Why do some patients struggle with anesthesia, requiring multiple cartridges just to get numb? Could your TMD patients have an underlying systemic condition that's been missed? Are you overlooking the signs of a connective tissue disorder? https://youtu.be/gaoJKPTV_Z0 Watch PDP222 on Youtube ”When you can't connect the issue, think connective tissue!” Dr. Audrey Kershaw joins Jaz for a fascinating deep dive into the world of connective tissue disorders and their hidden impact on dentistry. Together, they explore how hypermobility, unexplained joint issues, and even a history of spontaneous injuries could be key indicators of an underlying disorder. They also break down why dentists play a crucial role in screening and identifying these conditions, ensuring better patient outcomes and a more holistic approach to care. Because sometimes, when things don't seem connected… they actually are. Protrusive Dental Pearl: Don't just take a "relevant" medical history—take a comprehensive one! Encourage patients to share all health issues, even those they don't think relate to dentistry. You might uncover important clues about conditions like connective tissue disorders or sleep-disordered breathing, leading to better care and stronger patient trust. Key Take-aways Ehlers-Danlos Syndrome is often misunderstood and underdiagnosed. Patients with connective tissue disorders often face skepticism from healthcare providers. POTS is a common condition associated with EDS that affects blood pressure regulation. Many TMD patients may have undiagnosed connective tissue disorders. Awareness and education about EDS are crucial for better patient outcomes. The healthcare system can be challenging for patients seeking diagnoses. Research on local anesthetic effectiveness in EDS patients is lacking. Personal experiences can help in understanding and diagnosing connective tissue disorders. Collaboration between healthcare professionals is essential for patient care. Genetic testing is crucial for diagnosing rare types of Ehlers-Danlos. Dental professionals should be aware of the signs of connective tissue disorders. Diagnosis can empower patients to understand their health better. Holistic care is vital in managing symptoms associated with EDS and TMD. Medical histories should be seen as relevant in dental practice. The conference was the World Uyghur Congress and she was there to represent the Irish Uyghur Cultural Association which she had founded in 2024 as a way to bring Irish Uyghurs together. She was not entirely surprised.Zyden is a Uyghur, a Turkic Muslim from Xinjiang – a minority that has been subjected to massive surveillance and repression by the Chinese government over the past decade. The mother of three who has lived in Ireland since 2009 and is an Irish citizen, gets calls from the security services in China complaining about her political activities and suggesting she work with them. Her contact with her elderly mother in China is, she says, severely curtailed and monitored by the authorities.Moving away to live and work in Dublin has not protected her from the reach of the Chinese Communist Party. She explains how this impacts on her life and her determination to give a voice to the millions of Uyghurs in China who cannot defend themselves.Irish Times journalist Colm Keena with his colleagues at the International Consortium of Investigative Journalists has been investigating how the Chinese government monitors its citizens abroad for a major new report called China Targets. He explains how Irish citizen Naria Zyden became the victim of transnational repression.Presented by Bernice Harrison. Jeanne Lenzer is an award-winning independent medical investigative journalist and author whose hard-hitting investigations and analyses have appeared in medical journals, such as The BMJ and the Journal of Family Practice, and in outlets such as the New York Times, Washington Post, Smithsonian Magazine, the Atlantic, Washington Monthly, Newsweek Japan, Mother Jones, and Discover. Her first book, The Danger Within Us: America's Untested, Unregulated Medical Device Industry and One Man's Battle to Survive It, explores themes that have been at the heart of Lenzer's work over the past three decades: the intersection of money and medicine and how profiteering distorts medical science and undermines the public health, often by gaming or misrepresenting research to obtain a desired outcome. The book served as a basis for the International Consortium of Investigative Journalists' award-winning Implant Files project on medical devices and for the Netflix show, Bleeding Edge . It was used by John Oliver for his segment on medical devices as well as by the television show, The Resident for segments on the vagus nerve stimulator. It was favorably reviewed by Jerome Groopman in The New Yorker.  Her investigations have revealed hidden financial ties between industry and public institutions, including the U.S. Food and Drug Administration, the Centers for Disease Control and Prevention and the National Institutes of Health. In each instance, she documented flawed scientific recommendations that serve to protect profits over public health. Examples include the CDC's recommendation for oseltamivir (Roche, Tamiflu), a campaign that was paid for by Roche; and the FDA's approval of drugs over the (sometimes unanimous) recommendations of their own scientists - after being contacted by politicians beholden to manufacturers. http://www.jeannelenzer.com/

