Description: "When joint pain is present, the diagnosis of psoriatic arthritis needs to be made as soon as possible, ideally within six months to limit joint inflammation" Dr. Vinod Chandran mentions as he discusses efforts to identify a diagnostic test for those at risk of developing psoriatic arthritis. Join host Jeff Brown as he speaks with leading rheumatologist and clinician scientist Dr. Vinod Chandran, Director of the Gladman Krembil Psoriatic Arthritis Program, Schroeder Arthritis Institute, University Health Network and the Departments of Medicine, Laboratory Medicine and Pathobiology, and the Institute of Medical Science at the University of Toronto to learn more about the progress and promising results towards developing a psoriatic arthritis diagnostic test through multi-omic assays and identifying the distinct differences between psoriasis and psoriatic arthritis. This episode provides an update on the progress to date of the NPF PsA Diagnostic Test grant initiative which has shown promising results with a potential test entering prospective study in multiple sites soon. Thank you to Johnson and Johnson for their support of this program activity. Timestamps: (0:00) Intro to Psoriasis Uncovered & guest welcome rheumatologist Dr. Vinod Chandran. (0:52) It is challenging to diagnose psoriatic arthritis with many factors leading to a delay in diagnosis. (4:56) The start of Dr. Chandran's involvement with the PsA Diagnostic Test Grant project. (7:55) The different types of omics and the definition of multi-omic. (9:57) How the multi-omic approach is used to find biomarkers relative to a specific disease pattern. (11:08) Development of a predictive or prevention-based test using gene expression. (13:46) First year results identify 200 markers across different omic approaches that distinguish psoriatic arthritis from psoriasis. (14:58) The significance of MRNA vs mIcroRNA's use in development of a diagnostic test and how critical that is to dissemination of a potential test. (17:08) Identifying the skin-joint axis in relation to different types of arthritis. (20:20) Next steps to moving the diagnostic test research forward as a prospective study in multiple sites and the cost effectiveness of delivering the test. (23:13) If you have psoriasis, musculoskeletal, back, and joint pain think of psoriatic arthritis and be diagnosed early to maintain a good quality of life. Key Takeaways: · Given challenges associated with diagnosing psoriatic arthritis and the impact on quality of life, in 2019 NPF launched the PsA Diagnostic Test Grant project with the goal of developing an early stage test that would identify and diagnose those with psoriatic arthritis before debilitating joint damage begins. · Progress towards a PsA Diagnostic Test includes the study of multi-omic data sets where 200 distinct biomarkers have been identified leading to a greater understanding of the different pathways between psoriatic arthritis, psoriasis, and the skin joint axis. · A potential diagnostic test is now moving towards the prospective study phase. Until the test is available and if joint pain is present and you have psoriasis, ask your health care provider if it could be psoriatic arthritis and treat appropriately. Guest Bio: Vinod Chandran, MBBS, MD, DM, PhD is a rheumatologist, clinician scientist, and Director of the Gladman Krembil Psoriatic Arthritis Program, Schroeder Arthritis Institute, University Health Network and the Departments of Medicine, Laboratory Medicine and Pathobiology, and the Institute of Medical Science at the University of Toronto where he is also a Professor of Medicine. This week, Paul and Adam discuss the National Police Federation election controversy, the Attorney General's report on RCMP staffing levels, the RCMP's "expression of regret" for spying on Indigenous leaders (including former National Chief George Manuel) in the 1970's, and the arguments in the Supreme Court over Quebec's controversial legislation to prevent public servants from wearing religious symbols. Paul starts the show with a story of a meeting he had with a former biker gang leader, and then the guys discuss the resignations of two board members of the National Police Federation, over allegations that union leadership was trying to control nominations for the board of directors. This comes as the NPF is being sued over $3.39 million in payments to some of those board members. Also discussed this week, the Tiger Woods DUI arrest, the Justin King bail matter out of Newfoundland, Gladue Reports, and Halifax Police posing as homeless people begging for change in order to investigate traffic and criminal violations.

