry has a special place in this world -- it's her yoga mat. At first glance, it's just a piece of material, but once she is on her mat it becomes a portal to take her to another special place. The mat allows her to center herself. It's a place she can experience joy and agony; happiness and anger; tears and laughter; and everything in between. When Anna is on her mat she can take a break from the rest of the world so that she can deal with the rest of the world. It triggers a mind shift to put her in a place where she can process things and, well, do yoga. Lots of us have things like a yoga mat. Maybe it's a special blanket or a childhood toy. Maybe it's a trinket that takes us back to a trip we took years ago or it's a gift from a lover or friend. Perhaps it's a special chair that holds our memories of the past. Once we touch or engage with that thing -- whatever it is -- we can feel a change in our own energy. Is that a bit woo-woo? Not really. Perhaps it is a metaphysical portal to a different plane of time and energy. Or perhaps it's another example of the core principle of neuroplasticity -- cell that fire together, wire together. Our brains work on patterns and shortcuts. That's why therapy works. The more repetitions w get in PT, OT, or Speech Therapy, the more we drive new neural pathways so we can walk, speak, or bake cookies again. Repeating a process reinforces those connections until we can almost do it automatically. A special place -- like Anna's yoga mat -- can do that, too. Sitting down on that mat can kick off those routines in the brain that shift us to a different place. The object starts the program running in our brains, and our brains do the rest. And the really great thing about Anna's yoga mat is that when life dictates, she can roll up her special place and take it with her. Stroke, Naps, Gratitude, and Yoga with News Anchor Kristen Aguirre Kristen worked as a news anchor in Denver, survived a stroke, and was fired. She worked to pick up the pieces of her life, returned to the anchor desk back east and found gratitude is the key. Win of the Week Shelly shared her win of the week with us. You can hear it in the episode. Here's what she had to say: My name is Shelly, and I had a stroke four months ago. I was at the hospital for two weeks of acute rehab. I came home in a wheelchair, but I've been working really hard, and this week I did my farthest walk -- 4.2 miles. Things are still not 100% for sure. My arm isn't working that well. I can't feel in the arm. But the leg -- I've just been walking so much that I think things have improved. The more I've done, the more I think things improve. I didn't understand that when I first had the stroke. In physical therapy, when they would say that I could learn to walk without feeling, but as I've done it and now people can't always tell that I had a stroke when I walk, so that's been exciting for me. Hack of the Week Anna explained we need to take time to check in with ourselves. Yoga breathwork is one way to do that. It doesn't need to be yoga, though. Anna offered 3 key ways to do this. First, don't shy away from your feelings. A therapist can help you explore them further. Stroke is trauma. Grieving is natural. Clinical Depression is a common result of stroke (see http://Strokecast.com/depression for more information). It's a major life change and it's perfectly normal to feel feelings about it. Ignoring them won't make them better. Therapists can help. Neuropsychologists are also available with special training to help folks with brain injuries (see http://Strokecast.com/karen for more details). Second, acknowledge and recognize anger, anxiety, and other uncomfortable feelings. Those feelings are trying to tell you something. Talk to your anxiety like it's a small child. It wants attention and it's throwing a tantrum. Why? Third, offer yourself kindness and compassion. You're not an expert in this new body, brain, and life. You'll get things wrong, and that's okay. Dr. Christopher Nguyen is a renowned geriatric neuropsychologist.  He defines Neuropsychology and ways that neuropsychologists assess, diagnose, and treat cognitive and behavioral issues related to the brain.  He and several of his colleagues are translating assessment tools from English to Vietnamese and creating culturally relevant questions.  We also discuss barriers to help-seeking among individuals of Vietnamese and AAPI-descent, and how we can engage in conversations with our Asian elders about planning for their future care. ========================================== Bio: Dr. Chris Nguyen is a geriatric neuropsychologist and assistant professor at The Ohio State University Wexner Medical Center. He completed his doctorate degree at the University of Iowa, an internship at the Ann Arbor VA Healthcare System, and a fellowship at the University of Oklahoma Health Sciences Center. He conducts neuropsychological evaluations with a wide range of patient populations and has a particular interest in dementia and other neurodegenerative diseases.  His research interests include topics in cognitive aging, decision making, civil capacities, and cross-cultural considerations in neuropsychology. Chris is a member of the American Psychological Association Committee on Aging and was recently elected to serve as President-Elect of the Asian Neuropsychological Association.  In his free time, Chris enjoys a good cup of coffee and spends time with family and friends.

