Joseph Tuma, M.D., FACC, FSCAI, Interventional Cardiologist at Heart and Vascular Institute, joins Mark Houston for a fascinating conversation regarding peripheral vascular disease, which is when cholesterol builds up in arteries other than those that are in the heart. Knowing the symptoms and early detection is crucial. Among those particularly at risk are people who smoke, who have high blood pressure or who have diabetes. Dr. Tuma lists common symptoms, like wounds that won't heal, and claudication, which is cramping pain from walking that gets better when you rest. Dr. Tuma also covers a subtype of peripheral vascular disease, such as when there are weak spots in the arteries that begin to dilate, called aneurysmal disease, and some recent advancements in cardiovascular treatments that the HVI team has implemented. Today we are continuing our cholesterol series. In Part One we discussed what cholesterol actually is, as it is often vilified in conventional medicine. We learned that our body needs cholesterol in order to build healthy cells. Cholesterol is a part of the outer layer of our cells, the membrane of the cells. It is also the initial part of the steroidogenic pathway. In Part Two we dove deep into LDL and HDL. We discussed that cholesterol cannot travel throughout the body on its own – it needs a friend. LDL is responsible for moving cholesterol throughout the body to the tissues, whereas HDL's role is to bring cholesterol from the tissues back to the liver. Today we are going to discuss apolipoproteins, in particular, Apo B and Apo A. We are going to talk about what they are – the role they play in the body – the cardiovascular risks or benefits associated with higher levels. September is Peripheral Artery Disease (PAD) Awareness month so our conversation today gave us great information about the unfortunate consequences of ignoring PAD symptoms, especially if they're diabetic and have other risk factors. It is part of our community health partnership with Valley Health where we talk each month with administrators, physicians, and other Valley Health staff about health topics, events, and the community. Joining me today: Ayorinde Akinrinlola, MD, Valley Health Vascular Surgeons (who sees patients in Winchester, Front Royal and Martinsburg) and medical director of the Peripheral Vascular Disease Clinical Council at WMC Eric McBride, NP, wound care/limb preservation specialist and clinical manager, Center for Advanced Wound Care & Hyperbaric Medicine on WMC campus Dr. A. and Eric answered several questions about Peripheral Artery Disease (PAD) -- also known as Peripheral Vascular Disease (PVD) – what it is, how to know you've got it, treatment options, and most importantly how you can prevent it. PAD causes decreased blood flow in the legs and may cause leg pain with walking or even at rest. It can also cause non-healing ulcers or, ultimately, gangrene. These patients are at high risk for limb loss over time. PAD affects approximately 6.5 million people aged 40 and older in the United States. One in three people over the age of 50 with diabetes will develop PAD. An estimated 200,000 people (disproportionately from minority communities) suffer avoidable amputations every year. Major risk factors include hypertension (high blood pressure), hyperlipidemia (high cholesterol), diabetes, smoking, and kidney disease.