Hear leading dermatologist Dr. E. James Song and patient advocate Ana Rinck share their perspectives on the use of biosimilars to manage psoriasis and psoriatic arthritis. The use of biosimilars is growing with 24 currently on the market and more on the way for the treatment of psoriasis and psoriatic arthritis. What does this mean for you? Find out as we explore the use of biosimilars, the impact of interchangeability, what it's like switching to a biosimilar, and what questions to ask your health care provider with leading dermatologist and NPF Medical Board member Dr. E. James Song, Director of Clinical Research at Frontier Dermatology Partners, and patient advocate Ana Rinck who currently uses a biosimilar to treat her psoriasis and psoriatic arthritis. The intent of this episode is to offer an update on the use of biosimilars and to hear the experience of those who have used biosimilars to manage psoriatic disease. This episode is sponsored by Organon. Timestamps: (0:00) Intro to Psoriasis Uncovered and guest welcome to dermatologist Dr. E. James Song, Director of Clinical Research and Co-Chief Medical Officer, Frontier Dermatology, and patient advocate Ana Rinck, who presents her experience with using a biosimilar to treat psoriasis and psoriatic arthritis. (0:58) Definition of what is a biosimilar in comparison to a biologic which is also referred to as the original bio-originator or reference product. (3:10) What biosimilars are available and how they are used. (4:35) The term interchangeable and what it means for use of a biosimilar. (5:46) Ana's diagnosis to treatment options starting with use of topicals to ultimately use of a biosimilar. (7:57) Treatment discussion and choices when faced with diminishing effectiveness within a class of medications. Should a biosimilar be considered as an option? (9:50) Questions to consider when there's a change in treatment. (10:57) FDA announcement to simplify the development process for biosimilars and what that means for the future use of biosimilars. (13:55) What to consider when thinking of a biosimilar as a treatment option and where to find help when needed. (14:53) The hope and equity of access that biosimilars represent is here for the appropriate individual. Key Takeaways: · A biosimilar is an FDA-approved biologic product that behaves and functions like a brand biologic treatment which is also called the reference product or bio-originator. There are no clinically meaningful differences between biosimilars and their reference product. · The FDA has approved several biosimilars for the treatment of psoriasis and psoriatic arthritis. Some have been identified as interchangeable which means a biosimilar could be substituted for a reference product without a health care provider's approval pending state legislative rules. · Don't be afraid to ask questions or access resources to assist with preparing for a medication change should that change be initiated either by yourself, your healthcare provider, or your health insurance company. Guest Bios: E. James Song, M.D. is a nationally recognized dermatologist and clinical researcher who is also a Clinical Instructor at the University of Washington. Dr Song is recognized as a key opinion leader in treating psoriasis and atopic dermatitis. Dr. Song has been published extensively in peer review journals and regularly participates in meetings both as a lecturer and attendee. He is also a Medical Board member of the National Psoriasis Foundation. He believes that the best health outcomes come from shared decision making between the patient and physician. Dr. Song strives to help patients understand not only their condition, but all their treatment options. Ana Rinck is a patient advocate and volunteer for the National Psoriasis Foundation. She developed psoriasis in 2009 following symptoms she noticed for a few years which proved to be challenging to diagnose. She managed for many years on topicals and intralesional injections. In 2018 she was diagnosed with psoriatic arthritis and started use of a biologic realizing she needed to protect her joints from further damage. She now uses a biosimilar to manage her psoriatic disease. Ana owns her own private consulting business and is an Operations and Project Manager. Watch… https://youtu.be/jqE5vfTRewc About this episode… As advisory practices grow larger and more sophisticated, the definition of success is shifting. For many advisors, it's no longer just about income or payout. It's about ownership, alignment, and building something that carries real enterprise value. That shift raises important questions, such as: What does scale actually enable? How should advisors think about capital? And what does alignment really look like between firm leadership, capital providers, and the advisors they serve? To explore that, we invited three guests who see this from unique vantage points. James Poer, who leads Kestra Holdings, John Amore, who oversees the strategy and execution behind Kestra Financial's growth, and Fayez Muhtadie, who represents Stone Point Capital, Kestra's private equity partner. Kestra today operates one of the larger independent wealth management ecosystems in the country, supporting roughly 1,450 advisors and overseeing more than $160B in assets across its broker dealer and RIA platforms. Stone Point, for its part, is a financial services-focused private equity firm with decades