NEUMANN: And every time something was brought up, you know, we would show them this picture, his face would be as flat as can be but his physiological responses, his arousal of responses were really strong every time he saw one of these emotion stimuli. And it was amazing because just looking at his face there was nothing there. And he doesn't think anything is there. You know I had asked him, “What would you do if your wife divorced you?” And he said as flat as he, you know, just straight faced as he could possibly be, “Well she's got to do what she has to do.” Yet, this massive spike so you could tell he had this emotional response. And so it made me think that like these people who have this blunted affect, I think that it's just unlocking something. There's something there that we have to get through. We have, it's a barrier that we have to get through and help them learn there's a threshold. And it's, this threshold has become so high in some of these individuals that we've got to lower this threshold and their ability to tackle when something's going on. And kind of unlock some of these physiological responses and so that's what led me to kind of develop the intervention that we have for teaching people with brain injury to gain insight into their emotions, detect these physiological responses, connect physiological responses with an emotional label and what they're feeling and then being able to communicate and express those things. And then, so, if this neural plasticity, I believe that it's there. I believe that as long as these, you know, these connections aren't fully and completely destructed. You know, you can revive some of these and strengthen some of these connections. ZUPAN: What happens is when people first have a brain injury, there's often so many things that they need help with, right? And so you're focusing on let's get them back to eating, let's get them back to walking, let's get them the skills they need to go back to work or to their life. And so this whole emotion, empathy, social cognition side of things kind of gets forgotten about or, in some cases, isn't recognized as a problem, because when you're in a hospital or rehab situation, everything's structured and everything's happening with a specific purpose or reason so sometimes those interactional problems don't show up but also people are, um, too busy being focused on other things. So then these people go home and they no longer have this regular therapy and everyone thinks they're ready to go…you know they've the skills they need to be fairly successful and it all falls to pieces…And that's because, if you're not responding or interacting emotionally and using social communication the way people expect, then that creates huge problems… EPISODE SUMMARY: In this episode of NOGGINS & NEURONS: Stroke and TBI Recovery Simplified, we chat at length with Dr. Barbara Zupan and Dr. Dawn Neumann about emotion perception following traumatic brain injury/stroke. He then joined Ruth Lanius at University of Western Ontario, a specialist in neuroimaging of post-traumatic stress disorder (PTSD), where they uncovered a neurofeedback protocol that positively correlated with reductions in mind-wandering, together with enhanced functional connectivity in a key cognitive control network (Ros et al 2013, NeuroImage). This led to the first translational study investigating its impact on patients with PTSD, revealing a positive effect on well-being and a plastic modulation of salience/default-mode networks (Kluetsch et al 2014, Acta Scand Psych). His research interests at the University of Geneva was focused on the potential application of neurofeedback in the study of neurocognitive function as well as in the treatment of brain disorders (for example, major depression). His aim was to use multiple windows (fMRI, EEG and TMS) to explore the brain's remarkable plasticity, in the hope that the outcome of this research may inform future treatment options for psychiatry and neurorehabilitation. Episode Summary:   In this episode, I talk with a long-time colleague, and trusted friend, Dr. Lori Spohr. We have both worked in the field of Neuropsychology for over 25 years helping patients with various neurological disorders including traumatic brain injury. Dr. Spohr is a rehab neuropsychologist and works primarily with patients with illness or injury like stroke, traumatic brain injury, dementia, or patients with chronic difficulties like multiple sclerosis.   Frequently following traumatic brain injury or concussion, loved ones and family members do not have a good understanding of the neurological process, what symptoms might be experienced, a typical course of recovery, what they can expect during recovery, and how they might be able to assist with recovery to help optimize healing.   In this episode, Dr. Spohr and I discuss the advances in traumatic brain injury over the last 30 years, the biggest struggle for traumatic brain injury sufferers, and the biggest struggles for their caregivers. We also then discuss how the TBI patient can still hold on to hope, and what they can do to help maintain good self-care in the recovery process.   Quotables from the episode:   If you are someone who has suffered from a traumatic brain injury, or are a caregiver for someone with #TBI, then listen to this episode! #concussion Neuropsychology utilizes assessment and testing to help patients better understand themselves and work toward improving their quality of life. Neuropsychology focuses on neurological illness or injury and how that affects a person’s behavior and thought. Now, the majority of patients who get a traumatic brain injury survive it. The brain controls everything about us. Patients with a traumatic brain injury often have changes in physical functioning, emotional functioning (sometimes depression, irritability, anger) as well as changes in thinking skills (like making decisions and keeping themselves safe). A traumatic brain injury affects the whole family. Initially the caregivers want them to survive, then they focus on the physical challenges because they are the most obvious, then they are faced with the changes in behavior and cognition. It can be hard to adapt both for the traumatic brain injury patient and their caregivers because you can’t see the injury. This makes it hard for the caregivers to appreciate that there is an ongoing problem. Even mild brain injuries have changes in attention, organizing, and planning, and those can be very frustrating deficits. For a traumatic brain injury patient, it is a matter of being able to accept what is while going on to adapt to a new normal. A patient with traumatic brain injury often has difficulty returning to school, to work, and to their relationships, and it can be difficult for friends and caregivers can struggle because the patient is often unaware of their own deficits. Neuropsychologists, rehab psychologists, and health psychologists help the patient and family members adjust to such changes. We would encourage traumatic brain injury patient to reach out to other professionals for help such as rehab psychologists, good speech pathologists, occupational therapists, physical therapists, and good behavioral medicine psychologists and therapists. Brain injury recovery continues well after two years. Don’t grow impatient or lose hope, because it is likely that things will continue to improve. Frustration is normal in the recovery process. It is natural to feel irritable and overwhelmed during recovery. Feel free to take breaks as the frustration grows but go back at it and don’t give up on it. Those who do the best are those who will persevere through it. Those who maintain a combination of perseverance and optimism experience the greatest hope. Sometimes it is a matter of learning something new. Many traumatic brain injury patients move on to new and different roles, jobs, and vocations. There can be a lot of change. We all need a way to contribute, even if that is different from how a traumatic brain injury patient previously contributed to society. About 50% of traumatic brain injury patients may go through a major depressive disorder, and even more will experience what is called an adjustment disorder. It is very common for people who have experienced any major medical condition (e.g. stroke, heart attack, organ transplantation, cancer, etc.) to experience some change in their mental health. Sometimes respite care is a very helpful component of treatment for traumatic brain injury patient caregivers to be able to continue helping others. You must take care of yourself first. Giving yourself permission to take breaks and utilize resources can help you remain effective in the longer run. It can be difficult for a caregiver to not take it personally after caring for someone with a traumatic brain injury patient may take out their frustration on those who are caring for them. It’s helpful to step outside of it and realize that they aren’t personally attacking, but reacting out of their own frustration. Part of taking care of ourselves includes getting enough rest, eating right, and when able getting physical exercise. Neuropsychologists frequently address driving safety in their clinical evaluations.
