150 Years Ago (October 1874) The Husted Base Ball Club of Peekskill accepted a challenge from the Kellogg Base Ball Club of Cold Spring to compete for 10 gold badges that would be presented to the winner by Assembly Member Hamilton Fish Jr. When the Peekskill team arrived, Kellogg reported its local catcher was out of town and had been substituted with a Newburgh player. The Husted club protested; Kellogg offered to scratch him from the lineup if Peekskill removed a player who lived in Haverstraw. The visitors refused and left. Alexander Purdy of Breakneck, who had been walking his dog on a string, insisted he didn't hear any of the whistles blown by the northbound freight train whose cowcatcher tossed him over the south track and nearly into the river at Sandy Landing. Purdy suffered only bruises, but his dog was killed. The Cold Spring Recorder noted that District No. 3 spent $11 [$304] annually per pupil but District 13 spent $16.08 [$444], which the editor took as evidence that the schools should be merged. Officer McAndrew drove to the post office with a 3-year-old boy he had discovered playing near the tracks. The boy said his name was Charlie Hemway and that he lived in Matteawan [Beacon]. But suddenly he shouted "Mama!" and pointed to a woman on the sidewalk: Mrs. Daniel Henry of Nelsonville, who identified the boy as her son. He had been missing since breakfast. The schooner yacht Restless ran aground at Sandy Landing and had to be hauled off by the steamer Boardman two days later at high water. The wharf was covered with flagstones from Ulster County for Caleb Mekeel of Nelsonville to add to residences around the village. According to The Recorder, an express train cut off a portion of Mrs. McGwinny's dress when she was slow crossing the track. But Mrs. McGwinny insisted it wasn't true, and that "she is able to look out for herself." The annual hunting match took place on a rainy Saturday. A team captained by C.H. Ferris won with 922 beasts and birds. The day ended with a feast at Town Hall. J.Y. Dykeman was the individual champion with 302 kills. A dog was sent flying by the 8:12 p.m. express but got up and hobbled home. The Lone Star Club, a Black baseball team that the Poughkeepsie Press said was from Cold Spring, traveled to Poughkeepsie to play the Butterflies. The Lone Stars were actually from Newburgh, with The Recorder noting there had not been enough Black men in the village for a baseball team since the 18th century. Among the unusual items displayed at the Putnam County Fair in Carmel were a 200-year-old German trunk, 120-year-old shears and 143-year-old shoes worn in succession by Barnabas, Orpha, Ruth, Chloa, Heity, Polly, Daniel, Timothy, Samuel, Bizar, Jonathan and William Pierce. Four train cars left for the waterworks in Providence, Rhode Island, with iron piping from the Cold Spring Foundry that weighed 17 tons, 20 tons, 30 tons and 50 tons. A fifth pipe being cast weighed 62 tons. At a trustee vote for the Nelsonville school district, incumbent Stephen Mekeel received 19 votes and Isaac Riggs 17. But "owing to some informality," The Recorder said, "the chairman did not declare the result." In a second ballot, Riggs won, 22-17. Alex Skeene showed off a banana he had grown in a greenhouse. The little daughter of John Dillon was pulled off the track near the depot a moment before the southbound 1:51 p.m. train would have struck her. Mrs. Morris Englebride and her daughter were on their way to church when they passed a man walking unsteadily, with his hands in his pockets, across the Foundry Cove bridge. Soon after, they thought they heard a splash, but there was no cry for help. They told their story at church and several men investigated and found a body with "H.M." on the arm and a stamped rail ticket issued at Fishkill Landing [Beacon] for Sing Sing the day before. On this episode of Tent Show Radio, legendary Southern Rock band The Ozark Mountain Daredevils perform a career spanning set during their thrilling return to the Big Top stage. One of Southern rock's more inventive exponents, The Ozark Mountain Daredevils have always been indefinable in terms of music genres, producing sounds of country-rock and electric bluegrass all on the same record, writing lyrics both whimsical and poetic, and singing harmonies that send