of experience investing in banks, asset managers, insurers, and wealth platforms. Together, they represent a scaled, privately backed model that has become increasingly common in our industry. In this episode with Louis Diamond, they unpack what they describe as “multiple ways to win” actually means inside a platform of this size, including: The Kestra ecosystem—and how the firm has evolved from its founding to spin-off from NPF. The value of private equity ownership—and how common misconceptions impact the positive potential. The importance of cultural alignment—and how it can be preserved as firms grow. Growth and scale—and why James believes this business is not an income game, but a wealth game. Plus, the questions advisors should be asking when assessing their current firm or platform. If you're evaluating scale, ownership, or long-term enterprise value in your business, this is a conversation worth hearing. Want to learn more about where, why, and how advisors like you are moving? Description: Psoriatic disease affects far more than just the skin. Hear leading dermatologist Dr. April Armstrong and Dr. Benoît Guérrette discuss this and more with Jensen, a patient advocate. Psoriatic disease affects not only the skin but it can impact confidence, emotional and social well-being, and daily life. In this episode, join moderator Dr. Guy Eakin, Chief Scientific and Medical Officer at NPF, as we explore the disconnect between clinical classifications of psoriasis and what patients experience in real-life with leading dermatologist Dr. April Armstrong, Dr. Benoît Guérrette, Vice President of Dermatology & Rheumatology at Takeda, and Jensen, a NPF patient advocate and former Lead Youth Ambassador. Listen as we address the need for a more nuanced approach to classifying disease severity that accounts for the holistic needs of psoriatic disease, as well as share insights into how advocacy and awareness can drive change in treatment access and care standards. The intent of this episode is to identify how clinical severity classifications of psoriasis are evolving to meet the needs of those who live with the disease and how that change impacts overall management. This episode is sponsored by Takeda. Timestamps: (0:00) Intro to Psoriasis Uncovered and guest welcome to dermatologist Dr. April Armstrong, Vice President of Takeda, Dr. Benoît Guérette, and patient advocate Jensen, who discuss the unmet needs of people with moderate psoriasis and how as a community we can better serve those living with the disease. 2:22 How health care providers and the biopharmaceutical industry are coming together to address systemic eligibility and the unmet needs of people living with psoriasis. 4:25 Quality of life should be included when assessing clinical severity in psoriasis and identification of appropriate treatment choices. 6:52 The impact of misdiagnosis, inappropriate treatment, and effect on high impact sites can be life- altering. 8:30 How appropriate treatment and knowledge can make all the difference when diagnosed with plaque psoriasis. 9:40 Views on the psoriasis disease classification system and how it's evolving to include real life impact from physical and emotional needs, to more personalized care for those living with psoriasis, even when small body surface areas are involved. Wisdom from what I wish I had known previously. 16:52 Moving closer to care that truly reflects the lives and needs of those who live with psoriasis. Key Takeaways: · Severity of psoriasis isn't defined by skin coverage or body surface area (BSA) alone. The impact on quality of life should also be considered in the assessment, selection of treatment, and management of the disease. · The psoriasis disease classification system is evolving to be more of a patient centered approach. Many clinicians are now using the International Psoriasis Council (IPC) or 2 bucket approach to identify whether someone should receive a topical or systemic treatment based on location and response to treatment, as well as impact on quality of life. · With continued research and development, the next 5 to 10 years could see a shift in effective treatment options while also treating sooner to initiate better outcomes for people living with psoriasis and psoriatic arthritis. Dermatologist Dr. Erin Boh and patient advocate Brian Lehrschall discuss the challenge of managing weight, psoriasis, and psoriatic arthritis which share common inflammatory pathways and what can be done to improve all together. Join moderator Jennifer Bomberger as she asks what is the connection between psoriatic disease and excess weight and what can be done to counter the effects of inflammatory cytokines for both with leading dermatologist Dr. Erin Boh and Brian Lehrschall who has lost over 100 pounds using diet, exercise and a GLP-1 receptor agonist. Hear Brian's story along with what is a GLP-1, how they work, and side effects. The intent of this episode is to offer knowledge and an example of how a GLP-1 can be used successfully to improve