Yesterday, we featured a conversation with Jonathan Taplin about the dearth of high quality contemporary music and movies. The music writer, David Rowell, agrees with Taplin, but goes one step further, suggesting that we no longer even like new music. In his new book, The Endless Refrain, Rowell the rise of music streaming and the consolidation of digital platforms has so decimated the musical landscape that all we want to listen to now are the old hits of our youth. New music then, for Rowell, is being destroyed by new technology. The internet has frozen culture into a nostalgic mixtape of greatest hits. David Rowell grew up in North Carolina and graduated from the University of North Carolina at Chapel Hill. For nearly 25 years he was an editor at The Washington Post Magazine and has taught literary journalism in the MFA department at American University. He is currently a senior editor at the International Consortium of Investigative Journalists. Don't care all that much for new music these days? You're not alone and our guest is going to explain why. On the podcast is author David Rowell. His book is titled The Endless Refrain: Memory, Nostalgia, and the Threat to New Music. The veteran music journalist argues that the rise of music streaming and the consolidation of digital platforms is decimating the musical landscape, with dire consequences for the future of our culture. In The Endless Refrain, Rowell lays out how commercial and cultural forces have laid waste to the cultural ecosystems that have produced decades of great American music. From the scorched-earth demonetizing of artist revenue accomplished by Spotify and its ilk to the rise of dead artists “touring” via hologram, Rowell examines how a perfect storm of conditions have drained our shared musical landscape of vitality. Combining personal memoir, intimate on-the-ground reporting, industry research, and cultural criticism, Rowell's book is a powerful indictment of a music culture gone awry, driven by conformity and subverted by the ways the internet and media influence what we listen to and how we listen to it. A veteran music journalist argues that the rise of music streaming and the consolidation of digital platforms is decimating the musical landscape, with dire consequences for the future of our culture ... In The Endless Refrain: Memory, Nostalgia, and the Threat to New Music (Melville House, 2024), former Washington Post writer and editor David Rowell lays out how commercial and cultural forces have laid waste to the cultural ecosystems that have produced decades of great American music. From the scorched-earth demonetizing of artist revenue accomplished by Spotify and its ilk to the rise of dead artists "touring" via hologram, Rowell examines how a perfect storm of conditions have drained our shared musical landscape of vitality. Combining personal memoir, intimate on-the-ground reporting, industry research, and cultural criticism, Rowell's book is a powerful indictment of a music culture gone awry, driven by conformity and subverted by the ways the internet and media influence what we listen to and how we listen to it. If you live in Australia and require any support, please call the Butterfly Foundation on 1800 334 673.  Today's episode focuses on an important health topic that is rarely discussed, yet it's one that affects a number of women, and that's eating disorders in midlife and beyond. I'm sharing a conversation that I had with Associate Professor Gemma Sharp. Gemma leads a body image and eating disorders research program at Monash University in Melbourne, Australia. In her clinical work, she is a Senior Clinical Psychologist and Nationally Credentialed Eating Disorder Clinician in Australia.  She is very proud to have led the world's first comprehensive online resource addressing the intersection between eating disorders. This episode is absolutely fascinating; tune in as we talk about: How the hormonal changes in midlife impact your appetite and body composition. How it is for any age and time to start discussions around reproductive functioning, even as early as puberty. Breaking down what is an eating disorder and how it can look different for different women. How types of eating disorders seem to differ at different stages of life and for different genders. It's never too early to seek support, and how it can be hard to notice signs and symptoms. The different aspects of eating disorders and how they present themselves, for example, finding it difficult to engage in social eating, excessive time in the gym or body dissatisfaction. Talking to someone about an eating disorder. Changing body compositions in midlife and how we are not meant to stay the same weight our entire lives. Different cultures and attitudes towards eating disorders. As you'll hear throughout the interview, Gemma's drive and passion is leading to many world firsts that will benefit women for years to come. If you think it could benefit another woman, I would love for you to share this episode. Episode #182: “If you're a millionaire, or a billionaire, you want the best,” says Timo Schober, a German-based journalist who works at Papertrail Media, “and the best is natural grown teak [from Myanmar]...And that's what is driving the demand.”Schober joins the conversation along with Shirsho Dasgupta, an investigative reporter for the Miami Herald. They talk about the decimation of Burma's once vast forest lands which started during colonial times, and today is just 40% of what it once was. After the military coup in 2021, the US imposed sanctions on Myanmar Timber Enterprises (MTE), a military-linked corporation controlling the teak trade. Following a leak from Myanmar's tax department, the International Consortium of Investigative Journalists (ICIJ) collaborated with 39 media