Kia ora,Welcome to Thursday's Economy Watch where we follow the economic events and trends that affect Aotearoa/New Zealand.I'm David Chaston and this is the international edition from Interest.co.nz.And today we lead with news markets are awaiting signals from the minutes of the late January Fed meeting.In the US, mortgage applications fell rather sharply last week, down more than -10% from the prior week to be -13% lower than a year ago. A key reason for the sharpish pullback was an unexpected surge in mortgage interest rates which jumped +19 bps to 7.06% (plus points) for the benchmark 30 year fixed loan. That is their highest of 2024. These higher rates reflect the shift in market pricing as the chances of near-term Fed rate cuts recede.Another assessment of Fed rate trajectories will come this morning (8am NZT) when they release the minutes of the FOMC's January meeting. The next Fed meeting isn't until this time next month. (But there is an RBNZ one this time next week.)American retail sales rose +3.0% last week from a year ago at bricks & mortar stores in the Redbook survey. This is barely enough to keep up with inflation, a second straight week like this after nine weeks of significant volume growth. A hesitation was always on the cards.Sharply lower oil prices, and exports at a 14 month high have combined to deliver Japan a sharply lower January trade deficit. Those exports were on the basis of good demand from both the US and China.European consumer sentiment improved marginally in February even if it still remains quite negative - just less negative.In Indonesia, their central bank kept its policy rate unchanged at 6%.In Australia, wages rose +0.9% in the December quarter, and +4.2% for the full year, (marginally more than the CPI rise of +4.1%). That's its highest growth since 2008.The UST 10yr yield starts today at 4.32% and up +6 bps from this time yesterday. Wall Street in its Wednesday trade is down -0.4% on the S&P500. Overnight European markets were very mixed again with Frankfurt up +0.3% and London down -0.7%. Yesterday Tokyo ended its Wednesday session down -0.3%. But Hong Kong rose +1.6% in their Wednesday trade while Shanghai rose +1.0%. The China Securities Regulatory Commission has imposed a restriction that prevents sales in the first and last 30 minutes of trading for prices that are lower, part of increasingly drastic measures to prevent the Chinese stock-market slump from extending into a fourth year. The price of gold will start today down -US$3/oz from yesterday at US$2027/oz.Oil prices are +US$1/bbl firmer at just under US$78/bbl in the US while the international Brent price is up to US$82.50/bbl.The Kiwi dollar starts today at just on 61.7 USc and unchanged from yesterday. Against the Aussie we are marginally firmer at 94.3 AUc. Against the euro we are still at 57.1 euro cents. That all means our TWI-5 starts today at just under 71.2 and little-changed.The bitcoin price starts today at US$51,382 and down another minor -0.4% from this time yesterday. Prevent Falls was a post-hardcore band from NJ that existed from 1998-2003. Their members included Jeff, Nash, PJ, and Nick. PJ and Nash later went on to join Ben Jorgenson (episode 46) in Armor for Sleep. Americans with Disabilities Act of 1990 (ADA) established guidelines for wheelchair users and in- dividuals with disabilities in the community. ADA guidelines are designed to meet the needs of the majority of users, no matter what their ability or disability level is. Although it does not special- ly apply to residential homes, it is important to keep the ADA guidelines in mind when modifying homes for an individual’s need. ADA guidelines have already been proven to be effective for most people. Many homes in Erie County were built prior to 1976 and before the current, more standardized building codes. Those homes were not built for wheelchair use, walker access, and older adults. For these homes, home modification may be necessary using ADA guidelines.
Falls account for many occupational therapy patients. Let’s provide pre-care and advisement instead of post care and rehabilitation.
Approximately one third of older people over 65 years fall each year. Home modifications may decrease occurrence of falls. This study aims to determine the risk factors of falls for frail older persons and to evaluate the impact of home modifications by an occupational therapist on the occurrence of falls. We conducted a longitudinal study using a quasiexperimental design to examine occurrence of falls. All participants 65 years of age and older and were assessed at baseline and 6 months after the intervention. Bivariate analysis and logistic regression models were used to study the risk factors of falls and the effect of home modifications on the incidence of falls.