the outcomes and management of psoriasis and psoriatic arthritis. This episode is sponsored by Lilly. A Word of Warning This episode contains information about the body, like body size, weight management, healthy eating, and lifestyle changes. If you have a history of disordered eating or are struggling with body-related mental health challenges, you might wish to explore some of our other helpful tools. There are clinical terms used by our speakers about body size or body weight that you might find triggering. Key among them is the word ‘obesity.' While it is the scientifically correct name for a disease and a common clinical term, NPF acknowledges that this word is triggering and problematic, and it is used only for the clearest of accuracy. Timestamps: (0:23) Intro to Psoriasis Uncovered and guest welcome dermatologist Dr. Erin Boh and Brian Lehrschall, who has been living with psoriatic disease for over 30 years and has recently lost over 100 pounds. (1:32) Health challenges Brian faced while trying to manage his weight, psoriasis, and psoriatic arthritis. (4:08) Many factors contribute to psoriasis, psoriatic arthritis, and comorbidities. (5:56) Discussing the sensitive subject of losing weight and changing lifestyle. (7:57) The inflammatory pathways and connection between excess weight and psoriatic disease. (14:50) Tips for managing multiple diseases like psoriatic disease, depression, and excess weight together. Realize “it's not your fault”. (21:32) Are low calorie or anti-inflammatory diets effective with psoriasis? (25:21) The definition of a GLP-1 (receptor agonist), how they work, and benefits in resetting how the body functions. (30:02) Brian's experience and impact of using a GLP-1. (32:08) Current and future use of GLP-1 in the management of psoriatic disease, comorbidities, and excess weight. (34:49) Be an advocate for yourself to help do all the things that will make you feel better about living with psoriasis and psoriatic arthritis. Key Takeaways: · Obesity or excess weight is a disease driven by an overproduction of inflammatory cytokines. · Psoriasis, psoriatic arthritis, and excess weight have shared pro-inflammatory disease pathways that should be considered together when identifying an effective treatment plan. · GLP-1 receptor agonists can be used along with dietary changes and exercise to induce weight loss that improves the body's reaction to medications used to treat psoriasis and psoriatic arthritis. Guest Bios: Erin Boh, M.D., Ph.D. is the Joseph Chastain Endowed Chair of Clinical Dermatology at Tulane University, School of Medicine where she is also a Professor and Chairman of Dermatology. Her clinical and research focus includes photobiology, chronic diseases such as psoriasis, psoriatic arthritis, and skin cancers such as lymphoma. Dr. Boh is a past Medical Board member of the National Psoriasis Foundation and is currently a President's Council Member which represents an elite group of health care providers who are committed to the mission of the Foundation and finding a cure for psoriasis. Dr. Boh has published many articles including addressing today's topic about weight management and inflammation in psoriasis. Brian Lehrschall has lived with psoriatic disease for three decades. He was first diagnosed with psoriasis about thirty years ago, at the age of 13 and with psoriatic arthritis at age 15. With the diagnosis came bullying which led to emotional eating. Per Brian “The more I ate, the worse the weight got.” Like most people who are overweight Brian tried to improve his health with diet and exercise. Along the way he learned about the connection between psoriasis and excess body weight. Eventually a physician recommended he try the weight loss medication GLP-1 (Glucagon-like peptide-1). Using this medication he has lost over 100 pounds and is still losing weight along with improving his overall health which includes his psoriasis and psoriatic arthritis. Brian joined the National Psoriasis Foundation in 2000 as a volunteer and has become an advocate for improving access to care and sharing his story about living with psoriatic disease. Welcome to EO Radio Show – Your Nonprofit Legal Resource. I'm Cynthia Rowland, and episode 130 is our 22nd in a series of "Nonprofit Quick Tip" episodes focusing on the details of state registration of nonprofit corporations. Today, with the help of Joe Hilliard, we look at the basic state filing requirements for nonprofit corporations operating in Alabama and Arkansas. In previous episodes, we discussed filing in Delaware and California, which are the most frequently used by our clients, most of whom have a connection to California and several other states. Earlier episodes have covered 40 states and the District of Columbia. After today's episode, the only remaining states are Georgia, Idaho, Iowa, Kansas, Mississippi, Missouri, Montana, and Nebraska. You can find all prior episodes on our website or on Farella's YouTube channel, where a playlist is available for state filing requirements.