outlets to investigate teak exports and potential sanctions evasion, and their work is the subject of this episode. The international demand for high-quality teak remains strong, particularly among the ultra-rich. At least $400 million worth of teak has been exported since the coup, mainly to European and American markets.Traders argue that they don't buy directly from MTE but through private companies, and therefore their teak trade is legal. This creates complexities for customs officers in enforcing sanctions. Additionally, others believe their actions are legal as they bought teak before the sanctions, though officials state that the date of purchase does not exempt them.At the end of the day, Dasgupta reminds listeners that it comes back to finding a way to speak directly to the consumer about this murky business, as a way to discourage any involvement in teak. “A lot of luxury products that we covered, they do lead to conflict and the destruction of the environment, and we should keep in mind that even if we're not buying teak, we might be complicit in in other ways when we buy

The only way for the health care sector to sustainably contain costs and fulfill its mission is by putting the patient -- and the delivery of outcomes that matter to patients -- at the center of the industry's efforts. We must align incentives around the continuous improvement of health outcomes in a cost-effective matter. This is what value-based health care is all about – better health outcomes to patients for the money spent. Contrary to traditional approaches to health system reform that emphasize cost containment, value-based health care shifts the focus to continuous improvement in the outcomes delivered to patients. On the Race to Value this week, we are joined by two of the four authors from the new healthcare book, “The Patient Priority”. With this book, they wrote a practical step-by-step guide for clinicians, payers, policymakers, and other industry stakeholders to lead patient-centered, value-based health care innovation. It presents case studies from leading innovators and provides a roadmap for the comprehensive value-based transformation of national health systems. On the show this week is Stefan Larsson, MD, PhD -- an independent advisor in health care and life sciences, and a senior advisor to Boston Consulting Group (BCG).  Joining him is Robert Howard, a former senior editor at Harvard Business Review and MIT Technology Review who collaborates with BCG on the topics of value-based care and health system transformation.   Episode Bookmarks: 01:30 Putting patients first is the only way for the health care sector to sustainably contain costs and fulfill its mission. 02:00 Introduction to Stefan Larsson, MD, PhD and Robert Howard and their new book, “The Patient Priority.” 03:00 Support Race to Value by subscribing to our weekly newsletter and leaving a review/rating on Apple Podcasts. 04:30 The Triple Crisis:  There is a Crisis of Value, a Crisis of Evidence, and a Crisis of Purpose in American Healthcare! 05:30 How the vicious cycle of how a continued explosion of medical research knowledge drives higher costs due to anxiety and lack of guidance. 07:00 The crisis of outcomes (e.g. obesity, declining life expectancy) have persisted over a long period of time. 07:30 “The health care crisis will only get worse unless there is a fundamental shift in how we lead, define success, and agree on what good looks like.” 08:00 The criticality of shifting to an outcomes measurement model that values what matters to patients. 08:30 “Outcomes that matter to patients is the essence of the future of health care.” 09:00 Why the value-based care movement should focus more on the system than the individual provider organizations. 09:30 “The only way individual organizations can sustain value-based change is if we transform health systems as a whole – on a regional, national, and even international level.” 10:00 Health care is a complex adaptive system, and you don't change it through rules-based policy. 11:00 “Measuring outcomes in accordance to what matters to patients should be the key metric to drive change in health care.” 12:30 This importance of clinical registries that collect comprehensive data on health outcomes in a population of patients with the same condition. 13:00 How a fragmented specialty-dominated healthcare system limits our ability to develop consensus standards for measuring outcomes that matter to patients. 15:00 “By measuring the outcomes that matter to patients, we take a holistic view of health care.  We can then integrate all the different professional capabilities in the most ultimate way.” 15:45 The important work of the International Consortium for Health Outcomes Measurement (ICHOM). 16:00 How Sweden is a driving force for the international adoption of patient-centered outcomes measures and clinical registries. 17:30 Why a health care comparison of Sweden to the United States is relevant. Documents illuminate how Uber used stealth technology to thwart regulators and law enforcement and how the company courted prominent political leaders, Russian oligarchs and media conglomerates as it sought footholds outside the United States. The project is based on more than 124,000 emails, text messages, memos and other records that a former top lobbyist for Uber, Mark MacGann, provided to the Guardian. It shared the material with the International Consortium of Investigative Journalists, which helped lead the project, and dozens of other news organizations, including The Washington Post. Journalists from 29 countries joined the effort to analyze the records over four months.