shivers up your spine. The Daredevils were formed in 1971 by the songwriting team of John Dillon, Randle Chowning, Larry Lee, and Steve Cash; with Michael "Supe" Granda and Buddy Brayfield completing the line-up. When the band emerged with their self-titled debut in 1973, they evoked not only the spirit of the age but the place from which they sprang: Springfield, Missouri, and the Ozarks—a place of pop, rock, country, mountain music, and much more. The critically acclaimed record, recorded in London under the direction of producer Glyn Johns, contained the US Top 30 single "If You Wanna Get To Heaven", while a second success, "Jackie Blue', which reached #3, came from the band's follow-up collection, “It'll Shine When It Shines.” Recorded at Randle Chowning's ranch, these records achieved “gold” status and showcased the group's strong harmonies and intuitive musicianship. Following a hiatus, the Daredevils were reactivated in the late 1980's by Dillon, Cash and Granda. The resulting album, “Modern History” found the band with a new lease on life as they carried on into the new millennium with a new line-up featuring Dillon, Cash, Granda, Ron Gremp (drums) and Bill Brown (guitar/vocals). In the years that followed, the Daredevils released a number of major label and independent record label recordings, traveled millions of miles performing throughout North America and Europe, and continued to grow and maintain a loyal and dedicated fan base throughout the world. Now more than 50 years since their founding, their songs are still regularly played on Classic Radio stations. On August 23. 2021 2 years ago today, a blood clot traveled up to my brain stopping the blood flow and nearly took my life. At 25 years old I had a stroke that left me paralyzed on my left side, blind in my left eye, short term memory loss, and paresthesia on the left side of my body from my toes to my head. I had to re-learn how to walk, use my left arm, and regain my cognitive function - all in silence. I was forced to quit my full time job, drop out of college, all while raising my boy as a single mother without any child support that year as I fought for not just my life but his. For 2 years I have masked my disability as I navigate family court in pursuit of full custody of my child. After years of strenuous physical, occupational, and speech therapy I am now at a place where my recovery has “peaked” according to science. (not God) Today I have lasting symptoms of numbness on my entire left side, no motor control in my left arm, and short term memory loss. But, I am alive. I am living proof that you do not need bruises to be abused. That emotional and psychological abuse can in fact kill you as the high amount of stress was a factor in my stroke. This is my living testimony and the miraculous ways in which God has weaved in and out of my journey with his handprints in everything. I want to thank Sarah and Austin Drake for adding joy to my hospital room, John Dillon for watching movies with me despite not being able to really “see” or “comprehend” it (lol) Amber for bringing me sushi so that I could get a break from rehab food. Pastor Jared Bridge and Anchor church for putting together meal trains and praying for me in the hospital. Nadia for bringing me Starbucks every week and being my friend in the high places and the lowest places. Kate and Justin for being willing to care long term for my son if I had died. My mom for caretaking over me and my son. Brayden for loving me and showing me that my disability doesn't take away from my worth and pushing me to continue healing. Rebekah Choi for her encouragement as an OT to encourage me not to give up as my healing point has “peaked”. My neurologist Dr. Girotra at UNMH for caring for me and advocating for me. One of the most important challenges facing Fatty Liver stakeholders involves improving early diagnosis for patients with clinically relevant or advanced fibrosis (F2/3). Today, a significant percentage of patients learn they are living with cirrhosis in the Emergency Department during a decompensating event. Four in ten of these patients in the UK do not leave the hospital. In this episode From the Vault, panelists review a model that Ian Rowe and Richard Parker developed to determine the most cost-effective strategy for F2/3 diagnosis.As