One million inpatient falls occur annually in U.S. acute care hospitals. Sitters, also referred to as Continuous Patient Aids (CPA's) or safety attendants, are frequently used to prevent falls in high-risk patients. While it may make intuitive sense to use sitters to prevent falls, it does beg the question, what's the evidence that they work? We discussed with Drs. Adela Greeley and Paul Shekelle from the West Los Angeles Veterans Affairs Medical Center their recent systematic review published in Annals of Internal Medicine. Their review identified 20 studies looking at this issue (none of which are randomized trials). To sum up their findings, there were only two studies comparing sitters to usual care and they came up with conflicting conclusions (in one, the fall rate was lowered; in the other, it was not). In the other 18 studies, alternatives to sitter use were evaluated. The only thing that seems to have some evidence for was video monitoring (fall rates either stayed the same or improved, with a decrease in sitter usage). We also talk about multi-component interventions and how we should think about them. One intervention that is sometimes included in multicomponent interventions are bed alarms, which we discussed in our very first GeriPal podcast. It's also the podcast where we dreamed up the "anti-bed alarm" that would alert patients who haven't gotten out of bed yet. Here’s what tends to happen to someone who’s had a fall: they’re AFRAID of having another fall...this fear causes them to avoid activities that challenge their balance...as they become less active, their balance and strength get worse...as this happens, they become more likely to have another fall...meanwhile, they’re avoiding activities they used to enjoy, and they start to experience things like depression and anxiety...their “quality of life” is getting steadily worse over time, all because of one fall… If this sounds like you, this information should be very helpful! Topic #2: 3 Basic Steps to Maintaining Proper Balance. In this section, Luke gives you the overview of how balance works. Simply stated, here are the 3 steps: you must SENSE your balance, your brain must PROCESS this information, and your body must be able to RESPOND. Luke discusses how certain things can go wrong with these 3 steps. This knowledge helps you understand how to improve your balance. Topic #3: The “3 Balance Systems”. Learn all about how your body/brain sense your balance. The 3 systems include your vision, your sensation and your inner ear. Luke also explains a simple way to test the function of these 3 systems. Topic #4: Other Factors that Affect Your Balance. Balance is a multifaceted process, and there are several other factors that you need to understand and identify before trying to improve your balance. Topic #5: What is Vertigo? Have you ever experienced the sensation of movement, swaying or spinning? Luke will explain how to understand and address vertigo, including “BPPV”. Topic #6: How do we Successfully Treat Balance/Dizziness Issues? Now that you understand more about how proper balance works, as well as how many factors you need to consider, it’s time to talk about how to successfully improve your balance! Topic #7: What’s Next? To wrap things up, Luke shares 2 options with you if you want to take the next step towards improving your balance... We are confronted by violence in the news daily but who imagined that violence would reach the bedside? Violence caused by patients and visitors is an international problem that continues to worsen in its frequency and severity. In this episode, we talk to two advanced practice nurses who have led the charge in the Cleveland Clinic's response to violence in the workplace. GUESTS Dianna Copley, MSN, APRN, ACCNS-AG, CCRN serves as Clinical Nurse Specialist at the Cleveland Clinic in Ohio where she covers three high acuity Internal Medicine telemetry units. Since starting her nursing career at the Cleveland Clinic in 2009, she has served on a number of hospital committees and was involved in the development of a quality survey to explore nursing caregivers’ experience of workplace violence committed by patients and visitors. Catherine Skowronsky, MSN, RN, ACNS-BC, CMSRN is a clinical nurse specialist (CNS) for the Cleveland Clinic Health System. Catherine focuses on care of the medical patient with co-morbid behavioral health needs. Her role includes offering expert consultation on care planning, therapeutic communication and safety to the bedside nurse. This role requires her to act as a liaison between psychiatric and medical providers. She has written protocols to guide care of the patient who ingests foreign bodies and patients with substance use disorder. Catherine served as the medical-surgical adviser to develop national competencies for the medical-surgical nurse caring for the patient at risk for suicide. She is an experienced presenter and public speaker, often offering educational presentations within the health system and at local, national and international conferences. From 2011 through 2018 she traveled overseas to provide continuing education for the U.S Department of State. Skowronsky’s publishing experience entails authoring the manuscript “Close Observation of Medical Patients with Complex Conditions to Prevent Falls and Minimize the use of Patient Care Companions.” which was published in the Journal of Nursing Care Quality. She was a content reviewer for the AMSN Core Curriculum chapter on the respiratory system. Skowronsky is also the presenter of “But I’m not a Psych Nurse: Care of the psychiatric patient on the medical-surgical unit”. HOST Chelsea Parker RN, BSN, CMSRN was born and raised in Newport News, Virginia. She attended the University of Virginia and graduated in 2016 with a Bachelors of Science in Nursing. Since then she has pursued geriatric and palliative care nursing and developed a love for general medicine, which is where her foundation for med-surg nursing was built. As a young nurse looking for ways to bridge engagement between generations of nurses, she is excited about what this podcast will mean for AMSN.