this conversation starts, Louise Campbell notes that John Dillon reported in Barcelona that slightly over half of patients identified as having Fatty Liver via iLFT never returned for their appointments. Ian Rowe points out that this will reduce the costs in the model due to missed diagnoses but questions whether this cost reduction is truly beneficial since missed patient visits translate into non-treatment. More important, he reminds us that iLFT is under constant improvement so that data collected earlier in its lifecycle might not accurately reflect its costs and benefits today.Upon Roger Green's invitation for other questions Louise Campbell asks whether “we” (presumably the UK NHS) should do a better job supporting positive diet and exercise activity for healthcare professionals inside the system. On a more serious note, this leads Ian and Louise to focus on the policy issues that can support patients better (particularly those in the workforce). Ian points out that alcohol is issue #1 for many of these patients. He and Louise go on to point out the importance of public policy around things like soda and sweet food advertising in shifting consumers' focus or craving for bad foods. Roger suggests that this is a two-element issue: stopping advertising for soda and sweets will reduce disease over time but we also need to treat patients who already have developed disease. As he puts it, policy can “put your foot on the hose,” but even if we stop the in-flow of new Fatty Liver and metabolic patients, we still have “the patients in the hose” and they will need therapy.As a final thought before wrap-up, Ian shares his belief (which Louise also holds) that VCTE might be more effective in causing changes in patient behavior than blood tests, even if they point up the exact same patient need. The difference: feedback from VCTE comes in real-time where patients get face-to-face feedback and respond, whereas bloodwork results comeback to the patient several days later and delivered impersonally.From here, the group moves to the wrap-up question, which addresses research we should be doing and action steps/changes in behavior we should be promoting.

Given the vast amount of information and insight from The Liver Meeting, this episode sought to identify and explore a few key highlights. The panel (Jörn Schattenberg, William Alazawi, Naim Alkhouri, Laurent Castera, Ken Cusi, Wayne Eskridge and Roger Green) addresses several topics from the program.Naim begins this conversation with mentioning Labcorp's announcement that a reflex ELF test is available for patients with high FIB-4. The latter will be computed if the patient has a complete blood count (CBC) and a comprehensive metabolic panel (CMP). As Naim notes, this is similar to the Hep C paradigm where patients with a positive Hep C antibody test receive the Hep C RNA. For Naim, this is “a game changer” in primary care where patients with the basic CBC and CMP results can be flagged for advanced fibrosis using two widely-studied NITs.Will segues to discuss John Dillon's work with the intelligent Liver Function Test (iLFT), which was covered on the podcast earlier in the year. Since then, an abnormal iLFT result now leads to an ELF test. Like the Labcorp program, this follow-up makes it easier for primary care to identify patients with abnormal livers. Will cautions that more research on different ethnicities and disease levels ought to be conducted. At this point, Will exits the discussion due to a poor internet signal. While signing off he raises one last trend that has likewise surfaced on the podcast earlier this year. The encouraging note is that researchers are working to link NITs directly to outcomes instead of biopsy. In the remainder of this session, Ken discusses advances in the use of PPARs. He first notes the ongoing work with the pan-PPAR, lanifibranor, followed by Poxel's work with PXL-065. The latter is a deuterium-stabilized form of pioglitazone that generates greater activity of the r-enantiomer and less of the s-enantiomer. These are linked to mitochondrial benefit versus weight gain, respectively. Stephen Harrison discussed this molecule early last month in review of a recent string of press releases presaging some of the most promising data of the last decade in NASH drug development.

TRP Episode 82 on YouTube: Civil Rights attorney John Dillon, a prevailing attorney against California's so-called "assault weapon" definition and ban, reacts to the US Supreme Court's decision in NY Rifle & Pistol Assn v. Bruen (issued Thursday 23 June 2022). His victory in federal trial court against the so-called California Department of Justice came last summer, June of 2021, in Miller v. Bonta decided 4 June 2021. Interesting for our purposes here, the Judge Benitez's (Cuban ancestry) reasoning in striking down the ban as a violation of the Second Amendment was essentially the same reasoning used by the high Court in Bruen. Benitez used the Heller test, as he called it, before, and in addition to the flawed reasoning that the 9th Circuit had developed to counteract Heller, because they don't like Heller. He did not grow up as a "gun guy." He was a normal undegrad a UCLA. It's possible he partied a little too much. But he majored in History. He did well in school. He decided to go to Law School also in LA at Loyola. It was during law school there in LA that he went shooting at a club in LA and decided to buy his own firearm. There began his journey through the Alice in Wonderland type of web of irrational bureaucratic criminalization scheme that California and LA have enacted in the name of "public safety" and "police power." Police power, in these jurisdictions (like New York), could be symbolized with a fist in the air, power to the police, the police state. Innocence as a definition be damned. The once clear line between guilt and innocence in the California criminal law is now blurred, constantly moving, and sometimes not in line with anything approaching rationality or commonsense. This week, Surfing the NASH Tsunami returns to a subject we have explored from time to time over the past two years: helping patients with cirrhosis. While the immediate stimulus for doing so was the semaglutide late-breaker at #ILC2022, our more general interest is that many patients with cirrhosis will start to decompensate and decline in a fairly short period of time. This conversation focuses on a specific question: where is the next breakthrough in cirrhosis patient management likely to come?This conversation starts with Roger reflecting on a comment Lars had made during a previous appearance on the podcast, that we should spend more time focusing on what should be happening in the liver instead of what shouldn't. Lars notes one reason for this: when we look at other organ systems (heart or kidney, for example), we focus on how the organ functions. With the liver, we read static biopsies that do not measure actual function. He goes on to note that researchers are developing functional tests, many of which require labeled compounds and then record how they move through the body. At this point, Roger shifts the discussion by asking the panel where members anticipate the next major breakthrough in improving cirrhosis diagnosis and treatment. Louise responds first by noting the need to pick up patients earlier in the course of the disease as they move through the healthcare system. She envisions that this may come by adding. FIB-4 to every patient coming through the system or perhaps to something like the iLFT that John Dillon described at the Barcelona meeting. Even there, she notes, the system picks up only 50% of diseased patients. Jörn describes the need to improve diagnostic performance, both in terms of better tests and better ability to deliver both testing and results to patients. Lars questions whether the answer lies in providing imaging tests for every patient. Instead, he suggests, we need to link imaging results more closely to liquid biomarkers and then drive universal, early use of these less expensive liquid tests. As the conversation ends, Louise agrees with this point but notes again that we are assuming a level of patient communication and provider follow-through that does not exist today.

JOHN DILLON is recognized for his work as producer and co-host (along with his wife Vivian Nesbitt) of Art of the Song a one-hour program about songwriting and creativity heard across America on over 100 public radio stations. His book The 20-20 Creativity Solution gives readers a step-by-step process to enhance their creativity. As a luthier, John has built over 80 custom guitars for the likes of Hank Williams, Jr., Trisha Yearwood, the Mavericks and Steve Earle. His current project is HOPE ROAD - from Addiction to Recovery, an album of 12 songs that chronicle John's journey from being a carefree traveling hippie in the ‘60s to serious cocaine and alcohol addiction in the ‘80s. John recalls a ‘divine intervention' in the form of a run-in with the law. His true recovery journey began at an addiction treatment center in 1992. Subsequent songs on the album show how John has maintained sobriety and spiritual growth through gratitude, service and commitment to a 12-step program. John lives on a farm in upstate New York with his wife Vivian, her mother, two dogs, two horses and lots of guitars. John and his wife are going on 30 years of Recovery!! Injured in 2004 in a diving accident, this former chef had to pivot his professional life and ended up rediscovering his love of the outdoors, something that was important to him as a young man. In this episode, Jason shares the multiple accessible amenities that are available at John Dillon Park, including accessible lean-tos, which are cabins with three walls and and elevated bed that helps him camp independently, as well as the 3+ miles of accessible trails and wheelchair charging stations.
